    In Vitro Comparison Of Fluoride Gel Alone and in Combination With Er,Cr:YSGG Laser on Reducing White Spot Lesions in Primary Teeth

    Introduction: Laser therapy has been suggested as a method for caries-prevention, and ErCr:YSGG laser is increasingly used in dentistry. This study aimed to compare the efficacy of fluoride gel alone and in conjunction with Er,Cr:YSGG laser for remineralization of white spot lesions (WSLs) in primary teeth.Methods: This study was conducted on 20 primary teeth with WSLs extracted for orthodontic reasons. Three sections were made of each tooth at the site of WSLs. The surface area of WSLs was measured under a stereomicroscope at ×25 magnification. Samples were assigned to 3 groups of control (group 1), exposure to 1.23% APF gel for 4 minutes (group 2) and Er,Cr:YSGG laser (0.5 W power, 20 Hz frequency, 60% water, 40% air, pulse duration of 5±1 seconds) plus fluoride (group 3). All samples were stored in artificial saliva for 10 days and then the surface area of the WSLs was measured again under a stereomicroscope. Data were analyzed using the Kruskal-Wallis, Mann-Whitney and Wilcoxon signed rank tests.Results: The reduction in surface area of the WSLs was significantly different between the control and fluoride (P < 0.001) and also the control and laser plus fluoride groups (P < 0.001); the difference between fluoride and laser plus fluoride groups was not significant (P = 0.265). Comparison of the surface area of WSLs before and after the intervention showed no significant difference in group one (P = 0.737) while this difference in groups 2 (P < 0.001) and 3 (P < 0.001) was statistically significant.Conclusion: The results showed that Er,Cr:YSGG laser irradiation plus 1.23% APF gel was not significantly different from the application of fluoride gel alone in enhancing the remineralization of WSLs

    Crazing level after pin insertion in anterior primary teeth: a preliminary in vitro study

    Objectives This study aimed to investigate dentinal crack rate following parapulpal pin insertion in anterior primary teeth.Methods Thirteen sound freshly extracted primary canine teeth were horizontally sectioned 1 mm above the cementoenamel junction (CEJ). All samples were thoroughly inspected to ensure that the teeth had no cracks. The teeth were then mounted in acrylic blocks, and subjected to drilling and insertion of a single parapulpal pin in the prepared hole. The teeth were then sectioned perpendicular to the already prepared surface at 1, 2 and 3 mm depths for further evaluation under a stereomicroscope (x12 and x25 magnifications).Results No crack or crazing was observed in teeth in the control group while one out of 11 teeth in the case group had a crack.Conclusion The use of 0.53 mm diameter self-threading pin did not increase the risk of crack formation in dentin of anterior primary teeth prior to composite restoration

    Effects of Shark Cartilage on Heterotransplanted Oral Squamous Cell Carcinoma Induced in Nude Mice

    Objectives The use of shark cartilage as a supplementary treatment has a long yet unresolved history in the realm of complementary-alternative medicine. This study aimed to investigate the impact of concentrated and purified extracts from Persian Gulf shark cartilage (PGSC) on oral cavity squamous cell carcinoma (SCC) (specifically, the KB cell line) induced in an animal model. Methods Ectopic tumors of oral cavity SCC were induced in eight nude mice through the heterotransplantation of the KB cell line. Once the tumor volume reached 100 mm3, the mice were randomly divided into two groups: treatment and control. The treatment group received shark cartilage at a dosage of 50 mg/kg body weight, while the control group received a phosphate buffer. The drug was administered daily for four days via the intraperitoneal route. Following this, the drug administration was halted for a period of five days before resuming (as per the NCI protocol). After 54 days, the animals were sacrificed, and their tumors were sent for immunohistochemical evaluation using Ki-67 and CD34 markers. Results The findings revealed a significant reduction in intratumoral blood vessels in the treatment group compared to the control group (P-value = 0.001). While there was a decrease in both the size of the tumor and the proliferation of tumor cells, this reduction was not statistically significant. The average proliferation index was 13.33% for the treatment group and 33.33% for the control group. Conclusion Significant decrease in intra-tumoral vascularity can control tumor spreading and metastasis, potentially playing an important role in cancer management of oral cavity SCC

    Nano-Hydroxyapatite and Calcium-Enriched Mixture for Pulp Capping of Sound Primary Teeth: A Randomized Clinical Trial

    Introduction: Nano-hydroxyapatite (NHA) has been used for regeneration of osseous defects. Calcium-enriched mixture (CEM) cement is also used for various dental treatments. This trial compared the efficacy of NHA and CEM cement for direct pulp capping (DPC) of sound primary teeth. Methods and Materials: In this randomized clinical trial with split-mouth design, after attaining informed consent, 20 sound primary canines scheduled for orthodontic extraction, were selected. After mechanical pulp exposure, the exposed site was capped with either NHA or CEM cement and then immediately restored with glass-ionomer and resin composite. The teeth were extracted after two months and examined histologically. Parameters of hard tissue bridge (HTB) formation, its type and quality as well as pulpal inflammation scores were compared between the two experimental groups. The data were analyzed using the Mann Whitney U and Fisher’s exact test. The level of significance was set at 0.001. Results: All CEM specimens showed inflammation score of 0 (less than 10%). However, in NHA group, inflammation scores of 0 (less than 10%), 1 (10%-30%) and 2 (30%-50%) were observed in 2 (20%), 4 (40%) and 4 (40%) specimens, respectively (P<0.001). HTB was formed in all CEM specimens while it was developed in 2 specimens of NHA (20%; P<0.001). All CEM specimens showed normal pulp; only two cases in NHA group (20%) demonstrated uninflamed normal pulp. Conclusion: CEM cement was superior to NHA as a DPC agent in terms of HTB formation and pulp inflammation scores. It is a suitable material for the DPC of primary teeth

    Conservative Management of Periapical Cementoblastoma: A Case Report

    As a rare ectomesenchymal neoplasm, benign cementoblastoma comprises less than 6% of all odontogenic tumors. The typical treatment plan involves surgical excision and extraction of the affected tooth. Limited evidence, however, suggests the conservative management of this condition as the best treatment. This article discusses the case of a 32-year-old man who had benign cementoblastoma and underwent conservative treatment. The diagnosis was established based on clinical and radiological features analyses. Root canal therapy was performed on the tooth, followed by enucleation, curettage, apicoectomy, and guided bone regeneration (GBR) 30 days later. After a year of follow-up, there was no recurrence, and the tooth was in healthy conditions. These findings demonstrated that the tooth affected by cementoblastoma can be saved. It was treated conservatively to preserve the patient’s oral health and masticatory function

    Objective: The Ortho keratinized Odontogenic Cyst (OOC) is a rare lesion originates from dental lamina and clinically, it may be mistaken for many other odontogenic cysts and Tumors. Microscopically, It should be distinguished from KCOT because of differences in biologic behavior and histologic features.Case: An interesting case of OOC arising in the edentulous mandibular right first premolar region of a 55-year-old woman is reported. Under the initial clinical diagnosis of a residual cyst, the excisional biopsy was performed. Because of detection of an orthokeratinized epithelium lining, a definite diagnosis of OOC was made.Conclusion: Microscopic examination is crucial for making the correct diagnosis of such lesions, therefore establishing patients' prognosis accurately

    A 62-year-old Man with Chondroblastic Osteosarcoma of Maxilla: A Rare Case

    Osteosarcoma (OS) is a malignant tumor that induces formation of osteoid or immature bone and rarely occurs in maxilla. Maxillofacial OSs are reported to occur in the third to fifth decades that is 1-2 decades later than the mean age for OS of long bones. The present case is a 62-year-old man with facial asymmetry and pain in his left upper jaw. Intraoral examination showed a bony hard, tender swelling about 50 x 60 mm in diameter in the maxillary alveolar ridge extending unilaterally from the maxillary incisor to the molar region which had expanded the buccal and palatal aspects of alveolar ridge. Computed tomography showed a hyper dense mass in the left maxilla extending to maxillary sinus. The diagnosis of OS was confirmed by histopathologic evaluation. Following the initial surgery, a local recurrence developed in the facial region of the patient within 18 months. Radiographic evaluation of OS of maxilla is important for diagnosis. Early diagnosis and radical surgery are the keys to increasing survival rate

    Coincidence of Compound Odontoma and Cemento Ossifying Fibroma; A Rare Case Report

    Objective: Cemento-ossifying fibroma defines as a relative rare osteogenic neoplasm of the jaw. This tumor includes fibrous and osseous components. Odontoma is the most common odontogenic tumor containing enamel ,dentin ,cementum and pulp tissue. in this paper we report a  rare case of ossifying fibroma associated with compound odontoma in the mandible. Case: A 37-years-old woman was referred to Oral Medicine department , Shahid Beheshti Dental School with complaint of swelling in the anterior part of the mandible, over 6 years period. Clinical examination revealed mandibular enlargement in right –anterior region with labial and lingual expansion and canine missing . panoramic view showed a large mixed radiolucent -  radiopaque lesion associated with impacted canine.The differential diagnosis include calcifying odontogenic cyst (COC) and cemento-ossifying fibroma(COF). Histopathologic examination established diagnosis of COF with multiple compound odontoma .Conclusion: The relationship between the occurrence of these two lesions is not clear and more studies are needed to establish the relationship between them

    The Effect of Mineral Trioxide Aggregate Mixed with Chlorhexidine as Direct Pulp Capping Agent in Dogs Teeth: A Histologic Study

    Introduction: The aim of the present investigation was to compare the efficacy of mineral trioxide aggregate (MTA) and 0.2% chlorhexidine (CHX) mixture to pure MTA, as a pulp capping material. Methods and Materials: The pulp of 24 lateral incisors and canines from four dogs were exposed and capped either with MTA or MTA+0.2% CHX. After 2 months the animals were sacrificed and the teeth were prepared for histological evaluation in terms of calcified bridge formation, the degree of inflammation and presence of necrosis. The Fisher’s exact test was used for data analysis. Results: The results showed that formation of complete calcified bridge in MTA specimens was significantly more than MTA+CHX (P<0.05). No significant difference was found in the degree of inflammation and necrosis between MTA and MTA+CHX groups (P>0.05). Conclusion: Mixing MTA with CHX as pulp capping agent had a significant negative impact on formation of calcified bridge on directly capped dog’s teeth.Keywords: Chlorhexidine; Mineral Trioxide Aggregate; Pulp Capping; Vital Pulp Therap

    Efficacy of the Biomaterials 3 wt%-nanostrontium-hydroxyapatite-enhanced Calcium Phosphate Cement (nanoSr-CPC) and nanoSr-CPC-incorporated Simvastatin-loaded Poly(lactic-co-glycolic-acid) Microspheres in Osteogenesis Improvement

    Aims The purpose of this multi-phase explorative in vivo animal/surgical and in vitro multi-test experimental study was to (1) create a 3 wt%-nanostrontium hydroxyapatite-enhanced calcium phosphate cement (Sr-HA/CPC) for increasing bone formation and (2) creating a simvastatin-loaded poly(lactic-co-glycolic acid) (SIM-loaded PLGA) microspheres plus CPC composite (SIM-loaded PLGA + nanostrontium-CPC). The third goal was the extensive assessment of multiple in vitro and in vivo characteristics of the above experimental explorative products in vitro and in vivo (animal and surgical studies). Methods and results pertaining to Sr-HA/CPC Physical and chemical properties of the prepared Sr-HA/CPC were evaluated. MTT assay and alkaline phosphatase activities, and radiological and histological examinations of Sr-HA/CPC, CPC and negative control were compared. X-ray diffraction (XRD) indicated that crystallinity of the prepared cement increased by increasing the powder-to-liquid ratio. Incorporation of Sr-HA into CPC increased MTT assay (biocompatibility) and ALP activity (P \u3c 0.05). Histomorphometry showed greater bone formation after 4 weeks, after implantation of Sr-HA/CPC in 10 rats compared to implantations of CPC or empty defects in the same rats (n = 30, ANOVA P \u3c 0.05). Methods and results pertaining to SIM-loaded PLGA microspheres + nanostrontium-CPC composite After SEM assessment, the produced composite of microspheres and enhanced CPC were implanted for 8 weeks in 10 rabbits, along with positive and negative controls, enhanced CPC, and enhanced CPC plus SIM (n = 50). In the control group, only a small amount of bone had been regenerated (localized at the boundary of the defect); whereas, other groups showed new bone formation within and around the materials. A significant difference was found in the osteogenesis induced by the groups sham control (16.96 ± 1.01), bone materials (32.28 ± 4.03), nanostrontium-CPC (24.84 ± 2.6), nanostrontium-CPC-simvastatin (40.12 ± 3.29), and SIM-loaded PLGA + nanostrontium-CPC (44.8 ± 6.45) (ANOVA P \u3c 0.001). All the pairwise comparisons were significant (Tukey P \u3c 0.01), except that of nanostrontium-CPC-simvastatin and SIM-loaded PLGA + nanostrontium-CPC. This confirmed the efficacy of the SIM-loaded PLGA + nanostrontium-CPC composite, and its superiority over all materials except SIM-containing nanostrontium-CPC
