As improvements in health insurance coverage and the quality of medical services have increased life expectancy in South Korea, the elderly population has reached 15.8% of the total population in 2020, and the country is expected to become a super-aged society by 2025. In an aging society, issues such geriatric disease and medical expenses are becoming prominent social concerns, and one of the major factors threatening the quality of life of the elderly is falls. Socioeconomic losses, including costs related to medical treatments for fall-related injuriesโ€”which are common among the elderlyโ€”are also expected to continue increasing; therefore, it is important to identify the groups at highest risk of falls and properly manage risk factors to prevent them from occurring. This approach will improve the overall health of the elderly and reduce medical costs for society as a whole. The purpose of this study was to analyze the association between cognitive function and falls in middle-aged and elderly adults (aged 45 years and older). The participants were 4,126 individuals. Data on cognitive functioning and falls were obtained using the K-MMSE and the Korean Longitudinal Study of Aging (2006โ€“2018) conducted by the Ministry of Employment and Labor and the Korea Employment Information Service, respectively. In terms of statistical analyses performed, chi-square tests and t-tests were used to examine sociodemographic variables and health-related behavioral factors. Changes in K-MMSE scores over time were analyzed by calculating the mean, standard deviation, median, and quartiles of subjectsโ€™ K-MMSE scores by year. Then, subjects were divided into five groups according to the degree of change in the K-MMSE score and the frequency was measured in each group. Further, the odds ratio of falls was determined. Repeated measurements were taken from the subjects, and a generalized linear mixed model (GLMM) was applied using the LOGIT function for the link option and the binary function for the distribution option. SAS version 9.4 was used for all statistical analyses, and significance was defined as a p-value less than 0.05 in a two-tailed test. Results demonstrated that 3.2% of the subjects experienced falls, and the incidence rate of falls among females was significantly higher (males, 1.8%; females, 4.4%). Furthermore, the group that experienced falls displayed significantly higher K-MMSE scores compared to the group that did not. Subjects were divided into five groups according to the degree of change in cognitive function, and frequency was measured in each group. Additionally, the relationship between a change in cognitive status and the incidence rate of falls was measured. The results showed a significantly higher odds ratio of falls in groups with worse cognitive function, particularly in females, in the elderly compared to younger subjects, in obese subjects compared to those of average weight, and in separated, divorced, bereaved, or single subjects compared to subjects who were currently married. Findings of this study are valuable as they provide important evidence for the development of programs and nursing interventions to help prevent falls among middle-aged and elderly adults in the local community. As the survival rate improves thanks to the development of medical technology, the quality of life of breast cancer survivors is emphasized from a long-term perspective. Drastic body changes caused by breast cancer and its treatment have a negative effect on body image, which hinders the quality of life. As the acceptance of the altered body plays a decisive role in body image, body acceptance is highlighted as the key to improving body image. However, to date, the concept of body acceptance experienced by breast cancer women has not been established, and there is no specific tool for measurement. Therefore, this study analyzed the concept of body acceptance in breast cancer women and developed a psychometric scale for evaluating it. First, applying the hybrid model, the definition and attributes of body acceptance were derived. Based on the six categories identified from the concept analysis, 63 initial items were generated. Additionally, through the item review by three clinical and five empirical experts, items were revised and supplemented to 73 primitive items. Following the content validity test from six experts and a pilot test on 20 breast cancer women, primitive items were revised to 40 items. After the linguistic correction, the preliminary instrument consisting of six factors and 40 items was developed. For the main survey, the validity and reliability of the instrument were tested using a sample of 411 breast cancer women. As a result, the validity of the scale, including construct validity, cross-validity, control group validity, and criterion validity, was confirmed. The Cronbach's ฮฑ value for the instrument was .91 indicating very good internal reliability. Finally, the body acceptance scale was developed comprising 17 items derived from six factors: encountering, understanding, adaptation, return to daily life, successful transition, and being optimistic. The body acceptance scale consists of a 5-point Likert scale from 1 "not at all" to 5 "very much". The scores are added up after the reverse operation of the negation questions, and the possible total scores range from 17 to 85. Higher scores on the body acceptance scale indicate a higher degree of body acceptance in breast cancer women. The scale developed from this study is an instrument that has proven reliability and validity in assessing body acceptance among breast cancer women. Obesity is associated with dysregulation of vitamin D metabolism and impaired balance of CD4+ T cell subsets. mTOR pathway is known to regulate the proliferation and differentiation of CD4+ T cells. Obesity and invitro vitamin D treatment have been reported to affect the mTOR pathway, but the effect of dietary vitamin D supplementation on the mTOR pathway in obese mice T cells has not been reported. In this study, we investigated the effects of vitamin D on CD4+ T cell subsets and mTOR pathway. Ten-week-old-male C57BL/6N mice were divided into 4 groups and fed diets differing in fat (control or high-fat diet: CON or HFD) and vitamin D content (vitamin D control or vitamin D supplement diet: vDC or vDS) for 12 weeks. T cells purified by negative selection were stimulated with anti-CD3/anti-CD28 mAbs and cultured for 48h. The percentage of Th17 cells (CD4+IL-17+ T cells) and Treg cells (CD4+CD25+Foxp3+ T cells) were confirmed, and the expression of proteins related to the mTOR pathway (p70S6K, AKT, LC3, P62) were measured. Also, the expression of genes related to the vitamin D metabolism (Vdr, Ddit4, Cyp27b1, Cyp24a1) and glucose metabolism (Hif1 ฮฑ, Glut1) was analyzed. The percentage of Th17 was higher in vDS groups and tended to be higher in HFD groups than CON groups. The percentage of Treg was higher in HFD groups than CON groups. phospho-p70S6K/total-p70S6K ratio was lower in vDS groups than vDC groups, but phospho-AKT/total-AKT ratio was higher in vDS groups than vDC groups. The Hif1ฮฑ mRNA level was lower and the Glut1 mRNA level tended to be lower in vDS groups than vDC groups. These results indicated that vitamin D supplementation down-regulated mTORC1 activity and up-regulated mTORC2 activity in T cells. Given that mTORC1 promote glucose metabolism through HIF1ฮฑ, lower expression of Hif1ฮฑ and Glut1 seemed to be due to the lower mTORC1 activity which was down-regulated by vitamin D supplementation.
