Application of Canadian Experience for Supply Chain Strategy and Territorial Organization of Tourist Information Centres in the Regions of Russia

One of the countries that have a common and long-running TIC network is Canada. The experience of this large northern country in the formation of regional information visit-service systems is of interest to Russia, since the countries in their characteristics are similar in size, distances, and natural conditions based on the supply chain strategy. Consider the TIC systems in the four Atlantic Canadian provinces. Tourist Information Centres are a new branch of services and a significant component of tourism management in many countries. In Russia, tourist information centres (TIC) are organized mainly in the largest cities of the regions and at the main sites visited. The experience of countries that are close in terms of geographic conditions and advanced in relation to the tourist infrastructure is important for the Russian regions. The study of the territorial organization of the Visitor Centres in the Atlantic Canada allows us to determine some geographical patterns. They will be taken into account when developing the system of tourist information centres in the Permsky Krai, one of the internal regions of Russia, which begin to form a high-level tourist infrastructure

Brachytherapy procedure of choice in the localized prostate cancer treatment in patients with prostate benign hyperplasia

The article is devoted to the brachytherapy indication estimation as a procedure of choice in the localized prostate cancer treatment in patients with prostate benign hyperplasia. The study includes 26 patients with prostate adenocarcinoma T1c-T2b, undergone TURP prior the brachytherapy. The results of the study showed that brachytherapy in combination with TURP is an effective and safety method of localized prostate cancer treatment, causing no reduction of life quality.Статья посвящена определению показаний к брахитерапии, как методу выбора в лечении локализованного рака простаты у пациентов с доброкачественной гиперплазией предстательной железы. В исследование включено 26 пациентов с диагнозом аденокарцинома предстательной железы в стадиях Т1с-Т2Ь, которым была выполнена трансуретральная резекция предстательной железы и в последующем брахитерапия. Результаты проведенного исследования показали, что применение брахитерапии в комбинации с парциальной трансуретральной резекцией предстательной железы в лечении локализованных форм рака простаты является эффективным и безопасным методом лечения с отсутствием снижения качества жизни

Optimization of clinical approaches to early diagnostics of bladder cancer in Sverdlovsk region

The following article reveals analysis of Governor’s program “Urological health of man" achievements in eight cities of Sverdlovsk region. In described cities exists wide variety of different unfavorable factors, which affect on morbidity of bladder oncological diseases among man. Governor’s program positive influence on optimization of bladder cancer earlier diagnostics, based on mass questioning technique and newest medical technologies, is showed up.В работе представлены данные углубленных комплексных обследований 114989 мужчин, проведенных в двенадцати экспресс - диагностических урологических кабинетах в Свердловской области и в урологическом поликлиническом отделении областной клинической больницы № 1. У 94894 (82,5%) человек обнаружены различные урологические и нефрологические заболевания, в том числе у 3533 (3,1%) пациентов городов области установлен рак мочевого пузыря. Доказано, что организация специализированной ранней диагностики рака мочевого пузыря у мужского населения области при выполнении губернаторской программы за счет доступности и возможности у мужчин получить по месту жительства своевременную диагностику и лечение онкологических заболеваний позволяет медицинским работникам своевременно и качественно оказывать медицинскую помощь, диспансерное наблюдение, тем самым снизить летальность и количество осложнений

During the shown period 135 radical cystectomy operations with different urine derivation methods were done. За указанный период выполнено 135 радикальных цистэктомий с различными методами отведения мочи. Выбор метода деривации мочи является сложной, многофакторной и до конца не решённой проблемой

Minimally invasive open bladder resection

A new method of bladder cancer surgical treatment (minimally invasive open resection) was invented in urology department of Ural state medical academy. During yy. 2002-2007 39 patients (8 female, 31 male) were treated, following current methodic, considering indications and contraindications. Analysis of postoperative results displayed serious reduction department staying (less than 7 days) because of minimally invasive treatment, blood supply and innervation saving, ability of BCG instillations. Dispensary observation, during the period yy. 2002-2007 revealed considerable reduction of bladder cancer recurrences (15 patients - 39%) after microinvasive resection that is below between all amount of recurrences after traditional open operations.В клинике урологии Уральской государственной медицинской академии (УГМА) разработан новый метод оперативного лечения рака мочевого пузыря — малоинвазивная открытая резекция. По данной методике за период 2002-2007 г. было прооперировано 39 больных (8 женщин и 31 мужчина) с учетом определенных показаний и противопоказаний. Анализ результатов послеоперационного течения и диспансерного наблюдения за пациентами выявил значительное сокращение пребывания больных в стационаре (не более 7 дней) за счет малоинвазивности предложенного метода -сохранения кровоснабжения и иннервации прямых мышц живота, а также возможности проведения на более ранних сроках (30 сутки) курса внутрипузырных инстилляций вакцины БЦЖ. Диспансерное наблюдение за пациентами (2002-2007 гг.) выявило: рецидив рака мочевого пузыря был значительно реже — 15 больных (39%) после малоинвазивной резекции, что значительно ниже общего количества рецидивов, регистрируемых после традиционных открытых вмешательств

Is it necessary to apply the Tyumen experience of intracoronary interventions in the Chelyabinsk region

Aim: to evaluate the prevalence and characteristics of percutaneous coronary interventions (PCI) performed in the Ural federal district (in particular in Tyumen and Chelyabinsk regions) in 2010 and to assess the efficiency of PCI in CAD in the real clinical practice by the data of Tyumen Cardiology Center (TCC). In the Chelyabinsk region insufficient usage of PCI was noticed (124.8 PCI in 1 million inhabitants compared to 720.4 PCI in the Tyumen region). The efficacy of PCI in patients with CAD was proven by the improvement of long-term results and patients quality of life especially if regular medical check-ups are performed. Our data show that PCI should be used more frequently in the real clinical practice in all Russian regions, and in the Chelyabinsk region as well.Целью работы явился анализ частоты и особенностей выполнения чрескожных коронарных вмешательств (ЧКВ) в Уральском федеральном округе, в частности в Тюменской и Челябинской областях в 2010 г., а также оценка эффективности ЧКВ при ИБС в реальной клинической практике по данным Филиала НИИ кардиологии СО РАМН «Тюменский кардиологический центр» (ТКЦ). В Челябинской области отмечается недостаточная обеспеченность населения ЧКВ (124,8 операций на 1 млн. жителей, что значительно уступает количеству вмешательств в Тюменской области - 720,4 ЧКВ на 1 млн. населения). Эффективность операций ЧКВ в лечении ИБС, доказанная улучшением отдаленного прогноза и качества жизни пациентов, особенно при активной диспансеризации, диктует необходимость их более широкого внедрения в практику как во всех регионах России в целом, так и в Челябинской области в частности

Does botulinum therapy affect the urine microbiome of patients with neurogenic bladder? Metagenomic sequencing results

Introduction. Reorganization of the urinary microbiome in patients with neurogenic low urinary tract dysfunction (NLUTD) plays an important role in the pathogenesis of urinary tract infections (UTIs). However, we still do not know how the different types of NULTD therapy affect the microbiome.Purpose of the study. Was to assess the influence of botulinum toxin (BoNT) injections on the urinary microbiome.Materials and methods. Urine samples were obtained from 6 persons (4 females and 2 males) with NLUTD before BoNT therapy (200 UI) and 6 weeks after. Standard urine analysis, urine culture and 16S rRNA gene sequencing were performed. a total of 144,562 quality-filtered 16S rDNA sequence reads were processed through an Illumina MiSeq platform.Results. The results of the urine culture correlated with 16S rRNA sequencing results, but the greater number of bacterial taxa were detected. Analysis of the bacterial community in urine samples revealed between 2 and 21 species-level reads per individual. All patients before BoNT demonstrated abnormal urinary microbiome with a predominance of Enterobacteriales. In three cases in females, clinical and urodynamic improvement after botulinum therapy was associated with changing of the microbiome from Enterobacteriaceae to Lactobacillales. In one female and two males, no principal changes had happened.Conclusions. This is the first report comparing in pilot study the urinary microbiome before and after BoNT injections into the bladder wall. The results are restricted by the small number of participants. However, the promising observation about the influence of botulinum toxin therapy on urinary microbiome was obtained

X-RAY SPECTRA AND ENERGY BAND STRUCTURE OF SOME NOBLE AND TRANSITION METAL ALUMINIDES

On a étudié les bandes d'émission L2. 3 et Kβ d'aluminium dans ses alliages avec des métaux nobles et quelques métaux de transition. L'apparition des maxima de l'intensité du côté des grandes ondes dans les spectres des alliages a été liée avec des bandes électroniques tirantes leur origine des états d des atomes des métaux nobles et métaux de transition. On a supposé que la région d'énergie des bandes d-semblables à voisinage des atomes d'aluminium contient aussi le nombre considérable des états tirants leur origine des états s et p des atomes d'aluminium.The Al L2. 3- and Kβ-emission bands from noble and several transition metal aluminides have been investigated. The appearance of the long-wave intensity peaks in the alloy spectra is associated with the energy bands originated from the d-states of noble and transition metal atoms. An assumption has been made that the energy region of the d-like bands in the vicinity of aluminium atoms contains appreciable amount of states arising from s- and p- aluminium atom states
