    Analysis of the Dilemma and Strategies of Elderly Patients Access to Outpatient Services - Based on the Examples from three Grade A Tertiary Hospitals in Jiangxi Province

    Objective: To identify the dilemma of elderly patients' access to outpatient services, develop strategies to improve the environment and functions of the outpatient department, and encourage the elderly to access medical services independently. Methods: By observing and interviewing, this paper studies the environment, behavior, and experiences of elderly patients when accessing medical services, identifies and classifies the key issues, and provides corresponding suggestions. Results: Existing signs and voice prompt systems fail to guide elderly patients to access to medical services; Elderly patients have difficulty in finding places to transit and rest when accessing to outpatient services; Elderly patients have problems in using AI (artificial intelligence) technologies when they access to outpatient services; There are communication barriers between elderly patients and medical staffs. Conclusion: Optimizing the guiding signs and voice prompt systems according to the characteristics of elderly patients; Designing the areas of transition and rest reasonably; Enhancing the ability of elderly patients to use self-service equipment; Promoting the medical treatment process to the elderly in a humanized way

    Lactylation: The emerging frontier in post-translational modification

    Lactate, a metabolic byproduct, has gained recognition as a highly influential signaling molecule. Lactylation, an emerging form of post-translational modification derived from lactate, plays a crucial role in numerous cellular processes such as inflammation, embryonic development, tumor proliferation, and metabolism. However, the precise molecular mechanisms through which lactylation governs these biological functions in both physiological and pathological contexts remain elusive. Hence, it is imperative to provide a comprehensive overview of lactylation in order to elucidate its significance in biological processes and establish a foundation for forthcoming investigations. This review aims to succinctly outline the process of lactylation modification and the characterization of protein lactylation across diverse organisms. Additionally, A summary of the regulatory mechanisms of lactylation in cellular processes and specific diseases is presented. Finally, this review concludes by delineating existing research gaps in lactylation and proposing primary directions for future investigations

    Potential of Core-Collapse Supernova Neutrino Detection at JUNO

    JUNO is an underground neutrino observatory under construction in Jiangmen, China. It uses 20kton liquid scintillator as target, which enables it to detect supernova burst neutrinos of a large statistics for the next galactic core-collapse supernova (CCSN) and also pre-supernova neutrinos from the nearby CCSN progenitors. All flavors of supernova burst neutrinos can be detected by JUNO via several interaction channels, including inverse beta decay, elastic scattering on electron and proton, interactions on C12 nuclei, etc. This retains the possibility for JUNO to reconstruct the energy spectra of supernova burst neutrinos of all flavors. The real time monitoring systems based on FPGA and DAQ are under development in JUNO, which allow prompt alert and trigger-less data acquisition of CCSN events. The alert performances of both monitoring systems have been thoroughly studied using simulations. Moreover, once a CCSN is tagged, the system can give fast characterizations, such as directionality and light curve

    Detection of the Diffuse Supernova Neutrino Background with JUNO

    As an underground multi-purpose neutrino detector with 20 kton liquid scintillator, Jiangmen Underground Neutrino Observatory (JUNO) is competitive with and complementary to the water-Cherenkov detectors on the search for the diffuse supernova neutrino background (DSNB). Typical supernova models predict 2-4 events per year within the optimal observation window in the JUNO detector. The dominant background is from the neutral-current (NC) interaction of atmospheric neutrinos with 12C nuclei, which surpasses the DSNB by more than one order of magnitude. We evaluated the systematic uncertainty of NC background from the spread of a variety of data-driven models and further developed a method to determine NC background within 15\% with {\it{in}} {\it{situ}} measurements after ten years of running. Besides, the NC-like backgrounds can be effectively suppressed by the intrinsic pulse-shape discrimination (PSD) capabilities of liquid scintillators. In this talk, I will present in detail the improvements on NC background uncertainty evaluation, PSD discriminator development, and finally, the potential of DSNB sensitivity in JUNO

    Real-time Monitoring for the Next Core-Collapse Supernova in JUNO

    Core-collapse supernova (CCSN) is one of the most energetic astrophysical events in the Universe. The early and prompt detection of neutrinos before (pre-SN) and during the SN burst is a unique opportunity to realize the multi-messenger observation of the CCSN events. In this work, we describe the monitoring concept and present the sensitivity of the system to the pre-SN and SN neutrinos at the Jiangmen Underground Neutrino Observatory (JUNO), which is a 20 kton liquid scintillator detector under construction in South China. The real-time monitoring system is designed with both the prompt monitors on the electronic board and online monitors at the data acquisition stage, in order to ensure both the alert speed and alert coverage of progenitor stars. By assuming a false alert rate of 1 per year, this monitoring system can be sensitive to the pre-SN neutrinos up to the distance of about 1.6 (0.9) kpc and SN neutrinos up to about 370 (360) kpc for a progenitor mass of 30M⊙M_{\odot} for the case of normal (inverted) mass ordering. The pointing ability of the CCSN is evaluated by using the accumulated event anisotropy of the inverse beta decay interactions from pre-SN or SN neutrinos, which, along with the early alert, can play important roles for the followup multi-messenger observations of the next Galactic or nearby extragalactic CCSN.Comment: 24 pages, 9 figure

    Effects of cement-enhanced soil on the ultimate lateral resistance of composite pile in clayey soil

    The composite pile consisting of core-pile and surrounding cement-enhanced soil is a promising pile foundation in recent years. However, how and to what extent the cement-enhanced soil influences the ultimate lateral resistance has not been fully investigated. In this paper, the ultimate lateral resistance of the composite pile was studied by finite element limit analysis (FELA) and theoretical upper-bound analysis. The results of FELA and theoretical analysis revealed three failure modes of laterally loaded composite piles. The effects of the enhanced soil thickness, strength, and pile-enhanced soil interface characteristics on the ultimate lateral resistance were studied. The results show that increasing the enhanced soil thickness leads to a significant improvement on ultimate lateral resistance factor (NP), and there is a critical thickness beyond which the thickness no longer affects the NP. Increasing the enhanced soil strength induced 6.2%–232.6% increase of NP. However, no noticeable impact was detected when the enhanced soil strength was eight times higher than that of the natural soil. The maximum increment of NP is only 30.5% caused by the increase of interface adhesion factor (α). An empirical model was developed to calculate the NP of the composite pile, and the results show excellent agreement with the analytical results

    Deciphering the Biological Effects of Radiotherapy in Cancer Cells

    Radiotherapy remains an effective conventional method of treatment for patients with cancer. However, the clinical efficacy of radiotherapy is compromised by the development of radioresistance of the tumor cells during the treatment. Consequently, there is need for a comprehensive understanding of the regulatory mechanisms of tumor cells in response to radiation to improve radiotherapy efficacy. The current study aims to highlight new developments that illustrate various forms of cancer cell death after exposure to radiation. A summary of the cellular pathways and important target proteins that are responsible for tumor radioresistance and metastasis is also provided. Further, the study outlines several mechanistic descriptions of the interaction between ionizing radiation and the host immune system. Therefore, the current review provides a reference for future research studies on the biological effects of new radiotherapy technologies, such as ultra-high-dose-rate (FLASH) radiotherapy, proton therapy, and heavy-ion therapy
