    Cascading training the trainers in ophthalmology across Eastern, Central and Southern Africa.

    BACKGROUND: The Royal College of Ophthalmologists (RCOphth) and the College of Ophthalmology of Eastern Central and Southern Africa (COECSA) are collaborating to cascade a Training the Trainers (TTT) Programme across the COECSA Region. Within the VISION 2020 Links Programme, it aims to develop a skilled motivated workforce who can deliver high quality eye care. It will train a lead, faculty member and facilitator in 8 countries, who can cascade the programme to local trainers. METHODS: In phase 1 (2013/14) two 3-day courses were run for 16/17 selected delegates, by 3 UK Faculty. In phase 2 (2015/16) 1 UK Faculty Member ran 3 shorter courses, associated with COECSA events (Congress and Examination). A COECSA Lead was appointed after the first course, and selected delegates were promoted as Facilitators then Faculty Members on successive courses. They were given appropriate materials, preparation, training and mentoring. RESULTS: In 4 years the programme has trained 87 delegates, including 1 COECSA Lead, 4 Faculty Members and 7 Facilitators. Delegate feedback on the course was very good and Faculty were impressed with the progress made by delegates. A questionnaire completed by delegates after 6-42 months demonstrated how successfully they were implementing new skills in teaching and supervision. The impact was assessed using the number of eye-care workers that delegates had trained, and the number of patients seen by those workers each year. The figures suggested that approaching 1 million patients per year were treated by eye-care workers who had benefited from training delivered by those who had been on the courses. Development of the Programme in Africa initially followed the UK model, but the need to address more extensive challenges overseas, stimulated new ideas for the UK courses. CONCLUSIONS: The Programme has developed a pyramid of trainers capable of cascading knowledge, skills and teaching in training with RCOphth support. The third phase will extend the number of facilitators and faculty, develop on-line preparatory and teaching materials, and design training processes and tools for its assessment. The final phase will see local cascade of the TTT Programme in all 8 countries, and sustainability as UK support is withdrawn

    The Cyprus Women’s Health Research (COHERE) initiative: normative data from the SF-36v2 questionnaire for reproductive aged women from the Eastern Mediterranean

    Purpose: Describe the health-related quality of life for a representative cohort of women aged 18–55 in Northern Cyprus. Methods: We utilised the SF-36-Health-Survey-version-2 (SF-36v2) questionnaire as part of the COHERE Initiative study to calculate the eight physical and mental subscale scores, as well as the two overall summary measures for physical and mental health, where we present results using Cyprus-specific scoring as well as scores based on the test developers’ algorithms. We examined associations between sociodemographic characteristics for both scores. Results: A total of 7089 women fully completed the SF-36v2 questionnaire (mean age = 36.9), which was reliable and valid in this population. We observed better physical health in ages 18–25 compared to 46–55 (53.32 vs. 46.72 (p < 0.001)) and better mental health in women aged 46–55 compared to 18–25 (52.07 vs. 47.95 (p < 0.001)). Women in employment had better physical and mental health compared to those who were unemployed (physical: 50.25 vs 49.95, p < 0.001 and mental: 50.25 vs 49.24, p = 0.083) and scores increased as educational attainment increased (physical: 47.55 for primary to 51.58 for postgraduate, mental: 48.88 to 50.59, p < 0.001). Turkish Cypriot women had higher scores than Turkish women (physical: 50.42 vs 49.30, mental: 50.43 vs 49.10, p < 0.001). Conclusion: These are the first population normative values published from a large representative sample of women between 18 and 55 years from the Eastern Mediterranean region. We found better physical health in younger women and better mental health in older women. Turkish Cypriot women and non-migrant women had better mental health, and HRQOL was highest in those in paid employment and those with a higher educational achievement

Review of scientific instruments. 2005;76(3): 033704.Near-field optical applications require the fast, stable, and reproducible fabrication of scanning near-field optical microscopy (SNOM) aperture probes in the submicrometer range. We have developed a stand-alone device for the electrolytic etching of nanoapertures with an integrated current and optical transmission monitoring and control. Probes with an aperture ranging from 50 to 100 nm were reproducibly fabricated with great reliability. With these probes, high resolution SNOM images of 100 nm test patterns and single dye molecules (Rhodamine 6G in poly(vinyl alcohol)) are measured and presented. Not requiring a SNOM setup, the stand-alone device is not only inexpensive and compact, but also insensitive to external disturbances

    Intravascular ultrasonography allows accurate assessment of abdominal aortic aneurysm: An in vitro validation study

    AbstractObjective: The objective of this study was to acquire insight into the interpretation of intravascular ultrasound images of the abdominal aorta and to assess to what extent this technique can provide useful parameters for the endovascular treatment of patients with abdominal aortic aneurysm. Study Design: This was a descriptive study. Methods: Fifteen abdominal aortic specimens (normal, atherosclerotic, or aneurysmal) were studied. Ultrasonic images and corresponding histologic sections were compared for vessel wall characteristics, lesion morphologic characteristics, and lumen diameter. The length of the aneurysm and the length of the proximal and distal neck were measured and compared with external measurements. Tomographic images were reconstructed to a three-dimensional format. Results: Normal aortic wall was seen as a two- or three-layered structure corresponding with intima, media, and adventitia. A distinction could be made among fibrous lesion, calcified lesion, and thrombus and between normal and aneurysmal aorta. Correlation between the histologic specimens and intravascular ultrasonography for lumen diameter measurements was high (r = 0.93; p < 0.001). In a similar fashion, correlation between external measurements and intravascular ultrasound measurements on the length of the aneurysm and its proximal and distal neck was high (r = 0.99; p < 0.001). Three-dimensional analysis enhanced interpretation of the tomographic images by visualizing the spatial position of anatomic structures and contributed to understanding the shape and dimensions of the aneurysm. Conclusions: Intravascular ultrasonography provides accurate information on the vessel wall, lesion morphologic characteristics, and quantitative parameters of the abdominal aorta. Spatial information supplied by three-dimensional analysis contributes to a more realistic interpretation of the tomographic images. (J Vasc Surg 1998;27:347-53.
