Tuberculosis caused by XDR resistant Mycobacterium tuberculosis in Poland. Microbiological and molecular analysis
Wstęp: Przedmiotem badania było poszukiwanie szczepów Mycobacterium tuberculosis o oporności XDR ( XDR = MDR + oporność na: fluorochinolon + amikacyna i/lub kapreomycyna) wśród chorych nowo wykrytych i leczonych w Polsce
w latach 1997-2004.
Materiał i metody: Materiał do badań stanowiło 10 913 chorych zakażonych i wydalających prątki gruźlicy wrażliwe
i lekooporne. Chorzy ci byli analizowani pod kątem występowania u nich lekooporności pierwotnej i nabytej w 3 programach
WHO Drug resistance surveillance obejmujących zasięgiem cały kraj.
Wyniki: Obecnie dokonano analizy występowania lekooporności typu MDR i XDR. Wśród szczepów o oporności MDR zbadano
lekooporność na leki dodatkowe. Wśród chorych stwierdzono 1 przypadek gruźlicy wywołanej szczepem o oporności XDR, co
stanowiło 0,4% wszystkich przypadków gruźlicy o wzorze MDR. Analiza genetyczna wykazała, że szczep XDR należy do rodziny
molekularnej opisanej w międzynarodowej bazie jako spoligotyp H1 1558. Do tej samej rodziny molekularnej należały ponadto
szczepy wyizolowane od 3 innych chorych z tego samego województwa. Ich wzory lekooporności nie pozwoliły na zaliczenie ich
do prątków typu XDR. W rodzinie molekularnej H1 1558 dominują "polskie szczepy" - 13 (87%) szczepów wyizolowanych od
polskich chorych na ogółem 15 zgłoszonych do międzynarodowej bazy.
Wnioski: Z analizy lekooporności na leki dodatkowe prątków gruźlicy typu MDR wynika, że zjawisko oporności na
fluorochinolony, amikacynę i kapreomycynę występuje w Polsce rzadko.Introduction: The retrospective analysis of frequency of drug resistant tuberculosis XDR (XDR = MDR + resistance to:
fluoroquinolones + amikacin and/or capreomycin) in Poland have been tested.
Material and methods: Pattern of resistance to first, second and third line drugs has been tested among new and treated
patients. The total number of 10 913 Mycobacterium tuberculosis strains isolated from the same number of the patients
surveyed in 1997-2004 in WHO programme Drug resistance surveillance countrywide study.
Results: One HIV-negative patient (43 years old men) was infected by XDR (0,4% among new and treated MDR cases). Molecular
analysis of the XDR strain by spoligotyping has been shown the T11558 cluster. Three others tuberculosis patients living in the
same region of Poland excreted the Mycobacterium tuberculosis strains belonged to the same T11558 cluster, but they had not
pattern of XDR resistance. In the same T11558 molecular cluster 13/15 - 87% strains have been isolated from polish patients.
Conclusions: In Poland Mycobacterium tuberculosis resistance to fluoroquinolones, capreomycin and amikacine among
MDR strains has been observed very rarely
Genetic evaluation of relationship between mutations in rpoB and resistance of Mycobacterium tuberculosis to rifampin
<p>Abstract</p> <p>Background</p> <p>Rifampin is a first line antituberculosis drug active against bacilli in logarithmic and stationary phase, which interferes with RNA synthesis by binding to bacterial RNA polymerase. Tubercle bacilli achieve resistance to rifampin by accumulation of mutations in a short-81 bp region of the <it>rpoB </it>gene. Among many mutations identified in the <it>rpo</it>B gene, few were verified by molecular genetic methods as responsible for resistance to rifampin (RMP).</p> <p>Results</p> <p>In this study eight different mutations identified in an 81 bp section of a "hot spot" region of the <it>rpo</it>B gene of RMP resistant <it>Mycobacterium tuberculosis </it>clinical strains were evaluated in respect to drug resistance. It was found that: mutations in positions 526 (H/D), 516 (D/V) and 531 (S/L) result in high level resistance to rifampin; mutations in positions 516 (D/Y), 515 (M/I), 510 (Q/H) or a double mutation in codons 512 (S/I) and 516 (D/G) relate to low level of resistance. Gene <it>rpo</it>B carrying mutations in codon 513 (Q/L) introduced into an <it>M. tuberculosis </it>laboratory strain did not cause resistance to rifampin, however the same gene introduced into two different clinical strains did, with the level of resistance depending on the host strain.</p> <p>Conclusion</p> <p>Mutations in an 81 bp "hot spot" region of the <it>rpoB </it>of <it>M. tuberculosis </it>lead to different levels of resistance to rifampin. Some mutations in this "hot spot" region of <it>rpoB </it>require a specific genetic background for the host strain to develop resistance to rifampin. Therefore, the identification of such mutations in a clinical <it>M. tuberculosis </it>strain is not enough to classify the given strain as resistant to rifampin.</p
Phenotypic characterization of pyrazinamide-resistant Mycobacterium tuberculosis isolated in Poland
Introduction: Pyrazinamide (PZA) is an important first-line antituberculous drug, which is applied together with INH, RMP,
EMB and SM. This drug plays a unique role in the first phase of TB therapy because it is active within macrophages and kills
tubercule bacilli.
Testing of the resistibility of Mycobacterium tuberculosis to PZA is technically difficult because PZA is active only at
acid pH. Therefore routine drug resistibility testing of M. tuberculosis for PZA is not performed in many laboratories.
The objective of our study was to estimate the resistibility for PZA among M. tuberculosis isolates from polish patients
in 2000-2008 years.
Material and methods: We analyzed M. tuberculosis strains with different resistibility to first-line antituberculous drugs.
The strains were isolated from 1909 patients with tuberculosis. The strains were examined for PZA resistibility by the
radiometric Bactec 460-TB method. The PZA-resistant strains were examined for following MIC PZA for drug concentration:
100, 300, 600, 900 μg/mL.
Results: PZA resistance among M. tuberculosis strains was found in 6.7% untreated patients and in 22.2% previously
treated patients (p < 0.001). In both groups resistance to PZA was correlated with drug resistance for INH + RMP + SM + EMB in 32.7% untreated patients and in 34.5% previously treated ones (p < 0.8). The PZA-monoresistant strains were observed in
20.8% untreated patients groups.
Among resistant strains: in 3.4% MIC for PZA was > 100 μg/mL, in 11.6% ≥ 300 μg/mL, in 8.9% ≥ 600 μg/mL and in 76%
≥ 900 μg /mL.
Conclusions: Among M. tuberculosis strains PZA resistance was found in 6.7% of untreated patients and in 22.2% of
previously treated patients.
Among the PZA-resistant strains very high MIC value for PZA (≥ 900 μg/mL) was revealed for 76% M. tuberculosis strains.

EMB i SM. Lek ten odgrywa kluczową rolę w pierwszej fazie leczenia choroby, ponieważ działa silnie wewnątrz makrofagów,
gdzie przeżywają prątki gruźlicy. Określenie fenotypu oporności na PZA jest trudne ze względu na aktywność leku jedynie
w kwaśnym środowisku. Głównie z tego powodu ten test jest wykonywany w niewielu laboratoriach na świecie,
a informacje o PZA-oporności są fragmentaryczne.
Celem pracy była analiza oporności na PZA szczepów Mycobacterium tuberculosis wyizolowanych od polskich chorych
w latach 2000-2008.
Materiał i metody: Analizie poddano szczepy M. tuberculosis o różnym fenotypie oporności na 4 podstawowe leki wyizolowane
od 1909 chorych. Fenotyp oporności na PZA określono metodą radiometryczną Bactec 460-TB. Szczepom opornym na
PZA określono MIC dla stężeń PZA: 100, 300, 600, 900 μg/ml.
Wyniki: W grupie chorych nowo wykrytych 6,7% wydalało szczepy oporne na PZA. W grupie chorych wcześniej
leczonych szczepy oporne na PZA wydalało 22,2% (p < 0,001). W obu grupach chorych oporność na PZA była
skorelowana z opornością na 4 leki INH + RMP + SM + EMB (32,7% u chorych nowo wykrytych i u 34,5% chorych
wcześniej leczonych [p < 0,8]). Szczepy oporne tylko na PZA dominowały w grupie chorych nowo wykrytych (20,8%).
Dla 3,4% szczepów wartość MIC PZA wynosiła > 100 μg/ml, dla 11,6% ≥ 300 μg/ml, dla 8,9% ≥ 600 μg/ml i aż dla
76% ≥ 900 μg/ml.
Wnioski: Wśród szczepów M. tuberculosis stwierdzono wysokie odsetki oporności na PZA (6,7% u chorych nowo wykrytych,
22,2% u chorych wcześniej leczonych). Większość szczepów opornych na PZA (76%) posiadała wysokie miana
oporności - MIC PZA ≥ 900 μg/ml.

The biological role of prokaryotic and eukaryotic N-acetyltransferase
The N-acetyltransferases (NAT; E.C.2.3.1.5) are involved in the metabolism of drugs and environmental toxins. They catalyse the acetyl transfer from acetyl coenzyme A to an aromatic amine, heterocyclic amine, or hydrazine compound. NAT homologues are present in numerous species from bacteria to human. Sequence variations in the human NAT1 and NAT2 result in the production of NAT proteins with variable enzyme activity or stability, leading to slow or rapid acetylation. Therefore, genetic polymorphisms in NAT1 and NAT2 influence drug metabolism and drug-related toxicity. Epidemiological studies suggest that the NAT1 and NAT2 acetylation polymorphisms modify the risk of developing cancers of the urinary bladder, colorectal, breast, head and neck, and lung.N-acetylotransferazy (NAT; EC2.3.1.5) biorą czynny udział w metabolizowaniu leków i toksyn środowiskowych. Katalizują przeniesienie grupy acetylowej z acetylokoenzymu A do terminalnej grupy aminowej aryloamin, arylohydrazyn i niektórych amin heterocyklicznych. Enzymy arylamino N-acetylotransferazy zostały zidentyfikowane u wielu organizmów eukariotycznych i prokariotycznych. Polimorfizm genetyczny N-acetylotransferaz powoduje powstanie enzymów o zmienionej sekwencji aminokwasowej, która przyczynia się do obniżenia ich aktywności i stabilności. Występowanie polimorfizmu w aktywności enzymu N-acetylotransferazy 2 jest przyczyną występowania dwóch odmiennych fenotypowo grup: wolnych i szybkich acetylatorów. Szybkość metabolizowania leków oraz związków kancerogennych zależnych od N-acetylotransferaz wpływa na skuteczność i efekt toksyczny tych leków oraz może mieć związek ze zwiększonym ryzykiem wystąpienia niektórych jednostek chorobowych. Badania epidemiologiczne sugerują, że polimorfizm N-acetylotransferaz może mieć wpływ na ryzyko zachorowania na raka pęcherza moczowego, jelita grubego, piersi, głowy i szyi oraz płuc
The results of molecular epidemiological investigations in patients infected with strains of the genus Acinetobacter
Wstęp: Pałeczki z rodzaju
Acinetobacter są zaliczane do ważnych patogenów oportunistycznych odpowiedzialnych za wzrost
liczby zakażeń szpitalnych. Stanowią poważny problem epidemiologiczny ze względu na trudności w leczeniu spowodowane
opornością na wiele grup antybiotyków. Celem pracy było zbadanie lokalnych zakażeń wywołanych przez różne gatunki
z rodzaju Acinetobacter, występujących na oddziałach IGiChP w okresach ich wzmożonego występowania - w sierpniu
2007 roku oraz w lutym i marcu 2008 roku.
Materiał i metody: Materiał badań stanowiły 23 szczepy Acinetobacter spp. pochodzące od 19 chorych hospitalizowanych
w IGiChP w tym samym okresie (2007 rok - 13 szczepów od 11 chorych; 2008 rok - 10 szczepów od 8 chorych). Analizie
poddano cechy fenotypowe oraz pokrewieństwo genetyczne wszystkich szczepów.
Wyniki: Szczepy Acinetobacter (n = 23) charakteryzowały się dużą opornością na badane leki. Zastosowana metoda
AP-PCR wykazała istnienie 10 genotypów wśród wszystkich analizowanych szczepów. Szczepy Acinetobacter spp. wyhodowane
w 2007 i 2008 roku należące do jednego genotypu, pochodziły od chorych hospitalizowanych na tych samych
oddziałach, co potwierdza transmisję zakażenia w obrębie miejsca pobytu pacjenta.
Wnioski: Jednakowy wzór molekularny szczepów Acinetobacter baumannii wyizolowanych od dwóch chorych w 2007 roku
oraz od dwóch chorych w 2008 roku świadczy o występowaniu tego drobnoustroju w środowisku szpitalnym. W niniejszej
pracy pokazano przydatność metody AP-PCR w molekularnych dochodzeniach epidemiologicznych.

infections. The majority of infection are of epidemic origin, and treatment has become difficult because many strains are
resistant to a wide range of antibiotics. The aim of this study was to investigate the local infections caused by various
species of the genus Acinetobacter, occurring in the hospital wards IGiChP in periods of increased prevalence: August 2007
and February and March 2008.
Material and methods: Twenty three strains of Acinetobacter spp. were isolated from 19 patients residing in the same
period and the same hospital ward (2007 - 13 strains from 11 patients, 2008 - 10 strains from 8 patients). Acinetobacter
isolates obtained from these patients were characterized by phenotypically methods and genotypically by arbitrarily primed
PCR (AP-PCR).
Results: All strains of Acinetobacter (n = 23) were multi-drug resistant. Used AP-PCR method showed 10 genotypes among
the all strains. Acinetobacter spp. strains cultivated in 2007 and 2008 belonged to one genotype, came from patients
hospitalized on the same wards, which confirms the transmission of infection in the patient’s residence.
Conclusions: The same genotype Acinetobacter baumannii strains isolated from two patients in 2007, and two patients in
2008, showed the presence of bacteria in the hospital environment. In the present study, we also established the usefulness
of AP-PCR in molecular epidemiological investigations.

The significance of spoligotyping method in epidemiological investigations of tuberculosis
Introduction: The control of tuberculosis (TB) requires methods for rapid detection and tracing sources of infection, so that
further transmission can be arrested. Recent developments in molecular biology have resulted in techniques that allow
prompt identification and tracking specific strains of M. tuberculosis as they spread through the population. Most of these
techniques take advantage of M. tuberculosis DNA polymorphism and are based on various repetitive DNA elements as
genetic markers. Each method yields strain-specific genetic profiles (fingerprints). Strains showing identical fingerprints are
referred to as clustered and are usually associated with recent transmission, whereas strains whose fingerprints are unique
are presumed to represent remote transmission, a reactivation of infection acquired in the distant past.
Material and methods: In recent years, spoligotyping has become one of the most widely used genotyping method for
epidemiological studies of TB. Spoligotyping is a PCR-based method allowing to analyze strain-dependent polymorphisms
observed in spacer sequences present within the direct repeat (DR) genomic region of M. tuberculosis complex strains.
Spoligotyping provides some important advantages over other genotyping techniques. These are simplicity, rapidity, high
reproducibility and stability of the results, with the latter being expressed in a simple digital pattern, readily named and
databased, and the ability to perform spoligotyping directly on clinical samples, without the need for prior culture. However,
spoligotyping has relatively low discriminatory capacity, which makes it necessary to use secondary fingerprinting methods
to prove clonality between isolates.
Results: The aim of this study was to evaluate the usefulness of spoligotyping in epidemiological investigations of TB by
analyzing 16 isoniazid-resistant M. tuberculosis strains isolated from patients with pulmonary TB in the central region of
Poland.
A total of 11 distinct spoligopatterns were obtained. 9 isolates were represented by a unique pattern, whereas 7 were
clustered in 2 groups of 5 and 2 isolates, respectively. When compared with an international spoligodatabase SpolDB4, 13
isolates shared already described spoligotypes, whereas 3 did not match any existing spoligopattern in database and were
defined as orphans.
Spoligotyping overestimated the number of clustered isolates in one of its two clusters when compared to IS6110 Mtb1/
/Mtb2 PCR. Strains clustered using the latter method were assumed to be closely epidemiologically related.
Conclusion: This report demonstrates the utility of spoligotyping as an initial screening technique, to be supplemented by
another typing method of greater discriminatory power, such as the IS6110 Mtb1/Mtb2 PCR in order to better recognize the
epidemiological links between TB patients.Wstęp: Obserwowany w ostatnich latach intensywny rozwój nowoczesnych metod biologii molekularnej umożliwił ich
zastosowanie do badań w epidemiologicznych dochodzeniach gruźlicy. Techniki te polegają na wykrywaniu w genomie
prątków sekwencji insercyjnych lub powtórzonych sekwencji DNA, których liczba i zmienność genetyczna stanowią marker
identyfikacyjny. Każda z metod wykorzystujących właściwości jednego lub kilku markerów genetycznych prowadzi do
uzyskania charakterystycznego dla danego szczepu wzoru genetycznego - fingerprint. Szczepy prątków gruźlicy, których
wzory typowania genetycznego są identyczne lub bardzo do siebie podobne, są grupowane (clustering) i mogą mieć wspólne
źródło pochodzenia. Szczepy takie uznaje się jako transmitowane między ludźmi, podczas gdy szczepy o różnych wzorach
wykazują odmienne źródło transmisji, mogą również pochodzić z reaktywacji dużo wcześniejszego zakażenia.
Materiał i metody: W ostatnich latach metoda spoligotyping stała się jedną z powszechnie stosowanych metod genetycznych
w gruźlicy. Metoda ta oparta na PCR wykorzystuje polimorfizm chromosomalnego regionu DR występującego
u prątków należących do M. tuberculosis complex. Jest to metoda szybka, łatwa w wykonaniu i wydajna. Cechuje ją wysoka
powtarzalność wyników. Może być stosowana do wykrywania i identyfikacji M. tuberculosis complex bezpośrednio
w materiale klinicznym z pominięciem hodowli. Ponieważ jednak metoda ma pewne ograniczenia, szczepy posiadające te
same spoligotypy muszą być dalej różnicowane inną metodą molekularną.
Wyniki: Badania przeprowadzone przez autorów pracy dotyczyły szczepów prątków gruźlicy opornych na izoniazyd, które
wyizolowano od 16 chorych z centralnego regionu Polski. Otrzymano 11 różnych wzorów spoligo, w tym 9 szczepów
posiadało unikalne (niepodobne do siebie) wzory, natomiast 7 należało do dwóch rodzin molekularnych. Otrzymane wzory
sprawdzano w międzynarodowej bazie opisanych wzorów molekularnych SpolDB4. Szczepy od 13 chorych posiadały swoje
wzorce w bazie, natomiast 3 szczepy zdefiniowano jako sieroce (orphans), to znaczy niezgłoszone jeszcze do bazy. Zastosowanie
metody IS6110 Mtb1/Mtb2 PCR pozwoliło na zróżnicowanie szczepów posiadających takie same wzory spoligo.
Wnioski: Praca potwierdza przydatność metody spoligotyping jako metody przesiewowej w dochodzeniach epidemiologicznych
w gruźlicy. Ze względu na ograniczoną zdolność różnicowania, szczepy o tym samym spoligotypie muszą być poddane
dodatkowej analizie metodą IS6110 Mtb1/Mtb2 PCR
Detection of mutation in NAT II gene as a method of determination of izoniazyd (INH) acetylation type in human population
Wstęp: Izoniazyd (INH) jest lekiem powszechnie stosowanym w leczeniu gruźlicy, metabolizowanym w wątrobie do acetyloizoniazydu
przez enzym N-acetylotransferazę (NAT). Szybkość acetylacji leku jest cechą zdeterminowaną genetycznie.
Enzym jest kodowany przez dwa geny: NAT1 i NAT2, który jest odpowiedzialny za występowanie różnic w szybkości
acetylacji różnych związków. W przedstawionej pracy badano zastosowanie metody genotypowania w określaniu typu
acetylacji (szybki, wolny acetylator).
Materiał i metody: Stężenie izoniazydu w surowicy określano metodą biologiczną, gwarantującą wysoką dokładność
i powtarzalność wyników. Genomowe DNA izolowano przy użyciu kitu firmy Qiagen, a następnie poddawano reakcji amplifikacji
według Spurr [1]. Produkty PCR trawiono oddzielnie 4 enzymami restrykcyjnymi: Dde1, Kpn1, Tag1 i BamH1.
Wyniki: W badanej grupie zidentyfikowano występowanie 4 alleli genu NAT2. Obecność dwóch zmutowanych alleli identyfikowała
wolny typ acetylacji, natomiast szybcy acetylatorzy mieli 1 lub 2 allele dzikie (NAT2*4).
Wnioski: Zastosowanie metody genotypowania w określaniu mutacji w NAT2 u człowieka umożliwia szybkie określanie
typu acetylacji w monitorowaniu biodostępności izoniazydu.Introduction: Isoniazid (INH) is a drug widely used in the treatment of tuberculosis. INH is metabolized to acetylisoniazid by
N-acetyltransferase (NAT) in the liver. The rate of INH acetylation is genetically determined. NAT isozymes are encoded at
2 loci; one encodes NAT1, formerly known as the monomorphic form of the enzyme, while the other encodes the polymorphic
NAT2, which is responsible for individual differences in the ability to acetylate certain compounds. The objective of the
present study was to apply the genotyping of the fast and slow acetylators for personalized therapeutic dose.
Material and methods: Plasma concentrations of INH were determined with biological method in the authors modification.
This method warrants high accuracy and secured repeatable results. Genomic DNA was isolated from the blood samples.
DNA extracted by Blood DNA Kit and amplified by PCR by Spurr [1] with two primers. PCR product was cut separately with
4 different restriction enzymes: Dde1, Kpn1, Tag1, and BamH1.
Results: Four different NAT 2 alleles were detected in the study population. The presence of any 2 mutant alleles defines the
slow-acetylator genotype, whereas rapid acetylators have 1 or 2 wild-type NAT2*4 alleles.
Conclusion: On the basis of our results we suggest the using of NAT 2 genotyping for discrimination of the fast and slow
acetylators in monitoring of tuberculosis therapy
Identification and analysis of mutations in the katG gene in multidrug-resistant Mycobacterium tuberculosis clinical isolates
WSTĘP: Główną rolę w kształtowaniu oporności prątków gruźlicy (Mycobacterium tuberculosis) na izoniazyd (INH) przypisuje się mutacjom w genie katG kodującym białko katalazy/peroksydazy, enzym niezbędny do otrzymania aktywnej farmakologicznie formy leku. Analiza występowania mutacji w genie katG w szczepach M. tuberculosis może być podstawą dla opracowania wiarygodnych i szybkich testów wykrywania INH-oporności. Celem pracy jest dentyfikacja i charakterystyka mutacji w genie katG w szczepach klinicznych M. tuberculosis o wielolekooporności typu MDR.MATERIAŁ I METODY: Badanie objęło 46 szczepów wyizolowanych od chorych na gruźlicę MDR w Polsce w 2004 roku. Mutacje wykrywano, analizując sekwencje genu katG (2 223 pz) w porównaniu z sekwencją genu katG w szczepie M. tuberculosis H37Rv. Wyniki sekwencjonowania interpretowano w kontekście miana oporności na INH oraz aktywności katalazowej badanych szczepów.WYNIKI: Mutacje w genie katG wykryto w 43 (93%) szczepach. Najczęściej obserwowaną była mutacja w kodonie 315, którą stwierdzono w 34 (74%) szczepach. Mutacje w innych kodonach występowały znacznie rzadziej; w 4 szczepach wykryto zmianę w kodonie 463, w 2 — w kodonie 131, a w innych 2 — w kodonie 234. Mutacje w kodonach 68, 91, 101, 126, 128 i 194 dotyczyły pojedynczych szczepów. W 2 szczepach wykryto pojedyncze mutacje typu nonsens, które, wprowadzając przedwczesne kodony terminacji translacji, powodowały powstanie skróconego, niefunkcjonalnego białka katalazy. Szczepy te charakteryzowały się najwyższym mianem oporności na INH (MIC = 80 i 100 μg/ml) oraz całkowitą utratą aktywności katalazy. Dla pozostałych 41 szczepów niosących mutacje w genie katG, wartości MIC INH mieściły się w zakresie 0.2–10 μg/ml. Mutacje typu missens nie wpływają na aktywność katalazy oraz miano oporności na INH. Mutacje typu nonsens skutkują wysokim mianem INH-oporności oraz utratą aktywności katalazy.INTRODUCTION: A major role in the development of resistance of Mycobacterium tuberculosis to isoniazid (INH) is attributed to mutations in the katG gene coding for the catalase/peroxidase, an enzyme required for obtaining a pharmacologically active form of the drug. Analysis of mutations in the katG gene in M. tuberculosis strains may contribute to the development of reliable and rapid tests for detection of INH resistance. The aim of the study was to identify and characterize mutations in the katG gene in multidrug-resistant M. tuberculosis clinical isolates.MATERIAL AND METHODS: The study included 46 strains of M. tuberculosis, recovered from MDR-TB patients in Poland in 2004. Mutations in the katG gene were detected by comparing DNA sequences with the corresponding sequence of a wild-type reference laboratory strain (M. tuberculosis H37Rv). The obtained results were interpreted in the context of MIC values of INH and catalase activity of the strains tested.RESULTS: A total of 43 (93%) strains contained mutations in the katG gene. The most frequently observed were mutations at codon 315, found in 34 (74%) strains. Mutations at other codons were rare: 4 strains contained mutations at codon 463, 2 at codon 131 and another 2 at codon 234. Mutations at codons 68, 91, 101, 126, 128 and 194 were found in single strains only. Two strains, for which no mutations at codon 315 of the katG gene were identified, had a unique translation termination mutation, which would invariably result in polypeptide truncation leading to the generation of dysfunctional catalase polypeptides. Both these strains presented the highest MIC values for INH (80 and 100 μg/mL) and showed a complete loss of catalase activity. For the remaining 41 strains with katG mutations, the MICs of INH were within the range 0.2–10 μg/mL. Thirty-six (88%) of those strains retained their catalase activity.CONCLUSIONS: Mutations at codon 315 within the katG gene, depending on their type might be useful for the prediction of INH resistance. Whereas the missense mutations do not affect the catalase activity or the level of INH resistance, the nonsense mutations result in high-level resistance to INH and a total loss of catalase activity
Molecular analysis of strains from tuberculosis patients in Polish prisons in 2004–2008. Initial analysis of the project
Introduction: Correctional facilities are recognised breeding ground for infectious diseases. As The World Health Organization
reported, the incidence of infectious diseases in prisons population is 10100 times higher than in general population.
The incidence of tuberculosis among correctional inmates in Poland in 2008 was 270/100000, that is around 10 times higher
than among non-prisoners.
Materials and methods: The study included 57 M. tuberculosis isolates from patients in Polish prisons in 20042008 (5% of
all diagnosed TB patient in Polish prisons 20042008). Primary isolation was performed with Löwenstein-Jensen (L-J)
medium, species identification was done with the niacin test and gene probes test. Bacterial DNA was extracted from the
L-J medium slants with the cetyltrimethylammonium bromide (CTAB) method. Mycobacterium tuberculosis strains were
analyzed with two methods: screening for epidemiological discrimination of M. tuberculosis spoligotyping and highthroughput
MIRU/VNTR.
Results: Isolates that are grouped in clusters (33 isolates) were analyzed by means of MIRU/VNTRs. In MIRU/VNTRs all
strains showed different genetic patterns. Most isolates of the prisoners were grouped into two clusters: T1 53 and H3 50.
Conclusions:
1. MIRU/VNTR is a high-throughput method.
2. MIRU/VNTR is a promising method to diagnose TB transmission in Polish jails.
3. To identify the probable source of transmission, molecular analysis of strains from patients of the general population is
needed.Wstęp: Więzienia są uznawane za środowisko, w którym szczególnie łatwo dochodzi do transmisji chorób zakaźnych, w tym
również gruźlicy. Według danych Światowej Organizacji Zdrowia częstość ich występowania w środowisku więziennym jest
10–100 razy większa niż w populacji ogólnej. W Polsce w 2008 roku zapadalność na gruźlicę wśród więźniów wynosiła 270
na 100 tys. i była około 10 razy wyższa niż w populacji ogólnej.
Materiał i metody: Badaniu poddano 57 (5%) szczepów gruźlicy wyizolowanych od polskich więźniów w latach 2004–2008.
Szczepy Mycobacterium tuberculosis identyfikowano za pomocą sondy genetycznej i testu niacynowego. Bakteryjne DNA
izolowano za pomocą bromku cetylotrimetyloamoniowego. Szczepy M. tuberculosis analizowano dwiema metodami
genetycznymi: przesiewową spoligotyping i wysoce różnicującą metodą MIRU/VNTR.
Wyniki: Szczepy od więźniów, klastrujące się w metodzie spoligotyping (33 szczepy), analizowano metodą MIRU/VNTR.
Dwie najliczniejsze grupy szczepów reprezentowały spoligotypy T1 53 i H3 50. Analiza ta pokazała, że wszystkie szczepy
prątków gruźlicy od osadzonych posiadały różne wzory molekularne w metodzie MIRU/VNTR.
Wnioski:
1. Metoda MIRU/VNTR jest metodą wysoce różnicującą.
2. MIRU/VNTR jest dobrą metodą do poszukiwania źródeł transmisji gruźlicy.
3. Zaplanowano dalszą analizę molekularną w populacji ogólnej, zgodnej z miejscem pochodzenia poszczególnych więźniów.
Wyniki tych badań mają na celu ustalenie prawdopodobnego źródła zakażenia, poprzez analizę porównawczą szczepów
więziennych i populacji ogólnej
