    Efficacy of consensus interferon in treatment of HbeAg-positive chronic hepatitis B: a multicentre, randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Consensus interferon (CIFN) is a newly developed type I interferon.</p> <p>Aims</p> <p>This multicentre, controlled trial was conducted to determine the efficacy of CIFN and to compare it with alpha-1b-interferon (IFN-α1b) in the treatment of patients with hepatitis B e antigen (HBeAg)-positive chronic hepatitis B.</p> <p>Methods</p> <p>144 Patients were randomly assigned to receive 9 Όg CIFN (CIFN group) or 50 Όg INF-α1b (IFN-alpha group) subcutaneously 3 times weekly for 24 weeks, followed by 24 weeks of observation. Efficacy was assessed by normalization of serum alanine transaminase (ALT) levels and the non-detectability of serum hepatitis B virus DNA or HBeAg at the end of treatment and 24 weeks after stopping treatment.</p> <p>Results</p> <p>There was no statistically significant difference in the serological, virological and biochemical parameters between CIFN and IFN-α1b groups at the end of the therapy and follow-up period (p > 0.05). Overall, at the end of treatment, 7.0% (5/71) and 35.2% (25/71) of patients in the CIFN group showed a complete or partial response compared with 7.4% (5/68) and 33.8% (23/68) of the IFN-alpha group (p = 0.10). At 24 weeks after stopping treatment, 6.9% (5/72) and 37.5% (27/72) of patients in the CIFN group showed complete response or partial response compared with 7.1% (5/70) and 34.3% (24/70) of the IFN-alpha group (p = 0.10).</p> <p>Conclusion</p> <p>These findings suggest that 9 Όg CIFN is effective in the treatment of patients with HBeAg-positive chronic hepatitis B. It can gradually induce ALT normalization and HBV DNA clearance and HBeAg loss or HBeAg/HBeAb seroconversion.</p

    The Zakharov equations, which involve the interactions between Langmuir and ion acoustic waves in plasma, are analytically studied. By using the method of Exp-function, the periodic wave, bisoliton, Akhmediev breather, Ma breather, and Peregrine breather of the Zakharov equations are obtained. These results presented in this paper enrich the diversity of solution structures of the Zakharov equations. Furthermore, based on the numerical simulations of these solutions, some physics analysis of bisolitons and various breathers are given

    Nonlinear Dynamics and Exact Traveling Wave Solutions of the Higher-Order Nonlinear Schrödinger Equation with Derivative Non-Kerr Nonlinear Terms

    By using the method of dynamical system, the exact travelling wave solutions of the higher-order nonlinear Schrödinger equation with derivative non-Kerr nonlinear terms are studied. Based on this method, all phase portraits of the system in the parametric space are given with the aid of the Maple software. All possible bounded travelling wave solutions, such as solitary wave solutions, kink and anti-kink wave solutions, and periodic travelling wave solutions, are obtained, respectively. The results presented in this paper improve the related previous conclusions

    Simulating Flow and Dispersion by Using WRF-CFD Coupled Model in a Built-Up Area of Shenyang, China

    Results are presented from a series of numerical studies designed to investigate the atmospheric boundary layer structure, ambient wind, and pollutant source location and their impacts on the wind field and pollutant distribution within the built-up areas of Shenyang, China. Two models, namely, Open Source Field Operation and Manipulation (OpenFOAM) software package and Weather Research and Forecasting (WRF) model, are used in the present study. Then the high resolution computational fluid dynamics (CFD) numerical experiments were performed under the typical simulated atmospheric boundary conditions. It was found that the atmospheric boundary structure played a crucial role in the pollution within the building cluster, which determined the potential turbulent diffusion ability of the atmospheric surface layer; the change of the ambient wind direction can significantly affect the dispersion pattern of pollutants, which was a more sensitive factor than the ambient wind speed; under a given atmospheric state, the location of the pollution sources would dramatically determine the pollution patterns within built-up areas. The WRF-CFD numerical evaluation is a reliable method to understand the complicated flow and dispersion within built-up areas

    Association of glycemic variability and the presence and severity of coronary artery disease in patients with type 2 diabetes

    <p>Abstract</p> <p>Background</p> <p>Glucose variability is one of components of the dysglycemia in diabetes and may play an important role in development of diabetic vascular complications. The objective of this study was to assess the relationship between glycemic variability determined by a continuous glucose monitoring (CGM) system and the presence and severity of coronary artery disease (CAD) in patients with type 2 diabetes mellitus (T2DM).</p> <p>Methods</p> <p>In 344 T2DM patients with chest pain, coronary angiography revealed CAD (coronary stenosis ≄ 50% luminal diameter narrowing) in 252 patients and 92 patients without CAD. Gensini score was used to assess the severity of CAD. All participants' CGM parameters and biochemical characteristics were measured at baseline.</p> <p>Results</p> <p>Diabetic patients with CAD were older, and more were male and cigarette smokers compared with the controls. Levels of the mean amplitude of glycemic excursions (MAGE) (3.7 ± 1.4 mmol/L vs. 3.2 ± 1.2 mmol/L, p < 0.001), postprandial glucose excursion (PPGE) (3.9 ± 1.6 mmol/L vs. 3.6 ± 1.4 mmol/L, p = 0.036), serum high-sensitive C-reactive protein (hs-CRP) (10.7 ± 12.4 mg/L vs. 5.8 ± 6.7 mg/L, p < 0.001) and creatinine (Cr) (87 ± 23 mmol/L vs. 77 ± 14 mmol/L, p < 0.001) were significantly higher in patients with CAD than in patients without CAD. Gensini score closely correlated with age, MAGE, PPGE, hemoglobin A<sub>1c </sub>(HbA<sub>1c</sub>), hs-CRP and total cholesterol (TC). Multivariate analysis indicated that age (p < 0.001), MAGE (p < 0.001), serum levels of HbA<sub>1c </sub>(p = 0.022) and hs-CRP (p = 0.005) were independent determinants for Gensini score. Logistic regression analysis revealed that MAGE ≄ 3.4 mmol/L was an independent predictor for CAD. The area under the receiver-operating characteristic curve for MAGE (0.618, p = 0.001) was superior to that for HbA<sub>1c </sub>(0.554, p = 0.129).</p> <p>Conclusions</p> <p>The intraday glycemic variability is associated with the presence and severity of CAD in patients with T2DM. Effects of glycemic excursions on vascular complications should not be neglected in diabetes.</p
