Clinical and molecular aspects of gastrointestinal stromal tumors
Nowotwory podścieliskowe przewodu pokarmowego (GIST) to grupa najczęstszych nowotworów pochodzenia
mezenchymalnego w obrębie przewodu pokarmowego. Postępy w poznaniu ich mechanizmów
molekularnych spowodowały opracowanie metod leczenia, które stały się wzorcem leczenia ukierunkowanego
molekularnie w onkologii. Wprowadzenie do terapii metanosulfonianu imatynibu (hamującego
m.in. kinazy KIT/PDGFRA) zrewolucjonizowało terapię zaawansowanych (nieoperacyjnych i/lub
przerzutowych) GIST. Lek ten zarejestrowano również do leczenia uzupełniającego po wycięciu pierwotnego
GIST o znaczącym ryzyku nawrotu. U większości chorych leczonych imatynibem ostatecznie
dochodzi do rozwoju oporności. Postępy wiedzy w ciągu ostatnich lat pozwoliły na częściowe wyjaśnienie
mechanizmów progresji choroby podczas terapii imatynibem, które w większości przypadków wiążą
się z powstawaniem wtórnych mutacji w genach KIT i/lub PDGFRA. Obecnie jedynym lekiem zarejestrowanym
do terapii drugiej linii w GIST jest jabłczan sunitynibu - inhibitor wielokinazowy hamujący między
innymi KIT, PDGFRA/B, receptory naczyniowo-śródbłonkowego czynnika wzrostu (VEGFR), FLT3,
RET i CSF-1R . Trwają również badania nad innymi inhibitorami kinaz tyrozynowych nowej generacji, jak
również lekami działającymi w aternatywnych miejscach szlaków przekazywania sygnału (np. białko
wstrząsu termicznego 90, mTOR, receptor insulinopodobnego czynnika wzrostu).Gastrointestinal stromal tumors (GIST) comprise the entity of the most common mesenchymal neoplasms
of gastrointestinal tract. Advances in knowledge of molecular mechanisms of GISTs led to development
of therapy methods, which are the model of molecular targeted therapy in oncology. Introduction
to clinical practice imatinib mesylate [inhibiting kinases - KIT/PDGFRA (platelet-derived growth
factor receptor-α)] revolutionized therapy of advanced (inoperable and/or metastatic) GISTs. Imatinib
has been recently registered also for adjuvant therapy after resection of primary GIST with significant
risk of recurrence. In majority of patients treated with imatinib the resistance to this therapy developes.
Scientific advances allowed for partial explanation of mechanisms of disease progression during imatinib
therapy. The development of secondary mutations in KIT and/or PDGFRA genes is the most common
mechanism of resistance. The only registered drug for second line therapy in GIST is sunitinib
mesylate - multikinase inhibitor for e.g. KIT, PDGFRA/B, vascular endothelial growth factor receptors
(VEGFR), FLT3, RET i colony stimulating factor 1 receptor (CSF-1R). Many other tyrosine kinase inhibitors and drugs acting on alternative signaling pathways (as heat shock protein 90, mTOR, insuline-like
growth factor receptor) are currently investigated
Panitumumab and inhibition of the epidermal growth factor receptor signaling in the treatment of metastatic colorectal cancer
Panitumumab is a human monoclonal antibody that targets epidermal growth factor receptor (EGFR)
approved for the treatment of patients with metastatic colorectal cancer (mCRC). The current use of
panitumumab is, however, restricted to the management of patients who have failed chemotherapy
with irinotecan and oxaliplatin. The results of a phase III studies published in 2010 made it possible to
demonstrate clinical benefit from using panitumumab in combination with first- or second-line chemotherapy
in mCRC, at the same time confirming in prospective setting the benefits of using anti-EGFR
treatments only in the group of patients without the KRAS mutation.
We present the results of these studies and discuss selected communications from ASCO 2010 and
WCGI 2010 related to anti-EGFR treatment in mCRC.

czynnika wzrostu (EGFR), zarejestrowanym do stosowania u pacjentów z przerzutowym rakiem jelita
grubego. Obecnie stosowanie panitumumabu ogranicza się do leczenia chorych po niepowodzeniu
chemioterapii z wykorzystaniem irynotekanu i oksaliplatyny. Wyniki opublikowanych w 2010 badań
III fazy pozwoliły na wykazanie korzyści klinicznych ze stosowania panitumumabu w połączeniu z chemioterapią
w pierwszej i drugiej linii leczenia raka jelita grubego z przerzutami, jednocześnie potwierdzając
w badaniach prospektywnych korzyść z leczenia anty-EGFR wyłącznie w grupie pacjentów, u których nie
występuje mutacja w obrębie KRAS.
W niniejszym artykule przedstawiono wyniki tych badań, a także omówiono wybrane doniesienia z konferencji
ASCO 2010 i WCGI 2010 dotyczące problematyki leczenia anty-EGFR w przerzutowym raku
jelita grubego.

Adjuvant chemotherapy in patients with early breast cancer — the current state of knowledge
Uzupełniająca chemioterapia jest jednym z elementów skojarzonego leczenia chorych na niezaawansowanego raka piersi. Decyzja o jej zastosowaniu i dobór odpowiedniego schematu powinny mieć indywidualny charakter w zależności od czynników związanych z nowotworem (stopień zaawansowania, stan receptorów hormonalnych i HER2, stopień złośliwości guza, wskaźnik proliferacji Ki-67), stanem chorej (wiek biologiczny, choroby współistniejące) i jej preferencjami. Dotychczas przeprowadzono wiele badań klinicznych oceniających skuteczność różnych schematów uzupełniającej chemioterapii. W artykule omówiono wyniki najważniejszych badań z uwzględnieniem ich ostatnio opublikowanych aktualizacji po dłuższym czasie obserwacji oraz przedstawiono obecne zasady postępowania u chorych poddanych uzupełniającej terapii z powodu raka piersi. Ponadto przedstawiono zagadnienia wymagające dalszych badań (np. ocena skuteczności konkretnych schematów w zależności od biologicznego podtypu raka piersi).Adjuvant chemotherapy is one of the elements used in combined treatment of patients with early breast cancer. The decision if chemotherapy should be given and the choice of regimen must be individualized depending on the nature of the tumor (stage, hormonal receptor and HER2 status, grade, Ki-67 expression) and patient’s characteristic (biological age, comorbidities) and the patient’s preference. A number of clinical trials evaluating the effectiveness of different regimens in adjuvant setting were performed. We discuss the results of the most important studies including their recently published updates which have a longer follow-up period. We present the current management of breast cancer patients receiving adjuvant therapy. We also discuss the issues requiring further research (e.g. the assessment of specific regimens’ efficacy depending on biological subtype of breast cancer)
Capecitabine in the treatment of advanced gastric cancer
Leczenie zaawansowanego raka żołądka ewoluowało w ciągu ostatnich 20 lat od leczenia wspomagającego
do chemioterapii wielolekowej, co wiąże się z poprawą mediany przeżycia całkowitego z 3 do
11 miesięcy. W praktyce klinicznej, ale także w zaleceniach ekspertów, w dalszym ciągu brakuje konsensusu
odnośnie wskazań do leczenia paliatywnego, składu chemioterapii, a także złożoności programów
chemioterapii (programy dwu- czy trójlekowe) oraz zasadności leczenia kolejnych linii.
Klasyczne programy chemioterapii paliatywnej raka żołądka opierają się na przedłużonym wielodobowym
stosowaniu 5-fluorouracylu (5-FU). W tym zakresie istotną odmianę wprowadziło zastosowanie kapecytabiny
— doustnej fluoropirymidyny mającej charakter proleku, która dzięki postaci farmaceutycznej i metabolizmowi
w tkankach guza nowotworowego pozwala na uzyskanie wysokiej aktywności przeciwnowotworowej.
W badaniach III fazy REAL-2 i ML17032 wykazano, że w połączeniu z pochodnymi platyny jest ona nie
mniej skuteczna od 5-FU w analogicznych schematach leczenia u chorych z zaawansowanym rakiem
żołądka. Dodatkowo metaanaliza powyższych badań wykazała, iż stosowanie kapecytabiny zamiast 5-FU
istotnie wydłuża przeżycie całkowite, nie wpływając znacznie na profil toksyczności i czas do progresji.
W pracy przedstawiono aktualny stan wiedzy na temat miejsca kapecytabiny w leczeniu raka żołądka
w kontekście badań rejestracyjnych oraz pozarejestracyjnych, zidentyfikowanych w ramach przeglądu
systematycznego literatury.
Onkol. Prak. Klin. 2011; 7, 3: 119–126Treatment of advanced gastric cancer has evolved over the past 20 years and includes adjuvant treatment
as well as multidrug chemotherapy. Such treatments are associated with improved median overall
survival rates between 3 and 11 months. In clinical practice, and also according to expert opinion, there
is still no consensus regarding: the indications for the use of palliative treatment; the composition of
chemotherapy; the complexity of chemotherapy programs (double- or triple-drug combinations); and
the merits of second line treatments.
Classical programs of palliative chemotherapy of gastric cancer are based on an extended multiday
use of 5-fluorouracil (5-FU). In this respect, a significant change was brought about with the use of
capecitabine, which is an orally-available prodrug form of fluoropyrimidine. The active metabolites of
capecitabine display high anticancer activity.
In Phase III of REAL-2 and ML17032 it was demonstrated that, in combination with platinum derivatives,
capecitabine is not less effective than 5-FU in the same regimens when used for patients with advanced
gastric cancer. A meta-analysis of these studies showed that the use of capecitabine, in place of 5-FU,
significantly extended overall survival. Furthermore, the toxicity profile and time to progression appeared
the same when the two drugs were compared.
The paper presents the current state of knowledge relating to the use of capecitabine in the treatment
of gastric cancer. To this end, both pre-registration and post-registration clinical studies were identified
in the systematic review of the literature.

The role of 18F-FDG PET/CT in the diagnosis of recurrent colorectal cancer
Rak jelita grubego jest jednym z najczęściej występujących nowotworów złośliwych w Polsce i na świecie. Coraz częściej stosowaną i uznaną w onkologii metodą diagnostyczną jest badanie PET/CT — jedna z najnowocześniejszych metod diagnostyki obrazowej. Dzięki połączeniu obrazowania anatomicznego (CT) z obrazowaniem metabolicznym (PET) umożliwia wczesną identyfikację zmian rozrostowych, dokładną ocenę stopnia zaawansowania nowotworu. W diagnostyce wznowy raka jelita grubego badanie PET/CT z zastosowaniem fluorodeoksyglukozy (18F-FDG) ma udowodnioną wartość kliniczną. Informacje uzyskane w badaniu w istotnym stopniu wpływają na podejmowane decyzje terapeutyczne. W przypadku planowanej metastazektomii metoda ta pozwala precyzyjniej niż inne ocenić stopień zaawansowania wznowy, zlokalizować wszystkie ogniska nawrotu, a co za tym idzie, pozwala uniknąć niepotrzebnych zabiegów. Standardowo wykonywane badanie 18F-FDG PET/CT, polegające na wykonaniu badania tomografi i komputerowej tzw. low dose bez podania dożylnego środka kontrastującego, a następnie rejestracji PET i wykonaniu fuzji obu obrazów, nie dostarcza jednak wystarczających informacji dotyczących oceny stosunku guza bądź nacieku do narządów sąsiednich oraz naczyń, niezbędnych do pełnej oceny operacyjności wznowy, dostępnych w badaniu tomografi ikomputerowej z zastosowaniem dożylnego środka kontrastującego. Wykonanie badania 18F-FDG PET/CT wg schematu low dose CT, PET, a następnie CT z dożylnym podaniem środka kontrastującego umożliwia uzyskanie na podstawie jednego badania wszystkich niezbędnych informacji dotyczących operacyjności zmiany i może w przyszłości stać się badaniem z wyboru w diagnostyce wznowy raka jelita grubego.Colorectal cancer is one of the most common cancers worldwide, including Poland. PET/CT is a relatively new diagnostic imaging method which has gained wide acceptance in oncology. The combination of anatomical imaging (CT) and metabolic imaging (PET) enables early identification of proliferative changes and thus an accurate staging of cancer. In the diagnosis of recurrent colorectal cancer, PET/CT with fluorodeoxyglucose (18F-FDG) has proven to be of high clinical value. The information obtained in this study has a significant impact on patient management. When there is suspicion of relapse, it is important to use an effective restaging tool to accurately define the extent of the recurrence and to plan the proper therapy. In the standard protocol for the 18F-FDG PET/CT study, fused images of low dose CT (without intravenous contrastagent) and PET are performed. This approach does not provide sufficient information of local tumor or invasion to adjacent organs and vessels to enable assessment of the suitability for resectability. This information is, however, available from computed tomography when intravenous contrast agent is used. Implementation of the PET/CT protocol consisting of low dose CT, with PET followed by CT with intravenous contrast agent in the corresponding region, provides in a single study all the necessary information required to qualify patients for surgery. It may in the future become the modality of choice in the diagnosis of colorectal cancer recurrence
Recommendations on the treatment of basal cell carcinoma and squamous cell carcinoma prepared by the Oncology Department of the Polish Dermatology Society and the Melanoma Academy Department of the Polish Society of Oncological Surgery
Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are the most frequent malignant neoplasms among Caucasian patients. Despite the fact that they seldom metastasise and are not directly fatal, they constitute a significant clinical issue. Such cancers infiltrate surrounding tissues and destroy the surrounding structures, e.g. bones and cartilages, as a result of which such structures develop into severe aesthetic defects and significantly deteriorate the life quality of the patients. Among patients from the high-risk group (namely patients under chronic immunosuppression or those genetically predisposed to develop UV-induced skin cancers) skin cancers may be aggressive and fatal. The Oncology Department of the Polish Dermatology Society (Polish: Sekcja Onkologiczna Polskiego Towarzystwa Dermatologicznego — PTD) and the Melanoma Academy Department of the Polish Society of Oncological Surgery (Polish: Sekcja Akademia Czerniaka Polskiego Towarzystwa Chirurgii Onkologicznej — PTChO), based on the current European guidelines, American recommendations of the National Comprehensive Cancer Network (revision 1.2015), and interventional reviews of publications elaborated by the Cochrane Skin Group, attempted to systemise the diagnostic and therapeutic procedures and determine homogenous rules of primary and secondary prevention in patients suspected/diagnosed with a basal cell or squamous cell carcinoma. This paper presents actual recommendations regarding skin cancer diagnosis and treatment, taking all related benefits and challenges into consideration, as well as recommendations for post-treatment patient monitoring
Diagnosis and treatment of basal cell and squamous cell carcinoma and melanoma of the skin recommended by the Oncology Section of the Polish Dermatology Society as well as the Melanoma Academy Section of the Polish Society of Oncological Surgery
Rak podstawnokomórkowy (BCC— basal cell carcinoma) oraz rak kolczystokomórkowy (SCC — squamous cell carcinoma) są najczęściej występującymi nowotworami złośliwymi występującymi u osób rasy kaukaskiej. Pomimo że rzadko prowadzą do powstawania przerzutów i zgonu chorego, to stanowią jednak bardzo istotny problem kliniczny, ponieważ charakteryzują się naciekaniem i niszczeniem sąsiadujących tkanek i struktur, co sprawia, że stają się powodem poważnych defektów estetycznych i znacząco upośledzają jakość życia chorych. U pacjentów w trakcie przewlekłej immunosupresji lub z genetyczną predyspozycją mogą mieć jednak agresywny i prowadzący do śmierci przebieg. Z tego powodu Sekcja Onkologiczna Polskiego Towarzystwa Dermatologicznego oraz sekcja Akademia Czerniaka Polskiego Towarzystwa Chirurgii Onkologicznej, na podstawie aktualnych wytycznych europejskich, rekomendacji amerykańskich National Comprehensive Cancer Network (wersja 1.2015) oraz interwencyjnych recenzji publikacji opracowanych przez Cochrane Skin Group, podjęła próbę usystematyzowania zasad postępowania diagnostyczno-terapeutycznego oraz ustalenia jednorodnych zasad prewencji pierwotnej i wtórnej u pacjentów z podejrzeniem/rozpoznaniem raka podstawnokomórkowego lub kolczystokomórkowego. W pracy przedstawiono aktualne zalecenia dotyczące rozpoznania i leczenia raków skory, uwzględniając korzyści oraz problemy z nim związane, a także wytyczne monitorowania pacjentów po terapii.
