    Acute intestinal obstruction revealing synchronous gastrointestinal stromal tumors in a small bowel diverticulum and mucinous adenocarcinoma of the colon: a case report

    Gastrointestinalstromal tumors are rare neoplasms and represent 0,1% to 3% of all gastrointestinal cancers. They are the most frequent mesenchymal neoplasms of the gastrointestinal tract with a malignant potential and unpredictable behavior. The synchronous association with other primary gastrointestinal carcinoma has been rarely reported in the literature with increasing number in the last ten years. The associated Gastrointestinalstromal tumor is usually discovered incidentally during surgery for carcinoma. The limited number of these cases cannot confirm the existence of a common factor in tumorigenesis of these different tumors and other studies are needed to clarify the possible association. We report the first case in the literature of synchronous primary Gastrointestinalstromal tumors developed in small bowel diverticulum and mucinousadenocarcinoma of the colon.     Does the Formulation of Oral Solid Dosage Forms Affect Acceptance and Adherence in Older Patients?:A Mixed Methods Systematic Review

    OBJECTIVES: Age-related changes mean that the older population can encounter barriers toward taking medication orally. Further work is needed to identify the characteristics of oral solid dosage forms that will improve patient acceptance and adherence. The aim of this systematic review was to identify if and how formulation aspects of oral solid dosage forms affect acceptance and adherence in older people. DESIGN: Mixed methods systematic review using a data-based convergent synthesis design. SETTING AND PARTICIPANTS: Articles were selected if they included participants aged 60 years and older, or included health care professionals, social care professionals, and informal carers of patients aged 60 years and older. METHODS: A systematic search of the following databases was undertaken: Web of Science, MEDLINE, Scopus, and The Cochrane Databases. The search of databases was supplemented by a search of gray literature, and reference lists of included papers were manually searched. RESULTS: A total of 16 studies were included in the final synthesis. Three themes were generated from the thematic analysis: (1) dimensions, (2) palatability, and (3) appearance. The dimensions and palatability are often modified to improve swallowability by breaking tablets in half or taste masking with food. Polypharmacy can lead to patients using the appearance to identify tablets; however, this can lead to confusion when products appear similar. No study was identified that explored formulation characteristics across all 3 categories directly in the older population. CONCLUSION AND IMPLICATIONS: Manufacturers should take into account practical problems older people may encounter when considering the dimensions, palatability, and appearance of the final drug product. These characteristics should be optimized to aid visual identification and swallowability. Medical providers and pharmacists have an important role in ensuring that these patient-centric drug products are prescribed and dispensed appropriately so that patients receive the most suitable formulation

    Zonas ripárias são áreas de saturação hídrica, permanente ou temporária, cuja principal função é a proteção dos recursos hídricos de uma microbacia. Essa pesquisa comparou a adequação do uso do solo de dois cenários de planejamento agrícola de uma microbacia: o cenário convencional, representando o método usualmente empregado, que apenas considera as classes de capacidade de uso da terra, e o cenário hidrológico, que inclui a delimitação e avaliação das zonas ripárias. Um estudo de caso foi realizado na Microbacia do Ribeirão São João (3.656 ha), no município de Mineiros do Tietê (São Paulo, Brasil). Mapas de Classe de Capacidade de Uso da Terra e de Adequação do Uso do Solo foram elaborados, utilizando o Sistema de Informação Geográfica (SIG), para a construção dos cenários convencional e do proposto. Excluindo a Área de Preservação Permanente (APP), o cenário convencional indicou que 59,0% da área destinada à agricultura está adequadamente utilizada, 28,2% está subutilizada e 2,6% está sobreutilizada. O cenário proposto ou hidrológico, com inclusão da identificação da zona ripária (24,9% da microbacia) mostrou que muitas áreas que, no cenário convencional, possuem pouca restrição para o cultivo intensivo, como as classes II e III, são zonas ripárias, de sensibilidade hidrológica. Existem dentro dos limites da zona ripária 38,9% de classe de capacidade de uso III e 49,5% de classe IV. O planejador, desconsiderando a zona ripária, pode colocar em risco áreas vitais que, se degradadas, representam danos para a saúde e resiliência da microbacia

    Electronic-Cigarette Vehicles and Flavoring Affect Lung Function and Immune Responses in a Murine Model

    The use of electronic nicotine delivery systems (ENDS), also known as electronic-cigarettes (e-cigs), has raised serious public health concerns, especially in light of the 2019 outbreak of e-cig or vaping product use-associated acute lung injury (EVALI). While these cases have mostly been linked to ENDS that contain vitamin E acetate, there is limited research that has focused on the chronic pulmonary effects of the delivery vehicles (i.e., without nicotine and flavoring). Thus, we investigated lung function and immune responses in a mouse model following exposure to the nearly ubiquitous e-cig delivery vehicles, vegetable glycerin (VG) and propylene glycol (PG), used with a specific 70%/30% ratio, with or without vanilla flavoring. We hypothesized that mice exposed sub-acutely to these e-cig aerosols would exhibit lung inflammation and altered lung function. Adult female C57BL/6 mice (n= 11-12 per group) were exposed to filtered air, 70%/30% VG/PG, or 70%/30% VG/PG with a French vanilla flavoring for 2 h a day for 6 weeks. Prior to sacrifice, lung function was assessed. At sacrifice, broncho-alveolar lavage fluid and lung tissue were collected for lipid mediator analysis, flow cytometry, histopathology, and gene expression analyses. Exposures to VG/PG + vanilla e-cig aerosol increased lung tidal and minute volumes and tissue damping. Immunophenotyping of lung immune cells revealed an increased number of dendritic cells, CD4+ T cells, and CD19+ B cells in the VG/PG-exposed group compared to air, irrespective of the presence of vanilla flavoring. Quantification of bioactive lung lipids demonstrated a \u3e3-fold increase of 2-arachidonoylglycerol (2-AG), an anti-inflammatory mediator, and a 2-fold increase of 12-hydroxyeicosatetraenoic acid (12-HETE), another inflammatory mediator, following VG/PG exposure, with or without vanilla flavoring. This suggests that e-cig aerosol vehicles may affect immunoregulatory molecules. We also found that the two e-cig aerosols dysregulated the expression of lung genes. Ingenuity Pathway Analysis revealed that the gene networks that are dysregulated by the VG/PG e-cig aerosol are associated with metabolism of cellular proteins and lipids. Overall, our findings demonstrate that VG and PG, the main constituents of e-liquid formulations, when aerosolized through an e-cig device, are not harmless to the lungs, since they disrupt immune homeostasis

    Computational and Experimental Study on Undoped and Er-Doped Lithium Tantalate Nanofluorescent Probes

    We present a combined density functional theory (DFT) and experimental work on lithium tantalate LiTaO3 (LT) and its Er-doped counterparts. We calculate the electronic and optical properties for both LT and LT:Er+3, with Er occupying either Li or Ta sites, at 4.167 mol%. The generalized gradient approximation (GGA) calculations show that the Er-4 f bands appear closer to the conduction band bottom and to the valance band top, for the first and second doped configurations, respectively. This agrees with changes in the imaginary part of the frequency dependent dielectric function between the doped configurations. There are striking differences between the GGA and the hybrid functional HSE06 calculations for the band structures of the doped configurations. HSE06 accurately predicts the location in energy for all Er-4 f orbitals: These are now spread in energy and appear above and below the Fermi energy. We synthesized LT:Er+3 nanoparticles, validated through X-ray diffraction and Scanning Electron Microscopy. Differential scanning calorimetry and thermogravimetric analysis confirmed increases in the activation energy and lowering of the reaction temperature due to Er+3 doping. The LT:Er+3photoluminescence showed strong f–f emission in the visible and near-infrared regions, in an excellent agreement with the HSE06 electronic information

    Literature review of medication administration problems in paediatrics by parent/caregiver and the role of health literacy

    Objective: To identify studies that highlighted medication administration problems experienced by parents and children, which also looked at health literacy aspect using a validated tool to assess for literacy. Study design: Ten electronic databases were systematically searched and supplemented by hand searching through reference lists using the following search terms: (1) paediatric, (2) medication error including dosing error, medication administration error, medication safety and medication optimisation and (3) health literacy. Results: Of the (1230) records screened, 14 studies were eligible for inclusion. Three analytical themes emerged from the synthesis. The review highlighted that frequencies and magnitudes of dosing errors vary by the measurement tools used, the dose prescribed and by the administration instruction provided. Parent's sociodemographic, such as health literacy and language, is a key factor to be considered when designing an intervention aimed at averting medication administration errors at home. The review summarised some potential strategies that could help in reducing medication administration errors among children at home. Among these recommendations is to show the prescribed dose to the parents or young people along with the verbal instructions, as well as to match the prescribed dose with the measuring tool dispensed, to provide an explicit dose intervals and pictographic dosing instructions. Conclusion: The findings suggest that in order to optimise medication use by parents, further work is needed to address the nature of these issues at home. Counselling, medication administration instructions and measurement tools are some of the areas in addition to the sociodemographic characteristics of parents and young people that need to be considered when designing any future potential intervention aimed at reducing medication errors among children and young people at home

    Cervical cancer awareness, perception, and attitude among tertiary health institution students in northeastern Nigeria

    BackgroundThe devastating scourge of cervical cancer in Africa is largely due to the absence of preventive interventions, driven by low awareness and poor perception of the disease in the continent. This work is a preliminary effort toward understanding key social drivers promoting this disease in our immediate environment with a view to mitigating it.MethodFemale students of two tertiary health institutions in Azare, northeastern Nigeria, were approached to participate in this cross-sectional descriptive study. A structured self-administered questionnaire was administered to consenting participants and covered questions on their socio-demographics, awareness, perception, and attitude about/toward cervical cancer and its prevention. The responses were scrutinized for coherency and categorized into themes using summary statistics, while a chi-square test was used to determine the association between awareness of cervical cancer and participant age, marital status, religion, screening uptake, and willingness to undergo screen.ResultsAwareness of cervical cancer was recorded among 174/230 (75.7%) respondents who enrolled in this study; 117 (67.2%) knew that it was preventable, but only three (1.3%) respondents had undergone screening. Among the aware participants, 91 (52.3%) and 131 (75.3%) knew that sexual intercourse and multiple sexual partners are risk factors for the disease, respectively. In contrast, knowledge of the etiology was poor; 82 (47.1%) respondents who knew it was preventable had heard about human papillomavirus (HPV), while 72 (41.4%) knew that HPV causes cervical cancer. Most (78%) of the participants expressed willingness to take a human papillomavirus vaccine or undergo screening (84.6%) if made available to them. Awareness was significantly associated with participants’ age (p = 0.022) and willingness to undergo screening (p = 0.016).ConclusionThis study revealed discordance between awareness and knowledge about cervical cancer. Educational initiatives reflective of population perception/knowledge of cervical cancer are needed to mitigate the rising incidence of this disease, especially among female healthcare providers
