A Microstructure-Based Mechanistic Approach to Detect Degeneration Effects on Potential Damage Zones and Morphology of Young and Old Human Intervertebral Discs

Kyphoplastie par greffe osseuse autologue (un nouveau traitement mini invasif des fractures du rachis thoraco lombaire)
Introduction : Les fractures du rachis thoracolombaire représentent une préoccupation importante dans la chirurgie rachidienne. Il n y a aujourd hui pas de consensus pour les fractures du rachis de type A selon la classification de Magerl. La kyphoplastie avec injection de ciment PMMA couplée à l ostéosynthèse rachidienne percutanée est aujourd hui une indication validée dans ce contexte. Pour pallier aux complications induites par l injection de ciment PMMA l objectif de ce travail est d évaluer une technique de kyphoplastie par injection de greffe osseuse autologue iliaque. Matériels et méthodes : Nous avons effectué le suivi d une série prospective de 15 patients opérés d une kyphoplastie avec injection d os autologue associée à une ostéosynthèse percutanée pour une fracture de type A dans le service de neurochirurgie au CHRU de Lille de novembre 2012 à avril 2013. Toutes les données opératoires, cliniques et radiologiques sont rapportées. Résultats : Le temps d intervention moyen a été de 145,5 minutes avec un saignement inférieur à 100mL. Il a été noté une amélioration significative de la douleur avec une EVA passant de 6,9 à 3,1. Les angles de cyphose vertébrale et d angulation régionale traumatique ont eux aussi été améliorés passant respectivement de 14,1 à 3,6 et de 12,9 à 1,3. Il n est rapporté qu un seul cas d hyperthermie isolée postopératoire résolutif après traitement antibiotique adapté et sans complication. Conclusion : La kyphoplastie par injection de greffe osseuse autologue couplée à l ostéosynthèse percutanée paraît être une technique sûre et efficace pour le traitement des fractures de type A. L évaluation prolongée d une cohorte plus importante dans le cadre d une étude randomisée pourra apporter une validation supplémentaire à cette technique.LILLE2-BU Santé-Recherche (593502101) / SudocSudocFranceF

Modeling strain-induced dual-phase transformation in semicrystalline polylactide
International audiencePolylactic acid or polylactide (PLA) can form amorphous, semicrystalline, or highly crystalline structure. In this paper, we provide a model for semicrystalline PLA under straininduced dual-phase transformation, between mesomorphization and crystallization. In the proposed constitutive model, we use an internal state variable approach by considering initial microstructure, strain-induced dual-phase transformation, intermolecular features, and preferred network orientation-induced anisotropy. The effective contribution of the newly created phases is considered in both the elastic-viscoplastic intermolecular resistance and the viscohyperelastic network resistance using a micro-macro transition approach. We find a good agreement between the model and experimental data for the semicrystalline PLA in terms of progressive evolution of the mesomorphic/crystalline phases along with stressstrain behavior up to very large strains and over a wide range of crystallinities. The relationship between initial crystallinity, strain-induced molecular ordering, newly created phases, and mechanical response is discussed
