Preliminary Study About The Application Of Gravicorder For Weight-Shifting and-Bearing Exercise As a New Educational Device of Physiotherapists

Background: To establish an objective evaluation of basic physiotherapy technique, we recorded a mechanical aspect during a lateral weight-shifting (WS) using gravicorder. Methods: Subjects were 18 physiotherapists divided into three groups based on experience year. Our originally developed software converted center of gravity data during WS to 3-dimensional mechanical value. Results: All direction data showed significantly lower in the long experience year group (p<0.05). The coefficient of variation was high vertical for all three groups. Conclusion: In the group with more years of experience, it was confirmed that it was compactly implemented. In the vertical direction, it is thought to be implemented by empirical rules because there is no constant normal pattern or common recognition among physiotherapists

Human T-cell leukemia virus type 1 may invalidate T-SPOT.TB assay results in rheumatoid arthritis patients: A retrospective case-control observational study.

BACKGROUND:CD4-positive T cells are the main target of human T-cell leukemia virus type 1 (HTLV-1). Interferon-γ release assays rely on the fact that T-lymphocytes release this cytokine when exposed to tuberculosis-specific antigens and are useful in testing for latent tuberculosis infection before initiating biologic therapy, such as anti-tumor necrosis factor agents. However, the reliability of interferon-γ release assays in detecting tuberculosis infection among HTLV-1-positive patients with rheumatoid arthritis (RA) remains unclear. The present study aimed to evaluate the use of the T-SPOT.TB assay in HTLV-1-positive RA patients. METHODS:Overall, 29 HTLV-1-positive RA patients and 87 age- and sex-matched HTLV-1-negative RA patients (controls) were included from the HTLV-1 RA Miyazaki Cohort Study. Results of the T-SPOT.TB assay for latent tuberculosis infection screening were collected from medical records of patients. RESULTS:Approximately 55% of the HTLV-1-positive RA patients showed invalid T-SPOT.TB assay results (odds ratio: 108, 95% confidence interval: 13.1-890, p 10 in the negative controls. HTLV-1 proviral load values were significantly higher in patients with invalid results compared with those without invalid results (p = 0.003). CONCLUSION:HTLV-1 infection affects T-SPOT.TB assay results in RA patients. Assay results in HTLV-1 endemic regions should be interpreted with caution when screening for latent tuberculosis infection before initiation of biologic therapy

レビー小体型痴呆の一症例

レビー小体型痴呆の患者の示す三大症状としては,緩やかに進行する痴呆,幻覚,パーキンソニズムがある。今回,私達はレビー小体型痴呆患者を受け持ったので,この様な患者の看護における要点について報告する。患者は80才男性で,幻視等の精神症状のため本院に入院となった。5年前より痴呆およびパーキンソニズムを呈していた。精神症状に対し少量のカルバマゼピンおよび塩酸チアプリドが投与された。投与数日後より他の患者との関係も改善された。痴呆とパーキンソニズムは存在したが,精神症状の改善により日常生活活動も改良された。この様な患者は時には抗精神病薬に著しく反応することがあるので,この様な点を考慮に入れて薬剤の不利な作用を厳重に観察することが重要である。Patients of dementia with Lewy bodies (DLB) show three characteristic symptoms : slowly progressive dementia, hallucinations, and parkinsonism, which are the features of this disorder. We treated a patient with this disorder, and here report the key points in nursing such patients. The patient, a 80 year old man, was admitted to our hospital because of visual hallucinations and violent tendencies. He had suffered from dementia and gait disturbance for the previous 5 years. In order to control the psychiatric symptoms, small amounts of carbamazepine and tiaprid were prescribed. Hallucinations were quickly alleviated by these drugs and the relationship between this patient and other patients soon improved markedly. Although dementia and parkinsonism are still evident, the patient is able to function much better in conducting the activities of daily life since his psychiatric state ameliorated. Patients of this disorder sometimes show pronounced responses to antipsychotic drugs, and therefore it is important to take this point into account and strictly observe the adverse effects of prescribed medications
