    HIV Treatment as Prevention: Contradictory Perspectives from Dynamic Mathematical Models

    The preventative effects of antiretroviral therapy for people with HIV have been debated since they were first raised. Models commenced studying the preventive effects of treatment in the 1990s, prior to initial public reports. However, the outcomes of the preventive effects of antiretroviral use were not consistent. Some outcomes of dynamic models were based on unfeasible assumptions, such as no consideration of drug resistance, behavior disinhibition, or economic inputs in poor countries, and unrealistic input variables, for example, overstated initiation time, adherence, coverage, and efficacy of treatment. This paper reviewed dynamic mathematical models to ascertain the complex effects of ART on HIV transmission. This review discusses more conservative inputs and outcomes relative to antiretroviral use in HIV infections in dynamic mathematical models. ART alone cannot eliminate HIV transmission

    Syndemics of syphilis, HCV infection, and methamphetamine use along the east coast of China

    An upsurge in club drug use has been observed in recent years in some cities of China, especially methamphetamine, which is quickly replacing heroin to become the most widespread drug across the nation. This study investigated the type of drugs used, syphilis and hepatitis C virus (HCV) infection and the correlates for syphilis, HCV and unprotected commercial sex behavior among drug users in two cities along the east coast of China. A cross-sectional survey conducted in 2010 provided demographics, sexual and drug use behaviors, HIV knowledge and the utilization of intervention services among drug users. Blood samples were tested for HIV, syphilis, and HCV infection. Of 805 eligible participants, 0.2% were infected with HIV, 3.7% with HCV, and 9.6% with syphilis. Of the participants, 96.6% were methamphetamine users, 11.9% reported ever having used ≥2 types of these drugs, and 11.4% reported ever injecting drugs. In the multivariable logistic regression analysis, participants infected with syphilis were more likely to be female (adjusted odds ratio (AOR)=2.8, 95% confidence interval (CI): 1.2-6.5), have ever had commercial sex in the past 12 months (AOR=2.0, 95% CI: 1.0-3.9), be infected with HCV (AOR=12.1, 95% CI: 4.1-20.3) and less likely to have ever had sex with regular partners in the past 12 months (AOR=0.2, 95% CI: 0.1-0.6). Participants infected with HCV were more likely to have ever injected drugs (AOR=2.7, 95% CI: 1.1-6.5) and be infected with syphilis (AOR=8.0, 95% CI: 3.5-18.0). Participants who had unprotected sex with commercial sex partners in the last sexual encounter were more likely to be female (AOR=2.9, 95% CI:1.7-4.9), have middle school or lower level education (AOR=3.4, 95% CI:2.0-5.5), never have received intervention in the last year (AOR=2.1, 95%CI:1.2-3.6) and be infected with syphilis (AOR=4.2, 95% CI:2.4-7.4). Methamphetamine is the predominant drug used among the drug users, the prevalence of syphilis and HCV infection are alarmingly high, and unprotected commercial sex was common among this group. The findings highlight the need for effective, multifaceted interventions addressing sexual and drug use-related risky behaviors among this group.     Estimation of HIV-1 incidence among five focal populations in Dehong, Yunnan: a hard hit area along a major drug trafficking route

    <p>Abstract</p> <p>Background</p> <p>Since 1989 when the first 146 HIV positives in China were identified, Dehong Prefecture had been one of the areas hardest-hit by HIV in China. The local and national governments have put substantial financial resources into tackling the HIV epidemic in Dehong from 2004. The objective of this study was to track dynamic changes in HIV-1 prevalence and incidence among five focal populations in Dehong and to assess the impact of HIV prevention and control efforts.</p> <p>Methods</p> <p>Consecutive cross-sectional surveys conducted in five focal populations between 2004 and 2008. Specimens seropositive for HIV were tested with the BED IgG capture enzyme immunoassay to identify recent seroconversions (median, 155 days) using normalized optical density of 0.8 and adjustments.</p> <p>Results</p> <p>From 2004 to 2008, estimated annual HIV incidence among injecting drug users (IDUs) decreased significantly [from 15.0% (95% CI = 11.4%-18.5%) in 2004 to 4.3% (95% CI = 2.4%-6.2%) in 2008; trend test P < 0.0001]. The incidence among other focal populations, such as HIV discordant couples (varying from 5.5% to 4.7%), female sex workers (varying from 1.4% to 1.3%), pregnant women (0.1%), and pre-marital couples (0.2 to 0.1%) remained stable. Overall, the proportion of recent HIV-1 infections was higher among females than males (P < 0.0001).</p> <p>Conclusions</p> <p>The HIV epidemic in Dehong continued to expand during a five-year period but at a slowing rate among IDUs, and HIV incidence remains high among IDUs and discordant couples. Intensive prevention measures should target sub-groups at highest risk to further slow the epidemic and control the migration of HIV to other areas of China, and multivariate analysis is needed to explore which measures are more effective for different populations.</p

    Influence of counter electrode material during accelerated durability test of non-precious metal electrocatalysts in acidic medium

    Significant progress has been made in the development of non-precious metal electrocatalysts (NPMEs) during the past decade. Correspondingly, there is an urgent demand for an appropriate measurement method to be established for the reliable evaluation of NPMEs. In this study, platinum and graphite counter electrodes were used to investigate the impact of counter electrode material on the accelerated durability testing (ADT) of NPMEs in acidic medium. Platinum used as the counter electrode in a traditional three-electrode electrochemical cell was found to dissolve in acidic medium and re-deposit on NPME coated on the working electrode during ADT. Such re-deposition causes the oxygen reduction reaction (ORR) performance of NPMEs to remarkably improve, and thus will seriously mislead our judgment of NPMEs if we are unaware of it.     Power Grid Structure Performance Evaluation Based on Complex Network Cascade Failure Analysis

    A safe and stable operation power system is very important for the maintenance of national industrial security and social economy. However, with the increasing complexity of the power grid topology and its operation, new challenges in estimating and evaluating the grid structure performance have received significant attention. Complex network theory transfers the power grid to a network with nodes and links, which helps evaluate the system conveniently with a global view. In this paper, we employ the complex network method to address the cascade failure process and grid structure performance assessment simultaneously. Firstly, a grid cascade failure model based on network topology and power system characteristics is constructed. Then, a set of performance evaluation indicators, including invulnerability, reliability, and vulnerability, is proposed based on the actual functional properties of the grid by renewing the power-weighted degree, medium, and clustering coefficients according to the network cascade failure. Finally, a comprehensive network performance evaluation index, which combines the invulnerability, reliability, and vulnerability indicators with an entropy-based objective weighting method, is put forward in this study. In order to confirm the approach’s efficacy, an IEEE-30 bus system is employed for a case study. Numerical results show that the weighted integrated index with a functional network could better evaluate the power grid performance than the unweighted index with a topology network, which demonstrates and validates the effectiveness of the method proposed in this paper

    Facile synthesis and electrochemical properties of MnO2/carbon nanotubes

    The poor electrical conductivity of MnO2 limits its use as an electrode material. To overcome this limitation, we report an easy and rapid approach to deposit nanosized MnO2 onto multi-wall carbon nanotubes functionalized with hydroxyl groups (MWCNTs-OH) by chemical reduction of KMnO4 with MnSO4 in aqueous solution under ambient conditions. Characterization with XRD and TEM reveals that the obtained MnO2/MWCNTs-OH composite is nanocrystalline and partially covered by MnO2 nanosheets with a thickness of 1-3 nm at a MnO2 loading of 20 wt%. Cyclic voltammetry (CV) and galvanostatic charge-discharge measurements reveal that the MnO2/MWCNTs-OH composite with a MnO2 loading of 20 wt% has a relatively high specific capacitance of 234 F/g at a scan rate of 2 mV/s and exhibits good cycling stability. Furthermore, the oxygen reduction reaction (ORR) shows that MnO2/MWCNTs-OH composite may have potential applications as a non-noble metal electrocatalyst in fuel cells and metal-air batteries.
