    The Waveform Fluctuation and the Clinical Factors of the Initial and Sustained Erythropoietic Response to Continuous Erythropoietin Receptor Activator in Hemodialysis Patients

    Objectives. Erythropoiesis-stimulating agents (ESA) are the main treatment for anemia in hemodialysis (HD) patients. We evaluated factors determining the response after treatment of a new ESA (continuous erythropoietin erythropoietin receptor activator (CERA)). Methods. 61 HD patients were classified by their response at two different timings. First, patients whose hematocrit (Hct) increased 1.5% in the first week were defined as initial responders (IR, n = 16). We compared several parameters between IR and the rest of the study subjects (non-IR, n = 45). Second, patients whose Hct increased 2% in the 4th week were defined as sustained responders (SR, n = 12), and we did a similar comparison. Results. The Hct showed a waveform fluctuation. Compared with the rest, IR had significantly lower platelet counts and higher levels of ferritin, total protein, total bilirubin, and serum sodium, while SR had significantly lower levels of C-reactive protein and low-density lipoprotein (All P < 0.05). In comparison with the rest, higher Hct persisted for 10 weeks in SR but only for two separate weeks (the 1st and 7th week) in IR. Conclusions. The initial and sustained erythropoietic responses are independent from each other and are associated with different factors. Treatment focusing on these factors may improve the response

    Safety-based Injection Strategy for Carbon Dioxide Geological Sequestration in a Deep Saline Aquifer with Complex Sandstone-shale Sequences: A Case Study from Taiwan

    AbstractThe purpose of this study was to decide the best injection strategy for CO2 geo-sequestration in a deep saline aquifer with complex bedded sandstone-shale sequences. The best injection strategy is decided based on the estimates of the safety index (SFI). Numerical simulation method was used in this study. The major conclusions from this study are: (1) Safe trapping mechanisms contribute to a lower risk of CO2 leakage by trapping CO2 as immobile blobs or changing the phase of CO2 from supercritical phase to aqueous, ionic, and mineral phases in the post-injection period. (2) For an aquifer with complex sandstone-shale sequences, the best injection strategy should be decided by the results of risk evaluation and the SFI estimation. (3) The well location affected the injection strategy. The risk of CO2 leakage was lower using a down-dip injection well than an up-dip well. (4) The best strategy for this case study was to use the down-dip well to inject CO2 into the bottom sandstone layer. The SFI for this scenario reached 0.99 at the storage time of 1000 years, which meant that the probability of CO2 leakage occurring was nearly zero

    The role of thrombomodulin lectin-like domain in inflammation

    Thrombomodulin (TM) is a cell surface glycoprotein which is widely expressed in a variety of cell types. It is a cofactor for thrombin binding that mediates protein C activation and inhibits thrombin activity. In addition to its anticoagulant activity, recent evidence has revealed that TM, especially its lectin-like domain, has potent anti-inflammatory function through a variety of molecular mechanisms. The lectin-like domain of TM plays an important role in suppressing inflammation independent of the TM anticoagulant activity. This article makes an extensive review of the role of TM in inflammation. The molecular targets of TM lectin-like domain have also been elucidated. Recombinant TM protein, especially the TM lectin-like domain may play a promising role in the management of sepsis, glomerulonephritis and arthritis. These data demonstrated the potential therapeutic role of TM in the treatment of inflammatory diseases

    Concurrent Use in Taiwan of Chinese Herbal Medicine Therapies among Hormone Users Aged 55 Years to 79 Years and Its Association with Breast Cancer Risk: A Population-Based Study

    Background. The purpose of the present study was to analyze the concurrent use of Chinese herbal products (CHPs) among women aged 55 to 79 years who had also been prescribed hormonal therapies (HT) and its association with breast cancer risk. Methods. The use, frequency of service, and CHP prescribed among 17,583 HT users were evaluated from a random sample of 1 million beneficiaries from the National Health Insurance Research Database. A logistic regression method was used to identify the factors that were associated with the coprescription of a CHP and HT. Cox proportional hazards regressions were performed to calculate the hazard ratios (HRs) of breast cancer between the TCM nonusers and women who had undergone coadministration of HT and a CHP or CHPs. Results. More than one out of every five study subjects used a CHP concurrently with HT (CHTCHP patients). Shu-Jing-Huo-Xie-Tang was the most commonly used CHP coadministered with HT. In comparison to HT-alone users, the HRs for invasive breast cancer among CHTCHP patients were not significantly increased either in E-alone group or in mixed regimen group. Conclusions. The coadministration of hormone regimen and CHPs did not increase the risk of breast cancer

    Critical Success Factors of Alignment between Websites and OCAs in the Manufacturing Industry

    In this study, a list containing twelve organizational critical activities (OCAs) of manufacturing industry is found out by the interviews with executives and revision of two experts. The analysis on secondary data and the deeper interviews are carried out to obtain the critical success factors of the alignment between websites and OCAs in the manufacturing industry. Six common critical success factors of the three companies are: Support of Executives, Excellent and Professional Team, Full Communication, Harmonic Website Build Process, Technical Support, and Complete Information System Architecture. Finally, this study hopes that companies in the manufacturing industry can improve the competitiveness according to the result of this study
