    We present a case of odontogenic maxillary sinusitis with marked perisinuous bone resorption. A 29-year-old man visited to our hospital because of pain in buccal region on left side after extraction of teeth at a dental practitioner. Orthopantomography and CT were performed and revealed inflammatory thickening of maxillary sinus mucosa and marked perisinuous bone resorption. In the blood examination, the values of WBCs and CRP were elevated abnormally. Moreover, bacterial examination was performed using the exudate from maxillary sinus through a fistula after extraction of teeth; α-Streptococcus, Prevotella buccae, Prevotella intermedia, Prevotella melaninogenica and Acinetobacter baumannii were detected. First, medication of antibiotics and irrigation of maxillary sinus was worked out under a clinical diagnosis of odontogenic maxillary sinusitis. These therapies didn't change perisinuous bone resorption for the better. Sequestrum and mucosa of maxillary sinus were biopsied to examine thoroughly; diagnosis of odontogenic maxillary sinusitis was confirmed histopathologically. Moreover, biochemical examination of blood showed normal serum level of specific markers for bone metabolism or Aspergilli. The similar therapies were continued from then and perisinuous bone resorption stopped suddenly for unknown reasons 3 months after these therapies. Time course of perisinuous bone regeneration was found by CT. Finally, operation to close the antrooral fistula with palatal flap was performed. There has been no relapse of inflammation for more than 5 years

    Filtration properties of nanofiber/microfiber mixed filter and prediction of its performance

    There is an increasing demand of air filters with a high collection performance, i.e., high collection efficiency and low pressure drop, for the application to indoor air cleaning. Air filters consisting of nanofibers have attracted great interests since they may have a low pressure drop because of slip flow effect and high collection efficiency due to interception. Although various nanofiber filters are available on the market, their collection performance is not as high as expected by the conventional filtration theory because non-uniform packing of fibers plays a significant role. In the present work, the collection performance of nanofiber (780 nm) and microfiber (11.2 µm) mixed filters with various mixing fractions was studied in order to maximize the quality factor of filter, qF, as a function of mixing fraction of nanofibers. The collection performance of mixed fiber filters was predicted by using theoretical equations reported by Bao et al. (1998) for bimodal distribution of fibers. As a result, it was found that the mixed fiber filters had a uniform fiber packing compared to laminated filters and that the collection efficiency was well predicted by introducing the inhomogeneity factor calculated for the filter consisting of two distinct fiber sizes. Furthermore, we found that the mixed fiber filter with the nanofiber mixing fraction of 5% in mass had the highest quality factor. © Taiwan Association for Aerosol Research

    Changes in Image Examinations at the Department of Oral Radiology : The Number of Intraoral and Panoramic Radiographs

    After the computed radiography (CR) system was developed in 1986, it became possible to acquire, view and save digital images for medical diagnosis. Around that time, panoramic radiography was digitized by the CR system, but it was limited to large hospitals. From 1994, it has been digitized at large hospitals and dental clinics. We examined the changes in the number of intraoral and panoramic radiographs in dentistry, and based on the relationships among the numbers of radiographs, patients and dental operations, we discussed the clinical significance of digitization. We extracted intraoral and panoramic radiography data, the number of dental operations and the number of patients from the database of Tokushima University Hospital from 2002 to 2017. Then, we extracted nationwide data from Statistics of Medical Care Activities in Public Health by the Ministry of Health, Labour and Welfare from 2002 to 2017. As a result, although the numbers of patients and dental operations at Tokushima University Hospital have remained constant, the number of intraoral radiographs has decreased (72%), whereas the number of panoramic radiographs has increased annually since 2011 (131%). In addition, although the number of patients and nationwide insurance points increased in a different manner than that at Tokushima University Hospital, a decrease in the number of intraoral radiographs and an increase in the number of panoramic radiographs were noted across Japan. As panoramic imaging has been improved and adapted to different settings, panoramic radiography has been used instead of intraoral radiography, and it has been widely used in screening applications such as for perioperative patients. Although digitization is progressing, the analog images are still used (intraoral radiography analog rate, 29.7%, panoramic radiography analog rate, 12.4% in 2017)

    Normative consciousness of young children can be observed on almost any occasion during everyday activities and play, especially in "game play" where there are clearly defined rules and activities, and outcomes are determined by interactions during the game. Young children sometimes exhibit unfair behavior or behavior deviating from the rules when they are in danger of experiencing a negative outcome in the game. Among children, when such behavior is pointed out by their peers, the word "cheating" is used in most situations. In the present research, a survey among preschool teachers employed in kindergartens and nursery schools was conducted to reveal if the behavior of young children judged by the authors to be "cheating" was also recognized as "cheating" by the preschool teachers. Further, the authors discuss whether the preschool teachers\u27 judgment of what constituted "cheating" was associated with their consciousness of nurturing young children\u27s normative consciousness. The research revealed that preschool teachers who were predisposed to be affirmative in order to nurture children\u27s normative consciousness are more likely to judge behaviors the authors judged as cheating to be cheating

    Identification and structural analysis of C-terminally truncated collapsin response mediator protein-2 in a murine model of prion diseases

    <p>Abstract</p> <p>Background</p> <p>Prion diseases are fatal neurodegenerative disorders that accompany an accumulation of the disease-associated form(s) of prion protein (PrP<sup>Sc</sup>) in the central nervous system. The neuropathological changes in the brain begin with focal deposits of PrP<sup>Sc</sup>, followed by pathomorphological abnormalities of axon terminal degeneration, synaptic loss, atrophy of dendritic trees, and eventual neuronal cell death in the lesions. However, the underlying molecular basis for these neuropathogenic abnormalities is not fully understood.</p> <p>Results</p> <p>In a proteomic analysis of soluble proteins in the brains of mice challenged intracerebrally with scrapie prion (Obihiro I strain), we found that the amount of the full-length form of collapsin response mediator protein-2 (CRMP-2; 61 kDa) decreased in the late stages of the disease, while the amount of its truncated form (56 kDa) increased to comparable levels observed for the full-length form. Detailed analysis by liquid chromatography-electrospray ionization-tandem mass spectrometry showed that the 56-kDa form (named CRMP-2-ΔC) lacked the sequence from serine<sup>518 </sup>to the C-terminus, including the C-terminal phosphorylation sites important for the regulation of axonal growth and axon-dendrite specification in developing neurons. The invariable size of the mRNA transcript in Northern blot analysis suggested that the truncation was due to post-translational proteolysis. By overexpression of CRMP-2-ΔC in primary cultured neurons, we observed the augmentation of the development of neurite branch tips to the same levels as for CRMP-2<sup>T514A/T555A</sup>, a non-phosphorylated mimic of the full-length protein. This suggests that the increased level of CRMP-2-ΔC in the brain modulates the integrity of neurons, and may be involved in the pathogenesis of the neuronal abnormalities observed in the late stages of the disease.</p> <p>Conclusions</p> <p>We identified the presence of CRMP-2-ΔC in the brain of a murine model of prion disease. Of note, C-terminal truncations of CRMP-2 have been recently observed in models for neurodegenerative disorders such as ischemia, traumatic brain injury, and Wallerian degeneration. While the structural identity of CRMP-2-ΔC in those models remains unknown, the present study should provide clues to the molecular pathology of degenerating neurons in prion diseases in connection with other neurodegenerative disorders.</p

    Optimal Control Designs for Systems with Input Saturations and Rate Limiters

    Most systems in practice have some input saturations due to physical restrictions. But taking input saturations into account directly is not easy in ordinary control design methods. In this report, we show that the method recently developed by one of the authors is useful to approximate the exact solutions of Hamilton-Jacobi equations for systems including input saturations. First, we propose a method to design nonlinear optimal controllers for systems with input saturations by solving Hamilton-Jacobi equations. Next, it will be extend to a design method of nonlinear optimal controllers for systems with rate limited actuators. Two numerical examples are illustrated for the input saturation and input rate limited problems
