Tokushima City Medical Association has conducted memory screening by“primary caredoctors”since fiscal year 2004 to detect dementia in an early stage and to take its appropriatemedical care. The Association would like to report 4-year result of its activities till fiscal 2007.30,250 people went through primary medical examination in four years by an interview sheetconsisting of 10 questionnaire items. Among them, 8,009 people needed further detailed examination.4,271 people requiring detailed examination took secondary medical examination by MMSE.1,014 people were suspected of abnormality, and 681 people out of them were diagnosed as dementia.There were 20 juvenile dementia patients who were less than 65 years of age. These resultsshow the significance and importance of memory screening by“primary care doctors”who seepatients and their family in their community
Prevalence of Amiodarone-Induced Thyrotoxicosis and Associated Risk Factors in Japanese Patients
Amiodarone is a widely used agent for life-threatening arrhythmias. Although amiodarone-induced thyrotoxicosis (AIT) is a major adverse effect that can cause recurrence of arrhythmias and exacerbation of heart failure, risk factors for AIT among amiodarone-treated Japanese patients have not been elucidated. Here, we investigated the prevalence and predictive factors for AIT. The study subjects were 225 patients treated with amiodarone between 2008 and 2012, who were euthyroid before amiodarone therapy. All patients with AIT were diagnosed by measurement of thyroid hormones and ultrasonography. Among the 225 subjects, 13 patients (5.8%) developed AIT and all the patients were classified as Type 2 AIT. Baseline features of patients with AIT were not different from those who did not develop AIT, except for age (AIT, 55.1 ± 13.8, non-AIT, 68.1 ± 12.0 years, P<0.001). Multivariate analyses using the Cox proportional hazard model identified age as the sole determinant of AIT (hazard ratio: 0.927, 95% confidence interval: 0.891–0.964). Receiver operating characteristic curve analysis identified age of 63.5 years as the cutoff value for AIT with sensitivity of 70.3% and specificity of 69.2%. In summary, this study showed that the prevalence of AIT is 5.8% in Japanese patients treated with amiodarone and that young age is a risk factor for AIT
Insights into Candida tropicalis nosocomial infections and virulence factors
Candida tropicalis is considered the first or the second non-Candida albicans Candida (NCAC) species most frequently isolated from candidosis, mainly in patients admitted in intensive care units (ICUs), especially with cancer, requiring prolonged catheterization, or receiving broad-spectrum antibiotics. The proportion of candiduria and candidemia caused by C. tropicalis varies widely with geographical area and patient group. Actually, in certain countries, C. tropicalis is more prevalent, even compared with C. albicans or other NCAC species. Although prophylactic treatments with fluconazole cause a decrease in the frequency of candidosis caused by C. tropicalis, it is increasingly showing a moderate level of fluconazole resistance. The propensity of C. tropicalis for dissemination and the high mortality associated with its infections might be strongly related to the potential of virulence factors exhibited by this species, such as adhesion to different host surfaces, biofilm formation, infection and dissemination, and enzymes secretion. Single-crystalline rutile TiO2 hollow spheres: Room-temperature synthesis, tailored visible-light-extinction, and effective scattering layer for quantum dot-sensitized solar cells
A general synthesis of inorganic single-crystalline hollow spheres has been achieved through a mechanism analogous to the Kirkendall effect, based on a simple one-step laser process performed at room temperature. Taking TiO(2) as an example, we describe the laser process by investigating the influence of experimental parameters, for example, laser wavelength, laser fluence/irradiation time, liquid medium, and concentration of starting materials, on the formation of hollow spheres. It was found that the size-tailored TiO(2) hollow spheres demonstrate tunable light scattering over a wide visible-light range. Inspired by the effect of light scattering, we introduced the TiO(2) hollow sphere's scattering layer in quantum dot-sensitized solar cells and achieved a current notable 10% improvement of solar-to-electric conversion efficiency, indicating that TiO(2) hollow spheres are potential candidates in optical and optoelectronic devices
Effects of ABCB1 gene polymorphisms on autonomic nervous system activity during atypical antipsychotic treatment in schizophrenia
Abstract Background There are interindividual differences in the adverse effects of atypical antipsychotics, which include autonomic nervous system (ANS) dysfunction. Accordingly, to clarify the interindividual differences in the adverse effects of specific atypical antipsychotics in schizophrenia, we investigated the association between ANS dysfunction and ATP-binding cassette transport sub-family B member 1 (ABCB1) gene polymorphisms in patients with schizophrenia. Methods In total, 233 Japanese patients with schizophrenia participated in this study. All of the participants received an atypical antipsychotic as monotherapy: 89 participants received risperidone, 69 olanzapine, 48 aripiprazole, and 27 quetiapine. ANS activity was assessed by means of a power spectral analysis of heart rate variability. Four single nucleotide polymorphisms (SNPs) in ABCB1 (rs1045642, rs1128503, rs2032582, and rs2235048) were genotyped using the TaqMan method. Results For aripiprazole, sympathetic and total autonomic nervous activities were significantly lower in the rs1045642 T allele carrier–rs2235048 C allele carrier group than in the rs1045642 non-T allele carrier–rs2235048 non-C allele carrier group. In addition, in the aripiprazole group, the T-C-T-A haplotype (rs1045642-rs2235048-rs1128503-rs2032582) was associated with decreased ANS activity. However, there were no significant associations between ANS activity and ABCB1 gene polymorphisms in the risperidone, olanzapine, and quetiapine groups. Multiple regression analysis revealed that sympathetic and total nervous activities were significantly associated with the ABCB1 rs1045642–rs2235048 genotype and the T-C-T-A haplotype (rs1045642-rs2235048-rs1128503-rs2032582). Conclusion We suggest that ABCB1 genetic polymorphisms affect aripiprazole-related ANS dysfunction but do not affect risperidone-, olanzapine-, or quetiapine-related ANS dysfunction
