    A novel modified hanging maneuver in laparoscopic left hemihepatectomy

    Introduction The liver hanging maneuver is an essential technique for controlling bleeding in hepatectomy, however it is often difficult in laparoscopic major hepatectomy. The present study describes a novel modified hanging maneuver in laparoscopic left hemihepatectomy. Presentation of case A 29-year-old female underwent laparoscopic left hemihepatectomy for mucinous cystic neoplasm. After mobilizing the left lobe, the liver parenchyma was dissected along the demarcation line. For the hanging technique, the upper edge of the hanging tape was placed on the lateral side of the left hepatic vein, and fixed with the Falciform ligament. The lower edge of the tape was extracted outside the abdomen. Accordingly the hanging tape can be controlled extraperitoneally during the liver parenchyma dissection. Discussion This technique includes several advantages including no need of assistance using forceps, easy control of the hanging tape extraperitoneally, outflow control, better exposure of surgical field, and helpful guide of the liver dissection line toward the root of the left hepatic vein. Conclusion Our novel modified hanging maneuver is easy and reproducible to use in laparoscopic left hemihepatectomy. Moreover, this technique can be applied to other laparoscopic hepatectomy

    Favorable control of hepatocellular carcinoma with peritoneal dissemination by surgical resection using indocyanine green fluorescence imaging: a case report and review of the literature

    Background The optimal management for peritoneal dissemination in patients with hepatocellular carcinoma remains unclear. Although several reports have described the usefulness of surgical resection, the indications should be carefully considered. Herein, we report the case of a patient with hepatocellular carcinoma with peritoneal recurrence who underwent surgical resection using an indocyanine green fluorescence navigation system and achieved favorable disease control. Case presentation A 45-year-old Asian woman underwent left hemihepatectomy for a ruptured hepatocellular carcinoma. Seventeen months after the initial surgery, a single nodule near the cut surface of the liver was detected on computed tomography, along with elevation of tumor markers. The patient was diagnosed with peritoneal metastasis and underwent a surgical resection. Twelve months later, a single nodule on the dorsal side of the right hepatic lobe was detected on computed tomography, and we performed surgical resection. Indocyanine green (0.5 mg/kg) was intravenously administered 3 days before surgery, and the indocyanine green fluorescence imaging system revealed clear green fluorescence in the tumor, which helped us perform complete resection. Indocyanine green fluorescence enabled the detection of additional lesions that could not be identified by preoperative imaging, especially in the second metastasectomy. There was no further recurrence at 3 months postoperatively. Conclusion When considering surgical intervention for peritoneal recurrence in patients with hepatocellular carcinoma, complete resection is mandatory. Given that disseminated nodules are sometimes too small to be detected by preoperative imaging studies, intraoperative indocyanine green fluorescence may be an essential tool for determining the indications for surgical resection

    Isolation and Phenotypic Characterization of Lotus japonicus Mutants Specifically Defective in Arbuscular Mycorrhizal Formation

    Several symbiotic mutants of legume plants defective in nodulation have also been shown to be mutants related to arbuscular mycorrhizal (AM) symbiosis. The origin of the AM symbiosis can be traced back to the early land plants. It has therefore been postulated that the older system of AM symbiosis was partially incorporated into the newer system of legume-rhizobium symbiosis. To unravel the genetic basis of the establishment of AM symbiosis, we screened about 34,000 plants derived from ethyl methanesulfonate (EMS)-mutagenized Lotus japonicus seeds by microscopic observation. As a result, three lines (ME778, ME966 and ME2329) were isolated as AM-specific mutants that exhibit clear AM-defective phenotypes but form normal effective root nodules with rhizobial infection. In the ME2329 mutant, AM fungi spread their hyphae into the intercellular space of the cortex and formed trunk hyphae in the cortical cells, but the development of fine branches in the arbuscules was arrested. The ME2329 mutant carried a nonsense mutation in the STR-homolog gene, implying that the line may be an str mutant in L. japonicus. On the ME778 and ME966 mutant roots, the entry of AM fungal hyphae was blocked between two adjacent epidermal cells. Occasionally, hyphal colonization accompanied by arbuscules was observed in the two mutants. The genes responsible for the ME778 and ME966 mutants were independently located on chromosome 2.     Surgical resection of mixed neuroendocrine-non-neuroendocrine neoplasm in the biliary system: a report of two cases

    Background Mixed neuroendocrine-non-neuroendocrine neoplasm (MINEN) is a rare disease and there is scarce literature on its diagnosis, treatment, and prognosis. We encountered two unusual cases of MINEN in the biliary tract, one in the ampulla of Vater and the other in the distal bile duct. In this report, we describe the clinical course of these two cases in detail. Case presentation Case 1: A 69-year-old woman presented with a chief complaint of epigastric pain. When endoscopic sphincterotomy and retrograde biliary drainage were performed for gallstone pancreatitis, an ulcerated lesion was found in the ampulla of the Vater. Based on the biopsy results, the lesion was diagnosed as the ampulla of Vater carcinoma and subtotal stomach-preserving pancreatoduodenectomy (SSPPD) was performed. Postoperative histopathological examination revealed the coexistence of adenocarcinoma and neuroendocrine carcinoma components, consistent with the diagnosis of MINEN. In addition, lymph node metastasis was found on the dorsal side of the pancreas and the metastatic component was adenocarcinoma. Adjuvant chemotherapy with etoposide and cisplatin was administered for 6 months, and presently the patient is alive without recurrence 64 months after surgery. Case 2: A 79-year-old man presented with a chief complaint of anorexia. Cholangiography showed severe stenosis of the distal bile duct. A biopsy was conducted from the stenotic lesion and it revealed the lesion to be adenocarcinoma. A diagnosis of distal bile duct carcinoma was made, and SSPPD was performed. Histopathological examination revealed the coexistence of adenocarcinoma and neuroendocrine carcinoma components, and the tumor was confirmed as MINEN of the distal bile duct. No adjuvant chemotherapy was administered due to the poor performance status. 7 months later, the patient was found to have a liver metastasis. Conclusion We experienced two valuable cases of biliary MINEN. To identify better treatments, it is important to consider the diversity of individual cases and to continue sharing a variety of cases with different presentations

    Diagnostic value of computed high b-value whole-body diffusion-weighted imaging for primary prostate cancer

    Purpose: To investigate the utility of post-acquisition computed diffusion-weighted imaging (cDWI) for primary prostate cancer (PCa) evaluation in biparametric whole-body MRI (bpWB-MRI). Methods: Patients who underwent pelvic MRI for PCa screening and subsequent bpWB-MRI for staging were included. Two radiologists assessed the diagnostic performance of the following datasets for clinically significant PCa diagnosis (grade group >= 2 according to the Prostate Imaging-Reporting and Data System, version 2.1): bpMRI(2000) (axial DWI scans with a b-value of 2,000 s/mm(2) + axial T2WI scans from pre-biopsy pelvic MRI), computed bpWB-MRI2000 (computed WB-DWI scans with a b-value of 2,000 s/mm(2) + axial WB-T2WI scans), and native bpWB-MRI1000 (native axial WB-DWI scans with a b-value of 1,000 s/mm(2) + axial WB-T2WI scans). Systemic biopsy was used as reference standard. Results: Fifty-one patients with PCa were included. The areas under the curve (AUCs) of bpMRI(2000) (0.89 for reader 1 and 0.86 for reader 2) and computed bpWB-MRI2000 (0.86 for reader 1 and 0.83 for reader 2) were significantly higher (p < 0.001) than those of native bpWB-MRI1000 (0.67 for both readers). No significant difference was observed between the AUCs of bpMRI(2000) and computed bpWB-MRI2000 (p = 0.10 for reader 1 and p = 0.25 for reader 2). Conclusions: The diagnostic performance of computed bpWB-MRI2000 was similar to that of dedicated pelvic bpMRI(2000) for primary PCa evaluation. cDWI can be recommended for implementation in standard WB-MRI protocols to facilitate a one-step evaluation for concurrent detection of primary and metastatic PCa

    Spin frustration in antiperovskite systems: (TTF˙+or TSF˙+)3[(Mo6X14)2−Y−]

    Two novel antiperovskite charge-transfer (CT) solids composed of a tetraselenafulvalene radical cation (TSF˙+), a dianionic molybdenum cluster unit [Mo6X14]2−, and a halogen anion (Y−) (X, Y = Cl, Br) were prepared by electrocrystallization. Their crystal structures and magnetic properties with regard to spin frustration are discussed together with those of isostructural tetrathiafulvalene (TTF) CT solids previously reported. Both TSF and TTF salts have an apex sharing distorted octahedral spin lattice with a rhombohedral R[3 with combining macron] space group. The calculated overlap integrals based on the crystal structures and insulating nature of the TSF salts indicate that they are Mott insulators. Their spin susceptibilities obeyed the Curie–Weiss law and exhibited an antiferromagnetic ordering at lower temperatures for the TSF salts (Néel temperature, TN = 3.0 K for X = Y = Cl and 5.5 K for X = Y = Br) than the TTF salts. The Curie–Weiss temperatures (|ΘCW| ∼ 1.6–6.3 K) for the TSF salts are lower than those of the TTF salts. For the TSF salts, spin-flop behavior was detected at 3.2 T for X = Y = Cl and 1.5 T for X = Y = Br at 1.9 K. Due to both the distortion of the octahedral geometry of the spin lattice and the anisotropic molecular orientation, the geometrical spin frustrations in TSF and TTF systems are weakened

    Stellar Populations of Lyman-alpha Emitters at z=4.86: A Comparison to z5z\sim5 LBGs

    (abridged) We present a study of stellar population of LAEs at z=4.86 in GOODS-N and its flanking field. With the publicly available IRAC data in GOODS-N and further IRAC observations in the flanking fields, we select five LAEs which are not contaminated by neighboring objects in IRAC images and construct their observed SEDs with I_c, z', IRAC 3.6micron, and 4.5micron band photometry. The SEDs cover the rest-frame UV to optical wavelengths. We derive stellar masses, ages, color excesses, and star formation rates of five LAEs using SED fitting method. Assuming the constant star formation history, we find that the stellar masses range from 10^8 to $10^{10} Msun with the median value of 2.5x10^9 Msun. The derived ages range from very young ages (7.4 Myr) to 437 Myr with a median age of 25 Myr. The color excess E(B-V) are between 0.1-0.4 mag. Star formation rates are 55-209 Msun/yr. A comparison of the stellar populations is made between three LAEs and 88 LBGs selected at the same redshift, in the same observed field, and down to the same limit of the rest-frame UV luminosity. These three LAEs are the brightest and reddest samples among the whole LAE samples at z=4.86. The LAEs distribute at the relatively faint part of UV-luminosity distribution of LBGs. Deriving the stellar properties of the LBGs by fitting their SEDs with the same model ensures that model difference does not affect the comparison. It is found that the stellar properties of the LAEs lie on distributions of those of LBGs. On average, the LAEs show less dust extinction, and lower star formation rates than LBGs, while the stellar mass of LAEs nearly lies in the middle part of the mass distribution of LBGs. However, the stellar properties of LAEs and LBGs are similar at the fixed UV or optical luminosity.     Diagnostic Value of the Vesical Imaging-Reporting and Data System in Bladder Urothelial Carcinoma with Variant Histology

    The value of the Vesicle Imaging-Reporting and Data System (VI-RADS) in the diagnosis of muscle-invasive bladder cancer (MIBC) for urothelial carcinoma with variant histology (VUC) remains unknown. We retrospectively evaluated 360 consecutive patients with bladder cancer (255 pure urothelial carcinoma [PUC] and 69 VUC) who underwent multiparametric magnetic resonance imaging between 2011 and 2019. VI-RADS scores assigned by four readers were significantly higher for the VUC group than for the PUC group (p &lt; 0.05). In the cohort of 122 pair-matched patients, there was no significant difference in VI-RADS score distribution between the PUC and VUC groups for all readers (p &gt; 0.05). The area under the receiver operating characteristic curve for MIBC diagnosis via overall VI-RADS score was 0.93-0.94 for PUC and 0.89-0.92 for VUC, with no significant difference between the PUC and VUC groups (p = 0.32-0.60). These data suggests that VI-RADS scores achieved high diagnostic performance for detection of muscle invasion in both PUC and VUC. PATIENT SUMMARY: The Vesical Imaging-Reporting and Data System (VI-RADS) is a standardized system for reporting on detection of muscle-invasive bladder cancer via magnetic resonance imaging (MRI) scans. Our study shows that VI-RADS is also highly accurate for diagnosis for different variants of muscle-invasive bladder cancer, with good inter-reader agreement.</p
