    NF-κB Hyper-Activation by HTLV-1 Tax Induces Cellular Senescence, but Can Be Alleviated by the Viral Anti-Sense Protein HBZ

    Activation of I-κB kinases (IKKs) and NF-κB by the human T lymphotropic virus type 1 (HTLV-1) trans-activator/oncoprotein, Tax, is thought to promote cell proliferation and transformation. Paradoxically, expression of Tax in most cells leads to drastic up-regulation of cyclin-dependent kinase inhibitors, p21CIP1/WAF1 and p27KIP1, which cause p53-/pRb-independent cellular senescence. Here we demonstrate that p21CIP1/WAF1-/p27KIP1-mediated senescence constitutes a checkpoint against IKK/NF-κB hyper-activation. Senescence induced by Tax in HeLa cells is attenuated by mutations in Tax that reduce IKK/NF-κB activation and prevented by blocking NF-κB using a degradation-resistant mutant of I-κBα despite constitutive IKK activation. Small hairpin RNA-mediated knockdown indicates that RelA induces this senescence program by acting upstream of the anaphase promoting complex and RelB to stabilize p27KIP1 protein and p21CIP1/WAF1 mRNA respectively. Finally, we show that down-regulation of NF-κB by the HTLV-1 anti-sense protein, HBZ, delay or prevent the onset of Tax-induced senescence. We propose that the balance between Tax and HBZ expression determines the outcome of HTLV-1 infection. Robust HTLV-1 replication and elevated Tax expression drive IKK/NF-κB hyper-activation and trigger senescence. HBZ, however, modulates Tax-mediated viral replication and NF-κB activation, thus allowing HTLV-1-infected cells to proliferate, persist, and evolve. Finally, inactivation of the senescence checkpoint can facilitate persistent NF-κB activation and leukemogenesis

    A circumplex model of affect and its relation to personality : a five-language study

    Are there aspects of affect that can be generalized across different languages? Are there consistent patterns of associations between self-reported affect and personality across groups speaking different languages? In the present dissertation, I explore these two questions in five different language samples. Studies of current self-reported affect in English suggest that Russell's (1980), Thayer's (1989), Larsen and Diener's (1992), and Watson and Tellegen's (1985) models of affect variables can be integrated and summarized by a two-dimensional space defined by Pleasant vs Unpleasant and Activated vs Deactivated axes. To assess the cross-language generalizability of this integrated structure, data on translations of the English affect scales (N for Spanish = 233, N for Chinese = 487, N for Japanese = 450, N for Korean = 365) were compared with the structure in English ON = 535). Systematic and random errors were controlled through multi-format measurements (Green, Goldman, & Salovey, 1993) and structural equation modeling. Individual measurement models as defined in English received support in all five languages, although revisions of these scales in non-English samples provided an even closer approximation to the two-dimensional structure in English. In all five languages, the two dimensions explained most, but not all, of the reliable variance in other affect variables (mean = 88%). The four structural models fit comfortably within the integrated two-dimensional space. In fact, the variables fell at different angles on the integrated space, suggesting a new circumplex structure. In prior studies conducted in English, the personality traits of Neuroticism and Extraversion were most predictive of affect and they aligned with the Pleasant Activated and Unpleasant Activated states. To clarify and extend the previous findings, participants in all five samples also completed NEO FFI (Costa & McCrae, 1992), a measure for the Five Factor Model of personality (FFM). Again, Neuroticism and Extraversion were most predictive of affect, accounting for, on average, 10% of the variance. The remaining three factors of the FFM contributed, on average, 2%. In all five languages, the FFM dimensions did not align with the two predicted affective dimensions.     Change amid stability: rethinking the planning and design guidelines for 'area-wide' conservation in HongKong

    This dissertation seeks to solve a long-standing conundrum for heritage conservation in Hong Kong, regarding the ‘area-wide’ mistreatment of our historic places and their settings. While Hong Kong has the legal framework and policies in place, it has been let down by an outdated and incomprehensive set of ‘area-wide’ conservation guidance in the Hong Kong Planning Standards and Guidelines (‘HKPSG’) – an important manual that has received scant attention in the conservation field. Through a review of international charters, this dissertation will offer the first extensive critique on the ‘area-wide’ conservation provisions of this infamous document. Drawing lessons from international best practices, this dissertation will go further to recommend possible remedies for the Hong Kong context.     Ethnic-wise population forecast for Singapore using Lee Carter model for mortality, fertility and migration.

    In this study, we investigate Singapore population using the variant of Lee-Carter model proposed by Hyndman and Ullah. The mortality rates, fertility rates and net migration numbers which form the basis of the population are analysed. Coherent functional forecasting method is adopted to restrict the divergence of forecast mortality rates between male and female. The model is applied on Singapore demographics data from 1982 to 2008 to forecast the population from 2009 to 2029. The accuracy of the estimates is assessed by comparing the forecast result against actual data from year 2001 to 2009. Next, the forecast demographic components (mortality, fertility and net migration) between Chinese, Malay and Indian are compared and discussed. Possible evolution in ethnic population trend within Singapore is then justified. The results show that there is an upward trend in Singapore population between years 2009 to 2029. Ethnic-wise forecasts show that Indian population will outnumber Malay population by 2021. There is also increasing proportion of elderly population within the forecast periods. Finally, limitations of the model are discussed and concluding remarks are made at the end of the report with recommendations on possible future research.BUSINES

    Urine Metabolome Dynamics Discriminate Influenza Vaccination Response

    Influenza represents a major and ongoing public health hazard. Current collaborative efforts are aimed toward creating a universal flu vaccine with the goals of both improving responses to vaccination and increasing the breadth of protection against multiple strains and clades from a single vaccine. As an intermediate step toward these goals, the current work is focused on evaluating the systemic host response to vaccination in both normal and high-risk populations, such as the obese and geriatric populations, which have been linked to poor responses to vaccination. We therefore employed a metabolomics approach using a time-course (n = 5 time points) of the response to human vaccination against influenza from the time before vaccination (pre) to 90 days following vaccination. We analyzed the urinary profiles of a cohort of subjects (n = 179) designed to evenly sample across age, sex, BMI, and other demographic factors, stratifying their responses to vaccination as “High”, “Low”, or “None” based on the seroconversion measured by hemagglutination inhibition assay (HAI) from plasma samples at day 28 post-vaccination. Overall, we putatively identified 15,903 distinct, named, small-molecule structures (4473 at 10% FDR) among the 895 samples analyzed, with the aim of identifying metabolite correlates of the vaccine response, as well as prognostic and diagnostic markers from the periods before and after vaccination, respectively. Notably, we found that the metabolic profiles could unbiasedly separate the high-risk High-responders from the high-risk None-responders (obese/geriatric) within 3 days post-vaccination. The purine metabolites Guanine and Hypoxanthine were negatively associated with high seroconversion (p = 0.0032, p < 0.0001, respectively), while Acetyl-Leucine and 5-Aminovaleric acid were positively associated. Further changes in Cystine, Glutamic acid, Kynurenine and other metabolites implicated early oxidative stress (3 days) after vaccination as a hallmark of the High-responders. Ongoing efforts are aimed toward validating these putative markers using a ferret model of influenza infection, as well as an independent cohort of human seasonal vaccination and human challenge studies with live virus

    Recent and comprehensive research of gout in the Pacific region and Hawai'i is significantly lacking. This study was conducted to improve the understanding of the healthcare utilization of gout patients within a single health care system in Hawai'i. The objective was to examine gout inpatient, outpatient and emergency department care within a single health care system in Hawai'i. This study was a retrospective chart review of patients, ≥ 18 years admitted to three Hawai'i Pacific Health facilities for a primary diagnosis of gout or rheumatoid arthritis (RA) from 2011 to 2017. Population data for the State of Hawai'i was used to calculate visit rates per 1,000 Hawai'i adults. Trend analysis was performed to compare changes over time. We studied gout health care utilization concurrently with RA to provide an internal comparison group for the healthcare utilization patterns of interest. Gout patients were primarily managed in the outpatient setting with high rates of emergency department visits. In contrast, RA patients were primarily managed in the outpatient setting, with low rates of emergency department visits. Both gout and RA patients had low rates of inpatient admissions. The cost of gout emergency department visits was approximately 3.4 times higher than gout outpatient visits. The rates for gout emergency department visits, outpatient visits, inpatients visits, and RA outpatient visits in 2017 were trending downward and significantly changed from 2011 (p <0.05). The rates for RA emergency department visits and inpatient visits were not significantly changed from 2011-2017. Gout care in Hawai'i remains suboptimal with higher rates of emergency department visits, as compared to RA. Because emergency department visits are associated with higher cost, efforts should be made to reduce these emergency department visits to improve the quality of care

    Clinicopathological significance of synchronous carcinoma in colorectal cancer

    Background: \ud Synchronous colorectal carcinoma has seldom been studied in large series. The study was designed to examine the significance of colorectal synchronous carcinoma in a large cohort of patients.\ud \ud Methods: \ud The clinicopathological features of 102 patients with synchronous colorectal carcinoma were compared with 1,793 patients with solitary colorectal carcinoma.\ud \ud Results: \ud The prevalence of synchronous colorectal carcinoma was 3.6%. In these patients, 4% had FAP, 6% had hyperplastic polyposis, and 2% had ulcerative colitis. The index carcinoma was more likely to have higher histological grade and T stage than other carcinoma(s) in the same patient. When compared with solitary colorectal carcinoma, synchronous colorectal carcinoma was more often noted in males with coexisting FAP and in proximal location. The 5-year survival rate of patients with synchronous colorectal carcinoma was 53% and was similar to those with solitary colorectal carcinoma.\ud \ud Conclusions: We examined the clinicopathological features of patients with synchronous colorectal carcinomas in a large cohort of patients.
