Change Management: A Theoretical Framework
Organizations are dynamic and open systems that exist in mutual affection with their environment and deliver output to environment from which they receive inputs. Organizations need to make organizational regulations against internal and external occurring changes. Therefore, organizations have to gain new features and abilities in order to get along with the change. Today, changes we face in all areas affect also health system. The increase of the society’s health level is attributed to well-mannered health policy, and to give good preventive and remedial health services. Health executive have important roles in implementing these services, as well. Thus, in this article study, evaluations depending on healthcare organizations are done as focusing on the changes and the change management process in organizations. The importance of change management in health organizations and the factors that were required for its efficiency are mentioned. The success of health organizations can be attributed to their ability to adapt the changes occurred their around depending on the conceptual studies and obtained study findings. In order to have an effective change management in health organizations, firstly it needed to understand the importance of change and its necessity, to train the employees who work for the organization, to include them to change preparation and implementation processes and to give support to them about the change. In this article, suggestions are presented for health organizations as discussing in question topic, change management implementations.Keywords. Örgütsel Bağlılık ile Cinsiyet Arasındaki İlişki: Hastanelerde Çalışan Sağlık Personeli Üzerinde Bir Araştırma
This study examines organizational commitment concept and evaluates the gender factor as being an individual factor that was related with organizational commitment. As a result of the literature study and based on the conceptual background, it was suggested that gender would be a factor determining the organizational commitment. For examining the relationship between gender and organizational commitment, a research study was performed on the health care staff working in hospitals. The sample of the study was composed of the health care staff from two selected hospital in Istanbul. According to the results, a significant relationship between gender and organizational commitment was found. Besides, it was seen that there were significant relationships between the sub dimension of organizational commitment and gender factor. It was found that the dimension of organizational commitment differed in terms of the gender difference
Liderlik ve Motivasyon Teorileri: Kuramsal Bir Çerçeve
Within the framework of leadership and motivation theories, this article study has two main objectives. The first objective is to perform a theoretical research on the concept of leadership and leadership theories. Initially, the leadership concept was defined, the basic leadership theories were examined and the importance of leadership was discussed. In particular, an assessment was done for the behavioral approaches on leadership theory. As such, the leadership concept and leadership theories were described and in the light of the extant literature, the evaluations on behavioral approaches of leadership were made.
The second objective of this article study is to perform a theoretical research on the concept of motivation and motivation theories. Initially, the motivation concept was defined, the basic motivation theories were examined and the importance of motivation was discussed. In particular, an assessment was done for the health organizations and hospitals and the motivation of health care employees was evaluated. As such, the motivation concept and motivation theories were described and in the light of the extant literature, the relations of motivation with other concepts were examined. Examination of perception dimension of engineering technology and ergonomics principles in terms of healthcare personnel: a scale development study
In general terms, ergonomics is the conformity of human and
equipment. Unconformity between human and equipment is more likely to
give harm either to the equipment or to human. This conformity may go for
both daily domestic tasks and work life. Domestic furniture and equipment
are designed by taking average features of individuals into consideration.
For example; chairs, tables, beds and similar things are manufactured with
the measurements which are accepted as standard all over the world. People
spend most of their time at their homes and workplaces. Similarly, the
ergonomics considered in the manufacture of domestic furniture takes an
important place in working life, as well. This is highly significant in terms
of both health of workers and efficiency of the work done. On the other
hand, ergonomics possesses importance in terms of patients except from
workers and work efficiency in health sector. Working in non-ergonomic
settings may both endanger health of healthcare personnel and cause decrease
in work efficiency as well as posing negative effects for recovery of the
patients. From this point of view, ergonomics in health sector becomes
more important in many aspects. In this study, it was aimed to develop a
scale with regard to finding out perceptions of the healthcare personnel on
engineering technologies and ergonomics principles
SAĞLIK KURUMLARINDA ÖRGÜT İKLİMİ İLE ÇALIŞANLARIN İŞ PERFORMANSI ARASINDAKİ İLİŞKİNİN İNCELENMESİ
Bu çalışmanın amacı, sağlık kurumlarında görev yapan çalışanların algıladıkları örgütsel iklimin iş performansları ile ilişkisini ve iş performansı üzerindeki etkisini tespit etmektir. Bu amaçla, öncelikle çalışmada temel kavramlar tanımlanmış ve yapılan alanyazın incelemesiyle kavramlar arasındaki ilişkiler öncül çalışmalar ile desteklenmeye çalışılmıştır. Kuramsal çerçevenin oluşturulması ile araştırmanın hipotezi geliştirilmiştir. Bu amaçla oluşturulan anket formu ile İstanbul’da bulunan iki hastanede görev yapan çalışanlar üzerinde araştırma gerçekleştirilmiştir. Elde edilen veriler tanımlayıcı analizler, korelasyon ve regresyon analizleri ile değerlendirilmiştir. Yapılan analizler neticesinde çalışanların olumlu örgüt iklimi algılamalarının iş performansı düzeyi ile anlamlı ve pozitif yönde ilişkili olduğu belirlenmiştir
Medroxyprogesterone Acetate Plus Metformin to Prevent Persistent Endometrial Hyperplasia
OBJECTIVE: To determine the effect of metformin on treatment response in simple endometrial hyperplasia without atypia.
STUDY DESIGN: In this study, we identified 134 women with simple endometrial hyperplasia without atypia. Seventy two of these women were administered cylic oral medroxyprogesterone acetate 10 mg/day for 3 months, 62 of these women were also receiving metformin 1000 mg/day due to preexisting insulin resistance and and all subjects underwent control endometrial sampling after treatment. All subjects were evaluated in terms of age, gravidity, parity, body mass index (BMI), menstrual cycle, luteal phase endometrial thickness, uterine fibroids, ovarian cysts, serum CA 125 levels, systemic disorders, cigarette smoking. All parameters and metformin were assessed for the effects on treatment success.
RESULTS: Out of 72 women who were administered medroxyprogesterone acetate 10 mg/day for 3 months, 15 of them were diagnosed as endometrial hyperplasia in control endometrial sampling while only 5 women had persistent endometrial hyperplasia in group with receiving both medroxyprogesterone acetate 10 mg/day and metformin 1000 mg/day (P<0.05). Age, gravidity, serum CA125, BMI, pretreatment endometrial thickness were comparable between groups (P>0.05).
CONCLUSION: Medroxyprogesterone acetate and metformin may be used as an adjunctive therapy for persistent endometrial hyperplasias especially in women with high body mass index
