    Infectious Diseases of the Upper Gastrointestinal Tract

    Infectious diseases of the upper gastrointestinal tract are rare, but certain bacteria including Treponema pallidum and Mycobacterium tuberculosis may infect the esophagus, stomach, and duodenum even in an immunocompetent individual. Gastric syphilis is difficult to diagnose because it presents with non-specific symptoms and diverse endoscopic findings. Nevertheless, gastric syphilis should be considered in the differential diagnosis when a patient presents with chronic inflammatory gastric lesions such as multiple erosive/ulcerative lesions and stricture or with other evidence of syphilis. Histological evaluation and specific serological tests should be performed if syphilis is suspected. Esophageal and gastroduodenal tuberculosis also exhibits non-specific clinical manifestations. The diagnosis is confirmed by mucosal biopsy or aspiration cytology revealing the presence of caseating granulomata and/or acid-fast bacilli. Mycobacterial culture and polymerase chain reaction should be incorporated into routine diagnostic studies to improve the diagnostic sensitivity. The diagnosis of tuberculosis is occasionally confirmed indirectly by an excellent response of the patient to anti-tubercular therapy

    GaIA: Graphical Information Gain based Attention Network for Weakly Supervised Point Cloud Semantic Segmentation

    While point cloud semantic segmentation is a significant task in 3D scene understanding, this task demands a time-consuming process of fully annotating labels. To address this problem, recent studies adopt a weakly supervised learning approach under the sparse annotation. Different from the existing studies, this study aims to reduce the epistemic uncertainty measured by the entropy for a precise semantic segmentation. We propose the graphical information gain based attention network called GaIA, which alleviates the entropy of each point based on the reliable information. The graphical information gain discriminates the reliable point by employing relative entropy between target point and its neighborhoods. We further introduce anchor-based additive angular margin loss, ArcPoint. The ArcPoint optimizes the unlabeled points containing high entropy towards semantically similar classes of the labeled points on hypersphere space. Experimental results on S3DIS and ScanNet-v2 datasets demonstrate our framework outperforms the existing weakly supervised methods.     Optimal Multiuser Diversity in Multi-Cell MIMO Uplink Networks: User Scaling Law and Beamforming Design

    We introduce a distributed protocol to achieve multiuser diversity in a multicell multiple-input multiple-output (MIMO) uplink network, referred to as a MIMO interfering multiple-access channel (IMAC). Assuming both no information exchange among base stations (BS) and local channel state information at the transmitters for the MIMO IMAC, we propose a joint beamforming and user scheduling protocol, and then show that the proposed protocol can achieve the optimal multiuser diversity gain, i.e., KM log (SNR log N), as long as the number of mobile stations (MSs) in a cell, N, scales faster than SNRKM-L/1-epsilon for a small constant epsilon > 0, where M, L, K, and SNR denote the number of receive antennas at each BS, the number of transmit antennas at each MS, the number of cells, and the signal-to-noise ratio, respectively. Our result indicates that multiuser diversity can be achieved in the presence of intra-cell and inter-cell interference even in a distributed fashion. As a result, vital information on how to design distributed algorithms in interference-limited cellular environments is provided

    Characteristics of poisoning in younger children according to different forms of the drugs

    Purpose This study aimed to investigate the characteristics of poisoning drug ingested by younger children, and to compare the clinical outcome by drug forms. Methods This was a retrospective analysis based on medical records from the Emergency Department based Injury In-depth Surveillance (EDIIS) registry in Korea from January to December 2015. Patients aged 7 years or younger visiting the emergency department (ED) with drug poisoning were included. We classified the forms of drugs as tablets or syrup, and analyzed the characteristics by size, color, and shape. In addition, clinical outcomes and ED length of stay were compared according to the drug forms. Results A total of 308 cases were collected, and 202 patients finally were analyzed. Tablets and capsules (TACs) were more common than syrup (67.3% vs. 32.7%). Regarding clinical outcomes, patients who took TACs had higher admission rate (18.4% vs. 7.6%, P = 0.040) without a significant difference in ED length of stay compared to those who took syrups. While commonly ingested drugs in TACs were hormones, sedative and analgesics, frequent drugs in syrup were antihistamines and cold drugs. In 136 case of TACs, median long and short axes were 0.85 cm (interquartile range [IQR], 0.7-1.1 cm) and 0.72 cm (IQR, 0.59-0.82 cm), respectively. Chromatic TACs were 80 cases (52.8%) and more common than achromatic TACs. Round shapes were preferred than angular ones (96.3% vs. 3.7%). Conclusion In younger children poisonings, the TACs showed higher incidence and admission rate compared to syrups. Especially, chromatic TACs and round shapes were preferred. Therefore, drugs with these characteristics need to be stored more carefully

    Antibiotic resistance mechanisms inform discovery: identification and characterization of a novel amycolatopsis strain producing ristocetin.

    Discovering new antibiotics is a major scientific challenge, made increasingly urgent by the continued development of resistance in bacterial pathogens. A fundamental understanding of the mechanisms of bacterial antibiotic resistance will be vital for the future discovery or design of new, more effective antibiotics. We have exploited our intimate knowledge of the molecular mechanism of glycopeptide antibiotic resistance in the harmless bacterium Streptomyces coelicolor to develop a new two-step cell wall bioactivity screen, which efficiently identified a new actinomycete strain containing a previously uncharacterized glycopeptide biosynthetic gene cluster. The screen first identifies natural product extracts capable of triggering a generalized cell wall stress response and then specifically selects for glycopeptide antibacterials by assaying for the induction of glycopeptide resistance genes. In this study, we established a diverse natural product extract library from actinomycete strains isolated from locations with widely varying climates and ecologies, and we screened them using the novel two-step bioassay system. The bioassay ultimately identified a single strain harboring the previously unidentified biosynthetic gene cluster for the glycopeptide ristocetin, providing a proof of principle for the effectiveness of the screen. This is the first report of the ristocetin biosynthetic gene cluster, which is predicted to include some interesting and previously uncharacterized enzymes.
