    All students in the nursing care department must take the general graduate examination. The result does not afect students\u27 acquisition of the national license now. However, it is expected that each student must pass the examination to get the license in the future. We, professors in the nursing care department, educate and encourage them through various opportunities to become good care workers. The goal of passing the examination becomes more diicult year after year as a whole, for the acknowledged level of students lowers year after year. However, the knowledge of nursing care is critically important in providing appropriate nursing care. Therefore all the professors of the department determined to educate all the students in order to pass the examination. It is a hard object for the professors to attain. We must improve our teaching skills through our faculty development activities. Last year we had many special lessons for our students to prepare the examination. The number of passing the examination improved amazingly last year, compared to the previous year. We will continue to improve our teaching skills to attain our goal, to make sure all the students pass the examinat ion

    Pain experienced by neonates on the NICUs: A survey of nurses' perception and actual nursing care

    本研究の目的は、新生児看護に従事する看護婦の新生児の痛みに対する認識や看護実践状況を調査し,今後の課題を明らかにすることである.質問内容は,痛みに対する考え方,痛みを伴う処置,痛みのサイン,痛みへの介入とし,構成型質問紙を553人の新生児看護婦に配布し,260人から有効回答を得た(回収率47%).その結果,1)「新生児は日齢が少ないほうが痛みを感じない」との回答が約9割を占めた;2) ……新生児の痛みの看護を発展させ,新生児を痛みから解放するためには,次の3点が重要な課題と考えられた:1)痛みの観察指標,特に行動学的指標を開発する;2)痛みの緩和法を再評価し,効果的な介入を行なう;薬理学的介入の開発に看護者も取り組む.The purpose of this survey was to explore nurses' perception and practices with regard to assessment and management of pain in neonates. A survey questionnaire comprised of fixed-alternative items was developed. The questions asked were the following:1) beliefs regarding pain, 2) painful treatment, 3) clinical signs used to assess pain, and 4) interventions used for pain. A questionnaire was distributed to 553 neonatal nurses and 260 responses (47%) were analyzed.The results were as follows:1) 224 nurses (90%) believed that younger neonates will not respond more sensitively to painful stimuli than older.2) ……Nurses need appropriate methods for assessing pain of critically ill neonates. These infants often cannot demonstrate behavioral signs of pain, and physiologic signs are difficult to interpret because of immaturity of the autonomic nervous system. Comfort measures having been used by nurses may be ineffective for relieving severe pain. Guidelines of pain medication need also need to be developed for neonates

    A survey of actual care and proposal for a tentative inetervention model of neonatal agitation

    新生児のagitationに対する認識と介入について,NICU看護婦110人を対象に質問紙調査を実施した.その結果,次の6点が明らかにされた:1)大多数の看護婦がagitationは看護上の問題であると認識していた,2)agitationが問題となる理由は,「エネルギーを消耗させる」が最も多かった,3)agitationがみられる時期は,正期産児は生後2.1±3.9日,早期産児は修正33.3±4.1週であった,4)agitationを起こす新生児の特徴は,慢性肺疾患およびその治療に伴う状況(水分摂取量の制限,不適当な換気条件)であった,5)agitationに対して看護婦がよく行う介入は,なでる,おしゃぶりを与えるなどのなだめの行為であった.逆に,環境調整は少なかった,6)agitationに対して薬が使用されると回答したのは約60%で,催眠・鎮静薬が最も多く使用されていた。これらの結果から、新生児のagitationに対する介入モデル試案を作成することができた。110 NICU nurses completed a written questionnaire regarding perception and intervention of neonatal agitation.The results were the following 6 points:l)By most NICU nurses,agitation was identified as a problem of caring,2)"Exhaustion of energy" was often considered as problematic feature of agitation,3)The age of the infants in which agitation was most commonly seen in their units was 2.1±3.9 days after birth in full term infants and 33.3±4.1weeks gestational age in premature infants,4)Characteristic features associated with agitated infants were chronic lung disease and associated secondary problems (limitation of fluid intake, insufficient respiration), 5)The nursing intervention most commonly used to manage agitation was soothing,such as touch or giving the infant apacifier.On the other hand,environmental management was minimal,6)About 60 percentresponded that medication was administered to infants for agitation most often a hypnotic or sedative. As a result, we were able to draw up a tentative intervention model of neonatal agitation
