    Whole exome sequencing of well-differentiated liposarcoma and dedifferentiated liposarcoma in older woman: a case report

    BackgroundCommon kinds of soft tissue sarcomas (STS) include well-differentiated liposarcoma (WDLPS) and dedifferentiated liposarcoma (DDLPS). In this case, we present a comprehensive clinical profile of a patient who underwent multiple recurrences during the progression from WDLPS to DDLPS.Case presentationA 62-year-old Asian female underwent retroperitoneal resection of a large tumor 11 years ago, the initial pathology revealed a fibrolipoma-like lesion. Over the next six years, the patient underwent three resections for recurrence of abdominal tumors. Postoperative histology shows mature adipose tissue with scattered “adipoblast”-like cells with moderate-to-severe heterogeneous spindle cells, pleomorphic cells, or tumor giant cells. Immunohistochemistry (IHC) demonstrated positive staining for MDM2 and CDK4, confirming that the abdominal tumor was WDLPS and gradually progressing to DDLPS. Post-operative targeted sequencing and IHC confirmed the POC1B::ROS1 fusion gene in DDLPS. Whole-exome sequencing (WES) revealed that WDLPS and DDLPS shared similar somatic mutations and copy number variations (CNVs), whereas DDLPS had more mutated genes and a higher and more concentrated amplification of the chromosome 12q region. Furthermore, somatic mutations in DDLPS were significantly reduced after treatment with CDK4 inhibitors, while CNVs remained elevated.ConclusionDue to the high likelihood of recurrence of liposarcoma, various effective treatments should be taken into consideration even if surgery is the primary treatment for recurrent liposarcoma. To effectively control the course of the disease following surgery, combination targeted therapy may be a viable alternative to chemotherapy and radiotherapy in the treatment of liposarcoma

    Mucocele of the appendix presenting as an exacerbated chronic tubo-ovarian abscess: A case report and comprehensive review of the literature.

    Appendiceal mucocele is a rare entity of mucinous cystic dilatation of the appendix. It has no typical clinical presentation and is considered a potentially premalignant condition. We present a case of accidental intraoperative finding of an appendiceal mucocele in a 54-year old woman that clinically presented with an exacerbated chronic tubo-ovarian abscess. Trans-vaginal ultrasonography showed an encapsulated, oval, unilocular mass above the uterus with a heteroechogenic structure, homogeneous fluid content, and smooth regular walls without inner proliferation. The histopathologic diagnosis was consistent with an appendiceal cystadenoma. The patient underwent a simple appendectomy. There were no clinical, biochemical or imaging signs of the disease recurrence at 6 months follow up. To our knowledge, this is the only well-documented case of appendiceal mucocele mimicking exacerbated chronic tubo-ovarian abscess reported in the literature. Awareness of a rare entity such as an appendiceal mucocele, which is frequently misdiagnosed as a potential cause of acute abdomen, is necessary for the appropriate management strategy in order to prevent complications

    Outcomes of elective liver surgery worldwide: a global, prospective, multicenter, cross-sectional study

    Background: The outcomes of liver surgery worldwide remain unknown. The true population-based outcomes are likely different to those vastly reported that reflect the activity of highly specialized academic centers. The aim of this study was to measure the true worldwide practice of liver surgery and associated outcomes by recruiting from centers across the globe. The geographic distribution of liver surgery activity and complexity was also evaluated to further understand variations in outcomes. Methods: LiverGroup.org was an international, prospective, multicenter, cross-sectional study following the Global Surgery Collaborative Snapshot Research approach with a 3-month prospective, consecutive patient enrollment within January–December 2019. Each patient was followed up for 90 days postoperatively. All patients undergoing liver surgery at their respective centers were eligible for study inclusion. Basic demographics, patient and operation characteristics were collected. Morbidity was recorded according to the Clavien–Dindo Classification of Surgical Complications. Country-based and hospital-based data were collected, including the Human Development Index (HDI). (NCT03768141). Results: A total of 2159 patients were included from six continents. Surgery was performed for cancer in 1785 (83%) patients. Of all patients, 912 (42%) experienced a postoperative complication of any severity, while the major complication rate was 16% (341/2159). The overall 90-day mortality rate after liver surgery was 3.8% (82/2,159). The overall failure to rescue rate was 11% (82/ 722) ranging from 5 to 35% among the higher and lower HDI groups, respectively. Conclusions: This is the first to our knowledge global surgery study specifically designed and conducted for specialized liver surgery. The authors identified failure to rescue as a significant potentially modifiable factor for mortality after liver surgery, mostly related to lower Human Development Index countries.     Wpływ rozkładu wad lokalnych na charakterystykę drgań łożysk kulkowych

    Local defects in ball bearings may occur at the center line of raceway and its surrounding areas. However, most current studies were concentrated in studying the influence of the local defect located at the centerline of raceway on the bearing vibrations, where the effects of local defects surround the centerline were ignored. To overcome this problem, based on Hertzian point contact theory and multi-body dynamic algorithm, a multi-body dynamic model considering the offset and angular distributions for a ball bearing with a local defect on its outer raceway is established. The influences of offset distance and skew angle between the geometric center of local defect and the centerline of outer raceway on the bearing vibrations are investigated. The relationship between the offset distance and the impulse waveform characteristics is obtained, as well as that between the skew angle and the impulse waveform characteristics. The results show that the offset distance and skew angle of the local defect have a great influence on the time-domain impulse waveform characteristics of the bearing accelerations. This paper can provide a useful guidance for the accurate diagnosis of early local fault in the ball bearings.Wady lokalne łożysk kulkowych mogą występować na linii środkowej bieżni oraz w otaczających ją obszarach. Jednak większość dotychczasowych badań nad wpływem wad lokalnych na drgania łożyska koncentruje się na wadach linii środkowej bieżni ignorując oddziaływanie wad zlokalizowanych w obszarach otaczających tę linię. Aby rozwiązać ten problem, w przedstawionej pracy wykorzystano teorię kontaktu Hertza oraz algorytm do analizy dynamiki układów wieloczłonowych, co pozwoliło na utworzenie modelu dynamiki układu wieloczłonowego uwzględniającego przesunięcie i rozkłady kątowe łożyska kulkowego z lokalną wadą na bieżni zewnętrznej. Badano wpływ wartości przesunięcia i kąta nachylenia między geometrycznym środkiem wady lokalnej a linią środkową bieżni zewnętrznej na drgania łożyska. Otrzymano zależności pomiędzy wartością przesunięcia a charakterystyką przebiegu impulsu, a także między kątem nachylenia a charakterystyką przebiegu impulsu. Wyniki pokazują, że wartość przesunięcia i kąt nachylenia wady lokalnej mają duży wpływ na przebieg przyspieszeń łożyska w dziedzinie czasu. Praca dostarcza pożytecznych wskazówek na temat trafnego diagnozowania lokalnych uszkodzeń łożysk kulkowych w ich wczesnych stadiach

    Expounding the Effect of Harvest Management on Rice (<i>Oryza sativa</i> L.) Yield and Latent Loss Based on the Accurate Measurement of Grain Data

    Due to the impact of global environment and climate change, determining how to ensure food production and reduce food loss has become an important research topic for many countries, especially developing countries, and can provide key information for China’s grain harvest management. This article mainly examines the impact of harvesting period on rice yield, the existence of latent losses, and their management value. From 2019 to 2022, our team conducted experiments on the growth curve of rice grains, plants, and populations to investigate the existence of latent losses by establishing the relationship between the standard moisture weight and the days after heading. The results showed that the weight of the rice gradually decreased as the harvest time was delayed, and there were latent losses, of which the dry matter loss was about 3.5%. With the addition of grain shattering loss, the latent loss rate was about 7.0%. In summary, if rice management is strengthened, including harvesting at the optimal time, 4.67% of the loss can be recovered. The timing of the harvest significantly impacts rice yield. Understanding the process and causes of new types of rice losses, increasing the basis for judging the timely harvest period, and providing the best management measures can prevent the post-harvest losses caused by traditional methods and increase the amount of fertile land available
