Metformin in peritoneal dialysis patients — more benefit or risk?
Wbrew wcześniejszym obostrzeniom metforminę od niedawna zaleca się chorym na cukrzycę powikłaną umiarkowaną niewydolnością nerek (eGFR 30–60 ml/min/1,73 m2). Jest to wynik badań wskazujących na jej korzystny wpływ na przeżycie również w tej populacji chorych oraz na znikome ryzyko kwasicy mleczanowej, jeśli zredukuje się dawkowanie i przestrzega przeciwwskazań do przyjmowania leku. Ostatnio pojawiły się propozycje, aby metforminą leczyć również chorych na schyłkową niewydolność nerek, w tym dializowanych otrzewnowo. Za stosowaniem leku mógłby przemawiać korzystny wpływ wywierany na układ sercowo-naczyniowy, polegający na zmniejszeniu stopnia uszkodzenia niedokrwienno-reperfuzyjnego i przerostu mięśnia sercowego oraz na poprawie funkcji śródbłonka. Przeciwko wydaje się świadczyć nasilanie przez metforminę komórkowych przemian katabolicznych. To działanie, pożądane u pacjentów z otyłością i zespołem metabolicznym, może być szkodliwe w populacji chorych ze schyłkową niewydolnością nerek, u których za zwiększoną śmiertelność odpowiada w dużej mierze wyniszczenie białkowo-energetyczne. Metformina mogłaby znaleźć zastosowanie w tej populacji raczej w wybranych grupach chorych, na przykład obciążonych wysokim ryzykiem lub już obecną chorobą nowotworową, czy też wśród pacjentek z zaburzeniami owulacji (istnieją doniesienia o korzystnym oddziaływaniu leku w tych stanach chorobowych). Ryzyko kwasicy mleczanowej z powodu kumulacji metforminy przy nakładaniu się na schyłkową niewydolność nerek ostrych schorzeń zmniejszających metabolizm tlenowy nakazuje jednakże pogłębione badania farmakodynamiki leku, zanim zacznie się go podawać tym chorym.Contrary to previous precautions, metformin has been recently recommended in patients with diabetes complicated by moderate chronic kidney disease (eGFR 30–60 mL/min/1.73 m2). This resulted from studies that showed improved survivals with metformin treatment also in this population, and negligible risk of lactic acidosis with dosing reduction and adherence to known contraindications. Recently, metformin use has been advocated in patients with end-stage renal disease, including those treated with peritoneal dialysis. Beneficial effects of the drug on cardiovascular system speak in favor of its applicability — alleviation of cardiac ischemia-reperfusion injury and hypertrophy, as well as improvement of endothelial function, among others. On the other hand, metformin-induced enhancement of cellular catabolism seems to counter the concept. This latter impact, which is desired in patients with obesity and metabolic syndrome, may be harmful in those affected with end-stage kidney disease, whose increased mortality results largely from protein-energy wasting. Metformin might be rather useful in certain subgroups of this population, like patients at high risk of or with cancer, or female patients with ovulation disorders, given favorable effects of the drug in these conditions. Nevertheless, the risk of lactic acidosis due to metformin accumulation in settings of end-stage kidney failure overlapped with acute disorders diminishing oxidative metabolism, warrants further studies of the drug pharmacodynamics before it can be allowed in these patients
Znaczenie insulinooporności w rozwoju uszkodzenia nerek
Diabetes is complicated by diabetic nephropathy in ~50% of cases. Its occurrence is attributed to long-term influence of hyperglycemia on the kidneys. Nevertheless, studies of the recent 10 years have shown that pathogenesis of diabetic nephropathy also involves other disorders accompanying insulin resistance of peripheral tissues, as well as insulin resistance of kidney cells themselves. Insulin at physiological blood concentrations is vital for the intact structure of cytoskeleton, and thus conformation and function of the podocyte. Among the elements of the insulin resistance syndrome, hyperleptinemia augments glomerular fibrosis, whereas hypertriglyceridemia, plausibly along with hyperglycemia and hyperinsulinemia, deteriorates sensitivity of podocytes to insulin. The latter evokes functional and histological abnormalities typical for diabetic nephropathy. Another potential contributor to kidney damage in the course of diabetes is inadequate action of insulin on proximal tubule cells, which results in disturbed albumin endocytosis in experimental settings. The research performed so far, as well as that awaited, shall augment chances for kidney protection in diabetic patients.Nefropatia cukrzycowa stanowi powikłanie w około 50% przypadków cukrzycy. Jej wystąpienie jest wiązane z długotrwałym oddziaływaniem hiperglikemii na nerki. Badania z ostatnich 10 lat dowodzą, że w patogenezie nefropatii cukrzycowej istotną rolę odgrywają również powikłania towarzyszące insulinooporności tkanek obwodowych, a także insulinooporność samych nerek. Insulina w stężeniach fizjologicznych jest niezbędna dla prawidłowej struktury cytoszkieletu, a tym samym budowy i funkcji podocytu. Spośród składowych zespołu insulinooporności hiperleptynemia nasila włóknienie kłębuszka nerkowego, hipertriglicerydemia zaś — najpewniej wspólnie z hiperglikemią i hiperinsulinemią — upośledza wrażliwość podocytów na insulinę. Wywołuje to zaburzenia czynnościowe i zmiany histologiczne typowe dla nefropatii cukrzycowej. Przedmiotem badań jest również rola w patogenezie nefropatii cukrzycowej nieadekwatnego oddziaływania insuliny na komórki kanalika proksymalnego, co w warunkach doświadczalnych skutkuje zaburzeniem endocytozy albuminy. Zarówno badania już przeprowadzone, jak i te spodziewane niosą potencjał szans na znalezienie nowych sposobów ochrony nerek u chorych na cukrzycę
Prevalence and characteristic of adrenal glands’ abnormalities assessed by computed tomography in hypertensive patients with elevated plasma aldosterone to renin activity ratio
Wstęp Wartość współczynnika aldosteronowo-reninowego
(ALDO/ARO) większa niż 50 u chorych na
nadciśnienie tętnicze (w tym również u chorych bez
towarzyszącej hipokaliemii) jest uznawana przez
wielu badaczy za wystarczające kryterium do rozpoznania
pierwotnego hiperaldosteronizmu. Celem
pracy była ocena częstości występowania i charakteru
zmian morfologicznych w nadnerczach wykrywanych
przy użyciu tomografii komputerowej (CT)
spiralnej u chorych na nadciśnienie tętnicze z podwyższonym
(≥ 50) współczynnikiem ALDO/PRA.Materiał i metody U 1367 pacjentów dwukrotnie
oznaczano aktywność reninową osocza (PRA) — po
3 dniach stosowania diety zawierającej 100–120 mmol
sodu na dobę i 8-godzinnym przebywaniu w pozycji
leżącej (PRA1) oraz po kolejnych 3 dniach diety
z ograniczeniem sodu do 10–20 mmol/dobę i 3-godzinnej
pionizacji ciała (PRA2). Stężenie aldosteronu
w osoczu oznaczano tylko u pacjentów z niskoreninowym
profilem nadciśnienia tętniczego (PRA1 < 1 ng/ml/h
i PRA2/PRA1 < 1,5 ng/ml/h), również dwukrotnie
— przed stymulacją i po stymulacji dietą ubogosodową
i pionizacją (ALDO1 i ALDO2). U pacjentów,
u których stwierdzono wartości współczynnika ≥ 50
wykonano badanie CT jamy brzusznej metodą spiralną
oraz test dożylnego obciążenia solą.
Wyniki U 36 pacjentów stwierdzono wartość współczynnika
ALDO/PRA ≥ 50 (2,6% wszystkich badanych
chorych na nadciśnienie tętnicze). W tej podgrupie
znamiennie częściej obserwowano hipokaliemię
(< 3,5 mmol/l). U 34 pacjentów wykonano CT. Najczęściej
stwierdzanymi zmianami były: guz i obustronny
przerost nadnerczy. U 44,1% chorych (n = 15) nie
uwidoczniono żadnej patologii w obrębie gruczołów
nadnerczowych. U 24 pacjentów wykonano test obciążenia
solą. Wynik dodatni uzyskano w 15 przypadkach.
Analiza ROC wykazała, że przy wartości współczynnika
powyżej 147 zmiany w CT stwierdza się u 66,7%
chorych, a jedynie 21,1% zmian pozostaje niewykrytych.
Wnioski 1. Częstość hiperaldosteronizmu pierwotnego
w ogólnej populacji chorych na nadciśnienie
tętnicze oceniana wyłącznie na podstawie podwyższonej
wartości współczynnika ALDO/PRA (≥ 50)
wynosi 2,6%. 2. Współczynnik ALDO/PRA jest
szczególnie przydatny w rozpoznawaniu zespołu
Conna o niewielkim stopniu zaawansowania klinicznego
i bez podwyższonego stężenia aldosteronu
w surowicy. 3. Wartość współczynnika ALDO/PRA
powyżej 147 jest charakterystyczna dla chorych ze
zmianami morfologicznymi nadnerczy wykrywanymi
za pomocą CT spiralnej.Background The plasma aldosterone to renin activity
(ALDO/PRA) ratio over 50 in hypertensive patients is
widely recognized as a cut-off value for identification of
primary hyperaldosteronism (especially in patients without
hypokalaemia). The aim of this study was to evaluate
prevalence and characteristic of adrenal glands’ abnormalities
in hypertensive patients with an elevated plasma
aldosteron to plasma renin activity ratio.
Material and methods In 1367 subsequent adult patients
with arterial hypertension plasma renin activity was estimated
twice: first in patients receiving normally salted diet
(100–120 mmol sodium per day) after 8 hours of recumbent
position (PRA1) and a second time after 3 days of
sodium restriction (10–20 mmol sodium per day) and
3 hours of upright position (PRA2). Plasma aldosterone
concentration was assessed only in patients with low PRA1
(< 1 ng/ml/h) and without or only marginal reaction of PRA
to salt restriction and upright position (PRA2/PRA1 < 1.5).
Plasma aldosteron concentration was assessed also twice
on normally salted diet and after sodium retriction
(ALDO1 and ALDO2). In patients with ALDO/PRA ≥ 50
a CT scan of adrenal glands and a saline infusion test were
performed.
Results ALDO/PRA ≥ 50 was found in 36 patients (2.6%
of the entire hypertensive group). In that subgroup frequency
of hypokaliaemia (< 3,5 mmol/l) was significantly
higher than in hypertensive patients with a lower ALDO/PRA ratio. Computed tomography was performed in
34 out of these 36 patients. The scans revealed no abnormalities
in 44.1% (n = 15). The most frequent abnormalities
were: isolated adrenal tumour and bilateral adrenal
enlargement. Saline infusion test was performed in 24 patients.
A positive result was obtained in 15 cases. The ROC
analysis revealed, that a ALDO/PRA ratio over 147 is connected with adrenal abnormalities in 66.7% of patients and
only 21.1% changes remain unrevealed.
Conclusions 1. The prevalence of primary hyperaldosteronism
in general population of hypertensive patients
estimated according to the raised plasma aldosterone to
plasma renin activity ratio (≥ 50) is 2.6%. 2. The ALDO/PRA ratio is especially useful in diagnosis of Conn syndrome
in hypertensive patients without elevated serum aldosterone
concentration. 3. The ALDO/PRA over 147 is
highly specific for patients with morfological changes observed
in computed tomography
Pentoxifylline and Methylprednisolone Additively Alleviate Kidney Failure and Prolong Survival of Rats after Renal Warm Ischemia-Reperfusion
Renal ischemia-reperfusion injury (IRI) induces local inflammation leading to kidney damage. Since pentoxifylline (PTX) and steroids have distinct immunomodulatory properties, we aimed to evaluate for the first time their combined use in IRI-induced acute kidney injury (AKI) and chronic kidney disease (CKD) in rats. In two experiments, PTX (100 mg/kg body weight subcutaneously) was administered 90 min prior to renal IRI or/and methylprednisolone (MP; 100 mg/kg body weight intramuscularly) was infused 60 min after reperfusion of a solitary kidney (AKI model: 45 min ischemia, 48 male Sprague-Dawley rats) or one kidney with excision of contralateral kidney 2 weeks later (CKD model: 90 min ischemia, 38 rats). Saline was infused in place of PTX or/and MP depending on the group. Renal function (diuresis, serum creatinine, creatinine clearance, sodium and potassium excretion, and urine protein/creatinine) was assessed at 48 h and 120 h post-IRI (AKI model) or 4, 16 and 24 weeks after IRI, along with survival analysis (CKD model). More evidently at early stages of AKI or CKD, treated animals showed higher glomerular filtration and diminished tubular loss of electrolytes, more so with PTX + MP than PTX or MP (serum creatinine (μmol/L) at 48 h of AKI: 60.9 ± 19.1 vs. 131.1 ± 94.4 vs. 233.4 ± 137.0, respectively, vs. 451.5 ± 114.4 in controls, all p < 0.05; and at 4 weeks of CKD: 89.0 ± 31.9 vs. 118.1 ± 64.5 vs. 156.9 ± 72.6, respectively, vs. 222.9 ± 91.4 in controls, p < 0.05 for PTX or PTX + MP vs. controls and PTX + MP vs. MP). Survival was better by >2-fold with PTX + MP (89%) vs. controls (40%; p < 0.05). PTX + MP largely protect from IRI-induced AKI and CKD and subsequent mortality in rats. This calls for clinical investigations, especially in kidney transplantation
Mesenchymal Stem Cells—Potential Applications in Kidney Diseases
Mesenchymal stem cells constitute a pool of cells present throughout the lifetime in numerous niches, characteristic of unlimited replication potential and the ability to differentiate into mature cells of mesodermal tissues in vitro. The therapeutic potential of these cells is, however, primarily associated with their capabilities of inhibiting inflammation and initiating tissue regeneration. Owing to these properties, mesenchymal stem cells (derived from the bone marrow, subcutaneous adipose tissue, and increasingly urine) are the subject of research in the settings of kidney diseases in which inflammation plays the key role. The most advanced studies, with the first clinical trials, apply to ischemic acute kidney injury, renal transplantation, lupus and diabetic nephropathies, in which beneficial clinical effects of cells themselves, as well as their culture medium, were observed. The study findings imply that mesenchymal stem cells act predominantly through secreted factors, including, above all, microRNAs contained within extracellular vesicles. Research over the coming years will focus on this secretome as a possible therapeutic agent void of the potential carcinogenicity of the cells

Selected microRNA Expression and Protein Regulator Secretion by Adipose Tissue-Derived Mesenchymal Stem Cells and Metabolic Syndrome
The role of adipose mesenchymal stem cells (Ad-MSCs) in metabolic syndrome remains unclear. We aimed to assess the expression of selected microRNAs in Ad-MSCs of non-diabetic adults in relation to Ad-MSC secretion of protein regulators and basic metabolic parameters. Ten obese, eight overweight, and five normal weight subjects were enrolled: 19 females and 4 males; aged 43.0 ± 8.9 years. Ad-MSCs were harvested from abdominal subcutaneous fat. Ad-MSC cellular expressions of four microRNAs (2−ΔCt values) and concentrations of IL-6, IL-10, VEGF, and IGF-1 in the Ad-MSC-conditioned medium were assessed. The expressions of miR-21, miR-122, or miR-192 did not correlate with clinical parameters (age, sex, BMI, visceral fat, HOMA-IR, fasting glycemia, HbA1c, serum lipids, CRP, and eGFR). Conversely, the expression of miR-155 was lowest in obese subjects (3.69 ± 2.67 × 10−3 vs. 7.07 ± 4.42 × 10−3 in overweight and 10.25 ± 7.05 × 10−3 in normal weight ones, p = 0.04). The expression of miR-155 correlated inversely with BMI (sex-adjusted r = −0.64; p p = 0.03), and serum CRP (r = −0.63; p p = 0.02). Moreover, miR-155 synthesis was associated marginally negatively with Ad-MSC secretion of IGF-1 (r = −0.42; p = 0.05), and positively with that of IL-10 (r = 0.40; p = 0.06). Ad-MSC expression of miR-155 appears blunted in visceral obesity, which correlates with Ad-MSC IGF-1 hypersecretion and IL-10 hyposecretion, systemic microinflammation, and HDL dyslipidemia. Genetic imputation of kidney transcriptome, proteome and multi-omics illuminates new blood pressure and hypertension targets
Genetic mechanisms of blood pressure (BP) regulation remain poorly defined. Using kidney-specific epigenomic annotations and 3D genome information we generated and validated gene expression prediction models for the purpose of transcriptome-wide association studies in 700 human kidneys. We identified 889 kidney genes associated with BP of which 399 were prioritised as contributors to BP regulation. Imputation of kidney proteome and microRNAome uncovered 97 renal proteins and 11 miRNAs associated with BP. Integration with plasma proteomics and metabolomics illuminated circulating levels of myo-inositol, 4-guanidinobutanoate and angiotensinogen as downstream effectors of several kidney BP genes (SLC5A11, AGMAT, AGT, respectively). We showed that genetically determined reduction in renal expression may mimic the effects of rare loss-of-function variants on kidney mRNA/protein and lead to an increase in BP (e.g., ENPEP). We demonstrated a strong correlation (r = 0.81) in expression of protein-coding genes between cells harvested from urine and the kidney highlighting a diagnostic potential of urinary cell transcriptomics. We uncovered adenylyl cyclase activators as a repurposing opportunity for hypertension and illustrated examples of BP-elevating effects of anticancer drugs (e.g. tubulin polymerisation inhibitors). Collectively, our studies provide new biological insights into genetic regulation of BP with potential to drive clinical translation in hypertension.</p
