    A Fatal Case of Cor Pulmonale with Undetected Chronic Hypoventilation in an Infant with a Known Congenital Myopathy

    The authors of this paper wish to present a case of fatal cor pulmonale with right ventricular hypertrophy complicated by a congenital myopathy. It is our intention to demonstrate the importance of vigilant clinical assessment of children with a congenital myopathy, regardless of the exact etiology of their disease, or family history of disease severity. This case highlights the risk for fatal complications if hypoventilation and respiratory insufficiency go unrecognized in myopathic children. Consequently, we recommend respiratory and cardiac monitoring surveillance as well as appropriate referral to specialists in the management of such children

    Beginning in March of 2020, public school educators at all levels were thrown into a situation in which they were required to drastically alter pedagogical approaches to teaching and learning. The move from classroom-based instruction to a virtual platform, whether synchronous or not, was too often done on-the-fly and with mixed results. With the realization and acceptance, albeit grudgingly, that web-based, internet-based, and/or virtual platforms need to be greatly expanded to the population and may become the norm for public education, there clearly exists a need for an expanded definition of instructional leadership. The purpose of this study is to explore perceptions of school administrators regarding the role of the professionally trained school librarian in the K-12 environment with respect to campus change processes and activities that relate to collaboration between the librarian and the classroom teacher. To investigate this purpose, the researchers elicited data from specific questions on the Principal Technology Leadership Assessment Survey that were overtly related to the purpose of the research

    Selective Expression of Immune-Associated Surface Antigens by Keratinocytes in Irritant Contact Dermatitis

    The expression of three immunoregulatory surface antigens by epidermal keratinocytes was studied in irritant contact dermatitis (lCD), in order to assess whether keratinocytes have a modulatory role in the pathogenesis of this disorder. Biopsies were taken from 48-h patch test reactions to six structurally unrelated irritants, and frozen sections immunolabeled with monoclonal antibodies to the major histocompatibility complex class II antigen, HLA-DR, intercellular adhesion molecule-i (ICAM-1), and the 88-Kd glycoprotein CD36 (OKM5), as well as to the CD3 (T cells) and CD11a (lymphocyte function associated antigen-1, LFA-1) antigens. We found that there was very limited expression of HLA- DR by keratinocytes, with no correlation between the extent of HLA-DR positivity and the degree of T cell infiltration into the epidermis and dermis, suggesting that interferon gamma may not be a significant mediator of lCD at 48h. In contrast, keratinocytes showed extensive upregulation of ICAM-1, with an excellent spatial association between ICAM-1 expression and LFA-1 positive leucocytes in the epidermis. This indicates that keratinocyte JCAM-1 induction is not restricted to diseases in which antigen presentation is pivotal, but that it has a generalized role in cutaneous inflammatory reactions, promoting the infiltration of leucocytes into the epidermis. Immunolabeling with OKM5 revealed that CD36 is present to a variable degree on keratinocytes in normal skin. Differential changes in the pattern of keratinocyte expression occurred between irritants, in a manner that suggested that the CD36 antigen does not act as an adhesion molecule in lCD, but rather that its expression is related to the proliferative state of the epidermis. The results of this study demonstrate that immune-associated antigens are selectively expressed on the surface of keratinocytes in 48-h ICD biopsies, implying that these cells play an important regulatory role in the development of the inflammatory response to irritant chemicals

    Social Media Practices of School Administrators: The Time is Now

    The educational disruption caused by COVID-19 increased awareness of the value of organizational resiliency/sustainability and the critical need for administrators to provide timely communication among all stakeholders. If preparation programs are to provide administrators with the skills and knowledge to effectively use social media as a communication tool, we must explore the practices and perceptions of school administrators’ use of social media to communicate with stakeholders and to identify their recommended social media communication practices. The Social Media as a Tool to Effectively Communicate with Stakeholders Survey and interviews were used for data collection. Findings are focused on comfort levels, perceived effectiveness, benefits, concerns, and recommended practices reported by the school administrators who participated in the study. While, social media was found to be an effective communication tool that can promote and support positive public relations, there were noted gaps in self-efficacy and best practices of the reporting administrators. Research literature, preparation programs, and practicing administrators will equally benefit from the outcomes of this study

    A Natural Plasmid Uniquely Encodes Two Biosynthetic Pathways Creating a Potent Anti-MRSA Antibiotic

    Background Understanding how complex antibiotics are synthesised by their producer bacteria is essential for creation of new families of bioactive compounds. Thiomarinols, produced by marine bacteria belonging to the genus Pseudoalteromonas, are hybrids of two independently active species: the pseudomonic acid mixture, mupirocin, which is used clinically against MRSA, and the pyrrothine core of holomycin. Methodology/Principal Findings High throughput DNA sequencing of the complete genome of the producer bacterium revealed a novel 97 kb plasmid, pTML1, consisting almost entirely of two distinct gene clusters. Targeted gene knockouts confirmed the role of these clusters in biosynthesis of the two separate components, pseudomonic acid and the pyrrothine, and identified a putative amide synthetase that joins them together. Feeding mupirocin to a mutant unable to make the endogenous pseudomonic acid created a novel hybrid with the pyrrothine via “mutasynthesis” that allows inhibition of mupirocin-resistant isoleucyl-tRNA synthetase, the mupirocin target. A mutant defective in pyrrothine biosynthesis was also able to incorporate alternative amine substrates. Conclusions/Significance Plasmid pTML1 provides a paradigm for combining independent antibiotic biosynthetic pathways or using mutasynthesis to develop a new family of hybrid derivatives that may extend the effective use of mupirocin against MRSA
