Caffeine – common ingredient in a diet and its influence on human health
Kofeina jest składnikiem żywności regularnie spożywanym przez większość populacji. Jej głównym źródłem w diecie
jest kawa, herbata i napoje typu cola. W ostatnim okresie nastąpił znaczny rozwój rynku produktów z kofeiną, zwłaszcza
z grupy napojów energetyzujących i suplementów diety. Substancja ta od lat budzi wiele dyskusji, czy ma pozytywny,
czy negatywny wpływ na zdrowie, a grupą szczególnych obaw są dzieci. Działanie kofeiny w organizmie polega przede
wszystkim na pobudzeniu ośrodkowego układu nerwowego, dlatego jest ona uważana za najczęściej spożywaną substancję
psychoaktywną. Działanie fizjologiczne kofeiny i brak właściwości odżywczych powoduje duże zainteresowanie jej wpływem
na zdrowie, zwłaszcza w odniesieniu do ryzyka chorób sercowo-naczyniowych. Wyniki badań naukowych nie dają
jednoznacznej odpowiedzi, wskazując na wiele czynników endogennych i środowiskowych wpływających na metabolizm
kofeiny i indywidualne reakcje organizmu. Niemniej w świetle aktualnego stanu wiedzy umiarkowane spożycie kofeiny (do
400 mg dziennie) przez zdrowe osoby dorosłe nie jest związane z ujemnymi skutkami zdrowotnymi. Należy jednak brać
pod uwagę także inne, współistniejące zachowania zdrowotne stylu życia. Nadmierne spożycie kofeiny może wywoływać
niekorzystne efekty, takie jak zbytnią pobudliwość, bezsenność, ból głowy, problemy żołądkowe. Według Międzynarodowej
Klasyfikacji Chorób i Zaburzeń Stanu Zdrowia WHO objawy wywołane spożyciem dużych ilości kofeiny zakwalifikowane
są do działu zaburzeń umysłowych i zaburzeń zachowania, powodowanych przez substancje stymulujące. W organizmie
kobiet ciężarnych metabolizm kofeiny jest znacznie wydłużony, a kofeina i jej metabolity swobodnie przechodzą przez
łożysko do płodu. Z tego powodu kobietom w okresie ciąży zaleca się ograniczenie spożycia kofeiny. Kontrola ilości kofeiny
zalecana jest także w diecie dzieci i młodzieży, przede wszystkim z uwagi na jej oddziaływanie na ośrodkowy układ
nerwowy, znajdujący się w okresie intensywnego wzrostu i dojrzewania, gospodarkę wapniową w organizmie i czas snu.
Średnie spożycie kofeiny w krajach europejskich kształtuje się w granicach 280–490 mg dziennie. Największe jest w krajach
skandynawskich, co wynika z dużej konsumpcji kawy. Dane dotyczące spożycia kofeiny w Polsce są nieliczne. W latach
90-tych poprzedniego stulecia oszacowano, że statystyczny Polak spożywa 141 mg kofeiny dziennie. Obecnie w grupie
kobiet z regionu warszawskiego stwierdzono średnie spożycie na poziomie 251 mg, a 15% badanych kobiet spożywało ponad
400 mg kofeiny w ciągu dnia. Osoby palące tytoń spożywają więcej kofeiny niż niepalące, podobnie jak osoby cierpiące na
zaburzenia psychiczne. W odniesieniu do wielkości spożycia kofeiny należy pamiętać, że jej zawartość w naparach kawy
i herbaty w dużym stopniu uzależniona jest od metody parzenia, a zawartość kofeiny w napojach energetyzujących może
być znacznie zróżnicowana, co należy brać pod uwagę w dziennej puli kofeiny w diecie.Caffeine is widely consumed by people of all ages. In the last period a market of caffeine-containing products, particularly
energy drinks and food supplements increased. Caffeine for years is under discussion, whether has positive whether
adverse impact on health. Children are a group of special anxieties. Caffeine is a stimulant of central nervous system and
therefore is probably the most commonly used psychoactive substance in the world. The physiological effect of caffeine
and the lack of nutrition value causes a great interest its impact on health, especially with reference to the risk of cardiovascular
diseases. Results of scientific research are not clear. The influence of caffeine on the human body is conditioned
with the individual metabolism of caffeine which also depends on many endogenic and environmental factors. According to the current knowledge moderate caffeine intake by healthy adults at a dose level of 400 mg a day is not associated with
adverse effects, but it also depends on other health determinants of a lifestyle. Excessive caffeine consumption can cause
negative health consequences such as psychomotor agitation, insomnia, headache, gastrointestinal complaints. Adverse effect
of caffeine intoxication is classified in World Health Organization´s International Classification of Diseases (ICD-10).
Metabolism of caffeine by pregnant woman is slowed down. Caffeine and its metabolites pass freely across the placenta into
a fetus. For this reason pregnant women should limit caffeine intake. Children and adolescents should also limit daily caffeine
consumption. It results from the influence of caffeine on the central nervous system in the period of rapid growth and
the final stage of brain development, calcium balance and sleep duration. Average daily caffeine consumption in European
countries ranging from 280 – 490 mg. The highest caffeine intake is in Scandinavian countries what results from the great
consumption of the coffee. As far as caffeine consumption by Polish population is concerned there is very few data in this
subject so far. In the nineties of the previous century it was 141 mg per day, whereas according to recent survey daily caffeine
intake by women from the Warsaw region was 251 mg and 15 % of examined women consumed an excessive quantity
of caffeine (≥ 400 mg). Smokers consume more caffeine than nonsmokers, similarly to persons with mental illnesses. With
reference to the caffeine consumption it should be underline that caffeine content in coffee and tea beverages varies greatly
depending on the method of brewing whereas the content of caffeine in many brands of energy drinks can much vary. This
should be taken into account in the daily caffeine intake
Evaluation of prenatal vitamin-mineral preparations in the context of recommended dietary supplementation. Are pregnant women supplied with what they should get?
Background. The composition of preparations, intended for pregnant women is an important issue of proper dietary
supplementation. The range of such products on the market is very wide and their composition is not regulated by law.
Objectives. To evaluate the composition of preparations for pregnant women in the context of 2014 Polish Gynecological
Society (PTG) recommendation and the 2020 recommendation of the Polish Society of Gynecologists and Obstetricians
(PTGiP).
Materials and methods. A range of preparations was collected in pharmacies and e-pharmacies in 2019. The nutrient
content was determined based on the information on the unit packaging or the pharmacy's website. The content of folic
acid, vitamin D, iodine, DHA, and iron was assessed.
Results. There were 33 vitamin-mineral preparations (VMPs) on offer on the analyzed market. All preparations contained
folic acid, of which 55% contained precisely the dose recommended by the PTG, and 45% of the preparations is compliant
in this respect with the recommendation of the PTGiP. Ninety seven percent of VMPs contained vitamin D. Fifty percent
of them did not contain this vitamin at the dose recommended by the PTG, and half of the preparations do not meet the
PTGiP recommendation either. Ninety seven percent of VMPs contained iodine. Out of them 44% contained a too low
dose of iodine, by the PTG standards, but only 9% of preparations do not meet the PTGiP recommendation in this respect.
DHA was a component contained in 73% of VMPs. Among them, 33% contained the dose recommended by the PTG
for women who eat little fish and 88% of the preparations are in line with the new recommendation PTGiP. Eighty two
percent of preparations contained iron, which in the light of the PTGiP recommendation is debatable.
Conclusions. The composition of many VMPs did not reflect experts' recommendations regarding the type and amounts
of particular nutrients
Coffee consumption vs. cancer risk - a review of scientific data
Coffee and its impact on health continue to be the topic of much heated debate. Until recently, coffee consumption has been
believed to be associated with adverse effects, mainly cardiovascular problems. However, the vast majority of contemporary
sources not only emphasize a lack of detrimental effect, but also suggest a beneficial effect of coffee intake. According to
the current state of knowledge, coffee consumption is not associated with the majority of cancers although the results of
studies on bladder and lung cancer remain conflicting. In case of colorectal, liver and breast cancers, coffee drinking may
even have a protective effect. Coffee contains numerous compounds, potentially beneficial as well as harmful. The former
include polyphenols which inhibit harmful oxidation processes in the body, while the latter include acrylamide, whose high
intake in daily diet may have carcinogenic action. The impact of coffee on the human body is associated also with other
factors, e.g. the rate of metabolism and other individual features.Od lat trwa dyskusja na temat wpływu picia kawy na zdrowie. Jeszcze do niedawna przeważały poglądy o jej negatywnych
skutkach, w tym przede wszystkim w postaci zwiększonego ryzyka chorób sercowo-naczyniowych. W ostatnich latach
większość publikacji naukowych podkreśla jednak brak szkodliwego wpływu picia kawy na zdrowie, a niektóre sugerują
nawet jej działanie prozdrowotne. W świetle aktualnego stanu wiedzy obecność kawy w diecie nie ma związku z zachorowaniem
na większość nowotworów. Niejednoznaczne są dane dotyczące nowotworów pęcherza moczowego i płuc, natomiast
w przypadku jelita grubego, wątroby i piersi nie wyklucza się jej działania ochronnego. Kawa zawiera szereg składników,
zarówno potencjalnie korzystnych, jak i szkodliwych. Do pierwszej grupy należą przede wszystkim polifenole, hamujące
szkodliwe procesy utleniania w organizmie. Do drugiej należy zaliczyć akryloamid, którego duże spożycie może mieć
działanie kancerogenne. Wpływ kawy na organizm człowieka może zależeć także od innych czynników, w tym szybkości
jej metabolizmu i innych cech osobniczych
Pre-pregnancy nutritional status versus maternal weight gain and neonatal size
Background. Maternal weight during pregnancy may affect both, the course of pregnancy and the anthropometric parameters of the newborn. The steadily growing problem of excessive weight in reproductive-age women is associated with the risk for gestational obesity and its negative consequences for the infant.
Objectives. The aims of the study were to analyze the following: (i) maternal weight gain as compared to the pre-pregnancy BMI, and (ii) the link between maternal weight and other environmental factors versus neonatal size.
Material and methods. The study was conducted among 94 women in singleton term pregnancy, who delivered at the Department of Obstetrics, Gynecology and Oncology, Medical University of Warsaw. The American Institute of Medicine criteria for the recommended weight gain were followed. Multivariate logistic regression model and multiple regression analysis were used for statistical analysis.
Results. Normal weight gain was observed in only one-third of the subjects, while excess weight gain was detected in as many as 42.5% of the women. Active smokers were at a 4-fold higher risk for excess weight gain as compared to nonsmokers (OR 4.13, 95% CI 1.19 - 14.34, p = 0.026). Infants born to mothers with insufficient weight gain (24.5% of the mothers) were lighter by 302 g (p=0.0405) and shorter by 2.4 cm (p=0.0025) as compared to those born to mothers with normal weight gain. Conclusions. Maternal weight gain in most of our subjects was not compliant with the current recommendations, regardless of the pre-pregnancy BMI. Inadequate pregnancy weight gain negatively affects the anthropometric parameters of the newborn.Wprowadzenie. Masa ciała matki w okresie ciąży może mieć wpływ na przebieg ciąży i parametry antropometryczne noworodka. Narastający, już u kobiet w wieku rozrodczym problem nadmiernej masy ciała niesie ze sobą ryzyko otyłości ciężarnych i jej konsekwencji u dziecka.
Cel. Celem badania była analiza przyrostu masy ciała kobiet, w zależności od przedciążowego wskaźnika BMI oraz analiza związku, pomiędzy przyrostem masy ciała i innymi czynnikami środowiskowymi, a wielkością noworodków.
Materiał i metody. Badanie przeprowadzono wśród 94 kobiet w ciąży pojedynczej, które urodziły w terminie w Klinice Położnictwa, Chorób Kobiecych i Ginekologii Onkologicznej Warszawskiego Uniwersytetu Medycznego. Zalecany przyrost masy ciała przyjęto według kryteriów opracowanych przez amerykański Institute of Medicine. W analizie statystycznej wyników wykorzystano wielowymiarową analizę regresji logistycznej i wieloczynnikową analizę regresji.
Wyniki. Prawidłowy przyrost masy ciała miała tylko 1/3 pacjentek, a największy odsetek dotyczył przyrostu nadmiernego (42,5%). Kobiety palące w czasie ciąży miały ponad 4-krotnie większe ryzyko zbyt dużego przyrostu masy ciała, w porównaniu do kobiet ciężarnych niepalących (OR 4.13, 95% CI 1.19 - 14.34, p = 0,026). Noworodki matek z małym przyrostem masy ciała (24,5% matek) były o 302 g lżejsze (p=0,0405) i o 2,4 cm krótsze (p=0,0025), niż noworodki matek z przyrostem prawidłowym.
Wnioski. Ciążowy przyrost masy ciała większości badanych kobiet odbiegał od aktualnych zaleceń, niezależnie od przedciążowego wskaźnika BMI. Negatywny wpływ na parametry antropometryczne noworodków miał zbyt mały przyrost masy ciała matek
Evaluation of dairy products available on the Polish market in the context of nutrient profiles. Clear arguments for reformulation of foodstuffs
Background. Dairy products, which are one of the main groups of products in the diet, are expected to have high nutritional
value. The development of food technology often involves changing the nutritional parameters of foodstuffs.
Objective. The aim of this study was to determine the nutritional value of dairy products, in the context of nutrient profiles.
Material and Methods. The tested products included yoghurts, yoghurt drinks, kefir, cream cheeses and milk desserts
available on the Polish market, basing on the information from unit product packaging.
Results. The average sugar content in the group of all products was 11 g/100 g. Milk desserts contained the largest amount of sugar
(average 14.7 g/100g), whereas kefir – the smallest amount (average 6 g/100 g) (p <0.0001). Yoghurts and yoghurt drinks intended
for children had higher sugar content than products for the general population (14.4 g vs. 10.5 g/100 g) (p <0.0001). As many as
75% of products were sweetened. Taking into account the natural content of lactose, the estimated amount of added sugar ranged
from 6.9 - 12.3 g/100 g. The average fat content in the group of all products amounted to 3.8 g/100 g. In the view of WHO profiles,
71% of products exceeded the permissible sugar content (≤ 10 g/100 g) and 36% exceeded the permissible content of saturated fatty
acids (≤ 2 g/100 g). Using more liberal profiles developed by the food industry, the proportion of such products was smaller, but still
quite high (34% in case of sugar and 26% in case of saturated fatty acids). In terms of the sugar content, according to regulation
introduced in Poland, only 29% of the analyzed dairy products could be offer for children at schools. Among them there are all
natural (not sweetened) yoghurts and kefir and only 6% of other sweetened products.
Conclusions. There is a justified need for the reformulation of dairy products, especially in terms of added sugar.Wprowadzenie: Od przetworów mlecznych, które są jedną z podstawowych grup produktów w diecie człowieka oczekuje
się wysokiej wartości odżywczej. Rozwój technologii żywności często jednak pociąga za sobą zmiany parametrów
odżywczych produktów.
Cel: Charakterystyka wartości odżywczej przetworów mlecznych, w kontekście wymagań profili żywieniowych.
Materiał i metody: Ocenie poddano jogurty, napoje jogurtowe, kefiry, serki i desery mleczne dostępne na rynku polskim,
bazując na informacjach z opakowań jednostkowych produktów.
Wyniki: Średnia zawartość cukrów w produktach mlecznych wynosiła 11 g/100 g. Największą ilość cukru zawierały desery
mleczne (średnia 14,7 g/100 g), a najmniejszą kefiry (średnia 6 g/100 g) (p <0.0001). Jogurty i napoje jogurtowe adresowane
do dzieci miały większą zawartość cukru, niż produkty dla ogółu populacji (14,4 g vs. 10,5 g/100 g) (p<0,0001). Aż 75%
produktów zawierało dodatek różnych form cukru. Po uwzględnieniu zawartości naturalnej laktozy oszacowana ilość cukru
dodanego kształtowała się w przedziale 6,9 - 12,3 g/100 g. Średnia zawartość tłuszczu w grupie wszystkich produktów
wynosiła 3,8 g/100 g. Dopuszczalną, w świetle profili WHO, zawartość cukru (≤ 10g/100 g) przekroczyło 71% produktów,
a zawartość nasyconych kwasów tłuszczowych (≤ 2 g/100 g) 36% produktów. Przyjmując wartości profili opracowanych przez
przemysł spożywczy odsetek takich produktów był mniejszy, ale wciąż znaczny (34% w przypadku cukru i 26% w przypadku
tłuszczów nasyconych). W świetle wprowadzonych ostatnio wymagań, dotyczących wartości odżywczej żywności oferowanej
w szkołach, biorąc pod uwagę zawartość cukru tylko 29% analizowanych produktów mogłoby być przeznaczone dla dzieci.
Wśród nich byłyby wszystkie naturalne jogurty i kefiry, a tylko 6% stanowiłby inne słodzone produkty.
Wnioski: Istnieje uzasadniona potrzeba modyfikacji receptur przetworów mlecznych, szczególnie w zakresie ilości
dodawanego cukru
Dietary intake of DHA during pregnancy: a significant gap between the actual intake and current nutritional recommendations
Background: Docosahexaenoic acid (DHA) is indispensable to ensure proper development of the fetal central nervous
system and pregnancy duration. Daily intake of DHA should be at least 100 mg higher as compared to the pre-pregnancy
values.
Objectives: The aim of the study was to evaluate DHA intake during pregnancy and compare it to the current
recommendations.
Material and methods: A total of 100 pregnant women presenting for labor at the Department of Obstetrics, Gynecology
and Oncology, Medical University of Warsaw, were included in the study. DHA intake from the following sources: fish and
eggs, and DHA preparations collected based on the questionnaire was investigated.
Results: Low dietary DHA intake (median: 60 mg/day) was observed. The values were only slightly higher (median:
90 mg/day) after DHA preparations were taken into consideration because only 28% of the pregnant women used DHA
preparations. Overall, 92% of the subjects consumed <200 mg of DHA a day, which was the result of insufficient fish
consumption (mean: 15 g/day). Only 10% of the respondents ate fish twice a week, in which case the DHA dietary intake
was statistically significantly higher (median: 160 mg/day) (p=0.0232). Total median dietary DHA and EPA intake in the
study population was 79 mg/day.
Conclusions: The diet of pregnant women is largely deficient as far as DHA intake is concerned. Importantly, it is not
possible to comply with current recommendations without dietary supplementation.Wprowadzenie: Kwas dokozaheksaenowy (DHA) jest niezbędny dla prawidłowego rozwoju centralnego układu
nerwowego płodu, a także czasu trwania ciąży. Jego spożycie przez kobiety ciężarne powinno być co najmniej o 100 mg
większe, niż przed zajściem w ciążę.
Cel: Celem badania było oszacowanie wielkości spożycia DHA przez kobiety ciężarne, w kontekście aktualnych zaleceń
żywieniowych.
Materiał i metody: Badanie przeprowadzono wśród 100 kobiet ciężarnych, zgłaszających się do porodu w Klinice
Położnictwa, Chorób Kobiecych i Ginekologii Onkologicznej Warszawskiego Uniwersytetu Medycznego. Spośród
produktów spożywczych źródłami DHA były ryby i jaja, których częstość i wielkość spożycia ustalono w oparciu o ankietę.
Ponadto uwzględniono przyjmowanie przez kobiety ciężarne preparatów zawierających DHA.
Wyniki: Stwierdzono małe spożycie DHA z dietą (mediana: 60 mg dziennie). Po uwzględnieniu suplementacji diety
spożycie to wzrosło nieznacznie (mediana: 90 mg), ponieważ tylko 28% kobiet przyjmowało preparaty DHA. Ogółem
92% kobiet spożywało mniej, niż 200 mg DHA w ciągu dnia. Niewielkie spożycie DHA było konsekwencją zbyt małego
spożycia ryb – średnio 15 g/dzień. Tylko 10% kobiet jadło ryby dwa razy w tygodniu i w przypadku takiego spożycia
pobranie DHA z dietą było istotnie statystycznie większe (mediana: 160 mg dziennie) (p=0.0232). Łączne spożycie DHA
i EPA w całej grupie kobiet wynosiło – mediana 79 mg dziennie.
Wnioski: Dieta kobiet ciężarnych jest w dużym stopniu niedoborowa w DHA. Bez suplementacji diety nie jest możliwa
realizacja zaleceń, dotyczących spożycia tego składnika w okresie ciąży
Assessing patients’ attitudes towards dietary supplements
Background. There is currently many over the counter products on the market that exert nutritional or physiological effects
on the human body. The differences between dietary supplements and non-prescription drugs are however poorly understood
by the average consumer and may thus affect their expectations as to the desired effect produced on the body.
Objectives. To evaluate patients’ knowledge and attitudes towards dietary supplements as compared to non-prescription drugs.
Materials and Methods. Subjects were 335 patients of the Mazowiecki Voivodeship Hospital in Warsaw, Poland. The data
were collected from a face-to-face interview using a single and multiple choice questionnaire with 10 questions on dietary
supplements. Statistical analysis used the Chi-square (χ2) test.
Results. The majority of respondents were found to be familiar with the term ‘dietary supplements’, but had difficulties in
classifying these products into appropriate categories. Over 55% do not consider dietary supplements to be foodstuffs and
more than 40% considered such products to be drugs. Most respondents thought that the main purpose of taking dietary
supplements is to improve nutrition, but over one third expected them to also treat disease. Over 70% declared taking notice
to which category the non-prescription products they bought belongs to ie. whether non-prescription drugs (medicinal
products) or dietary supplements.
Conclusions. Many patients mistakenly believe that dietary supplements are drugs and can be used to treat disease and
health disorders.Wprowadzenie. W obrocie handlowym znajduje się wiele preparatów, dostępnych bez recepty, przeznaczonych do stosowania
w celach odżywczych lub fizjologicznych w organizmie. Przeciętny konsument może nie dostrzegać różnicy pomiędzy
suplementami diety a preparatami farmaceutycznymi, co z kolei może wpływać na jego oczekiwania dotyczące działania
tych preparatów.
Cel badań. Celem badań była ocena wiedzy i postaw pacjentów wobec suplementów diety oraz ocena tych produktów na
tle innych preparatów dostępnych bez recepty.
Materiał i metody. Badanie zostało przeprowadzone wśród 335 pacjentów Mazowieckiego Szpitala Wojewódzkiego
w Warszawie za pomocą bezpośredniego wywiadu.
Wyniki. Badania wykazały, że większości ankietowanym znane jest określenie „suplement diety”, ale trudności sprawiała
im kwalifikacja tych produktów do właściwej kategorii. Ponad 55% ankietowanych nie traktuje suplementów diety, jako
żywność, a ponad 40% zalicza je do leków. Najwięcej respondentów uważało, że głównym celem stosowania suplementów
diety jest poprawa sposobu odżywiania, ale ponad jedna trzecia z nich oczekuje również leczenia chorób. Ponad 70%
respondentów zadeklarowało, że kupując preparat bez recepty zwraca uwagę na kategorię (lek – suplement diety).
Wnioski. Wielu pacjentów błędnie sądzi, że suplementy diety to leki, które mogą być stosowane do leczenia chorób i zaburzeń
stanu zdrowia
Dielectric and Electrooptic Investigations of B Phases οf Banana-Shaped Thioesters
Electrooptic and dielectric measurements were done for B phases of two banana-shaped homologues 1,3-phenylene bis{4-[(4-nonylo-xy-benzoyl)sulfanyl]benzoate (9OSOR) and 1,3-phenylene bis{4-[(4-dodecylo-xy-benzoyl)sulfanyl]benzoate (12OSOR). Polarizing microscopy allowed to identify phase for 9OSOR and phase for 12OSOR on the basis of texture observation. Spontaneous polarization measurements were performed using reversal current method. The current response to applied triangular voltage shows that phase is a ferroelectric and phase - antiferroelectric one for which two well separated peaks were observed. Polarization for phase of 9OSOR is rather small and its temperature dependence is unusual for ferroelectric liquid crystals - it increases with temperature. Spontaneous polarization for phase of 12OSOR compound is of about 600 nC/. Dielectric spectra measured with bias field for phase of 12OSOR show two well separated relaxation processes. In the low frequency range the relaxation process is connected with fluctuations of ferroelectric domains. The relaxation process in the high frequency range appearing also without bias field is connected with molecular reorientation. The dielectric spectra measured for phase of 9OSOR with and without bias voltage showed only one dielectric relaxation process connected with molecular reorientation around the short axis
