    Clinical management of gastric cancer: results of a multicentre survey

    <p>Abstract</p> <p>Background</p> <p>The National Comprehensive Cancer Network clinical practice guidelines in oncology-gastric cancer guidelines have been widely used to provide appropriate recommendations for the treatment of patients with gastric cancer. The aim of this study was to examine the adherence of surgical oncologists, medical oncologists, and radiation oncologists' to the recommended guidelines.</p> <p>Methods</p> <p>A questionnaire asking the treatment options for gastric cancer cases was sent to 394 Chinese oncology specialists, including surgical oncologists, medical oncologists, and radiation oncologists working in hospitals joined in The Western Cooperative Gastrointestinal Oncology Group of China. The questionnaire involved a series of clinical scenarios regarding the interpretation of surgery, neoadjuvant, adjuvant, and advanced treatment planning of gastric cancer.</p> <p>Results</p> <p>Analysis of 358 respondents (91%) showed variations between each specialization and from the recommended guidelines in the management approaches to specific clinical scenarios. The majority of specialists admitted that less than 50% of patients received multidisciplinary evaluation before treatment. The participants gave different responses to questions involving adjuvant, neoadjuvant, and advanced settings, compared to the recommended guidelines.</p> <p>Conclusions</p> <p>These results highlight the heterogeneity of the treatment of gastric cancer. Surgical oncologists, medical oncologists, and radiation oncologists are not adhering to the recommended guidelines.</p

    Chitosan Modification and Pharmaceutical/Biomedical Applications

    Chitosan has received much attention as a functional biopolymer for diverse applications, especially in pharmaceutics and medicine. Our recent efforts focused on the chemical and biological modification of chitosan in order to increase its solubility in aqueous solutions and absorbability in the in vivo system, thus for a better use of chitosan. This review summarizes chitosan modification and its pharmaceutical/biomedical applications based on our achievements as well as the domestic and overseas developments: (1) enzymatic preparation of low molecular weight chitosans/chitooligosaccharides with their hypocholesterolemic and immuno-modulating effects; (2) the effects of chitin, chitosan and their derivatives on blood hemostasis; and (3) synthesis of a non-toxic ion ligand—D-Glucosaminic acid from Oxidation of D-Glucosamine for cancer and diabetes therapy

    Measurement and Evaluation of Convergence of Japan’s Marine Fisheries and Marine Tourism

    This study attempts to examine the convergence development of the marine fishery (MF) and marine tourism (MT) industries of Japan through the theory of industrial relevance. First, the current MF and MT situation in Japan is introduced to analyze the mechanism of the integration of the two industries. Second, a Vector Autoregression Model (VAR) is built to examine the relationship between MF and MT. In addition, the shock potential contributions of the MF and MT industries are identified using impulse response and variance decomposition. Results show that the impact of MF on MT is more significant than that of MT on MF. However, the interaction between MF and MT tends to stabilize in the long run. Third, the industrial integration case of Japan’s Himakajima Island is selected to analyze the MF and MT integration mechanism. The integration of MF and MT can reduce transaction costs, make full use of labor, and promote the development of the local economy. Therefore, attention should be paid to the integration of the MF and MT industries, rather than partial implementation, to balance the development of the marine economy. Finally, relevant suggestions and measures are presented for marine industry transformation and upgrading, industrial integration, and green ecological development

    Effect of Different Fertilization Measures on Soil Salinity and Nutrients in Salt-Affected Soils

    Saline soil from the coast is a valuable and readily available resource. It is also a valuable resource for reserving arable land. Adding organic fertilizers to salinized soils is an effective method of enhancement. However, saline soils cannot be improved using a single measure, and the effects of compound measures of organic fertilizers combined with mineral elements, such as humic acid, are significant and worthy of further examination. To explore the effects of various measures on the features of pH, electrical conductivity (EC), and nutrient changes in coastal salinized soils in Yancheng, Jiangsu Province, a ryegrass–alfalfa rotation with organic fertilizer and compound measures was designed. The findings indicated that the total nitrogen (TN) content of the soil increased and that all organic fertilizer composites decreased the electrical conductivity of the surface soil. However, the organic fertilizer with microbial fertilizer and humic acid was especially effective at regulating the pH and electrical conductivity of the surface soil when salts were prone to accumulation. In conclusion, our findings highlight new approaches to lowering salinity and boosting fertility in coastal saline soils: organic fertilizer with microbial fertilizers and humic acid, as well as organic fertilizer with attapulgite clay

    Optimal spinneret layout in Von Koch curves of fractal theory based needleless electrospinning process

    Needleless electrospinning technology is considered as a better avenue to produce nanofibrous materials at large scale, and electric field intensity and its distribution play an important role in controlling nanofiber diameter and quality of the nanofibrous web during electrospinning. In the current study, a novel needleless electrospinning method was proposed based on Von Koch curves of Fractal configuration, simulation and analysis on electric field intensity and distribution in the new electrospinning process were performed with Finite element analysis software, Comsol Multiphysics 4.4, based on linear and nonlinear Von Koch fractal curves (hereafter called fractal models). The result of simulation and analysis indicated that Second level fractal structure is the optimal linear electrospinning spinneret in terms of field intensity and uniformity. Further simulation and analysis showed that the circular type of Fractal spinneret has better field intensity and distribution compared to spiral type of Fractal spinneret in the nonlinear Fractal electrospinning technology. The electrospinning apparatus with the optimal Von Koch fractal spinneret was set up to verify the theoretical analysis results from Comsol simulation, achieving more uniform electric field distribution and lower energy cost, compared to the current needle and needleless electrospinning technologies

    Effects of Different Afforestation Years on Soil Properties and Quality

    As an important means of curbing soil degradation, afforestation has a profound impact on regional soil properties and quality. However, it is still unclear regarding how to conduct a systematic assessment of soil properties and soil quality and the impact of vegetation characteristics and plant functional traits in leguminous plantations with different afforestation years in drylands. Therefore, we investigated the vegetation characteristics and determined the functional traits of leaves and roots and the soil physicochemical properties of Caragana korshinskii plantations with 13, 35, and 55 years. The results showed that tree height; crown diameter (CD); root dry matter content; root water content; soil clay, silt, and sand contents; bulk density (BD); soil water content; soil organic carbon (SOC); total nitrogen (TN); available nitrogen; total phosphorus (TP); available phosphorus (AP); and soil quality index (SQI) changed significantly with an increase in afforestation years. Although the specific leaf area did not show a significant variation, it had a significant negative effect on soil properties and SQI, except for soil sand and BD. Soil sand and BD decreased with the afforestation succession, but the succession pattern of soil clay, silt, SOC, TN, TP, and AP was 13 years &lt; 35 years &lt; 55 years, and SQI increased from 0.20 (13 years) to 0.77 (55 years). This indicated that long-term legume afforestation led to the transformation of soil texture from silty loam to silt and significantly improved the soil properties and quality in the study area

    A system review of central nervous system tumors on children in China: epidemiology and clinical characteristics

    Abstract Background Central nervous system (CNS) tumors are the most common solid tumors in children and the leading cause of cancer-related death in the latter. Currently, the incidence rate exceeds that of leukemia and ranks first in the incidence of malignant tumors in children. Methods The epidemiological data on childhood CNS tumors were collected from the Chinese Cancer Registry Annual Report. The annual percent change (APC) of incidence and mortality-rate changes were estimated via Joinpoint regression. Due to a lack of pertinent data, we performed a system review on the clinical-pathological characteristics in Chinese publications. Results There was no significant increase in the incidence rate (APC: -0.1, 95% CI: -1.5 to 1.3), but there was a significant increase in the mortality rate (APC: 1.8, 95% CI: 0.3 to 3.4) for childhood CNS tumors. In the subgroup analysis, there were significant increases in both the incidence and mortality rates in rural areas (APC in the incidence: 6.2, 95% CI: 2.4 to 10.2; APC in mortality: 4.4, 95% CI: 0.4 to 8.4). The most common location and type of childhood CNS were, respectively, the cerebral hemisphere (25.5%, 95% CI: 21.7% to 29.4%) and astrocytomas (26.8%, 95% CI: 23.9% to 29.6%). Conclusions The epidemiological trends, and the relevant prediction, highlighted the need to pay continual attention to childhood CNS tumors, and the clinicopathology evinced its own distinctive characteristics. Timely detection and effective treatment must be further promoted regarding childhood CNS tumors with a view to decreasing the disease burden, especially in rural areas

    Factors Affecting the Survival of Patients with Oligometastatic Non-Small-Cell Lung Cancer: A Meta-Analysis

    Background. The aim was to investigate the potential factors related with overall survival of oligometastatic non-small-cell lung cancer (NSCLC) patients. Methods. A literature search was conducted in databases including PubMed, Embase, and Cochrane library up to March 2017. The hazard radio (HR) as well as the corresponding 95% confidence interval (CI) were calculated, and all the statistics analysis was performed by the R 3.12. Heterogeneity was analyzed using I-squared and Cochran Q tests. Furthermore, sensitivity analysis was performed to evaluate the stability of results. Results. In total, 6 articles were included in the meta-analysis. Nodal status was significantly correlated with the overall survival rate of NSCLC oligometastatic patients (HR: 1.69, 95% CI: 1.23–2.32, Z=3.20, P=0.001). No significant relationship was found between overall survival rate of NSCLC oligometastatic patients and the indicators including sex, stage, smoker, age, and histology. Notably, sensitivity analysis on data evaluating relationship between patients survival and the stage and histology showed that results were reversed after removing one of the studies. Conclusions. Nodal status might be associated with the overall survival of oligometastatic NSCLC patients
