    The Effect of Psychiatric Rehabilitation on the Activity and Participation Level of Clients with Long-Term Psychiatric Disabilities

    During the last decades of the 20th century, many psychiatric hospitals changed the living environments of their clients with long-term psychiatric disabilities. We investigated the effect of this environmental psychiatric rehabilitation and normalization process on the activity and participation level of such clients residing in one Dutch psychiatric hospital. The seven years of panel research demonstrated that more normal living environments have a positive effect on clients’ activity and participation level. This is controlled for the fact that younger clients, and clients with a relative high activity and participation level were selected for these normal living environments.

    Two-Step 3D-Guided Supramalleolar Osteotomy to Treat Varus Ankle osteoarthritis

    Background: Success of valgus-type supramalleolar osteotomy (SMOT) depends on adequate correction of malalignment, which can be hard to achieve with current 2-dimensional (2D) planning and operative techniques. A personalized digital 3-dimensional (3D) workflow to virtually plan and perform a 2-step 3D-guided medial opening (MO) SMOT has the potential to improve precision of correction. Methods: Computed tomography (CT)-based Proplan medical 3D models were made to virtually plan the desired MO SMOT, and exported to 3-Matic medical to develop patient-specific 2-step cutting and wedge guides. Workflow accuracy was tested in this limited clinical pilot study (3 patients) by comparing the virtual planned position of the osteotomized distal tibial fragment with the I -year post-MO SMOT configuration. Two millimeters or less translation deviation in every plane was defined as accurate. Results: Primary outcome analysis of the osteotomized distal tibial fragment deviation showed a median translation in all planes of 0.7 (range 0-8.2) mm (interquartile range 1.55) with an excellent interrater reliability of the measurements (intraclass correlation coefficient 0.998). There was a strong reduction in ankle pain as reflected by an increase of the AOFAS-AH score and decrease of NRS pain score with an unrestricted hindfoot motion 1 year after surgery. Conclusion: 3D virtually planned bone cutting and wedge guides is a promising approach associated with minimal postoperative deviation from the desired correction in medial opening supramalleolar osteotomy

    Ethnicity, youth cultural participation, and cultural reproduction in the Netherlands

    The purpose of this research is the explanation of differences in cultural participation of adolescents of different ethnic backgrounds. Six hundred ninety-eight Dutch and ethnic minority adolescents in a large city in the Netherlands filled in a questionnaire about their active cultural participation (e.g., playing musical instruments, dancing, acting, drawing) and receptive cultural participation (e.g., going to classical concerts, plays, museums). The expected lower cultural participation among minority youth (i.e., youth whose parents had both been born in Morocco, Turkey or former Dutch colonies such as Surinam and the Dutch Antilles) was limited toMoroccan and Turkish youths’ receptive cultural participation. Contrary to the acculturation hypothesis, we did not find any indication of a growing resemblance in youth cultural participation between the different ethnic groups. In keeping with Bourdieu’s reproduction theory, it was examined whether well-educated parents with ample cultural capital raise children who are also successful in acquiring educational and cultural capital. The findings suggest that mothers play a key role. Mothers exert a far greater cultural influence on their daughters than on their sons.Moreover, the cultural influence of mothers on daughters is much stronger than that of fathers. We conclude that intergenerational cultural reproduction affects Dutch and ethnic minority children in the same way

    Symptomatic and Functional Remission in Young Adults with a Psychotic Disorder in a Rehabilitation Focused Team

    The aim of this study is to assess symptomatic remission (SR) and functional remission (FR) in a rehabilitation focused program for young adults with a psychotic disorder in the Netherlands, and to investigate which individual and mental health care factors are associated with SR and/or FR, by using Routine Outcome Monitoring data and data on met needs and unmet needs for care. Data of 287 young adults were collected. Almost 40% achieved or maintained SR, 34% FR, and 26% achieved or maintained both. In addition to sociodemographic factors, living independently, paid employment, higher levels of compliance with treatment, and better fulfillment of unmet needs for care in relation to psychological distress, company and daytime activities were associated with better outcomes on SR and/or FR. Our findings underscore that to successfully improve and sustain remission in young adults with a psychotic disorder, it is needed to conduct specific research into the relationship between SR and FR

    CLASH: Extreme Emission Line Galaxies and Their Implication on Selection of High-Redshift Galaxies

    We utilize the CLASH (Cluster Lensing And Supernova survey with Hubble) observations of 25 clusters to search for extreme emission-line galaxies (EELGs). The selections are carried out in two central bands: F105W (Y105) and F125W (J125), as the flux of the central bands could be enhanced by the presence of [O III] 4959, 5007 at redshift of about 0.93-1.14 and 1.57-1.79, respectively. The multi-band observations help to constrain the equivalent widths of emission lines. Thanks to cluster lensing, we are able to identify 52 candidates down to an intrinsic limiting magnitude of 28.5 and to a rest-frame [O III] 4959,5007 equivalent width of about 3737 angstrom. Our samples include a number of EELGs at lower luminosities that are missed in other surveys, and the extremely high equivalent width can be only found in such faint galaxies. These EELGs can mimic the dropout feature similar to that of high redshift galaxies and contaminate the color-color selection of high redshift galaxies when the S/N ratio is limited or the band coverage is incomplete.     CLASH-X: A Comparison of Lensing and X-ray Techniques for Measuring the Mass Profiles of Galaxy Clusters

    We present profiles of temperature (Tx), gas mass, and hydrostatic mass estimated from new and archival X-ray observations of CLASH clusters. We compare measurements derived from XMM and Chandra observations with one another and compare both to gravitational lensing mass profiles derived with CLASH HST and ground-based lensing data. Radial profiles of Chandra and XMM electron density and enclosed gas mass are nearly identical, indicating that differences in hydrostatic masses inferred from X-ray observations arise from differences in Tx measurements. Encouragingly, cluster Txs are consistent with one another at ~100-200 kpc radii but XMM Tx systematically decline relative to Chandra Tx at larger radii. The angular dependence of the discrepancy suggests additional investigation on systematics such as the XMM point spread function correction, vignetting and off-axis responses. We present the CLASH-X mass-profile comparisons in the form of cosmology-independent and redshift-independent circular-velocity profiles. Ratios of Chandra HSE mass profiles to CLASH lensing profiles show no obvious radial dependence in the 0.3-0.8 Mpc range. However, the mean mass biases inferred from the WL and SaWLens data are different. e.g., the weighted-mean value at 0.5 Mpc is = 0.12 for the WL comparison and = -0.11 for the SaWLens comparison. The ratios of XMM HSE mass profiles to CLASH lensing profiles show a pronounced radial dependence in the 0.3-1.0 Mpc range, with a weighted mean mass bias of value rising to ~0.3 at ~1 Mpc for the WL comparison and of 0.25 for SaWLens comparison. The enclosed gas mass profiles from both Chandra and XMM rise to a value 1/8 times the total-mass profiles inferred from lensing at 0.5 Mpc and remain constant outside of that radius, suggesting that [8xMgas] profiles may be an excellent proxy for total-mass profiles at >0.5 Mpc in massive galaxy clusters.Comment: Accepted to ApJ; 24 pages; scheduled to appear in the Oct 10, 2014 issue.     A highly magnified candidate for a young galaxy seen when the Universe was 500 Myrs old

    The early Universe at redshift z\sim6-11 marks the reionization of the intergalactic medium, following the formation of the first generation of stars. However, those young galaxies at a cosmic age of \lesssim 500 million years (Myr, at z \gtrsim 10) remain largely unexplored as they are at or beyond the sensitivity limits of current large telescopes. Gravitational lensing by galaxy clusters enables the detection of high-redshift galaxies that are fainter than what otherwise could be found in the deepest images of the sky. We report the discovery of an object found in the multi-band observations of the cluster MACS1149+22 that has a high probability of being a gravitationally magnified object from the early universe. The object is firmly detected (12 sigma) in the two reddest bands of HST/WFC3, and not detected below 1.2 {\mu}m, matching the characteristics of z\sim9 objects. We derive a robust photometric redshift of z = 9.6 \pm 0.2, corresponding to a cosmic age of 490 \pm 15Myr (i.e., 3.6% of the age of the Universe). The large number of bands used to derive the redshift estimate make it one of the most accurate estimates ever obtained for such a distant object. The significant magnification by cluster lensing (a factor of \sim15) allows us to analyze the object's ultra-violet and optical luminosity in its rest-frame, thus enabling us to constrain on its stellar mass, star-formation rate and age. If the galaxy is indeed at such a large redshift, then its age is less than 200 Myr (at the 95% confidence level), implying a formation redshift of zf \lesssim 14. The object is the first z>9 candidate that is bright enough for detailed spectroscopic studies with JWST, demonstrating the unique potential of galaxy cluster fields for finding highly magnified, intrinsically faint galaxies at the highest redshifts.Comment: Submitted to the Nature Journal. 39 Pages, 13 figure

    The impact of bed rest on human skeletal muscle metabolism

    Insulin sensitivity and metabolic flexibility decrease in response to bed rest, but the temporal and causal adaptations in human skeletal muscle metabolism are not fully defined. Here, we use an integrative approach to assess human skeletal muscle metabolism during bed rest and provide a multi-system analysis of how skeletal muscle and the circulatory system adapt to short- and long-term bed rest (German Clinical Trials: DRKS00015677). We uncover that intracellular glycogen accumulation after short-term bed rest accompanies a rapid reduction in systemic insulin sensitivity and less GLUT4 localization at the muscle cell membrane, preventing further intracellular glycogen deposition after long-term bed rest. We provide evidence of a temporal link between the accumulation of intracellular triglycerides, lipotoxic ceramides, and sphingomyelins and an altered skeletal muscle mitochondrial structure and function after long-term bed rest. An intracellular nutrient overload therefore represents a crucial determinant for rapid skeletal muscle insulin insensitivity and mitochondrial alterations after prolonged bed rest
