    Disseminated Mycobacterium avium infection in a cat on long-term ciclosporin therapy and potential latent infection of an in-contact cat

    Case summary: An 8-year-old domestic shorthair cat receiving long-term ciclosporin treatment was evaluated for a history of weight loss and hyporexia. The main clinical finding was a cluster of enlarged mesenteric lymph nodes. Cytological examination of fine-needle aspirates showed granulomatous inflammation with abundant acid-fast bacilli. A diagnosis of Mycobacterium avium complex (MAC) infection was confirmed by PCR. The cat’s clinical condition deteriorated rapidly despite appropriate antibiotic treatment and it was euthanased 2 weeks after initial presentation due to development of severe paraparesis and ataxia. Post-mortem examination revealed granulomatous inflammation affecting multiple lymph nodes and other organs with intrahistiocytic acid-fast bacilli consistent with mycobacteria when stained using Ziehl–Neelsen stain. Another cat in the same household was screened for infection using the interferon gamma release assay (IGRA), with the result being consistent with infection by non-tuberculous mycobacteria (NTM), which includes MAC; however, it had no grossly detectable disease. Relevance and novel information: This case report is an unusual presentation of disseminated MAC infection in a cat, which remains a rare diagnosis. Clinicians should be aware of unusual and rare presentations of this infection. The clinical findings, progression of disease and histopathology results add to the current clinical database for feline disseminated MAC infections. Another cat in the same household tested positive for NTM by IGRA without any gross disease. This was suggestive of latent MAC infection which, to our knowledge, has not been previously reported in an in-contact cat

    Case report: Survival after deliberate strychnine self-poisoning, with toxicokinetic data

    INTRODUCTION: Strychnine poisoning is uncommon, and in most severe cases, the patient dies before reaching hospital. The management of strychnine poisoning is well documented, although there are few data on the kinetics of elimination of strychnine after overdose. CASE REPORT: A 42-year-old man presented shortly after ingestion of an unknown quantity of strychnine powder. After a respiratory arrest, with intensive supportive management requiring admission to an intensive care unit, he survived. Eight serum samples were taken over the first 5 days and analysed subsequently for strychnine concentrations. RESULTS: The initial concentration at 1.5 hours after ingestion was 4.73 mg/l, falling to 0.38 mg/l at 74 hours postingestion. Serum concentrations followed a monoexponential elimination curve with a calculated elimination half-life of 12 hours. DISCUSSION AND CONCLUSION: Strychnine poisoning presents with classical features, and with early diagnosis and supportive management, the patient can survive. The initial serum concentration of 4.73 mg/l is the highest reported concentration in a patient who has survived. Previous reports of the elimination half-life have suggested it is between 10 and 16 hours, which conforms to the elimination data in our case

    Polyphenol oxidase-mediated protection against oxidative stress is not associated with enhanced photosynthetic efficiency

    Background and Aims Polyphenol oxidases (PPOs) catalyse the oxidation of monophenols and/or o-diphenols to highly reactive o-quinones, which in turn interact with oxygen and proteins to form reactive oxygen species (ROS) and typical brown-pigmented complexes. Hence PPOs can affect local levels of oxygen and ROS. Although the currently known substrates are located in the vacuole, the enzyme is targeted to the thylakoid lumen, suggesting a role for PPOs in photosynthesis. The current study was designed to investigate the potential involvement of PPOs in the photosynthetic response to oxidative stress. Methods Photosynthesis (A, F(v)/F(m), ΦPSII, q(N), q(P), NPQ) was measured in leaves of a wild-type and a low-PPO mutant of red clover (Trifolium pratense ‘Milvus’) under control conditions and under a stress treatment designed to induce photooxidative stress: cold/high light (2 °C/580 µmol m(2 )s(–1)) or 0–10 µm methyl viologen. Foliar protein content and oxidation state were also determined. Key Results Photosynthetic performance, and chlorophyll and protein content during 4 d of cold/high light stress and 3 d of subsequent recovery under control growth conditions showed similar susceptibility to stress in both lines. However, more extensive oxidative damage to protein in mutants than wild-types was observed after treatment of attached leaves with methyl viologen. In addition, PPO activity could be associated with an increased capacity to dissipate excess energy, but only at relatively low methyl viologen doses. Conclusions The presence of PPO activity in leaves did not correspond to a direct role for the enzyme in the regulation or protection of photosynthesis under cold stress. However, an indication that PPO could be involved in cellular protection against low-level oxidative stress requires further investigation

    Interventions for Speech Perception in Noise in Children with Single-Sided Deafness: A Systematic Review

    Unilateral sensorineural hearing loss (UHL) in children can have a significant effect on language development, educational outcomes, and performance on localization tasks and speech perception in noise. Although the effects of bilateral hearing loss in children have been well-documented, less research has focused on the effects of unilateral hearing loss and single-sided deafness in this population. Children with UHL are unable to take advantage of binaural benefits, and therefore have shown decreased performance on tests of speech perception in noise and localization tasks (Reeder, Cadieux, & Firszt, 2015). These challenges are evident in their educational performance and speech and language outcomes (Bess & Tharpe, 1986; Lieu et al., 2010). Although several intervention options exist for UHL, in our experience the most common recommendation for children with UHL is preferential seating in the classroom combined with a "watch and wait" approach. For those with severe to profound UHL, also known as single-sided deafness, intervention in the form of air conduction hearing aids has often been unsuccessful. Current treatment options for SSD in adults include contralateral routing of signal (CROS) hearing aids and bone conduction hearing aids. Cochlear implantation is also becoming a more popular treatment for adults and investigations are beginning with children

    The Personal Genome Project-UK, an open access resource of human multi-omics data

    Integrative analysis of multi-omics data is a powerful approach for gaining functional insights into biological and medical processes. Conducting these multifaceted analyses on human samples is often complicated by the fact that the raw sequencing output is rarely available under open access. The Personal Genome Project UK (PGP-UK) is one of few resources that recruits its participants under open consent and makes the resulting multi-omics data freely and openly available. As part of this resource, we describe the PGP-UK multi-omics reference panel consisting of ten genomic, methylomic and transcriptomic data. Specifically, we outline the data processing, quality control and validation procedures which were implemented to ensure data integrity and exclude sample mix-ups. In addition, we provide a REST API to facilitate the download of the entire PGP-UK dataset. The data are also available from two cloud-based environments, providing platforms for free integrated analysis. In conclusion, the genotype-validated PGP-UK multi-omics human reference panel described here provides a valuable new open access resource for integrated analyses in support of personal and medical genomics

    Spectral Reflectance as a Covariate for Estimating Pasture Productivity and Composition

    Pasturelands are inherently variable. It is this variability that makes sampling as well as characterizing an entire pasture difficult. Measurement of plant canopy reflectance with a ground-based radiometer offers an indirect, rapid, and noninvasive characterization of pasture productivity and composition. The objectives of this study were (i) to determine the relationships between easily collected canopy reflectance data and pasture biomass and species composition and (ii) to determine if the use of pasture reflectance data as a covariate improved mapping accuracy of biomass, percentage of grass cover, and percentage of legume cover across three sampling schemes in a central Iowa pasture. Reflectance values for wavebands most highly correlated with biomass, percentage of grass cover, and percentage of legume cover were used as covariates. Cokriging was compared with kriging as a method for estimating these parameters for unsampled sites. The use of canopy reflectance as a covariate improved prediction of grass and legume percentage of cover in all three sampling schemes studied. The prediction of above-ground biomass was not as consistent given that improvement with cokriging was observed with only one of the sampling schemes because of the low amount of spatial continuity of biomass values. An overall improvement in root mean square error (RMSE) for predicting values for unsampled sites was observed when cokriging was implemented. Use of rapid and indirect methods for quantifying pasture variability could provide useful and convenient information for more accurate characterization of time consuming parameters, such as pasture composition
