    Motorcycle trauma is a serious issue in Queensland and throughout Australia; the fatality rate per 100 million kilometres travelled for motorcycle riders in Australia is nearly 30 times the rate for drivers of other vehicles (Australian Transport Safety Bureau, 2002). In 2009, the then Queensland Transport (later the Department of Transport and Main Roads or TMR) appointed CARRS-Q to provide a three-year program of Road Safety Research Services for Motorcycle Rider Safety. Funding for this research originated from the Motor Accident Insurance Commission. This program of research was undertaken to produce knowledge to assist TMR to improve motorcycle safety by further strengthening the licensing and training system to make learner riders safer by developing a pre-learner package (Deliverable 1), and by evaluating the Q-Ride CAP program to ensure that it is maximally effective and contributes to the best possible training for new riders (Deliverable 2), and identifying potential new licensing components that will reduce the incidence of risky riding and improve higher-order cognitive skills in new riders (Deliverable 3)

    PGC-1α mediated muscle aerobic adaptations to exercise, heat and cold exposure

    PGC-1α is regarded as a key regulator of mitochondrial biogenesis due to its central role in regulating the activity of key transcription factors associated with encoding mitochondrial components. Additionally, PGC-1α has shown to mediate adaptations that increase fat metabolism and angiogenesis, contributing to the overall oxidative phenotype of the muscle. While it is well established that exercise is a potent stimulator of PGC-1α, recent evidence indicates that heat and cold exposures may also influence mitochondrial biogenesis through the up-regulation of PGC-1α. This highlights the potential use of these modalities in conjunction with exercise to enhance training adaptations. As such, the purpose of this review is to describe the possible mechanisms and pathways by which exercise, as well as hot and cold exposures may influence mitochondrial biogenesis. It is clear that changes in intracellular calcium, oxidative stress and phosphorylation potential are major up-regulators of PGC-1α during exercise. Moreover, there is evidence implicating calcium signalling, in addition to β-adrenergic activation in cold-induced mitochondrial biogenesis, while PGC-1α during heat exposure is likely triggered by changes in phosphorylation potential and nitric oxide signalling. However, these mechanisms appear to change considerably when cold/heat is administered following exercise, and seem to be dependent on the experimental models used (i.e. in vitrovs. in vivo, rodent vs. human). Understanding the effects heat/cold exposure and its interaction with exercise may lead to the optimisation and development of temperature-related interventions to enhance training adaptations, or aid in the treatment of mitochondrial related diseases

Deliverable 3 of the program identified potential new licensing components that will reduce the incidence of risky riding and improve higher-order cognitive skills in new riders. While fatality and injury rates for learner car drivers are typically lower than for those with intermediate licences, this pattern is not found for learner motorcycle riders. Learner riders cannot be supervised as effectively as learner car drivers and errors are more likely to result in injury for learner riders than learner drivers. It is therefore imperative to improve safety for learner riders. Deliverable 1 examines the potential for improving the motorcycle learner and licence scheme by introducing a pre-learner motorcycle licensing and training scheme within Queensland. The tasks undertaken for Deliverable 1 were a literature review, analysis of learner motorcyclist crash and licensing data, and the development of a potential pre-learner motorcycle rider program

    Effect of liquid versus ice slurry ingestion on core temperature during simulated mining conditions

    Miners in Australia frequently perform physically demanding work under high ambient temperatures and humidity, often whilst wearing personal protective equipment, leading to heat-related illnesses. This study investigated effects of replacing 100% and 50% sweat losses with 5% carbohydrate liquid or ice-slurry solution on core temperature during simulated mining conditions. Five randomized treadmill trials were performed with: no fluid replacement (NF), 100% (100 ICE) and 50% (50 ICE) sweat loss replaced with ice-slurry (~-1°C) solution and 100% (100 LIQ) and 50% (50 LIQ) sweat loss replaced with liquid (~+4°C) solution. Time to exhaustion was longer in 100 ICE followed by 100 LIQ, 50 ICE, 50 LIQ and NF. Change in rectal temperature was least in 100 ICE followed by 100 LIQ, 50 ICE, 50 LIQ and NF. Ingestion of ice-slurry resulted in longer time to exhaustion and slower rates of change in rectal temperature. It is recommended that ice-slurry drinks be provided to personnel to lower heat strain during hot working conditions

    Hyperthermic Fatigue Precedes a Rapid Reduction in Serum Sodium in an Ironman Triathlete: A Case Report

    Purpose: To monitor the hydration, core temperature, and speed (pace) of a triathlete performing an Ironman triathlon. Methods: A 35-year-old experienced male triathlete participated in the Western Australian Ironman triathlon on December 1, 2006. The participant was monitored for blood Na+ concentration before the race (PRE), at the transitions (T1 and T2), halfway through the run (R21), and after the race (POST; 2hPOST). Core body temperature (Tc; pill telemetry) was recorded continuously, and running speed (s3 stride sensor) was measured during the run. Results: The participant completed the race in 11 h 38 min, in hot conditions (26.6 ± 5.8°C; 42 ± 19% rel. humidity). His Tc increased from 37.0 to 38.6°C during the 57-min swim, and averaged 38.4°C during the 335-min bike (33.5 km·h−1). After running at 12.4 km·h−1 for 50 min in the heat (33.1°C), Tc increased to 39.4°C, before slowing to 10.0 km·h−1 for 20 min. Tc decreased to 38.9°C until he experienced severe leg cramps, after which speed diminished to 6 km·h−1 and Tc fell to 38.0°C. The athlete’s blood Na+ was constant from PRE to T2 (139–140 mEq·L−1, but fell to 131 mEq·L−1 at R21, 133 mEq·L−1 at POST, and 128 mEq·L−1 at 2hPOST. The athlete consumed 9.25 L of fluid from PRE to T2, 6.25 L from T2 to POST, and lost 2% of his body mass, indicating sweat losses greater than 15.5 L. Conclusion: This athlete slowed during the run phase following attainment of a critically high Tc and experienced an unusually rapid reduction in blood Na+ that preceded cramping, despite presenting with signs of dehydration

    Effect of a 5 min cold water immersion recovery on exercise performance in the heat

    Background This study examined the effect of a 5-min cold-water immersion (14°C) recovery intervention on repeated cycling performance in the heat. Methods 10 male cyclists performed two bouts of a 25-min constant-paced (254 (22) W) cycling session followed by a 4-km time trial in hot conditions (35°C, 40% relative humidity). The two bouts were separated by either 15 min of seated recovery in the heat (control) or the same condition with 5-min cold-water immersion (5th—10th minute), using a counterbalanced cross-over design (CP1TT1 → CWI or CON → CP2TT2). Rectal temperature was measured immediately before and after both the constant-paced sessions and 4-km timed trials. Cycling economy and Vo2 were measured during the constant-paced sessions, and the average power output and completion times were recorded for each time trial. Results Compared with control, rectal temperature was significantly lower (0.5 (0.4)°C) in cold-water immersion before CP2 until the end of the second 4-km timed trial. However, the increase in rectal temperature (0.5 (0.2)°C) during CP2 was not significantly different between conditions. During the second 4-km timed trial, power output was significantly greater in cold-water immersion (327.9 (55.7) W) compared with control (288.0 (58.8) W), leading to a faster completion time in cold-water immersion (6.1 (0.3) min) compared with control (6.4 (0.5) min). Economy and Vo2 were not influenced by the cold-water immersion recovery intervention. Conclusion 5-min cold-water immersion recovery significantly lowered rectal temperature and maintained endurance performance during subsequent high-intensity exercise. These data indicate that repeated exercise performance in heat may be improved when a short period of cold-water immersion is applied during the recovery period

    Sedation for fibre optic bronchoscopy

    AbstractMost current sedative regimens for fibre optic bronchoscopy use an opioid, a benzodiazepine or a combination of both. This study compares midazolam (M) (a benzodiazepine), alfentanil (A) (an opioid) and a combination of both drugs (M + A).One hundred and three patients were randomized in double-blind manner into groups M(35), A(33) and M + A(35). The number of coughs, number of additional aliquots of lignocaine and duration of the procedures were recorded along with oxygen desaturation. The patient's level of discomfort was assessed by patient and bronchoscopist and expressed as a visual analogue score.There were significantly fewer coughs per minute in Group A compared with Group M (P=0·0053), and significantly less lignocaine was required in Group A (P=0·005) and in Groups M + A (P<0·002) compared with Group M. There was no significant difference in the assessment of discomfort between the groups. There was a trend for Group M + A to desaturate more than the other two with a significant difference between desaturation in Group M + A and Group A (P=0·033).Alfentanil is a more effective anti-tussive agent than midazolam for outpatient fibre optic bronchoscopy. The combination of alfentanil and midazolam does not provide any better anti-tussive effect and may have the risk of a greater degree of desaturation secondary to increased sedation

    Effects of Short-Term Training With Uncoupled Cranks in Trained Cyclists

    Purpose: Manufacturers of uncoupled cycling cranks claim that their use will increase economy of motion and gross efficiency. Purportedly, this occurs by altering the muscle-recruitment patterns contributing to the resistive forces occurring during the recovery phase of the pedal stroke. Uncoupled cranks use an independentclutch design by which each leg cycles independently of the other (ie, the cranks are not fixed together). However, research examining the efficacy of training with uncoupled cranks is equivocal. The purpose of this study was to determine the effect of short-term training with uncoupled cranks on the performance-related variables economy of motion, gross efficiency, maximal oxygen uptake (VO 2max), and muscle-activation patterns. Methods: Sixteen trained cyclists were matched-paired into either an uncoupled-crank or a normal-crank training group. Both groups performed 5 wk of training on their assigned cranks. Before and after training, participants completed a graded exercise test using normal cranks. Expired gases were collected to determine economy of motion, gross efficiency, and VO 2max, while integrated electromyography (iEMG) was used to examine muscle-activation patterns of the vastus lateralis, biceps femoris, and gastrocnemius. Results: No significant changes between groups were observed for economy of motion, gross efficiency, VO 2max, or iEMG in the uncoupled- or normal-crank group. Conclusions: Five weeks of training with uncoupled cycling cranks had no effect on economy of motion, gross efficiency, muscle recruitment, or VO 2max compared with training on normal cranks
