A two-way photonic interface for linking Sr+ transition at 422 nm to the telecommunications C-band
We report a single-stage bi-directional interface capable of linking Sr+
trapped ion qubits in a long-distance quantum network. Our interface converts
photons between the Sr+ emission wavelength at 422 nm and the telecoms C-band
to enable low-loss transmission over optical fiber. We have achieved both up-
and down-conversion at the single photon level with efficiencies of 9.4% and
1.1% respectively. Furthermore we demonstrate noise levels that are low enough
to allow for genuine quantum operation in the future.Comment: 5 pages, 4 figure
8x8 Reconfigurable quantum photonic processor based on silicon nitride waveguides
The development of large-scale optical quantum information processing
circuits ground on the stability and reconfigurability enabled by integrated
photonics. We demonstrate a reconfigurable 8x8 integrated linear optical
network based on silicon nitride waveguides for quantum information processing.
Our processor implements a novel optical architecture enabling any arbitrary
linear transformation and constitutes the largest programmable circuit reported
so far on this platform. We validate a variety of photonic quantum information
processing primitives, in the form of Hong-Ou-Mandel interference, bosonic
coalescence/anticoalescence and high-dimensional single-photon quantum gates.
We achieve fidelities that clearly demonstrate the promising future for
large-scale photonic quantum information processing using low-loss silicon

Identification of Low Surface Brightness Tidal Features in Galaxies Using Convolutional Neural Networks
Faint tidal features around galaxies record their merger and interaction
histories over cosmic time. Due to their low surface brightnesses and complex
morphologies, existing automated methods struggle to detect such features and
most work to date has heavily relied on visual inspection. This presents a
major obstacle to quantitative study of tidal debris features in large
statistical samples, and hence the ability to be able to use these features to
advance understanding of the galaxy population as a whole. This paper uses
convolutional neural networks (CNNs) with dropout and augmentation to identify
galaxies in the CFHTLS-Wide Survey that have faint tidal features. Evaluating
the performance of the CNNs against previously-published expert visual
classifications, we find that our method achieves high (76%) completeness and
low (20%) contamination, and also performs considerably better than other
automated methods recently applied in the literature. We argue that CNNs offer
a promising approach to effective automatic identification of low surface
brightness tidal debris features in and around galaxies. When applied to
forthcoming deep wide-field imaging surveys (e.g. LSST, Euclid), CNNs have the
potential to provide a several order-of-magnitude increase in the sample size
of morphologically-perturbed galaxies and thereby facilitate a much-anticipated

Clinical Pharmacokinetics and Dose Recommendations for Posaconazole in Infants and Children.
OBJECTIVES: The objectives of this study were to investigate the population pharmacokinetics of posaconazole in immunocompromised children, evaluate the influence of patient characteristics on posaconazole exposure and perform simulations to recommend optimal starting doses. METHODS: Posaconazole plasma concentrations from paediatric patients undergoing therapeutic drug monitoring were extracted from a tertiary paediatric hospital database. These were merged with covariates collected from electronic sources and case-note reviews. An allometrically scaled population-pharmacokinetic model was developed to investigate the effect of tablet and suspension relative bioavailability, nonlinear bioavailability of suspension, followed by a step-wise covariate model building exercise to identify other important sources of variability. RESULTS: A total of 338 posaconazole plasma concentrations samples were taken from 117 children aged 5Â months to 18Â years. A one-compartment model was used, with tablet apparent clearance standardised to a 70-kg individual of 15Â L/h. Suspension was found to have decreasing bioavailability with increasing dose; the estimated suspension dose to yield half the tablet bioavailability was 99Â mg/m2. Diarrhoea and proton pump inhibitors were also associated with reduced suspension bioavailability. CONCLUSIONS: In the largest population-pharmacokinetic study to date in children, we have found similar covariate effects to those seen in adults, but low bioavailability of suspension in patients with diarrhoea or those taking concurrent proton pump inhibitors, which may in particular limit the use of posaconazole in these patients

Pain Reduction with Oral Methotrexate in Knee Osteoarthritis; a Randomized Placebo-Controlled Clinical Trial
Background: Treatments for osteoarthritis are limited. Previous small studies suggest the anti-rheumatic drug methotrexate may be a potential treatment for osteoarthritis pain. Objective: To assess symptomatic benefits of methotrexate in knee osteoarthritis. Design: A multi-center, randomized, double-blind, placebo-controlled trial conducted between 13 June 2014 and 8 September 2017. Setting: Fifteen United Kingdom secondary-care musculoskeletal clinics. Participants: 207 participants with symptomatic, radiographic knee osteoarthritis, knee pain (severity ≥4/10) on most days in the last 3-months, with inadequate response to current medication were approached for inclusion. Interventions: Participants were randomized 1:1 to once-weekly oral methotrexate (6-week escalation 10mg-25mg) or matched placebo over 12-months and continued usual analgesia. Measurements: The primary endpoint was average knee pain (numerical rating scale (NRS) 0-10) at 6-months, with 12-month follow-up to assess longer-term response. Secondary endpoints included knee stiffness and function outcomes, and adverse events. Results: 155 participants (64% women, mean age 60.9 years, 50% Kellgren-Lawrence Grade 3-4) were randomized to methotrexate (n=77) or placebo (n=78). Follow-up was 86% (n=134; MTX 66, Placebo 68) at 6-months. Mean(SD) knee pain reduced from 6.4(1.80) at baseline to 5.1(2.32) at 6-months in the MTX group, and from 6.8(1.62) to 6.2(2.30) in the placebo group. The primary intention-to-treat analysis revealed a statistically significant pain reduction of 0.79 NRS points in favour of MTX (95%CI[0.08-1.51];p=0.030). There were also statistically significant treatment-group differences in favour of MTX at 6-months for WOMAC stiffness (0.60 points, 95%CI[0.01-1.18];p=0.045) and function (5.01 points, 95%CI[1.29-8.74],p=0.008). Treatment-compliance analysis supported a dose-response effect. Four unrelated serious adverse events were reported (methotrexate:2, placebo:2). Limitations: Not permitting oral methotrexate to be changed to subcutaneous delivery for intolerance. Conclusions: Oral methotrexate added to usual medications demonstrated statistically significant reduction in knee osteoarthritis pain, stiffness and function at 6-months.
