    Negotiations on a free trade agreement between India and the EU: ambitions, expectations, obstacles, and incentives

    In summer 2022, the European Union (EU) and India resumed negotiations on a free trade agreement to strengthen their strategic partnership. In addition, both sides are negotiating an investment protection agreement and an agreement on the protection of geographical indications. In this way, the EU wants to diversify its relations with the Indo-Pacific states and underscore India's prominent position. Through cooperation with the EU, India aims to advance its economic and technological modernisation, which is indispensable if the country is to play a greater international role. Unlike the negotiations that failed in 2013, the current negotiations are paradoxically both sim­pler and more complicated. They are simpler because the EU and India are now more aligned on geopolitical issues than ever, especially with regard to China. But they are also more complicated because the success of the negotiations still depends on diffi­cult concessions on both sides.     Untersammelsdorf Tunnel – Challenges, Special Measures and use of Special Shotcrete for Tunneling in Lacustrine Clay

    The 665 m long, twin-track Untersammelsdorf Tunnel is being built in the course of the construction of the Koralmbahn line between Graz and Klagenfurt. The tunnel is located in extremely challenging subsoil conditions consisting of silty to fine sandy lacustrine deposits. The developed tunneling concept provided wide-ranging special underground engineering measures as piling, jet grouting and temporary struts and represents a unique construction method to date. Accordingly, numerous challenges arose in the design phase, which could be verified by carrying out extensive trials and investigations in the preparatory period or for which fallback levels had to be provided for the support system.     Verhandlungen über ein Freihandelsabkommen zwischen Indien und der EU: Ambitionen, Erwartungen, Widerstände und Anreize

    Im Sommer 2022 haben die Europäische Union (EU) und Indien erneut Verhandlungen über ein Freihandelsabkommen aufgenommen, um ihre strategische Partnerschaft zu vertiefen. Darüber hinaus verhandeln die beiden Seiten über ein Investitionsschutzabkommen sowie ein Abkommen zum Schutz geographischer Herkunfts­angaben. Die EU möchte damit ihre Beziehungen zu den Staaten im Indo-Pazifik diversifizieren und unterstreicht Indiens herausgehobenen Stellenwert. Indien will durch die Koope­ra­tion mit der EU seine wirtschaftliche und technologische Modernisierung vorantreiben, die für die angestrebte größere internationale Rolle des Landes unabdingbar ist. Anders als die 2013 gescheiterten Gespräche sind die jetzigen Verhandlungen von dem Paradox gekennzeichnet, zugleich einfacher und komplizierter zu sein. Sie sind einfacher, weil die EU und Indien heute in geopolitischen Fragen vor allem mit Blick auf China mehr Übereinstimmung haben als je zuvor. Sie sind aber auch komplizierter, weil der Erfolg der Verhandlungen weiterhin von schwierigen Zugeständnissen auf beiden Seiten abhängt.     Lessons From the EThIGII Trial: Proper Putative Benefit Assessment of Low-Molecular-Weight Heparin Treatment in M2/ANXA5 Haplotype Carriers

    This study presents sample size considerations derived from the Efficacy of Thromboprophylaxis as an Intervention during Gravidity (EThIGII) trial (ClinicalTrials.gov: NCT00400387) to address the question of low-molecular-weight heparin (LMWH) treatment in women with recurrent pregnancy loss (RPL) depending on the M2/ANXA5 haplotype. To evaluate the possible influence of such treatment on miscarriage rates of trial participants, a post hoc analysis of ANXA5 promoter genotypes in the light of M2/ANXA5 (RPRGL3) distribution was performed using logistic models. DNA for genotyping was available from 129 LMWH and 95 control patients, 44 (19.6%) of whom were M2/ANXA5 carriers. Miscarriages occurred in 1 (4.0%) of 25 M2/ANXA5 carriers from the LMWH group compared to 4 (21.1%) of 19 in the control group, resulting in an odds ratio (95% confidence interval) for miscarriage of 0.16 (0.016-1.5) for women treated with LMWH. In noncarriers, miscarriage rates were 6 (5.8%) of 104 versus 7 (9.2%) of 76 for the LMWH and the control groups, respectively, corresponding to an odds ratio for miscarriage of 0.60 (0.19-1.9). The apparent beneficial effects of miscarriage rate reduction in M2/ANXA5 carriers with RPL concur with biological considerations about improvement in reduced ANXA5 function through LMWH treatment in an adequate murine model.     Depiction of pneumothoraces in a large animal model using x-ray dark-field radiography

    The aim of this study was to assess the diagnostic value of x-ray dark-field radiography to detect pneumothoraces in a pig model. Eight pigs were imaged with an experimental grating-based large-animal dark-field scanner before and after induction of a unilateral pneumothorax. Image contrast-to-noise ratios between lung tissue and the air-filled pleural cavity were quantified for transmission and dark-field radiograms. The projected area in the object plane of the inflated lung was measured in dark-field images to quantify the collapse of lung parenchyma due to a pneumothorax. Means and standard deviations for lung sizes and signal intensities from dark-field and transmission images were tested for statistical significance using Student’s two-tailed t-test for paired samples. The contrast-to-noise ratio between the air-filled pleural space of lateral pneumothoraces and lung tissue was significantly higher in the dark-field (3.65 ± 0.9) than in the transmission images (1.13 ± 1.1; p = 0.002). In case of dorsally located pneumothoraces, a significant decrease (−20.5%; p > 0.0001) in the projected area of inflated lung parenchyma was found after a pneumothorax was induced. Therefore, the detection of pneumothoraces in x-ray dark-field radiography was facilitated compared to transmission imaging in a large animal model

    Multispectral optoacoustic tomography for non-invasive disease phenotyping in pediatric spinal muscular atrophy patients

    Proximal spinal muscular atrophy (SMA) is a rare progressive, life limiting genetic motor neuron disease. While promising causal therapies are available, meaningful prognostic biomarkers for therapeutic monitoring are missing. We demonstrate handheld Multispectral Optoacoustic Tomography (MSOT) as a novel non-invasive imaging approach to visualize and quantify muscle wasting in pediatric SMA. While MSOT signals were distributed homogeneously in muscles of healthy volunteers (HVs), SMA patients showed moth-eaten optoacoustic signal patterns. Further signal quantification revealed greatest differences between groups at the isosbestic point for hemoglobin (SWL 800 nm). The SWL 800 nm signal intensities further correlated with clinical phenotype tested by standard motor outcome measures. Therefore, handheld MSOT could enable non-invasive assessment of disease burden in SMA patients

    A randomized, controlled, prospective trial to evaluate the haemostatic effect of Lyostypt versus Surgicel in arterial bypass anastomosis: "COBBANA" trial

    <p>Abstract</p> <p>Background</p> <p>The development of suture hole bleeding at peripheral arterial bypass anastomoses using PTFE graft prostheses is a common problem in peripheral vascular surgery. Traditionally the problem is managed by compression with surgical swabs and reversal heparin or by using several haemostatic device (e.g. different forms of collagen, oxidized cellulose, gelatine sponge, ethylcyanoacrylate glue or fibrin) with various success. Preclinical data suggest that the haemostatic effect of collagen is stronger than that of oxidized cellulose, but no direct clinical comparison of their hemostatic performance has been published so far.</p> <p>Design</p> <p>This randomized, controlled, prospective trial evaluates the haemostatic effect of Lyostypt versus Surgicel in arterial bypass anastomosis. 28 patients undergoing an elective peripheral vascular reconstruction due to peripheral vascular disease will be included. Suture hole bleeding occurring at the arterial bypass anastomosis using a PTFE prostheses will be stopped by the application of Lyostypt and/or Surgicel. The proximal anastomoses will be randomized intraoperatively. The patients will be allocated into 4 different treatment groups. Group1 Lyostypt distal/Surgicel proximal; Group 2: Lyostypt proximal/Surgicel distal; Group 3: Surgicel distal and proximal; Group 4: Lyostypt distal and proximal. Primary endpoint of the study is time to haemostasis. Secondary endpoints are the number of intraoperatively used haemostatic devices, postoperative mortality within 30 days as well as the intraoperative efficacy rating of the two devices evaluated by the surgeon. As a safety secondary parameter, the local and general complication occurring till 30 ± 10 days postoperatively will also be analysed. After hospital discharge the investigator will examine the enrolled patients again at 30 days after surgery.</p> <p>Discussion</p> <p>The COBBANA trial aims to assess, whether the haemostatic effect of Lyostypt is superior to Surgicel in suture hole bleedings of arterial bypass anastomoses.</p> <p>Trial registration</p> <p>NCT00837954</p
