Current status and legal/ethical problems in the research use of the tissues of aborted human fetuses in Japan
To date, there is no law regulating the research use of human aborted fetuses in Japan. The aim was to review the current status with historical background and legal/ethical problems limiting the research use of the tissues of aborted human fetuses. We reviewed literature via PubMed, Web of Science, Scopus, Japana Centra Revuo Medicina and CiNii, reports from various committees and research groups from Ministry of Health, Labour and Welfare (MHLW), and domestic books. Aborted human fetal tissues used for research purposes were first documented in the 1920s. The first guideline, the Peel Code was released in 1972. Since then, in Western countries, the research use of aborted fetuses has been less restricted compared with that of embryos, due to the following guidelines outlined by expert groups. Currently, aborted fetal tissues are commercially available for research purposes in the United States. In Japan, only four indications are presented in āa public statement permitting research use of deceased fetuses' and āneonatesā organs, etc.ā (1987). In the 2000s, expert committees of the MHLW concluded that research use of human aborted fetuses should be discontinued, and that comprehensive rules and independent regulations should be implemented. This issue has not been discussed in the Japanese legislature since 2003. Establishment of laws and guidelines for this issue is insufficient not only in Japan but also in other countries. Effects of a Supermassive Black Hole Binary on a Nuclear Gas Disk
We study influence of a galactic central supermassive black hole (SMBH)
binary on gas dynamics and star formation activity in a nuclear gas disk by
making three-dimensional Tree+SPH simulations. Due to orbital motions of SMBHs,
there are various resonances between gas motion and the SMBH binary motion. We
have shown that these resonances create some characteristic structures of gas
in the nuclear gas disk, for examples, gas elongated or filament structures,
formation of gaseous spiral arms, and small gas disks around SMBHs. In these
gaseous dense regions, active star formations are induced. As the result, many

AKARI Infrared Camera Survey of the Large Magellanic Cloud. I. Point Source Catalog
We present a near- to mid-infrared point source catalog of 5 photometric
bands at 3.2, 7, 11, 15 and 24 um for a 10 deg2 area of the Large Magellanic
Cloud (LMC) obtained with the Infrared Camera (IRC) onboard the AKARI
satellite. To cover the survey area the observations were carried out at 3
separate seasons from 2006 May to June, 2006 October to December, and 2007
March to July.
The 10-sigma limiting magnitudes of the present survey are 17.9, 13.8, 12.4,
9.9, and 8.6 mag at 3.2, 7, 11, 15 and 24 um, respectively. The photometric
accuracy is estimated to be about 0.1 mag at 3.2 um and 0.06--0.07 mag in the
other bands. The position accuracy is 0.3" at 3.2, 7 and 11um and 1.0" at 15
and 24 um. The sensitivities at 3.2, 7, and 24 um are roughly comparable to
those of the Spitzer SAGE LMC point source catalog, while the AKARI catalog
provides the data at 11 and 15 um, covering the mid-infrared spectral range
contiguously. Two types of catalog are provided: a Catalog and an Archive. The
Archive contains all the detected sources, while the Catalog only includes the
sources that have a counterpart in the Spitzer SAGE point source catalog. The
Archive contains about 650,000, 140,000, 97,000, 43,000, and 52,000 sources at
3.2, 7, 11, 15, and 24 um, respectively. Based on the catalog, we discuss the
luminosity functions at each band, the color-color diagram, and the
color-magnitude diagram using the 3.2, 7, and 11 um band data. Stars without
circumstellar envelopes, dusty C-rich and O-rich stars, young stellar objects,
and background galaxies are located at distinct regions in the diagrams,
suggesting that the present catalog is useful for the classification of objects

Management of Cirrhotic Ascites under the Add-on Administration of Tolvaptan
Tolvaptan is a recently available diuretic that blocks arginine vasopressin receptor 2 in the renal collecting duct. Its diuretic mechanism involves selective water reabsorption by affecting the water reabsorption receptor aquaporin 2. Given that liver cirrhosis patients exhibit hyponatremia due to their pseudo-aldosteronism and usage of natriuretic agents, a sodium maintaining agent, such as tolvaptan, is physiologically preferable. However, large scale studies indicating the patients for whom this would be effective and describing management under its use have been insufficient. The appropriate management of cirrhosis patients treated with tolvaptan should be investigated. In the present review, we collected articles investigating the effectiveness of tolvaptan and factors associated with survival and summarized their management reports. Earlier administration of tolvaptan before increasing the doses of natriuretic agents is recommended because this may preserve effective arterial blood volume
History of Transcatheter Arterial Chemoembolization Predicts the Efficacy of Hepatic Arterial Infusion Chemotherapy in Hepatocellular Carcinoma Patients
This study sought to identify factors that are predictive of a therapeutic response to hepatic arterial infusion chemotherapy (HAIC) by focusing on the number of prior transcatheter arterial chemoembolization (TACE) sessions. To determine the parameters predicting a good response to HAIC, we retrospectively analyzed 170 patients with hepatocellular carcinoma (HCC) who received HAIC regimens comprising low-dose cisplatin combined with 5-fluorouracil (LFP) or cisplatin (CDDP) for the first time. In both the LFP and CDDP regimens, the response rates were significantly lower in patients with three or more prior TACE sessions than in those with two or fewer prior TACE sessions (LFP 57% versus 28%; p=0.01, CDDP 27% versus 6%; p=0.01). Multivariable logistic regression analysis revealed that the number of prior TACE sessions (ā„ 3) was significantly associated with non-responder status (odds ratio 4.17, 95% Confidence Interval (CI) 1.76-9.86) in addition to the HAIC regimen. Multivariable analysis using the Cox proportional hazards model revealed that a larger number of prior TACE sessions (ā„ 3) was a significant risk factor for survival (hazard ratio 1.60, 95% CI 1.12-2.29) in addition to Child-Pugh class, serum alpha-fetoprotein concentration, and maximum diameter of HCC. HCC patients who receive fewer prior TACE sessions (ā¤ 2) were found to be better responders to HAIC
Oxidative stress-related markers as prognostic factors for patients with primary sclerosing cholangitis in Japan
Background/purpose Primary sclerosing cholangitis (PSC) is a rare chronic liver disease. The mechanisms and prediction of PSC progression are unclear. Recent investigations have shown that general conditions, such as oxidative stress, affect the course of chronic diseases. We investigated the clinical course and oxidative stress-related condition of PSC to determine prognostic factors.
Methods We recruited 58 patients with PSC (mean age; 37.4 years, mean observation period; 1382 days) who visited our department from 2003 to 2021. Clinical characteristics were investigated to define prognostic factors. Oxidative stress status was evaluated using two types of markers: an oxidative stress marker (serum reactive oxygen metabolite; dROM) and an antioxidant marker (serum OXY adsorbent test; OXY).
Results The revised Mayo risk, ChildāPugh, model for end-stage liver disease-sodium (MELD-Na) scores or fibrosis-related FIB-4 index significantly predicted poor overall survival. High intestinal immunoglobulin A (IgA) levels predicted poor survival. Among patients with high and intermediate revised Mayo risk scores, those with physiologically high dROM levels showed better survival than those with lower dROM levels. In this population, dROM was negatively correlated with AST and IgA, which are both correlated with survival.
Conclusions High and intermediate revised Mayo risk score group predicted a poor clinical course in PSC. Additionally, the ChildāPugh score, MELD-Na score, FIB-4 index, and serum IgA were significantly correlated with survival. In patients with high and intermediate revised Mayo risk scores, physiologically high oxidative stress status correlated with low IgA levels and a good prognosis.<br
A novel intraperitoneal therapy for gastric cancer with DFPā10825, a unique RNAi therapeutic targeting thymidylate synthase, in a peritoneally disseminated xenograft model
Purpose: In advanced gastric cancer, peritoneal dissemination is a lifeāthreatening mode of metastasis. Since the treatment options with conventional chemotherapy remain limited, any novel therapeutic strategy that could control such metastasis would improve the outcome of treatment. We recently developed a unique RNA interference therapeutic regimen (DFPā10825) consisting of short hairpin RNA against thymidylate synthase (TS shRNA) and cationic liposomes. The treatment with DFPā10825 has shown remarkable antitumor activity in peritoneally disseminated human ovarian cancerābearing mice via intraperitoneal administration. In this study, we expanded DFPā10825 to the treatment of peritoneally disseminated gastric cancer.
Methods: DFPā10825 was administered intraperitoneally into mice with intraperitoneally implanted human gastric cancer cells (MKN45 or NCIāN87). Antitumor activity and host survival benefits were monitored. Intraperitoneal distribution of fluorescenceālabeled DFPā10825 was monitored in this MKN45 peritoneally disseminated mouse model.
Results: Intraperitoneal injection of DFPā10825 suppressed tumor growth in two peritoneally disseminated cancer models (MKN45 and NCIāN87) and increased the survival time of the MKN45 model without severe side effects. Throughout the treatment regimen, no significant body weight loss was associated with the administration of DFPā10825. Interestingly, after intraperitoneal injection, fluorescenceālabeled DFPā10825 retained for more than 72 hours in the peritoneal cavity and selectively accumulated in disseminated tumors.
Conclusions: Intraperitoneal injection of DFPā10825 demonstrated effective antitumor activity without systemic severe adverse effects via the selective delivery of RNAi molecules into disseminated tumors in the peritoneal cavity. Our current study indicates that DFPā10825 could become an alternative option to improve the outcomes of patients with peritoneally disseminated gastric cancer
The Early Decline of alpha-Fetoprotein and Des-gamma-Carboxy Prothrombin Predicts the Response of Hepatic Arterial Infusion Chemotherapy in Hepatocellular Carcinoma Patients
Introduction: Molecular targeting drugs are recommended as second-line treatment for intrahepatic advanced hepatocellular carcinoma (HCC). However, in Asia, hepatic arterial infusion chemotherapy (HAIC) is also considered as a second-line treatment because it improves the survival of responders. The aim of this study was to predict responders and non-responders to HAIC with low-dose cisplatin plus 5-fluorouracil (LFP) using tumor markers.
Objective and Methods: The data of 47 patients who received LFP for the first time in our hospital were analyzed retrospectively. We evaluated the association between treatment response by Response Evaluation Criteria in Solid Tumors and the changing ratio of the serum concentration of alpha-fetoprotein (AFP),Lens culinarisagglutinin-reactive fraction of AFP (AFP-L3), and des-gamma-carboxy prothrombin (DCP) 2 weeks after LFP initiation.
Results: The number of patients showing a complete response (CR), a partial response (PR), stable disease (SD), and progressive disease (PD) was 0 (0%), 20 (43%), 18 (38%), and 9 (19%), respectively. The AFP ratio showed significant positive correlations for PR vs. SD (p= 0.004) and PR vs. PD (p= 0.003). The DCP ratio correlated significantly for PR vs. SD (p= 0.02). The optimal cutoff values for responders were 0.79 for the AFP ratio and 0.53 for the DCP ratio. Prediction using both or either cutoff value showed 93% sensitivity, 53% specificity, a 94% negative predictive value, and a 57% positive predictive value.
Conclusion: Optimal cutoff values for AFP and DCP ratios enable prediction of nonresponders to HAIC with LFP. This simple and early assessment method allows the use of HAIC and molecular targeting drugs for HCC treatment
Decreased Serum Antioxidant Marker is Predictive of Early Recurrence in the Same Segment after Radical Ablation for Hepatocellular Carcinoma
Radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC) is a promising method for controlling tumors, although it does not entirely eliminate recurrence. Oxidative stress is associated with the progression of hepatocarcinogenesis, while also acting as an anticancer response. The objective of the present study was to investigate the factors influencing post-RFA outcomes. We recruited 235 newly diagnosed HCC patients who received RFA for single tumors. The patients with recurrence were sub-grouped into early and segmental recurrence groups. The characteristics of the sub-grouped patients were evaluated, including by measuring oxidative stress marker reactive oxygen metabolites and antioxidant marker OXY-adsorbent tests. The factors associated with poor survival were a high Child-Pugh score and early recurrence within 2 years in the same segment. The patients who experienced recurrence within 2 years in the same segment showed a larger tumor diameter than did others. According to a multivariate analysis, the OXY values were also significantly low in these patients. In conclusion, maintaining the antioxidant reservoir function with a high OXY value might be necessary to prevent early recurrence within the RFA-treated segment
