Assessing complex PTSD and PTSD: validation of the German version of the International Trauma Interview (ITI)
Background: With the introduction of the ICD-11 into clinical practice, the reliable distinction between Posttraumatic Stress Disorder (PTSD) and Complex Posttraumatic Stress Disorder (CPTSD) becomes paramount. The semi-structured clinician-administered International Trauma Interview (ITI) aims to close this gap in clinical and research settings.
Objective: This study investigated the psychometric properties of the German version of the ITI among trauma-exposed clinical samples from Switzerland and Germany.
Method: Participants were 143 civilian and 100 military participants, aged M = 40.3 years, of whom 53.5% were male. Indicators of reliability and validity (latent structure, internal reliability, inter-rater agreement, convergent and discriminant validity) were evaluated. Confirmatory factor analysis (CFA) and partial correlation analysis were conducted separately for civilian and military participants.
Results: Prevalence of PTSD was 30% (civilian) and 33% (military) and prevalence of CPTSD was 53% (civilians) and 21% (military). Satisfactory internal consistency and inter-rater agreement were found. In the military sample, a parsimonious first-order six-factor model was preferred over a second-order two-factor CFA model of ITI PTSD and Disturbances in Self-Organization (DSO). Model fit was excellent among military participants but no solution was supported among civilian participants. Overall, convergent validity was supported by positive correlations of ITI PTSD and DSO with DSM-5 PTSD. Discriminant validity for PTSD symptoms was confirmed among civilians but low in the military sample.
Conclusions: The German ITI has shown potential as a clinician-administered diagnostic tool for assessing ICD-11 PTSD and CPTSD in primary care. However, further exploration of its latent structure and discriminant validity are indicated
An expression signature of syndecan-1 (CD138), E-cadherin and c-met is associated with factors of angiogenesis and lymphangiogenesis in ductal breast carcinoma in situ
INTRODUCTION: Heparan sulphate proteoglycan syndecan-1 modulates cell proliferation, adhesion, migration and angiogenesis. It is a coreceptor for the hepatocyte growth factor receptor c-met, and its coexpression with E-cadherin is synchronously regulated during epithelial-mesenchymal transition. In breast cancer, changes in the expression of syndecan-1, E-cadherin and c-met correlate with poor prognosis. In this study we evaluated whether coexpression of these functionally linked prognostic markers constitutes an expression signature in ductal carcinoma in situ (DCIS) of the breast that may promote cell proliferation and (lymph)angiogenesis. METHODS: Expression of syndecan-1, E-cadherin and c-met was detected immunohistochemically using a tissue microarray in tumour specimens from 200 DCIS patients. Results were correlated with the expression patterns of angiogenic and lymphangiogenic markers. Coexpression of the three prognostic markers was evaluated in human breast cancer cells by confocal immunofluorescence microscopy and RT-PCR. RESULTS: Coexpression and membrane colocalization of the three markers was confirmed in MCF-7 cells. E-cadherin expression decreased, and c-met expression increased progressively in more aggressive cell lines. Tissue microarray analysis revealed strong positive staining of tumour cells for syndecan-1 in 72%, E-cadherin in 67.8% and c-met in 48.6% of DCIS. E-cadherin expression was significantly associated with c-met and syndecan-1. Expression of c-met and syndecan-1 was significantly more frequent in the subgroup of patients with pure DCIS than in those with DCIS and a coexisting invasive carcinoma. Levels of c-met and syndecan-1 expression were associated with HER2 expression. Expression of c-met significantly correlated with expression of endothelin A and B receptors, vascular endothelial growth factor (VEGF)-A and fibroblast growth factor receptor-1, whereas E-cadherin expression correlated significantly with endothelin A receptor, VEGF-A and VEGF-C staining. CONCLUSION: Syndecan-1, E-cadherin and c-met constitute a marker signature associated with angiogenic and lymphangiogenic factors in DCIS. This coexpression may reflect a state of parallel activation of different signal transduction pathways, promoting tumour cell proliferation and angiogenesis. Our findings have implications for future therapeutic approaches in terms of a multiple target approach, which may be useful early in breast cancer progression

The Impact of Anatomical Characteristics on the Structural Integrity of Wood
The structural integrity of wood is closely related to its brittleness and thus to its suitability for numerous applications where dynamic loads, wear and abrasion occur. The structural integrity of wood is only vaguely correlated with its density, but affected by different chemical, physico-structural and anatomical characteristics, which are difficult to encompass as a whole. This study aimed to analyze the results from High-Energy Multiple Impact (HEMI) tests of a wide range of softwood and hardwood species with an average oven-dry wood density in a range between 0.25 and 0.99 g/cm³ and multifaceted anatomical features. Therefore, small clear specimens from a total of 40 different soft- and hardwood species were crushed in a heavy vibratory ball mill. The obtained particles were fractionated and used to calculate the ‘Resistance to Impact Milling (RIM)’ as a measure of the wood structural integrity. The differences in structural integrity and thus in brittleness were predominantly affected by anatomical characteristics. The size, density and distribution of vessels as well as the ray density of wood were found to have a significant impact on the structural integrity of hardwoods. The structural integrity of softwood was rather affected by the number of growth ring borders and the occurrence of resin canals. The density affected the Resistance to Impact Milling (RIM) of neither the softwoods nor the hardwoods
No evidence for seasonal variations of the incidence of testicular germ cell tumours in Germany.
The pathogenesis of testicular germ cell tumours (GCTs) is still incompletely understood. Any progress in its understanding must derive from observational studies. Recently, it has been suggested that the incidence of GCTs may follow a seasonal pattern based on circannual changes in the Vitamin D serum levels, with maximum incidence rates in winter months. To examine this promising hypothesis, we studied monthly incidence rates of testicular GCTs in Germany by analysing 30,988 GCT cases aged 15-69 years, diagnosed during 2009-2019. Monthly incident case numbers with data regarding histology and patient age were obtained from the Robert Koch Institut, Berlin, along with annual male population counts. We used precision weighting for deriving pooled monthly incidence rates for GCTs of the period 2009-2019. We stratified pooled rates by histology (seminoma and nonseminoma) and age (15-39 and 40-69 years). By assuming a cyclical effect, we used an estimator of the intensity of seasonal occurrence and report seasonal relative risks (RR). The mean monthly incidence rate was 11.93/105 person-months. The seasonal RR for testicular cancer over-all is 1.022 (95% CI 1.000-1.054). The highest seasonal RR was found in the subgroup of nonseminoma aged 15-39 years, with a RR 1.044 (95% CI 1.000-1.112). The comparison of the pooled monthly rates of the winter months (October-March) with the summer months (April-September) revealed a maximum relative difference of 5% (95% CI 1-10%) for nonseminoma, aged 15-39 years. We conclude that there is no evidence of a seasonal variation of incidence rates of testicular cancer. Our results are at odds with an Austrian study, but the present data appear sound because the results were obtained with precision weighted monthly incidence rates in a large population of GCT cases

Résumé Contexte Le testicule représente un lieu où le précurseur de la vitamine D est converti en sa forme active. Il a été suggéré que la perte d’un testicule pouvait induire une réduction des taux sériques de vitamine D. La carence en vitamine D représenterait un problème de santé important à long terme pour les patients présentant des tumeurs testiculaires à cellules germinales (TCG) puisque la plupart d’entre eux survivent. Le but de cette étude était de se tourner vers les taux sériques de 25(OH)-Vitamine D (25OHD) chez les patients présentant des TCG, avant et après orchidectomie. Au total, 177 patients avec TCG ont subi des mesures des taux sériques de 25OHD, dont 83 avec mesures préopératoires et 94 avec des mesures à six points de temps particuliers, de l’immédiat postopératoire jusqu’à plus de 24 mois. Des évaluations longitudinales des taux sériques de 25OHD ont été réalisées individuellement chez les patients avec des mesures répétées. Une seconde analyse a impliqué des cohortes de patients avec des mesures à six points de temps postopératoires. Les taux sériques des patients ont également été comparés à ceux de 2 groupes témoins, l’un composé de 84 patients atteints de maladies testiculaires non néoplasiques, et l’autre de 237 patients atteints de maladies urologiques non néoplasiques. Nous nous sommes également penchés sur les associations des taux de 25OHD avec les taux de testostérone, d’hormone folliculostimulante (FSH), l’âge, l’histologie de la TCG et la saison. Des méthodes statistiques descriptives ont été utilisées pour comparer les groupes et analyser les changements au fil du temps. Résultats Des taux sériques normaux de 25OHD ont été trouvés chez 21,7%, 23,1%, 20,2%, et 21,9% des patients avec GCT, respectivement en préopératoire et après >2 ans, ainsi que chez les patients du groupe témoin 1 et du groupe témoin 2. Les taux ont été significativement plus élevés au printemps et en été, mais aucune association n’a été observée avec d’autres paramètres. Nous avons retrouvé une diminution transitoire significative des taux de 25OHD avec un nadir à 6-12 mois après orchiectomie et un rétablissement par la suite. Conclusion Contrairement aux études précédentes, nous n’avons trouvé aucune réduction permanente des niveaux de sérum 25OHD après orchiectomie, mais une baisse postopératoire transitoire des taux de 25OHD. Il n’y avait pas d’associations entre les taux de 25OHD et l’âge, ni avec les taux de testostérone ou de FSH. Nos résultats peuvent mettre en avant un rôle particulier du testicule dans le métabolisme de la vitamine D et peuvent ainsi améliorer la compréhension des divers rôles physiologiques des testicules
Treating nightmares in posttraumatic stress disorder with dronabinol: study protocol of a multicenter randomized controlled study (THC PTSD-trial)
Abstract Background Distressing nightmares are a core symptom of posttraumatic stress disorder (PTSD) and contribute to psychiatric comorbidity, impaired physical health and decreased social functioning. No specific pharmacological treatment for PTSD-related nightmares is yet approved. Preliminary clinical data indicate that cannabinoid agonists can improve nightmares and overall PTSD symptoms in patients with PTSD. The primary objective of the study is to examine the efficacy of oral dronabinol (BX-1) versus placebo in reducing nightmares in patients with PTSD. The secondary objectives of the study are to examine the efficacy of oral BX-1 in reducing other PTSD symptoms. Methods The study is designed as a multi-centric, double-blind, randomized (1:1), placebo-controlled, parallel group interventional trial. Eligible patients will be randomized to BX-1 or placebo, receiving a once-daily oral dose before bedtime for 10 weeks. Primary efficacy endpoint is the Clinician-Administered PTSD Scale (CAPS-IV) B2 score for the last week, measuring frequency and intensity of nightmares. Secondary efficacy endpoints are other disorder-specific symptoms in patients with PTSD. Further, tolerability and safety of dronabinol will be assessed. Discussion This randomized controlled trial will provide evidence whether treating patients with PTSD and nightmares with dronabinol is safe and efficacious.
