Nutrición en cuidados paliativos: resumen de recomendaciones del Grupo de Trabajo de Ética de la SENPE
Los cuidados paliativos proporcionan una atención integral que tiene en cuenta los aspectos físicos, emocionales, sociales y espirituales del
paciente con enfermedad terminal y su entorno familiar. El tratamiento nutricional debe formar parte activa de los equipos de cuidados paliativos.
La necesidad de iniciar o no un tratamiento nutricional sigue siendo, desde hace décadas, uno de los principales problemas éticos a los que se
enfrentan los profesionales dedicados a la nutrición clínica. El origen de tal controversia radica, fundamentalmente, en cómo se consideran la
nutrición y la hidratación: cuidado/soporte o tratamiento médico.
Los objetivos fundamentales del tratamiento nutricional en los pacientes en cuidados paliativos deben ser otros: la mejoría de la calidad de vida,
de la supervivencia o de ambas. La decisión de indicar o no el tratamiento nutricional en cuidados paliativos debe tomarse tras considerar el
pronóstico, la calidad de vida y la relación “riesgo/beneficio”. En relación a la alimentación por vía oral (con o sin suplementos orales), prevalece
la idea de la “alimentación de confort”, que se basa en intentos de alimentación oral hasta que se produzcan la incomodidad y/o el rechazo
del paciente. No existen evidencias que justifiquen el uso de nutrientes específicos, aunque desde hace años se señala la posibilidad de lograr
beneficios cuando se utilizan ácidos grasos omega-3 en los pacientes con cáncer. En cuanto al tratamiento nutricional (enteral o parenteral), en
ausencia de evidencia, las decisiones sobre si iniciar una nutrición artificial en un paciente paliativo deben tomarse teniendo en cuenta los deseos
y creencias del paciente y sus familiares, y basarse en el consenso del equipo interdisciplinar sobre los objetivos que se persiguen al iniciarla
Breastfeeding is associated with higher adherence to the Mediterranean diet in a Spanish population of preschoolers: the SENDO project
Objective: To assess whether breastfeeding during the first months of life is associated with adherence to the Mediterranean dietary (MedDiet) pattern in preschool children. Design: The Seguimiento del Nino para un Desarrollo & Oacute; ptimo (SENDO) project is an ongoing pediatric cohort with open recruitment, started in 2015 in Spain. Participants, recruited when they are 4 to 5 years old at their primary local health center or school, are followed annually through online questionnaires. For this study, 941 SENDO participants with full data on study variables were included. Breastfeeding history was collected retrospectively at baseline. Adherence to the Mediterranean diet was assessed with the KIDMED index (range -3 to 12). Results: After accounting for multiple sociodemographic and lifestyle confounders, including parental attitudes and knowledge about dietary recommendations for children, breastfeeding was independently associated with a higher adherence to the MedDiet. Compared with children who were never breastfed, children breastfed for >= 6 months had a one-point increase on their mean KIDMED score (Mean difference +0.93, 95%confidence interval [CI]. 0.52-1.34, p for trend = 8) was 2.94 (95%CI 1.50-5.36) in children who were breastfed for at least 6 months, as compared to their peers who were never breastfeed. Children who were breastfed for less than 6 months exhibited intermediate levels of adherence (p for trend <0.01). Conclusion: Breastfeeding for 6 months or longer is associated with a higher adherence to the Mediterranean diet during the preschool years
Dietary and Lifestyle Patterns in the Spanish Pediatric Population (One to <10 Years Old): Design, Protocol, and Methodology of the EsNuPI Study
The interest in a healthy diet and lifestyle during the early stages of life increased, pointing out its role in the development of noncommunicable chronic diseases throughout adult life. Dietary habits and dietary patterns begin to be established in early childhood and persist during adulthood. Therefore, the EsNuPI ("Nutritional Study in Spanish Pediatric Population") study aims to depict the dietary patterns, physical activity, and sedentary behaviors in Spanish children aged from one to 50,000 inhabitants, stratified by Nielsen areas. Participants were involved in one face-to-face survey, followed by a telephone survey after at least one week. Information about dietary intake and habits was obtained using a quantitative food frequency questionnaire and two 24-h dietary recalls. Physical activity and sedentary behaviors were registered using a specific questionnaire based on a seven-day record. Data were processed and stratified by categorical variables to be statistically analyzed in order to meet the study objectives. This study is the first of its kind in a Spanish reference population of this age range and the first to evaluate whether the consumption of adapted milk formulas and dairy products is associated with healthier dietary patterns and better diet quality and lifestyles in this group
Guía de práctica clínica SENPE/SEGHNP/SEFH sobre nutrición parenteral pediátrica
Introduction: Parenteral nutrition (PN) in childhood is a treatment whose characteristics are highly variable depending on the age and pathology of the patient. Material and methods: The Standardization and Protocols Group of the Spanish Society for Parenteral and Enteral Nutrition (SENPE) is an interdisciplinary group formed by members of the SENPE, the Spanish Society of Gastroenterology, Hepatology and Pediatric Nutrition (SEGHNP) and the Spanish Society of Hospital Pharmacy (SEFH) that intends to update this issue. For this, a detailed review of the literature has been carried out, looking for the evidences that allow us to elaborate a Clinical Practice Guide following the criteria of the Oxford Center for Evidence-Based Medicine. Results: This manuscript summarizes the recommendations regarding indications, access routes, requirements, modifications in special situations, components of the mixtures, prescription and standardization, preparation, administration, monitoring, complications and home NP. The complete document is published as a monographic number. Conclusions: This guide is intended to support the prescription of pediatric PN. It provides the basis for rational decisions in the context of the existing evidence. No guidelines can take into account all of the often compelling individual clinical circumstances.Introducción: la nutrición parenteral (NP) en la infancia es un tratamiento cuyas características son muy variables en función de la edad y la patología que presente el paciente.
Material y métodos: el grupo de Estandarización y Protocolos de la Sociedad Española de Nutrición Parenteral y Enteral (SENPE) es un grupo interdisciplinar formado por miembros de la SENPE, Sociedad Española de Gastroenterología, Hepatología y Nutrición Pediátrica (SEGHNP) y Sociedad Española de Farmacia Hospitalaria (SEFH) que pretende poner al día este tema. Para ello, se ha realizado una revisión pormenorizada de la literatura buscando las evidencias que nos permiten elaborar una Guía de Práctica Clínica siguiendo los criterios del Oxford Centre for Evidence-Based Medicine.
Resultados: este manuscrito expone de forma resumida las recomendaciones en cuanto a indicaciones, vías de acceso, requerimientos, modificaciones en situaciones especiales, componentes de las mezclas, prescripción y estandarización, preparación, administración, monitorización, complicaciones y NP domiciliaria. El documento completo se publica como número monográfico.
Conclusiones: esta guía pretende servir de apoyo para la prescripción de la NP pediátrica. Constituye la base para tomar decisiones en el contexto de la evidencia existente. Ninguna guía puede tener en cuenta todas las circunstancias clínicas individuale
Declaración de Chinchón: decálogo sobre eldulcorantes sin y bajos en calorías (ESBC)
Multidisciplinary experts in the areas of nutrition and
health met in Chinchón, Madrid, on November 25-26, 2013
under the auspices of the Fundación para la Investigación
Nutricional (Nutrition Research Foundation) and with the
collaboration of the Madrid Regional Government’s Health
Ministry, the International Sweeteners Association and the
Carlos III Health Institute CIBER of Physiopathology of
Obesity and Nutrition. They analyzed the current status
of scientific knowledge on low- and no-calorie sweeteners
(LNCS) and developed a consensus Decalogue on their use;
this constitutes the Chinchón Declaration. Sweeteners,
including sugar, represent a subject of undeniable
interest and are currently a popular topic, although areas
relating to their safety and benefits remain unknown to
segments of academia and the general public. The nature
of LNCS makes them vulnerable to biased and even
contradictory information. They are food additives that
are broadly used as sugar substitutes to sweeten foods,
medicines and food supplements when non-nutritional or
non-caloric alternatives are needed. The Chinchón
Decalogue is the outcome of a meeting for reflection and
consensus by a group of experts with backgrounds in
different scientific disciplines (toxicology, clinical nutrition,
community nutrition, physiology, food science, public
health, pediatrics, endocrinology and nutrition, nursing,
pharmaceutical care and food legislation). The Decalogue
includes different aspects of LNCS related to regulation,
use, benefits and safety. In general, benefits of LNCS have
been traditionally neglected in comparison with the
tendency for emphasising unexisting or unproven possible
risks. The need to strengthen research on LNCS in Spain
was emphasized, as well as the need to educate both
professionals and the publicExpertos de carácter multidisciplinar de las áreas de
conocimiento de la nutrición y la salud reunidos en Chinchón,
Madrid, los días 25 y 26 de noviembre de 2013 , bajo
los auspicios de la Fundación para la Investigación Nutricional
y con la colaboración de la Consejería de Sanidad
del Gobierno de la Comunidad de Madrid, la International
Sweeteners Association y el CIBER de Fisiopatología de la
Obesidad y la Nutrición del Instituto de Salud Carlos III,
analizaron el estado actual del conocimiento científico en
torno a los Edulcorantes sin y bajos en calorías (ESBC) y
desarrollaron un Decálogo sobre su uso que constituye la
Declaración de Chinchón. Los edulcorantes, incluido el
azúcar, constituyen un elemento de indudable interés y
actualidad, aunque no exento de desconocimiento por
algunos sectores tanto académicos como de la población en
general. La propia naturaleza de los ESBC los hace susceptibles
de informaciones tergiversadas e incluso contradictorias.
Son aditivos alimentarios ampliamente utilizados
como sustitutivos del azúcar para endulzar alimentos,
medicamentos y complementos alimenticios cuando se
persiguen fines no nutritivos. El Decálogo de Chinchón es
fruto de una reunión de reflexión y consenso por parte de
un grupo de expertos procedentes de distintas disciplinas
científicas (toxicología, nutrición clínica, nutrición comunitaria,
fisiología, bromatología, salud pública, atención
primaria, pediatría, endocrinología y nutrición, enfermería,
atención farmacéutica y legislación alimentaria). El
decálogo incluye diferentes aspectos de los EBSC relacionados
con la legislación, uso, beneficios y seguridad. En
general, los beneficios de los EBSC han sido tradicionalmente
desatendidos en comparación con la tendencia de
destacar posibles riesgos inexistentes o que no han sido
probados. Hace especial hincapié en la necesidad de fortalecer
la investigación de los EBSC en España, así como la
necesidad de formar en este ámbito a los profesionales y a
los consumidores en genera
Acquired and Innate Immunity Impairment and Severe Disseminated Mycobacterium genavense Infection in a Patient With a NF-κB1 Deficiency
Background: NF-κB1 is a master regulator of both acquired and innate responses. NFKB1 loss-of-function mutations elicit a wide clinical phenotype with asymptomatic individuals at one end of the spectrum and patients with common variable immunodeficiency, combined immunodeficiency or autoinflammation at the other. Impairment of acquired and innate immunity and disseminated Mycobacterium genavense infection expands the clinical and immunological phenotype of NF-κB1 deficiency.Objective: Functional and molecular characterization of a patient with a novel phenotype of NF-κB1 deficiency.Methods: Circulating T, B, dendritic cell subsets and innate or unconventional T-cells were quantified. The cytokine production in stimulated whole blood samples was assessed and molecular characterization by next generation sequencing and gene expression assays were also performed.Results: We report a patient presenting with features of combined immunodeficiency (CID) and disseminated Mycobacterium genavense infection. Sequencing of genomic DNA identified a novel synonymous mutation (c.705G > A) in NFKB1 gene which resulted in exon 8 skipping and haploinsufficiency of the NF-κB1 subunit p50. The susceptibility to atypical mycobacterial infection has not been previously reported and may be the result of a dendritic cell deficiency. A selective deficiency of circulating follicular helper T (cTFH) cells responsible for mediating the differentiation of naive B cells into memory and plasma cells was also present in the patient. It could affect the maturation of innate or unconventional T cells where NF-κB1 could also be involved.Conclusion: These findings showed that the role of NF-κB1 in humans could be critical for the development of acquired and innate immunity and further highlights the role of human T cells in anti-mycobacterial immunity
Consideraciones éticas sobre el papel de la nutrición parenteral domiciliaria: Tratamiento conservador de la obstrucción intestinal en pacientes con cáncer avanzado
El uso de la nutrición artificial en pacientes con cáncer avanzado sin tratamiento curativo continúa siendo objeto de controversia y conlleva una considerable carga emocional. Sin embargo, este grupo de pacientes constituye la primera indicación en los programas de nutrición parenteral a domicilio (NPD), en la mayoría de series, a uno y otro lado del Atlántico. Objetivo: presentar las características de una serie de pacientes incluidos en un programa de NPD en los últimos diez años y hacer una reflexión, desde el punto de vista de la bioética clínica, de su empleo en este tipo de pacientes. Método: estudio retrospectivo de las historias clínicas de los once pacientes que recibieron NPD en este periodo. Se recogieron los datos demográficos y clínicos y las complicaciones y evolución y se compararon con los del grupo de pacientes con enfermedad benigna que recibieron NPD en el mismo periodo. Se hizo una revisión extensa de la literatura sobre el uso de la técnica en estos pacientes. Resultados: once pacientes recibieron NPD, nueve a causa de una obstrucción intestinal irresoluble y dos por una fístula de alto débito. La edad media al inicio de la NPD fue de 50,8 ± 12,7 años, frente a los 37,3 ± 17,2 del grupo con enfermedad benigna. La duración media de la NPD fue sensiblemente inferior en el grupo de pacientes con cáncer. Ninguno de los que fueron incluidos en el programa continúa vivo. La calidad de vida, medida mediante una escala de actividad, fue similar, al inicio de la NPD, en ambos grupos. Solo en uno de los pacientes se suspendió la NPD antes de los 5 días previos al fallecimiento, por deterioro clínico. Los datos de la revisión bibliográfica muestran resultados similares a los comunicados en esta serie. Conclusiones: la NPD ofrece al paciente con cáncer avanzado y grave disfunción intestinal la posibilidad de tratamiento en su domicilio, con un bajo índice de complicaciones. Si consideramos la corta duración media de la NPD, la inclusión en el programa ha de valorarse en forma individual y revisarse periódicamente
