    An Algorithm to Recover Shredded Random Matrices

    Given some binary matrix MM, suppose we are presented with the collection of its rows and columns in independent arbitrary orderings. From this information, are we able to recover the unique original orderings and matrix? We present an algorithm that identifies whether there is a unique ordering associated with a set of rows and columns, and outputs either the unique correct orderings for the rows and columns or the full collection of all valid orderings and valid matrices. We show that there is a constant c>0c > 0 such that the algorithm terminates in O(n2)O(n^2) time with high probability and in expectation for random n×nn \times n binary matrices with i.i.d.\ Bernoulli (p)(p) entries (mij)ij=1n(m_{ij})_{ij=1}^n such that clog2(n)n(loglog(n))2p12\frac{c\log^2(n)}{n(\log\log(n))^2} \leq p \leq \frac{1}{2}

    Role of the external drivers in the occurrence of low-latitude ionospheric scintillation revealed by multi-scale analysis

    We analyze the amplitude scintillation on L-band signals over San Miguel de Tucumán (Argentina), focusing on the multi-scale variability and speculating on the possible relationship between forcing factors from the geospace and the ionospheric response. The site is nominally located below the expected position of the southern crest of the Equatorial Ionospheric Anomaly (EIA). For this scope, we concentrate on the period 1?31 March 2011, during which one minor and one moderate storm characterize the first half of the month, while generally quiet conditions of the geospace stand for the second half. By leveraging on the Adaptive Local Iterative Filtering (ALIF) signal decomposition technique, weinvestigate the multi-scale properties of Global Navigation Satellite Systems (GNSS) amplitude scintillation and helio-geophysical parameters, looking for possible cause-effect mechanisms relating the former to the latter. Namely, we identify resonant modes in the Akasofu (e) parameter as likely related to the frequency components in the time evolution found for the amplitude scintillation index, hence modulating the scintillation itself.Fil: Spogli, Luca.     Il Mesolitico

    Dal punto di vista cronologico-culturale, le presenze archeologiche che testimoniano il popolamento mesolitico nelle Alpi nord-occidentali risultano distribuite in maniera disomogenea: ad aree caratterizzate da un elevato numero di siti se ne con- trappongono altre che conservano poche evidenze archeologiche. In dettaglio, all’inizio dell’Olocene, si moltiplicano i siti archeologici scoperti in territorio svizzero soprattutto ai margini del massiccio alpino, così come nelle Prealpi di Savoia si attesta un Mesolitico antico precoce, mentre nella seconda metà del Boreale la regione partecipa alla corrente culturale del Sauveterriano. In Italia nord-occidentale le ricerche hanno permesso di evidenziare una frequentazione caratterizzata da siti in quota: nei pressi del Passo dello Spluga al confi ne con la Svizzera, durante il Preboreale; a Dosso Gavia in alta Valtellina, a Cianciàvero nella conca di Alpe Veglia e al Mont Fallère in Valle d’Aosta, nel corso della prima parte dell’Olocene. Dai primi secoli dell’VIII millennio BP, il settore nord-occidentale delle Alpi partecipa alla progressiva diffusione di una tecnologia litica orientata alla produzione di lame/lamelle regolari e standardizzate, caratterizzanti il Mesolitico recente

    Phase II Study with Fludarabine and Cyclophosphamide Plus Rituximab (FC+R) in Relapsed Follicular Lymphoma Patients.

    Both Cyclophsphamide (C) and Fludarabine (F) have individual anti-lymphoma activity and the combination Rituximab (R) + F has been shown to have a synergistic activity against resistant lymphoma cell lines in vitro. In march 2000, we started a Phase II multicenter clinical trial to test safety and efficacy of FC+R combination in patients with relapsed follicular lymphoma. We have recently completed the enrollment of 48 pts in the trial. Patients received 4 doses of R (375 mg/sqm/d) in combination with 4 cycles of F (30 mg/sqm/d for 3 days) and C (300mg/sqm/d for 3 days) every 28 days. Patients characteristic: 45% males, 55% females; median age: 62 years (range 44-71); stage IV 47% pts; Systemic Symptoms 8,5% pts; elevated LDH 15% pts. Rearrangement of BCL2 gene was evaluated with PCR in 38pts (79%) on bone marrow or peripheral blood mononucleated cells; 22 patients showed BCL2 rearrangement (58%). Medium number of previous chemotherapy regimens was 1,7 (range 1-4); median duration of last remission before registration into the trial was 12 months (range 1-68). Thirty-nine patients were available for evaluation at the time of current analysis. One patient did not complete therapy due to severe cytopenia. The response rate in the intent-to-treat analysis was 97%, with 74%, Complete Responses (CR) and 23% Partial Responses (PR). Out of 18 patients with molecular monitoring of disease before and after chemotherapy, 15 patients (83%) obtained molecular remission in the bone marrow (BM) .After a median follow-up of 12 months (1-25), median duration of remission was 13 months; twenty percent of patients were only recently registered into the trial and had a short follow-up (less than 4 months). Overall, 8 patients relapsed (21%). One patient died due to severe neutropenia occurred during chemoterapy; two patients died due to lymphoma progression. Complete responses are ongoing in 28 patients. As far as toxicity is concerned WHO grade III-IV leukopenia was observed in 10 patients, with 4 patients presentig a WHO grade IV granulocytopenia. WHO grade IV infections were observed in only one patient who had a pulmonary infection after third cycle. Non-ematologic toxicity was minimal. During follow up, one patient had a renal cell tumor and one patient had myelodisplasia, 3 and 6 months after the end of treatment, respectively. In conclusion the combination FC+R is associated with acceptable toxicity and with an excellent response rate in this group of heavily pre-treated patients with relapsed follicular lymphoma. Furhter follow-up is required to evaluate response duration and survival in the whole group of patients

    Il seminario Villard:20 ha inteso proseguire la riflessione sulla ricostruzione intesa non soltanto come nuova edificazione o risanamento di un territorio colpito da eventi sismici, ma anche come reinterpretazione di territori fragili, le cui criticità sono acuite da rapidi processi di antropizzazione che sovente mettono in crisi l’identità dei luoghi. Il terremoto dell’estate 2017 ha riportato sotto i riflettori Ischia nella sua consistenza di vulcano attivo, evidenziandone l’elevato grado rischio. Un’ansia di ricostruzione pervade l’isola che, appiattita dai media in un’immagine di pericolo ed abusivismo, vede minacciata la sua principale economia: il turismo. Villard:20 assume le acque di Ischia come risorsa e chiave di lettura principale per la ricostruzione: la diffusione, la varietà e le proprietà terapeutiche delle acque ischitane costituiscono il corollario della essenza vulcanica che più ha inciso nel determinare la vocazione turistica dell’isola, differenziando e specificando la sua offerta rispetto alle vicine isole del golfo partenopeo. Le aree di progetto, individuate nel Comune di Ischia recano in sé temi progettuali specifici che nell’insieme hanno l’obiettivo di aprire l’isola ad un turismo sostenibile, responsabile, aderente alle specificità di un’isola dove le acque tornino ad ispirare declinazioni di spazi intimi o grandiosi, pubblici o privat

    The Italian Unitary Society of Colon-proctology (SIUCP: Società Italiana Unitaria di Colonproctologia) guidelines for the management of anal fissure

    Abstract Introduction The aim of these evidence-based guidelines is to present a consensus position from members of the Italian Unitary Society of Colon-Proctology (SIUCP: Società Italiana Unitaria di Colon-Proctologia) on the diagnosis and management of anal fissure, with the purpose to guide every physician in the choice of the best treatment option, according with the available literature. Methods A panel of experts was designed and charged by the Board of the SIUCP to develop key-questions on the main topics covering the management of anal fissure and to performe an accurate search on each topic in different databanks, in order to provide evidence-based answers to the questions and to summarize them in statements. All the clinical questions were discussed by the expert panel in different rounds through the Delphi approach and, for each statement, a consensus among the experts was reached. The questions were created according to the PICO criteria, and the statements developed adopting the GRADE methodology. Conclusions In patients with acute anal fissure the medical therapy with dietary and behavioral norms is indicated. In the chronic phase of disease, the conservative treatment with topical 0.3% nifedipine plus 1.5% lidocaine or nitrates may represent the first-line therapy, eventually associated with ointments with film-forming, anti-inflammatory and healing properties such as Propionibacterium extract gel. In case of first-line treatment failure, the surgical strategy (internal sphincterotomy or fissurectomy with flap), may be guided by the clinical findings, eventually supported by endoanal ultrasound and anal manometry

    Congenital Extrahepatic Portosystemic Shunts (Abernethy Malformation): An International Observational Study

    Congenital extrahepatic portosystemic shunt (CEPS) or Abernethy malformation is a rare condition in which splanchnic venous blood bypasses the liver draining directly into systemic circulation through a congenital shunt. Patients may develop hepatic encephalopathy (HE), pulmonary hypertension (PaHT), or liver tumors, among other complications. However, the actual incidence of such complications is unknown, mainly because of the lack of a protocolized approach to these patients. This study characterizes the clinical manifestations and outcome of a large cohort of CEPS patients with the aim of proposing a guide for their management. This is an observational, multicenter, international study. Sixty-six patients were included; median age at the end of follow-up was 30 years. Nineteen patients (28%) presented HE. Ten-, 20-, and 30-year HE incidence rates were 13%, 24%, and 28%, respectively. No clinical factors predicted HE. Twenty-five patients had benign nodular lesions. Ten patients developed adenomas (median age, 18 years), and another 8 developed HCC (median age, 39 years). Of 10 patients with dyspnea, PaHT was diagnosed in 8 and hepatopulmonary syndrome in 2. Pulmonary complications were only screened for in 19 asymptomatic patients, and PaHT was identified in 2. Six patients underwent liver transplantation for hepatocellular carcinoma or adenoma. Shunt closure was performed in 15 patients with improvement/stability/cure of CEPS manifestations. Conclusion: CEPS patients may develop severe complications. Screening for asymptomatic complications and close surveillance is needed. Shunt closure should be considered both as a therapeutic and prophylactic approach.status: publishe
