A Cold Nearby Cloud Inside the Local Bubble
The high-latitude Galactic H I cloud toward the extragalactic radio source 3C
225 is characterized by very narrow 21 cm emission and absorption indicative of
a very low H I spin temperature of about 20 K. Through high-resolution optical
spectroscopy, we report the detection of strong, very narrow Na I absorption
corresponding to this cloud toward a number of nearby stars. Assuming that the
turbulent H I and Na I motions are similar, we derive a cloud temperature of 20
(+6, -8) K (in complete agreement with the 21 cm results) and a line-of-sight
turbulent velocity of 0.37+/-0.08 km/s from a comparison of the H I and Na I
absorption linewidths. We also place a firm upper limit of 45 pc on the
distance of the cloud, which situates it well inside the Local Bubble in this
direction and makes it the nearest-known cold diffuse cloud discovered to date.Comment: 11 pages, 3 figures, accepted for publication in ApJ Letter
Introduction to Magnetic Monopoles
One of the most basic properties of magnetism is that a magnet always has two
poles, north and south, which cannot be separated into isolated poles, i.e.,
magnetic monopoles. However, there are strong theoretical arguments why
magnetic monopoles should exist. In spite of extensive searches they have not
been found, but they have nevertheless played a central role in our

A Music-Related Quality of Life measure to guide music rehabilitation for adult CI users
Purpose: A music-related quality of life (MuRQoL) questionnaire was developed for the evaluation of music rehabilitation for adult cochlear implant (CI) users. The present studies were aimed at refinement and validation. Method: Twenty-four experts reviewed the MuRQoL items for face validity. A refined version was completed by 147 adult CI users and psychometric techniques were used for item selection, assessment of reliability and definition of the factor structure. The same participants completed the Short Form Health Survey for construct validation. MuRQoL responses from 68 CI users were compared with those of a matched group of normal-hearing (NH) adults. Results: Eighteen items measuring music perception & engagement and 18 items measuring their importance were selected; they grouped together into two domains. The final questionnaire has high internal consistency and repeatability. Significant differences between CI users and NH adults and a correlation between music engagement and quality of life (QoL) support construct validity. Scores of music perception & engagement and importance for the 18 items can be combined to assess the impact of music on the QoL. Conclusion: The MuRQoL questionnaire is a reliable and valid measure of self-reported music perception, engagement and their importance for adult CI users with potential to guide music aural rehabilitation

A Very Sensitive 21cm Survey for Galactic High-Velocity HI
Very sensitive HI 21cm observations have been made in 860 directions at dec
>= -43deg in search of weak, Galactic, high-velocity HI emission lines at
moderate and high Galactic latitudes. One-third of the observations were made
toward extragalactic objects. The median 4-sigma detection level is NHI =
8x10^{17} cm^-2 over the 21' telescope beam. High-velocity HI emission is
detected in 37% of the directions; about half of the lines could not have been
seen in previous surveys. The median FWHM of detected lines is 30.3 km/s. High-
velocity HI lines are seen down to the sensitivity limit of the survey implying
that there are likely lines at still lower values of NHI. The weakest lines
have a kinematics and distribution on the sky similar to that of the strong
lines, and thus do not appear to be a new population. Most of the emission
originates from objects which are extended over several degrees; few appear to
be compact sources. At least 75%, and possibly as many as 90%, of the lines are
associated with one of the major high-velocity complexes. The Magellanic Stream
extends at least 10 deg to higher Galactic latitude than previously thought and
is more extended in longitude as well. Although there are many lines with low
column density, their numbers do not increase as rapidly as NHI^-1, so most of
the HI mass in the high-velocity cloud phenomenon likely resides in the more
prominent clouds. The bright HI features may be mere clumps within larger

A Super-Alfvenic Model of Dark Clouds
Supersonic random motions are observed in dark clouds and are traditionally
interpreted as Alfven waves, but the possibility that these motions are
super-Alfvenic has not been ruled out. In this work we report the results of
numerical experiments in two opposite regimes; M_a ~ 1 and M_a >> 1, where M_a
is the initial Alfvenic Mach number --the ratio of the rms velocity to the
Alfven speed. Our results show that models with M_a >> 1 are consistent with
the observed properties of molecular clouds that we have tested --statistics of
extinction measurements, Zeeman splitting measurements of magnetic field
strength, line width versus integrated antenna temperature of molecular
emission line spectra, statistical B-n relation, and scatter in that relation--
while models with M_a ~ 1 have properties that are in conflict with the
observations. We find that both the density and the magnetic field in molecular
clouds may be very intermittent. The statistical distributions of magnetic
field and gas density are related by a power law, with an index that decreases
with time in experiments with decaying turbulence. After about one dynamical
time it stabilizes at B ~ n^{0.4}. Magnetically dominated cores form early in
the evolution, while later on the intermittency in the density field wins out,
and also cores with weak field can be generated, by mass accretion along

Cyclopentenyl cytosine increases gemcitabine radiosensitisation in human pancreatic cancer cells
The deoxycytidine analogue 2′,2′-difluoro-2′-deoxycytidine (dFdC, gemcitabine) is a potent radiosensitiser, but has limited efficacy in combination with radiotherapy in patients with pancreatic cancer due to acute toxicity. We investigated whether cyclopentenyl cytosine (CPEC), targetting the ‘de novo' biosynthesis of cytidine triphosphate (CTP), could increase dFdC cytotoxicity alone or in combination with irradiation in a panel of human pancreatic cancer cells (Panc-1, Miapaca-2, BxPC-3). To investigate the role of deoxycytidine kinase (dCK), the rate-limiting enzyme in the activation of dFdC, human lung cancer cells without (dFdC-resistant SWg) and with an intact dCK gene (dFdC-sensitive SWp) were included. We found that CPEC (100–1000 nmol l−1) specifically reduced CTP levels in a dose-dependent manner that lasted up to 72 h in all cell lines. Preincubation with CPEC resulted in a dose-dependent increase in dFdC incorporated into the DNA only in dFdC-sensitive cells. Consequently, CPEC increased the effectiveness of dFdC (300 nmol l−1 for 4 h) only in dFdC-sensitive cells, which was accompanied by an increase in apoptosis. We also found that CPEC enhanced the radiosensitivity of cells treated with dFdC (30–300 nmol l−1 for 4 h). These results indicate that CPEC enhances the cytotoxicity of dFdC alone and in combination with irradiation in several human tumour cell lines with an intact dCK gene
Star Clusters in the Tidal Tails of Interacting Galaxies: Cluster Populations Across a Variety of Tail Environments
We have searched for compact stellar structures within 17 tidal tails in 13
different interacting galaxies using F606W- and F814W- band images from the
Wide Field Planetary Camera 2 (WFPC2) on the Hubble Space Telescope (HST). The
sample of tidal tails includes a diverse population of optical properties,
merging galaxy mass ratios, HI content, and ages. Combining our tail sample
with Knierman et al. (2003), we find evidence of star clusters formed in situ
with Mv < -8.5 and V-I < 2.0 in 10 of 23 tidal tails; we are able to identify
cluster candidates to Mv = -6.5 in the closest tails. Three tails offer clear
examples of "beads on a string" star formation morphology in V-I color maps.
Two tails present both tidal dwarf galaxy (TDG) candidates and cluster
candidates. Statistical diagnostics indicate that clusters in tidal tails may
be drawn from the same power-law luminosity functions (with logarithmic slopes
~ -2 - -2.5) found in quiescent spiral galaxies and the interiors of
interacting systems. We find that the tail regions with the largest number of
observable clusters are relatively young (< 250 Myr old) and bright (V < 24 mag
arcsec^(-2)), probably attributed to the strong bursts of star formation in
interacting systems soon after periapse. Otherwise, we find no statistical
difference between cluster-rich and cluster-poor tails in terms of many
observable characteristics, though this analysis suffers from complex,

Nurse-led follow-up of patients after oesophageal or gastric cardia cancer surgery: a randomised trial
Between January 2004 and February 2006, 109 patients after intentionally curative surgery for oesophageal or gastric cardia cancer were randomised to standard follow-up of surgeons at the outpatient clinic (standard follow-up; n=55) or by regular home visits of a specialist nurse (nurse-led follow-up; n=54). Longitudinal data on generic (EuroQuol-5D, European Organization for Research and Treatment of Cancer (EORTC) QLQ-C30) and disease-specific quality of life (EORTC QLQ-OES18), patient satisfaction and costs were collected at baseline and at 6 weeks and 4, 7 and 13 months afterwards. We found largely similar quality-of-life scores in the two follow-up groups over time. At 4 and 7 months, slightly more improvement on the EQ-VAS was noted in the nurse-led compared with the standard follow-up group (P=0.13 and 0.12, respectively). Small differences were also found in patient satisfaction between the two groups (P=0.14), with spouses being more satisfied with nurse-led follow-up (P=0.03). No differences were found in most medical outcomes. However, body weight of patients of the standard follow-up group deteriorated slightly (P=0.04), whereas body weight of patients of the nurse-led follow-up group remained stable. Medical costs were lower in the nurse-led follow-up group (€2600 vs €3800), however, due to the large variation between patients, this was not statistically significant (P=0.11). A cost effectiveness acceptability curve showed that the probability of being cost effective for costs per one point gain in general quality-of-life exceeded 90 and 75% after 4 and 13 months of follow-up, respectively. Nurse-led follow-up at home does not adversely affect quality of life or satisfaction of patients compared with standard follow-up by clinicians at the outpatient clinic. This type of care is very likely to be more cost effective than physician-led follow-up
