Aztreonam-avibactam synergy, a validation and comparison of diagnostic tools

IntroductionAntimicrobial resistance is a growing problem that necessitates the development of new therapeutic options. Cefiderocol and aztreonam (AT) are often the last active β-lactams for treating metallo-β-lactamases (MBL)-producing Gram-negative bacilli. In these difficult-to-treat bacterial strains, AT resistance is frequently attributed to the co-occurrence of other resistance mechanisms. In the case of β-lactamases they can often be inhibited by avibactam. In the present study, we evaluated the use of the double-disc synergy test (DDST) as a screening tool for the detection of synergy between AT-avibactam (ATA). We validated both the Gradient Diffusion Strips (GDSs) superposition method and the commercially available Liofilchem’s ATA GDS.Materials and methodsWe tested AT susceptibility in combination with ceftazidime-avibactam for 65 strains, including 18 Serine-β-Lactamase (SBL)- and 24 MBL-producing Enterobacterales, 12 MBL-producing P. aeruginosa, and 11 S. maltophilia isolates. Interpretation was done with EUCAST breakpoints (version 13.0), AT breakpoints being used for ATA. The accuracy and validity of the GDSs superposition method and ATA GDS were evaluated using an AT GDS applied on Mueller Hinton Agar plates supplemented with avibactam (MH-AV). A DDST was performed to screen for synergy between antibiotic combinations.ResultsUsing MH-AV, all SBL- and MBL-positive Enterobacterales were susceptible or susceptible at increased exposure to the combination AT-avibactam. In contrast, only 2 out of the 12 (17%) P. aeruginosa strains and 9/11 (82%) of the S. maltophilia strains were susceptible- or susceptible at increased exposure for the combination of AT-avibactam. The DDST detected all synergies, demonstrating a 100% sensitivity and 100% negative predictive value for all bacterial strains.ConclusionThe DDST is a sensitive tool for screening for antibiotic synergy. Unlike S. maltophilia and SBL- and MBL-positive Enterobacterales, most MBL-positive P. aeruginosa strains remain resistant to AT-avibactam. ATA GDS should be preferred for MIC determination of the AT-avibactam combination, while the GDSs superposition method can be used as an alternative to the commercial test

State Power and Other Powers : The Political Role of the Brussels Trades in the XVIIIth Century. The major issue developed in this paper deals with the sources of the actual power the Brussels trades gave themselves all along the centuries, and successfully preserved down to the end of the XVIIIth century. Four sources from which the trades derived most of their power and influence are analyzed : the tradition and long history of the Brabant dukedom's Third Estate and its constituting elements ; the massive support of nearly the whole population of Brussels ; the opportunity they had of developing a kind of clever opposition policy, mostly achieved through petitions and legal proceedings ; and the knowledge the authorities had of the considerable support the trades enjoyed in their initiatives and actions. The persistance of the Brussels trades seems then to be linked with the political opportunism of all the local powers in relation with the power of a central state still in its budding stage.La question principale de cet article concerne les sources du pouvoir effectif que les métiers de Bruxelles se sont attribué au cours des siècles, et qu'ils ont pu conserver jusqu'à la fin du XVIIIe siècle. Quatre sources desquelles les métiers puisaient essentiellement leur puissance et leur influence, sont abordées : la tradition et la longue histoire du tiers état du duché de Brabant et de ses éléments constitutifs ; le soutien considérable de la population bruxelloise quasi entière ; l'occasion de mener une sorte d'opposition savante, surtout réalisée par voie de requêtes et de procédures judiciaires ; et la connaissance qu'ont les autorités du support considérable pour les initiatives et les actions des métiers. La persistance des métiers bruxellois paraît plutôt être liée à l'opportu nisme politique de l'ensemble des puissances locales vis-à-vis du pouvoir d'un état central en pleine formation.Staats- en andere macht : de politieke rol van de Brusselse ambachten in de XVIIIde eeuw. In dit artikel wordt de oorsprong onderzocht van de werkelijke macht die de Brusselse ambachten zich in de loop der eeuwen toegeëigend hebben en die zij hebben kunnen bewaren tot het einde van de XVIIIde eeuw. Vier bronnen waaruit de ambachten hun macht en invloed ontleenden worden behandeld : de traditie en lange geschiedenis van de Derde Stand in het hertogdom Brabant en zijn structuur ; de belangrijke steun van zowat geheel de Brusselse bevolking ; het bij gelegenheid voeren van een verstandige vorm van oppositie aan de hand van verzoekschriften en andere rechtsmiddelen ; het besef bij de overheid van de aanzienlijke steun voor initiatieven en ondernemingen van de ambachten. Het voortbestaan van de Brusselse ambachten lijkt veeleer verbonden te zijn met het politiek opportunisme van het geheel van de plaatselijke machtsdragers tegenover het gezag van een in vorming zijnde gecentraliseerde staat.Van Honacker Karin. Collective actions in the Brabant capital cities should rather be studied in relation to the policy of centralization of the princes than to cyclical fluctuations and years of crisis. Even though they manifested themselves in a variety of forms and originated as a result of divergent causes, they often appeared to have the same motives, to wit, the struggle for the preservation of spheres of influence. Usually, it were the guild leaders, being part of the city administration, who directed the resistance.Les actions collectives dans les chefs-villes du Brabant doivent plutôt être étudiées en relation avec la politique de centralisation des souverains qu'avec les oscillations de la conjoncture et les années de crise. Bien qu'elles se manifestassent sous une variété de formes et qu'elles fassent leur apparition à la suite d'occasions diverses, elles parurent souvent avoir les mêmes mobiles, à savoir la lutte pour le maintien des zones d'influence.
