    Results of the standard set forpulmonary sarcoidosis: Feasibility and multicentre outcomes

    Our study presents findings on a previously developed standard set of clinical outcome data for pulmonary sarcoidosis patients. We aimed to assess whether changes in outcome varied between the different centres and to evaluate the feasibility of collecting the standard set retrospectively. This retrospective observational comparative benchmark study included six interstitial lung disease expert centres based in the Netherlands, Belgium, the UK and the USA. The standard set of outcome measures included 1) mortality, 2) changes in pulmonary function (forced vital capacity (FVC), forced expiratory volume in 1 s, diffusing capacity of the lung for carbon monoxide), 3) soluble interleukin-2 receptor (sIL-2R) change, 4) weight changes, 5) quality-of-life (QoL) measures, 6) osteoporosis and 7) clinical outcome status (COS). Data collection was considered feasible if the data were collected in ⩾80% of all patients. 509 patients were included in the retrospective cohort. In total six patients died, with a mean survival of 38±23.4 months after the diagnosis. Centres varied in mean baseline FVC, ranging from 110 (95% CI 92–124)% predicted to 99 (95% CI 97–123)% pred. Mean baseline body mass index (BMI) of patients in the different centres varied between 27 (95% CI 23.6–29.4) kg·m−2 and 31.8 (95% CI 28.1–35.6) kg·m−2. 310 (60.9%) patients were still on systemic therapy 2 years after the diagnosis. It was feasible to measure mortality, changes in pulmonary function, weight changes and COS. It is not (yet) feasible to retrospectively collect sIL-2R, osteoporosis and QoL data internationally. This study shows that data collection for the standard set of outcome measures for pulmonary sarcoidosis was feasible for four out of seven outcome measures. Trends in pulmonary function and BMI were similar for different hospitals when comparing different practices

    Vortex Lattice Melting into Disentangled Liquid Followed by the 3D-2D Decoupling Transition in YBa_2Cu_4O_8 Single Crystals

    A sharp resistance drop associated with vortex lattice melting was observed in high quality YBa_2Cu_4O_8 single crystals. The melting line is well described well by the anisotropic GL theory. Two thermally activated flux flow regions, which were separated by a crossover line B_cr=1406.5(1-T/T_c)/T (T_c=79.0 K, B_cr in T), were observed in the vortex liquid phase. Activation energy for each region was obtained and the corresponding dissipation mechanism was discussed. Our results suggest that the vortex lattice in YBa_2Cu_4O_8 single crystal melts into disentangled liquid, which then undergoes a 3D-2D decoupling transition.Comment: 5 pages, 4 eps figures, RevTex (Latex2.09

    Pre-clinical Evaluation of a Cyanine-Based SPECT Probe for Multimodal Tumor Necrosis Imaging

    Purpose: Recently we showed that a number of carboxylated near-infrared fluorescent (NIRF) cyanine dyes possess strong necrosis avid properties in vitro as well as in different mouse models of spontaneous and therapy-induced tumor necrosis, indicating their potential use for cancer diagnostic- and prognostic purposes. In the previous study, the detection of the cyanines was achieved by whole body optical imaging, a technique that, due to the limited penetration of near-infrared light, is not suitable for investigations deeper than 1 cm within the human body. Therefore, in order to facilitate clinical translation, the purpose of the present study was to generate a necrosis avid cyanine-based NIRF probe that could also be used for single photon emission computed tomography (SPECT). For this, the necrosis avid NIRF cyanine HQ4 was radiolabeled with 111indium, via the chelate diethylene triamine pentaacetic acid (DTPA). Procedures: The necrosis avid properties of the radiotracer [111In]DTPA-HQ4 were examined in vitro and in vivo in different breast tumor models in mice using SPECT and optical imaging. Moreover, biodistribution studies were performed to examine the pharmacokinetics of the probe in vivo. Results: Using optical imaging and radioactivity measurements, in vitro, we showed selective accumulation of [111In]DTPA-HQ4 in dead cells. Using SPECT and in biodistribution studies, the necrosis avidity of the radiotracer was confirmed in a 4T1 mouse breast cancer model of spontaneous tumor necrosis and in a MCF-7 human breast cancer model of chemotherapy-induced tumor necrosis. Conclusions: The radiotracer [111In]DTPA-HQ4 possessed strong and selective necrosis avidity in vitro and in various mouse models of tumor necrosis in vivo, indicating its potential to be clinically applied for diagnostic purposes and to monitor anti-cancer treatment efficacy

    Joint sequencing of human and pathogen genomes reveals the genetics of pneumococcal meningitis

    Streptococcus pneumoniae is a common nasopharyngeal colonizer, but can also cause lifethreatening invasive diseases such as empyema, bacteremia and meningitis. Genetic variation of host and pathogen is known to play a role in invasive pneumococcal disease, though to what extent is unknown. In a genome-wide association study of human and pathogen we show that human variation explains almost half of variation in susceptibility to pneumococcal meningitis and one-third of variation in severity, identifying variants in CCDC33 associated with susceptibility. Pneumococcal genetic variation explains a large amount of invasive potential (70%), but has no effect on severity. Serotype alone is insufficient to explain invasiveness, suggesting other pneumococcal factors are involved in progression to invasive disease. We identify pneumococcal genes involved

    Beer brewing, selling and consumption is, as in many African societies, of great importance among the Kapsiki. Brewing is a complex process that has at least two major variations, leading to the “red” and the “white” variety. Both are clearly distinguished and have quite different functions. The red "male" beer contrasts with the white “female” brew in many ways: ritual application versus market orientation, male versus female interests and symbolic versus economic motives. The last decade have shown a shift in availability of beer for market production, in which women have been able to get access to a formerly male domain of beer preparation. Shifts in production recipes and shifts in ritual use of beer have resulted from this “gender change” in beer. The same holds, to some extent, for the blacksmith women, who have gained an - admittedly small - proportion of the beer market.La fabrication, la vente et la consommation de bière de mil est très importante pour les Kapsiki comme pour d’autres sociétés africaines. La brasserie est une activité technique complexe qui se décline en deux variantes principales permettant d’obtenir deux qualités de bière de mil : la « rouge » et la « blanche ». Toutes deux sont clairement distinguées et ont des fonctions différentes. La bière rouge, « masculine », s’oppose à la blanche de plusieurs manières ; elle est utilisée dans les rituels alors que l’autre est destinée à la vente, elle relève du domaine masculin alors que la blanche constitue un attribut des femmes, elle est réservée aux utilisations rituelles alors que la bière blanche est associée au marché. Durant la dernière décennie, on a pu observer une évolution du marché des bières de mil qui a vu les femmes accéder à une spécialité exclusivement masculine de la fabrication de la bière rouge. Des transformations des recettes utilisées et des usages rituels de la bière ont conduit à une modification des relations de genre autour de la bière de mil. Cela est vérifié aussi, dans une certaine mesure, pour les femmes des « forgerons » endogames qui ont pu elles aussi obtenir un accès, encore peu important, au marché de la bière de mil
