    RISK FACTORS FOR EXTRAESOPHAGEAL SYMPTOMS OF GASTROESOPHAGEAL REFLUX DISEASE

    The aim of the study was to investigate the frequency and nature of extraesophageal symptoms in patients with gastroesophageal reflux disease. 122 patients with GERD without comorbidity have been examined (58 males, 64 females). The anthropometrical survey has been performed; estimated age, duration of the disease and smoking have been taken into account. Extraesophageal symptoms were diagnosed with the rabeprazole test, the frequency and intensity of the symptoms were evaluated using a Likert scale. All patients underwent endoscopic examination and the assessment of quality of life by means of the SF-36 questionnaire. Extraesophageal symptoms of GERD were diagnosed in 38 (31.1 %) patients, reflux laryngitis - in 14 (9.8 %) patients, reflux-pharyngitis - in 7 (4.1 %) patients, reflux cough - in 11 (7.3 %) patients, heart pain - in 6 (3.2 %) patients. In patients with extraesophageal manifestations compared with the patients having only physical symptoms, body mass index and the severity of heartburn on the Likert scale were significantly higher. Other indicators, such as disease duration, smoking, presence of erosive changes in the mucous membrane of the esophagus, hernia hiatal didn't statistically reveal any difference between the groups. When comparing the quality of life it has been revealed that the indicators of physical health had no statistical difference in the two groups, when mental health indices were lower in those with extraesophageal symptoms of GERD

    Evaluating the Efficacy of Translaminar Epidural Injections for Pain Resulting From the Lumbar Spine Degenerative Disease

    Background: Caused by the degenerative spine disease (DSD), the pain syndrome in the lumbosacral spine and lower extremities represents a socioeconomic problem. Nowadays, minimally invasive methods of treatment, in particular, epidural injections (EI), are commonly used.Objective: To do a literature review on translaminar EIs and their mechanism of action in treating patients with radicular and vertebrogenic pain symptoms arising from the DSD.Materials and methods: We analyzed the most recent literature data in which the efficacy of epidural corticosteroid injections for DSD was evaluated.Conclusions: Translaminar EIs with corticosteroids are effective in relieving symptoms in the short term (up to 6 months) and delaying the surgery, but the evidence of their long-term benefits is yet to be found

    Influence of the Pentaphylloides fruticosa l. liniment on TBA-active products and antioxidant status in wound injuries associated with experimental diabetes mellitus

    The aim of the work was to evaluate the influence of the linimentfrom Pentaphylloides fruticosa L. on lipid peroxidation in rats with experimental diabetes mellitus accompanied by wound process. The liniment was developed with the use of NanoDeBee30 device. Experiments were carried out on white male Wistar rats. Experimental diabetes mellitus was simulated by a single intraperitoneal introduction of alloxan in a dose of 100 mg/kg. The skin injury was simulated by inflicting planar wounds 2.0 x 2.0 cm in size. 5 mg of the 5%-liniment from Pentaphylloides fruticosa was put on the wounds of the first experimental group once a day for 14 days. The second experimental group was treated by the preparation of comparison - balsamic liniment. The wounds of the control group were treated with glycerin-lanolin ointment. On the 7th, 14th and 21st day the concentration of TBA-active products in the blood serum was determined to estimate the intensity of lipid peroxidation processes; the contents of reduced glutathione and catalase activity were determined to evaluate the antioxidant status. The results of the experiments verify the marked curative effect of the linimentfrom Pentaphylloides fruticosa L. on the healing of the wounds on the background of experimental diabetes mellitus in rat

    Microbial landscape of operated patients with limited sterile pancreatic necrosis in different periods of complex treatment

    The method of gas chromatography - mass spectrometry was studied for the first time in patients with sterile pancreatic necrosis. The parameters defined in the normal conditions are presented. We studied the microbial landscape of patients with sterile pancreatic necrosis in the dynamics of complex treatment. Qualitative analysis was based on the comparison of retention times and mass specter of the total corresponding pure compounds using a library of standard data and mixtures N1ST08.L. We detected elevated markers in patients with acute pancreatitis associated with anaerobic microflora (peptostreptococcus, staphylococci, eubacteria, fusobacterium, clostridium), and at the same time low level of lactobacilli, bifidobacteria, some ruminococcus, actinomycetes and other microorganisms that caused dysbiosis

    Influence of soft finished pharmaproducts based on Pentaphylloides fruticosa l. on the course of wound process

    The aim of the work was to estimate a wound-healing effect of soft drug products from the Pentaphylloides fruticosa L. shoots on the model of planar skin-fascial wound. The liniment was developed with the use of the NanoDeBee 30 device and the ointment was developed with the use of pharmaceutical technology. Experiments were carried out on white male Wistar rats. The skin injury was simulated by inflicting planar wounds 2.0 x 2.0 cm in size. The 5%-liniment and 5% ointment from P. fruticosa were put on the wounds of the first and second experimental groups respectively. In the third group the wounds were treated with glycerin-vaseline ointment. Bioptate of the wound surface was taken on the 7th, 14th and 21st day. Histological preparations were studied using standard methods. Cell elements of the first group animals were displayed as fibroblasts and fibrocytes; it was noted high level of collagen organization that testified the effective staging of the wound process in this case. The amount of fibrocytes and fibroblasts and ratio between juvenile and mature forms of collagen-producing cells vary in the first and second groups. Biopsy material obtained from the animals of the third group only on the 21st day corresponded to the picture observed in animals of the 1st group on the 14th day. The findings of the study verify that the use of the product obtained on NanoDeBee device provides the earlier forming of the scar as compared with the ointment developed using pharmaceutical technology

    A clinical analysis of 51 patient treated, in surgical ward of Republican. Clinical Hospital named, after N.A. Semashko during the period, of 1993—2011. All patients had. pancreatoduodenal resection for chronic pancreatitis (7) and pancreatic cancer (44). In the nearest postoperative period, mortality was 7,8 %. In the remote postoperative period, terms of survivability in 8 patients with cancer were 18—34 months. One patient lived 10 years

    Method of gas chromatography - mass spectrometry was used for the study of patients with microbial landscape pancreatonecrosis. In patients with necrotizing pancreatitis concentration of organisms was determined in serum at the admission to the hospital, in the middle of the treatment and at discharge. Increased concentrations of Helicobacter pylori, Herpes, anaerobic bacteria were detected in the blood serum of patients with limited sterile pancreatic necrosis who had medical diagnostic laparoscopy. This fact, in our opinion, extends the etiology of this disease. We found that patients with pancreatic necrosis has limited sterile dysbiosis, which can be adjusted over time by gas chromatography - mass spectrometry

    A centuries-old experience of Buryat branch of the traditional medicine in Russia

    The present work reviews the evolution of the traditional medicine in Buryatia; the contribution of Oriental medical schools and the Russian experience of herbal treatment were emphasized. This convergence process was developed in virtue of the geopolitical situation of the Republic. The evolution of the tradition and contribution of ethnic medical schools was schematically presented in the article. The aim of the work was to substantiate the originality of the Buryat traditional medicine. The data elucidating the pervasion of Eastern traditions due to the influence of Buddhism and the experience of the Russian settlers are given in the article. Peculiarity of the traditional medicine in this region involves the use of indigenous plants, original formularies and healing technologies. Approach to health and illness, principles of diagnostics and specific methods of treatment remain common to Eastern medical traditions. The article provides insight into the main findings of the study of this tradition. They include such aspects as the translation of written sources, development of new technologies and methods for the treatment and prevention of some diseases, new medicinal preparations and health-improving means recommended for practical use. Besides, there have been discussed the prospects for integration of rational methods and means from the array of the traditional medicine of Buryatia with modern medical and pharmaceutical technologies
