Management of diabetes during Ramadan: an update for Russian-speaking doctors
Fasting during the Islamic Ramadan month is one of the five obligatory pillars for each adult, healthy, and sane Muslim. People with severe illnesses, including type 1 and type 2 diabetes mellitus are exempt from fasting. However, many Muslims often insist on Ramadan participating despite any medical advises. It’s known that Muslims are the second largest religious group in Russia; thus, its crucial to have as much modern recommendation for management patients with type 1 and type 2 diabetes mellitus as possible. The aim of this narrative review is to evaluate physiological and pathophysiological metabolism changing during holy Ramadan month, to clarify the management of patients with type 1 and type 2 diabetes mellitus during Ramadan, to determine the frequency of glucose measuring during the day, and to understand when its time to interrupt the fasting. Moreover, we discuss specific recommendations in glucose-lowering therapy changing, nutrition, physical activities and education

On the Gibbs states of the noncritical Potts model on Z^2
We prove that all Gibbs states of the q-state nearest neighbor Potts model on
Z^2 below the critical temperature are convex combinations of the q pure
phases; in particular, they are all translation invariant. To achieve this
goal, we consider such models in large finite boxes with arbitrary boundary
condition, and prove that the center of the box lies deeply inside a pure phase
with high probability. Our estimate of the finite-volume error term is of
essentially optimal order, which stems from the Brownian scaling of fluctuating
interfaces. The results hold at any supercritical value of the inverse

Thyroid cancer: retrospective study of patients with surgical treatment (a single-center experience)
Introduction. Nodular goiter is one of the most common endocrine disorder, and thyroid cancer takes a leading place among malignancy of the endocrine system. The aim of our study is to evaluate the prevalence and risk factors of thyroid cancer. Materials and methods: this is a retrospective case-control study that included 140 patients operated with suspected thyroid cancer according to a cytological study of fine needle aspirates in single center in 2018. All patients underwent preoperative cytological examination with interpretation of the results according to the Bethesda system, thyroid ultrasound examination, thyroid status assessment. The results of the cytological examination, as well as clinical and laboratory data were compared with the data of histological analysis. Results: According to the preoperative cytological study, 76 nodules were reported as Bethesda IV, 4 – Bethesda III, 32 – Bethesda VI, 28 – Bethesda V. According to the results of a histological examination, thyroid cancer was confirmed in 70% of patients: in 89,3% of Bethesda V and 100% in the Bethesda VI group, 52,6% of Bethesda IV, 25% of the Bethesda III. The risk of nodule malignancy was associated with such ultrasound signs as hypoechoicity, calcifications and irregular contours. Conclusions: the improvement of the methods of preoperative diagnosis of thyroid cancer should help to choose the optimal surgical strategy, reduce the number of diagnostic and repeated thyroidectomy
[Arterial wide-section anastomoses: experimental study on animals].
A mechanical device for making arterial wide-section anastomoses was tested on a series of 15 laboratory rabbits. End-to-side anastomoses were made using a vein graft and they functioned in 93.1% of cases, as evidenced by intraoperative Doppler studies. When an anastomosis is made, the mean duration of temporary blood flow arrest in the rabbit aorta averaged 2 min. According to outcomes, the survived animals were divided into 3 groups: Group 1 comprised 2 (13.3%) rabbits that died within 10-12 hours after surgery and had significant symptoms of anastomotic dysfunction. Group 2 included 8 (53.3%) animals that died 2-3 days after surgery. They moved 5-6 hours following surgery without assistance, which was indicating of anastomotic functioning. Group 3 consisted of 3 (20%) rabbits that survived 2 weeks. Autoptic studies revealed no anastomic thrombosis in Group 2 animals. Group 3 animals were noted to have signs of allograft thrombosis and rejection. Thus, it is expedient to continue studies to make arterial wide-section anastomoses, by using the above device, in order to introduce this procedure into clinical practice
[Microsurgical treatment of large and giant aneurysms of the internal carotid artery: analysis of application of intravascular blood aspiration technique and future perspectives].
The article discusses results of surgical treatment of 82 patients with large and giant cerebral aneurysms operated in Burdenko Neurosurgical Institute (Moscow, Russia) during the recent 14 years. Special attention was given to benefits of intravascular blood aspiration (IBA) technique. This technique was proven to be very effective, allowing to clip the aneurysm in 87.8% of cases with good recovery in 41.5% and with moderate disability in 39%. Postoperative mortality was 4.9% (4 patients). The authors suggested original surgical classification of large and giant aneurysms. IBA was ineffective in cases of giant partially thrombosed aneurysms with sclerotic walls and giant aneurysms with very wide neck. These aneurysms require other surgical procedures such as embolization of ICA followed by extraintracranial bypass or high-flow bypass
