The possibility of hyperbaric oxygenation in the correction of the secretory impaired fertility in men

The infertility problem during a marriage affects demographic indicators significantly and has not only medical and biological but important social significance as well. In Russia, about 20 % of married couples remain infertile. With that, in half of cases this is caused with pathological changes in male reproductive system. Until now, a comprehensive andrologic examination is not capable of finding out the reason of pathospermia in 40–72 % of cases of male infertility and, as a result, of assigning the appropriate etiotropic treatment. A comparative dynamic study of spermograms of 107 patients with idiopathic pathozoospermia depending on various types of treatment has been held immediately, 1 month, and 2 months after completion of the therapy course. It has been determined that significantly greater and sustained improvement in quality and quantity of ejaculate is achieved in 1 month as compared to monotherapy with L-carnitine in combination with hyperbaric oxygenation by eliminating the effects of hypoxia and oxidativestress, and the effect is maintained during the whole term of observation

EVALUATION OF T CELL IMMUNOLOGICAL MEMORY BY MEANS OF CD45+ EXPRESSION IN HUMANS VACCINATED WITH LIVE REASSORTANT INFLUENZA VACCINE

Abstract. A full-scale development of post-vaccinal immunity is determined by T and B cell-dependent immunological memory which is formed in response to vaccine injection. However, a capacity of existing and newly developed vaccines to stimulate T cell-dependent immunological memory is very poorly studied. Present work is a first attempt of analyzing this issue, with respect to effects of live mucosal influenza vaccine.The study involved fifty-seven healthy young persons, in whom percentage of T cells with СD45RO+, CD45RA+, CD4+СD45RO+, CD4+CD45RA+, CD8+СD45RO+, and CD8+CD45RA+ phenotypes was determined pre- and post-vaccination.Following vaccination, increased average T cell levels of all the mentioned phenotypes was revealed in those volunteers who responded to the vaccine with elevated serum antibody titers. Individual data analysis has shown that, among individuals with antibody response, a percentage of persons with CD45RO+ T cells was 35 to 57%, as compared to vaccinated persons without such antibody response (11 to 22%, p &lt; 0.05; p &lt; 0.01). When using CD45RA+ marker, appropriate values among group 1 volunteers varied between 14 and 36%, whereas for group 2, only one case was detected with expression with significantly increased CD8+CD45RA+, expression on T cells post-vaccination.Hence, a single immunization with live influenza vaccine is followed by significantly increased levels of peripheral CTLs (CD8+) and Th (СD4+) with СD45R0+ (memory cells) and СD45RА+ (naive) phenotypes in sufficient cohort of immunized persons (35 to 60%). Certain relations exist between post-vaccinal accumulation of memory T cells in peripheral blood, and development of a systemic humoral response to vaccination. However, this connection is not absolute, since an increase in these cells post-vaccination occurs in some persons without such a response (up to 25% of immunized individuals). (Med. Diagnosis and treatment of erectile dysfunction in patients with benign prostatic hyperplasia

The efficacy and safety of the Russian phosphodiesterase type 5 inhibitor sildenafil-C3 in combination with a natural complex of the Russian multicomponent dietary supplement (DS) Erectogenon were investigated in 75 patients with erectile dysfunction (ED) and benign prostatic hyperplasia (BPH). Clinical and Doppler ultrasound examinations revealed moderate ED in 34 study group patients who had received combined treatment with sildenafil and the DS Erectogenon for 4 weeks. In the same perod, 21 patients from a comparative group (Group 2) had taken sildenafil only and 20 patients from a comparative group (Group 3) had used the natural biocomplex DS Erectogenon only. The investigation revealed mild ED in the patients from Groups 2 and 3. The immediate and late treatment results were assessed 4 and 12 weeks after the end of treatment, respectively. Combined therapy with sildenafil + Erectogenon was found to statistically significantly enhance its efficiency and to improve quality of sexual life in 85.3 % of the study group patients who had moderate ED. This patient group had a more marked positive clinical effect (a mean increase in erection quality by 7.0 scores by the International Index of Erectile Function, a rise in penile blood flow by 3.5 times, and a reduction in the International Prostatitis Symptom Index by 5 scores). Sildenafiil-C3 or Erectogenon monotherapy provided a lower effect in the comparative groups. The findings confirm that combined therapy with the Russian drugs sildenafil-C3 and DS Erectogenon is effective, safe, and well-compatible and causes no adverse reactions and the synergism of the components of the biocomplex DS Erectogenon prolongs the effect of daily low-dose sildenafil. The combined treatment may be recommended for the treatment of ED in patients with BPH, for this affects the pathogenesis of the disease, alleviates the progression of ED, and improves their quality of life. The treatment should be used in moderate ED and moderate reductions in urodynamic symptoms, by keeping in mind penile and prostatic Doppler ultrasound findings
