Opinions of Otorhinolaryngology Residents about Their Education Process
Objective:Our study was planned to get the views of residents about the Otorhinolaryngology (ORL) education process and to enlighten the studies to make this process more effective.Methods:A questionnaire was sent to the residents who were still in the residency program in all education clinics via “Google Drive”. Seventy-four of 354 residents responded and the answers were evaluated electronically.Results:Fifty residents (67.56%) gave an affirmative answer to the question about the use of “Resident Log Book” and no difference was seen among the clinics. While 9 residents (12.16%) were reporting that they did not read any scientific papers, 43 (58.1%) reported they read less than three per month. Forty-one residents thougt that they were having a good and sufficient education. Seventeen residents (51.51%) who thought they were not having a sufficient education reported that the education period should be longer. When they were wanted to evaluate the education process, while 66 of them (89.18%) said “Exhausting”, 52 (70.27%) said “Stressful”, it was seen that the ones who said “Instructive” and “Rewarding” were 26 (35.13%) and 17 (22.97%) respectively. Further, 43 of 48 residents (89.58%) who were over the third year of their residency program indicated that they were unable to perform at least one procedure listed in the questionnaire after finishing their education.Conclusion:This study is important because it is the first study about the opinion of ORL residents and will help determine the current status in Turkey. This study will be useful for the preparation of educational programs and guides in the future
Effect of formaldehyde formed temporalis muscle fascia in tympanoplasty
GİRİŞ: Bu çalışmanın amacı, timpanoplasti sırasında kullanılan temporal kas fasya greftlerinin formaldehit ile işlenmesinin operasyon sonrası başarı oranları üzerine etkisini araştırmaktır. GEREÇ VE YÖNTEM: Bu çalışma, Uludağ Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Anabilim Dalı’nda Ocak 2006 ile Ocak 2007 arasında timpanoplasti yapılan ve düzenli takipleri olan deney grubunda 24 hasta, kontrol grubunda ise 30 hasta olmak üzere toplam 54 hasta ile prospektif olarak gerçekleştirildi. Hastaların tüm kontrolleri otomikroskopi ile yapıldı. Altıncı aydaki kontrollerinde odyometrik testler yapıldı. BULGULAR: Çalışmamızda formaldehit kullanılan hastaların %79,2’ sinde, kontrol grubunun %73,3’ ünde perforasyonun kalıcı olarak onarıldığı görüldü. Her iki grupta da ortalama havayolu, kemik yolu açıklığında belirgin iyileşme sağlandı. Her iki grubun operasyon süreleri karşılaştırıldığında çalışma grubunda ortalama operasyon süresi bir miktar daha az olmakla birlikte istatistiksel olarak anlamlı bir fark olmadığı görüldü. (Çalışma grubu: 735sn.; kontrol grubu 775 sn.) SONUÇ: Temporal kas fasyası özellikle ilk kez opere edilen hastalarda en sık kullanılan greft materyalidir. Genel olarak başarı oranı %83,2-97,6 arasında bildirilmektedir. Çalışmamızda bu oranın düşük olması (ort. %75,9) kliniğimizin eğitim merkezi olması ve operasyonların deneyim kazanmakta olan kişilerce de yapılmasına bağlı olabilir. Ayrıca diğer çalışmalardan farklı olarak bu çalışmada greftin formaldehit ile işlenmesinin operasyon süresine etkisi olmadığı da görüldü. Ancak operasyon sırasında greftin daha rahat manipüle edilebildiği yönünde bir görüş oluştu. Sonuçlarımız formaldehit ile işlem yapmanın fasyanın direkt kurutularak kullanılmasına bir üstünlüğünü göstermemiştir. Her ne kadar başarı oranlarımız her iki grup için de düşük olsa da temporal kas fasyasının formaldehit ile işlenerek kullanılması daha küçük perforasyonların tedavisinde iyi bir alternatif olabilir.OBJECTIVES: The aim of this study is to determine the effect of formaldehyde formed type of temporalis muscle fascia grafts on postoperative success in tympanoplasty. MATERIAL and METHOD: This prospective study was performed in 54 patients who underwent tympanoplasty in Department of Otorhinolaryngology, Uludag University School of Medicine, between January 2006- January 2007 and who continued the routine follow-up for six months. In control examinations otomicroscopy was done for all patients and postoperative sixth month audiograms recorded. RESULTS: In this study 79,2% of patient in formaldehyde formed fascia group and 73,3% of patient in control group had permanent closure of perforation. Air bone gap in audiograms reduced significiently in both groups. Both group are compared by duration of operation and no diffirences observed statistically. (Study group: 735 sc, Control group: 775 sc) CONCLUSION: Temporalis muscle fascia is most common used graft material in tympanoplasty, especially in primary cases. In literature permanent closure of perforation rates are reported as 83,2-97,6%. In our study this rate is lower than the litterature. This may be a result of that our department is an educational center and some of the operations are performed by less experienced surgeons under supervision. Contrary to other studies, using formaldehyde formed fascia did not reduce operation time; but during the operation the manipulation of the graft was easier than the control group. Our results did not show us any superiority of formaldehyde formed fascia in small perforations. However regarding similar results obtained in both groups, formaldehyde formed fascia can be effective alternative for unexperienced surgeons because of its easier manipulability
