Hipofibrinogenemia como predictor de severidad en gestantes con hemorragia postparto atendidas en el hospital Víctor Lazarte Echegaray

Demostrar que la hipofibrinogenemia tiene valor como predictor de severidad en gestantes con hemorragia postparto atendidas en el Hospital Víctor Lazarte Echegaray. Material y métodos: Estudio de pruebas diagnósticas, retrospectivo, observacional, en 76 gestantes con hemorragia postparto. Se calcularon la sensibilidad, especificidad, valor predictivo positivo, negativo y exactitud pronóstica. Se aplicó el test de chi cuadrado y se compararon los promedios de fibrinógeno en ambos grupos por medio de t de student. Resultados: La sensibilidad, especificidad, valor predictivo positivo, negativo de la hipofibrinogenemia respecto a severidad de hemorragia postparto fue 86%; 88%; 43% y 98% respectivamente. La exactitud pronóstica de la hipofibrinogenemia respecto a severidad de hemorragia postparto fue de 88%. El promedio de fibrinógeno s+erico fue de 192.6 mg/dl en el grupo con hemorragia severa; significativamente inferior al promedio del grupo con hemorragia no severa con un promedio 236.8 mg/dl. Conclusiones: La hipofibrinogenemia tiene valor como predictor de severidad en gestantes con hemorragia postparto atendidas en el Hospital Víctor Lazarte Echegaray.Demonstrate if hypofibrinogenemia has value to predict severity in pregnant women with postpartum hemorrhage at the Victor Lazarte Echegaray Hospital. Material and methods: Retrospective, observational, diagnosis test study was made in 76 pregnant women with postpartum hemorrhage tests. Sensitivity, specificity, positive predictive value, negative and forecast accuracy were calculated .chi square test was applied and the average fibrinogen both groups were compared using Student's t. Results: Sensitivity, specificity, positive predictive value, negative hypofibrinogenemia regarding severity of postpartum hemorrhage was 86% ; 88% ; 43% and 98% respectively . The prognostic accuracy of hypofibrinogenemia regarding severity of postpartum hemorrhage was 88%. The mean serum fibrinogen was 192,6 mg/dl in the group with severe bleeding; significantly lower than the average of the group with no severe bleeding with 236.8 mg/dl Conclusions: Hypofibrinogenemia has value to predict severity in pregnant women with postpartum hemorrhage at the Victor Lazarte Echegaray Hospital
