The quality of the physician: assessment of the work and postgraduate professional education

The quality of the physician was examined as a social phenomenon reflecting the expectations of the society and the professional to medical activities, that is beyond the formal organizational assessments. In 2012 according to results of the questionnaire, 734 doctors and 172 heads of medical institutions recorded a high level of satisfaction with advanced training. However, the postgraduate professional education has little to do with the compensation of professional problems declared by doctors (low social status of the medical profession in Russian society, the increasing demand in the workplace). The effects of the institutional impact do not compensate for a positive evaluation of their work (altruism, self-actualization), which points to the need for an ordinary physician to look for alternative compensatory mechanisms to formal education.Качество деятельности врача рассмотрено как социальный феномен, отражающий реализацию ожиданий общества и профессионала к своей деятельности, что выходит за рамки формализованных организационных оценок. По результатам анкетирования в 2012 г. 734 врачей и 172 руководителей ЛПУ зафиксирован высокий уровень удовлетворенности повышением квалификации. Однако дополнительное профессиональное образование слабо связано с компенсацией профессиональных проблем, заявленных врачами (низкий социальный статус врачебной профессии в российском обществе, усиление требований на рабочем месте). Эффекты институционального влияния не компенсируют позитивные оценки своей работы (альтруизм, самореализация), что указывает на необходимость для рядового врача искать альтернативные формальному образованию компенсаторные механизмы

Predicting the development of cardiovascular disorders in children with bronchial asthma

The article gives the data about a condition of cardiovascular system of 7-12 year-old children, suffering from bronchial asthma (BA).As a result of the research it is established that all the children have changes of cardiovascular system in the form of heart rate changes, vegetovascular dysfunctions, small anomalies of heart, dysfunction of valve apparatus, a functioning foramen oval. As a rule all these dysfunctions do not depend on how heavy the BA is. According to the results obtained, a decisive rule for predicting cardiovascular disorders in children with asthma was obtained.В статье приводятся данные о состоянии сердечно-сосудистой системы у детей 7-12 лет, страдающих бронхиальной астмой (БА). В результате проведенных исследований установлено, что независимо от тяжести течения БА, все дети имеют отклонения со стороны сердечно-сосудистой системы в виде нарушений сердечного ритма, вегетативно-сосудистых дисфункций, малые аномалии сердца, дисфункции клапанного аппарата, функционирующее овальное окно. The analysis of students' satisfaction with the educational process of the 5 th year of the pediatric faculty. Results of the questionnaire

conducting monitoring studies within the framework of the quality management system is the basis for improving the educational process, as a result of which top-level cadres are issued in practical public health. One of the tools for improving quality is the questioning of students, analysis and implementation of corrective measures. To assess the quality of teaching discipline Outpatient and Emergency Pediatrics at the Department of Outpatient Pediatrics and Pediatrics of the FPK and PP, 113 5-year students of the pediatric faculty who were trained in the IX and X semesters were surveyed. During the survey, it turned out that 98.2% of respondents are satisfied with the quality of teaching the discipline. However, the students, as the main consumers of the educational process, made a number of comments and suggestions to improve the quality of teaching the discipline, in accordance with which corrective measures are planned.проведение мониторинговых исследований в рамках системы менеджмента качества является основой для совершенствования образовательного процесса, в результате которого в практическое здравоохранение выпускаются кадры высшей квалификации. Organization of preventive medical examinations of minors in the Sverdlovsk region: ways to improve quality

The goal is to analyze the health indicators of the children's population and assess the quality of preventive examinations of minors in the Sverdlovsk region. Materials of the research. Data on the health status of the children of the Sverdlovsk region on the results of preventive medical examinations (PE) for the period 2013-2017 are presented. for different age groups. A sample analysis of the primary medical records of children's polyclinics is performed. Results of the study. The analysis of the structure of pathological affection, depending on the age of the newly diagnosed diseases, the definition of the needs for additional consultations, studies, treatment, as well as the coverage of the data after the completion of the preventive examination. Deficiency of doctors of the experts participating in carrying out of preventive medical examinations of children is established. Identified defects in making conclusions on the results of medical examination by specialists, referral to additional consultations, treatment and treatment. Conclusions. A high level of coverage of the child population of the software was registered. Tendencies in changes in morbidity rates were typified by a decrease in pathological affection in most classes of diseases. The leading causes of morbidity in children 0-14 years old are as follows: diseases of the nervous system, digestive organs, musculoskeletal system, in adolescents - eye diseases, musculoskeletal system and digestive organs. In order to improve the quality of preventive examinations in the region, it is necessary to strengthen internal control over the completeness of the software, the correctness of recording the results, and the direction of the needy for the second stage of medical examination.Цель -провести анализ показателей здоровья детского населения и оценку качества профилактических осмотров несовершеннолетних в Свердловской области. Материалы исследования. Представлены данные о состоянии здоровья детского населения Свердловской области по результатам профилактических медицинских осмотров (По) за период 2013-2017 гг. по различным возрастным группам. Выполнен выборочный анализ первичной медицинской документации детских поликлиник. результаты исследования. Проведен анализ структуры патологической пораженности в зависимости от возраста, впервые выявленных заболеваний, распределения детей по группам здоровья и уровню физического развития, определена нуждаемость в дополнительных консультациях, исследованиях, лечении, а также охват данными услугами после завершения профилактического осмотра. Установлен дефицит врачей специалистов, участвующих в проведении профилактических медицинских осмотров несовершеннолетних. Выявлены дефекты при вынесении заключений по результатам осмотра врачами специалистами, направлении на дополнительные консультации, лечение и реабилитацию. Выводы. Зарегистрирован высокий уровень охвата детского населения По. тенденции в изменениях показателей заболеваемости характеризовались снижением патологической пораженности по большинству классов заболеваний. Ведущими причинами заболеваемости у детей 0-14 лет по-прежнему являются болезни нервной системы, органов пищеварения, костно-мышечной системы, у подростков – болезни глаза, костно-мышечной системы и органов пищеварения. В целях повышения качества проведения профилактических осмотров в регионе необходимо усилить внутренний контроль за полнотой проведения По, правильностью регистрации результатов и направлением нуждающихся на второй этап диспансеризации
