    The Unit Price Implication of Reinforcement Usage in Tie Beam Reinforced Concrete Construction

    The construction cost as one of the most important project resources should be planned and use efficiently. Cost estimation can be analysed using standard of unit price analysis according to Indonesian National Standard (SNI 7394:2008). Related to unit price analysis for tie beam reinforced concrete, the standard mentioned a specific analysis that combine all materials requirement include reinforcement (unit price analysis No. 6.29). In such analysis, reinforcement requirement is stated as much as 200 kg/m3 of concrete. Considering the diversity of dimension design of building structure caused by geographical location and building function, a further study required to response these problems. This research is aimed to provide information to what extent the unit price analysis related to tie beam reinforced concrete can be enforced in cost estimating. Research process initiated with secondary data collection to building construction located in zone 10 and 15 based on earthquake zone map (SNI 1726:2012) in Province of Aceh. The results of analysis informed that the ratio of reinforcement requirement of tie beam in zone 10 is 198.03 kg/m3 to 217.26 kg/m3 of concrete with average ratio 209.83 kg/m3. For zone 15, reinforcement requirement ratio is 203.76 kg/m3 to 233.83 kg/m3 of concrete with average ratio 215.17 kg/m3. The Potential inaccurate of cost estimation appears in the two review zones. Such inaccuracies may have an impact on the insufficient costs for the work. Thus, the use of the standard unit price analysis needs further assessment for proper application

    [1] During December 1999, polar stratospheric clouds (PSCs) were observed in the absence of conditions conducive to generation by topographic gravity waves. The possibility is explored that PSCs can be generated by inertia gravity waves (IGW) radiating from breaking synoptic-scale Rossby waves on the polar front jet. The aerosol features on 7 and 12 December are selected for comparison with theory and with simulations using the University of Wisconsin Nonhydrostatic Modeling System (UWNMS). Consistent with Rossby adjustment theory, a common feature in the UWNMS simulations is radiation of IGW from the tropopause polar front jet, especially from sectors which are evolving rapidly in the Rossby wave breaking process. Packets of gravity wave energy radiate upward and poleward into the cold pool, while individual wave crests propagate poleward and downward, causing mesoscale variations in vertical motion and temperature. On 12 December the eastbound DC-8 lidar observations exhibited a fairly uniform field of six waves in aerosol enhancement in the 14-20 km layer, consistent with vertical displacement by a field of IGW propagating antiparallel to the flow, with characteristic horizontal and vertical wavelengths of 300and300 and 10 km. UWNMS simulations show emanation of a field of IGW upward and southwestward from a northward incursion of the polar front jet. The orientation and evolution of the aerosol features on 7 December are consistent with a single PSC induced by an IGW packet propagating from a breaking Rossby wave over western Russia toward the northeast into the coldest part of the base of the polar vortex, with characteristic period 9hours,verticalwavelength9 hours, vertical wavelength 12 km, and horizontal wavelength 1000km.Lineartheoryshowsthatforbothofthesecases,IGWenergypropagatesupwardat1000 km. Linear theory shows that for both of these cases, IGW energy propagates upward at 1 km/hour and horizontally at 100km/hour,withcharacteristictracespeed100 km/hour, with characteristic trace speed 30 m/s. The spatial orientation of the PSC along IGW phase lines is contrasted with the nearly horizontal filamentary structures in the PSC, which are indicative of flow streamlines. It is suggested that vertical displacement is a crucial factor in determining whether a PSC will form and that most PSCs are relatable to specific synoptic and mesoscale motions

Kondisi ini dianalisis secara menyeluruh berdasarkan skala utama, kepentingan, kemampuan mahasiswa, kebutuhan masyarakat dan dilakukan sosialisasi mengenai PHBS (perilaku hidup bersih dan sehat) sebagai salah satu penanganan masalah stunting. Pendekatan penelitian ini digunakan model studi kasus yang merupakan pendekatan metode kualitatif dengan melakukan obsevasi, analisis situasi, perencanaan, sosialisasi dan evaluasi mandiri, yang dilakukan di Madrasah Ibditiyah Negeri 17 Takengon didesa Blang Mancung Bawah Kecamatan Ketol Aceh Tengah. observasi yang dilakukan dengan cara interaksi langsung (wawancara), sosialisasi dan penerapan perilaku hidup bersih dan sehat, membantu guru dalam memaparkan dan menjelaskan PHBS kepada siswa/i untuk melatih anak secara mandiri dalam memperhatikan kebersihan tubuhnya.     Multiarticular chronic tophaceous gout with severe and multiple ulcerations: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Gout is a common inflammatory arthritis caused by articular precipitation of monosodium urate crystals. It usually affects the first metatarsophalangeal joint of the foot and less commonly other joints, such as wrists, elbows, knees and ankles.</p> <p>Case presentation</p> <p>We report the case of a 75-year-old Caucasian man with tophaceous multiarticular gout, soft-tissue involvement and ulcerated tophi on the first metatarsophalangeal joint of the left foot, on the first interphalangeal joint of the right foot and on the left thumb.</p> <p>Conclusion</p> <p>Ulcers due to tophaceous gout are currently uncommon considering the positive effect of pharmaceutical treatment in controlling hyperuricemia. Surgical treatment is seldom required for gout and is usually reserved for cases of recurrent attacks with deformities, severe pain, infection and joint destruction.</p

    CubeSat Constellation Cloud Winds(C3Winds) A New Wind Observing System to Study Mesoscale Cloud Dynamics and Processes

    The CubeSat Constellation Cloud Winds (C3Winds) is a NASA Earth Venture Instrument (EV-I) concept with the primary objective to better understand mesoscale dynamics and their structures in severe weather systems. With potential catastrophic damage and loss of life, strong extratropical and tropical cyclones (ETCs and TCs) have profound three-dimensional impacts on the atmospheric dynamic and thermodynamic structures, producing complex cloud precipitation patterns, strong low-level winds, extensive tropopause folds, and intense stratosphere-troposphere exchange. Employing a compact, stereo IR-visible imaging technique from two formation-flying CubeSats, C3Winds seeks to measure and map high-resolution (2 km) cloud motion vectors (CMVs) and cloud geometric height (CGH) accurately by tracking cloud features within 5-15 min. Complementary to lidar wind observations from space, the high-resolution wind fields from C3Winds will allow detailed investigations on strong low-level wind formation in an occluded ETC development, structural variations of TC inner-core rotation, and impacts of tropopause folding events on tropospheric ozone and air quality. Together with scatterometer ocean surface winds, C3Winds will provide a more comprehensive depiction of atmosphere-boundary-layer dynamics and interactive processes. Built upon mature imaging technologies and long history of stereoscopic remote sensing, C3Winds provides an innovative, cost-effective solution to global wind observations with potential of increased diurnal sampling via CubeSat constellation

    Association of extent of cannabis use and psychotic like intoxication experiences in a multi-national sample of first episode psychosis patients and controls

    BackgroundFirst episode psychosis (FEP) patients who use cannabis experience more frequent psychotic and euphoric intoxication experiences compared to controls. It is not clear whether this is consequent to patients being more vulnerable to the effects of cannabis use or to their heavier pattern of use. We aimed to determine whether extent of use predicted psychotic-like and euphoric intoxication experiences in patients and controls and whether this differs between groups.MethodsWe analysed data on patients who had ever used cannabis (n = 655) and controls who had ever used cannabis (n = 654) across 15 sites from six countries in the EU-GEI study (2010-2015). We used multiple regression to model predictors of cannabis-induced experiences and to determine if there was an interaction between caseness and extent of use.ResultsCaseness, frequency of cannabis use and money spent on cannabis predicted psychotic-like and euphoric experiences (p ≤ 0.001). For psychotic-like experiences (PEs) there was a significant interaction for caseness × frequency of use (p &lt; 0.001) and caseness × money spent on cannabis (p = 0.001) such that FEP patients had increased experiences at increased levels of use compared to controls. There was no significant interaction for euphoric experiences (p &gt; 0.5).ConclusionsFEP patients are particularly sensitive to increased psychotic-like, but not euphoric experiences, at higher levels of cannabis use compared to controls. This suggests a specific psychotomimetic response in FEP patients related to heavy cannabis use. Clinicians should enquire regarding cannabis related PEs and advise that lower levels of cannabis use are associated with less frequent PEs

    Optimal functional outcome measures for assessing treatment for Dupuytren's disease: A systematic review and recommendations for future practice

Copyright © 2013 Ball et al.; licensee BioMed Central Ltd.Background: Dupuytren's disease of the hand is a common condition affecting the palmar fascia, resulting in progressive flexion deformities of the digits and hence limitation of hand function. The optimal treatment remains unclear as outcomes studies have used a variety of measures for assessment. Methods: A literature search was performed for all publications describing surgical treatment, percutaneous needle aponeurotomy or collagenase injection for primary or recurrent Dupuytren’s disease where outcomes had been monitored using functional measures. Results: Ninety-one studies met the inclusion criteria. Twenty-two studies reported outcomes using patient reported outcome measures (PROMs) ranging from validated questionnaires to self-reported measures for return to work and self-rated disability. The Disability of Arm, Shoulder and Hand (DASH) score was the most utilised patient-reported function measure (n=11). Patient satisfaction was reported by eighteen studies but no single method was used consistently. Range of movement was the most frequent physical measure and was reported in all 91 studies. However, the methods of measurement and reporting varied, with seventeen different techniques being used. Other physical measures included grip and pinch strength and sensibility, again with variations in measurement protocols. The mean follow-up time ranged from 2 weeks to 17 years. Conclusions: There is little consistency in the reporting of outcomes for interventions in patients with Dupuytren’s disease, making it impossible to compare the efficacy of different treatment modalities. Although there are limitations to the existing generic patient reported outcomes measures, a combination of these together with a disease-specific questionnaire, and physical measures of active and passive individual joint Range of movement (ROM), grip and sensibility using standardised protocols should be used for future outcomes studies. As Dupuytren’s disease tends to recur following treatment as well as extend to involve other areas of the hand, follow-up times should be standardised and designed to capture both short and long term outcomes

    What are we measuring? A critique of range of motion methods currently in use for Dupuytren's disease and recommendations for practice

    Background: Range of motion is the most frequently reported measure used in practice to evaluate outcomes. A goniometer is the most reliable tool to assess range of motion yet, the lack of consistency in reporting prevents comparison between studies. The aim of this study is to identify how range of motion is currently assessed and reported in Dupuytren’s disease literature. Following analysis recommendations for practice will be made to enable consistency in future studies for comparability. This paper highlights the variation in range of motion reporting in Dupuytren’s disease. Methods: A Participants, Intervention, Comparison, Outcomes and Study design format was used for the search strategy and search terms. Surgery, needle fasciotomy or collagenase injection for primary or recurrent Dupuytren’s disease in adults were included if outcomes were monitored using range of motion to record change. A literature search was performed in May 2013 using subject heading and free-text terms to also capture electronic publications ahead of print. In total 638 publications were identified and following screening 90 articles met the inclusion criteria. Data was extracted and entered onto a spreadsheet for analysis. A thematic analysis was carried out to establish any duplication, resulting in the final range of motion measures identified. Results: Range of motion measurement lacked clarity, with goniometry reportedly used in only 43 of the 90 studies, 16 stated the use of a range of motion protocol. A total of 24 different descriptors were identified describing range of motion in the 90 studies. While some studies reported active range of motion, others reported passive or were unclear. Eight of the 24 categories were identified through thematic analysis as possibly describing the same measure, ‘lack of joint extension’ and accounted for the most frequently used. Conclusions: Published studies lacked clarity in reporting range of motion, preventing data comparison and meta-analysis. Percentage change lacks context and without access to raw data, does not allow direct comparison of baseline characteristics. A clear description of what is being measured within each study was required.
