    The perioperative immune response

    PURPOSE OF REVIEW A host of immune modulators are now available in clinical practice. The perioperative period is characterized by profound alterations in host immunity, which can result in poor outcomes, which include infection, cancer recurrence and organ failure. Manipulation of the perioperative immune response has the potential to improve outcomes. A complete understanding of the mechanisms and clinical consequences of altered immune function in this setting is therefore imperative. RECENT FINDINGS Recent in-vivo data have emerged which further our understanding of the interaction between tissue damage, immune modulation and clinical outcomes by utilizing novel laboratory techniques capable of monitoring single-cell immune signatures. Traditional gene expression assays have continued to demonstrate their utility and have been instrumental in defining the host response to perioperative allogeneic blood transfusion. These mechanistic studies are complemented by large clinical studies describing associations between anaesthetic modalities and immune-related outcomes. SUMMARY Laboratory techniques are now available that can monitor the perioperative immune response and could be further developed to introduce personalized care pathways. Consideration must also be given to anaesthesia techniques and perioperative treatments that, although not immediately harmful, may be associated with poor outcomes temporally distant from the treatment, secondary to induced immunosuppression

    No evidence for associations between men's salivary testosterone and responses on the Intrasexual Competitiveness Scale

    Objectives: Many previous studies have investigated relationships between men’s competitiveness and testosterone. For example, the extent of changes in men’s testosterone levels following a competitive task predicts the likelihood of them choosing to compete again. Recent work investigating whether individual differences in men’s testosterone levels predict individual differences in their competitiveness have produced mixed results. Methods: In light of the above, we investigated whether men’s (N = 59) scores on the Intrasexual Competitiveness Scale were related to either within-subject changes or between-subject differences in men’s salivary testosterone levels. Results: Men’s responses on the Intrasexual Competitiveness Scale did not appear to track within-subject changes in testosterone. By contrast with one recent study, men’s Intrasexual Competitiveness Scale also did not appear to be related to individual differences in testosterone. Conclusions: Our results present no evidence for associations between men’s testosterone and their responses on the Intrasexual Competitiveness Scale

    The relative contributions of facial shape and surface information to perceptions of attractiveness and dominance

    Although many studies have investigated the facial characteristics that influence perceptions of others’ attractiveness and dominance, the majority of these studies have focused on either the effects of shape information or surface information alone. Consequently, the relative contributions of facial shape and surface characteristics to attractiveness and dominance perceptions are unclear. To address this issue, we investigated the relationships between ratings of original versions of faces and ratings of versions in which either surface information had been standardized (i.e., shape-only versions) or shape information had been standardized (i.e., surface-only versions). For attractiveness and dominance judgments of both male and female faces, ratings of shape-only and surface-only versions independently predicted ratings of the original versions of faces. The correlations between ratings of original and shape-only versions and between ratings of original and surface-only versions differed only in two instances. For male attractiveness, ratings of original versions were more strongly related to ratings of surface-only than shape-only versions, suggesting that surface information is particularly important for men’s facial attractiveness. The opposite was true for female physical dominance, suggesting that shape information is particularly important for women’s facial physical dominance. In summary, our results indicate that both facial shape and surface information contribute to judgments of others’ attractiveness and dominance, suggesting that it may be important to consider both sources of information in research on these topics

    Do sixth-grade writers need process strategies?

    Background: Strategy-focussed writing instruction trains students both to set explicit product goals and to adopt specific procedural strategies, particularly for planning text. A number of studies have demonstrated that strategy-focussed writing instruction is effective in developing writing performance. Aim: The present study aimed to determine whether teaching process strategies provides additional benefit over teaching students to set product goals. Sample: 94 typically developing Spanish sixth-grade (upper primary) students. Method: Students received 10 hours of instruction in one of three conditions: Strategy-focussed training in setting product goals and in writing procedures (planning and revision; Product-and-Process), strategy-focussed training in setting product goals (Product-Only), and product-focussed instruction (Control). Students' writing performance was assessed before, during, and after intervention with process measures based on probed self-report and holistic and text-analytic measures of text quality. Results: Training that included process instruction was successful in changing students' writing processes, with no equivalent process changes in the Product-Only or control conditions. Both Process-and-Product and Product-Only conditions resulted in substantial improvements in the quality of students' texts relative to controls, but with no evidence of benefits of process instruction over those provided by the Product-Only condition. Teaching process substantially increased time-on-task. Conclusions: Our findings confirm the value of strategy-focussed writing instruction, but question the value of training specific process strategies

    Theory of short-range magnetic order for the t-J model

    We present a self-consistent theory of magnetic short-range order based on a spin-rotation-invariant slave-boson representation of the 2D t-J model. In the functional-integral scheme, at the nearest-neighbour pair-approximation level, the bosonized t-J Lagrangian is transformed to a classical Heisenberg model with an effective (doping-dependent) exchange interaction which takes into account the interrelation of ``itinerant'' and ``localized'' magnetic behaviour. Evaluating the theory in the saddle-point approximation, we find a suppression of antiferromagnetic and incommensurate spiral long-range-ordered phases in the favour of a paramagnetic phase with pronounced antiferromagnetic short-range correlations.Comment: 2 pages, 1 Postscript figure, LTpaper.sty, Proc.     Analysis and clinical findings of cases positive for the novel synthetic cannabinoid receptor agonist MDMB-CHMICA

    Context: MDMB-CHMICA is a synthetic cannabinoid receptor agonist which has caused concern due to its presence in cases of adverse reaction and death. Method: 43 cases of suspected synthetic cannabinoid ingestion were identified from patients presenting at an Emergency Department and from post-mortem casework. These were subjected to liquid-liquid extraction using tertiary-butyl methyl ether and quantitatively analysed by Electospray Ionisation Liquid Chromatography – tandem Mass Spectrometry. For positive samples, case and clinical details were sought and interrogated. Results: 11 samples were found positive for MDMB-CHMICA. Concentrations found ranged from <1 – 22 ng/mL (mean: 6 ng/mL, median: 3 ng/mL). The age range was 15 – 44 years (mean: 26 years, median: 21 years), with the majority (82%) of positive results found in males. Clinical presentations included hypothermia, hypoglycaemia, syncope, recurrent vomiting, altered mental state and serotonin toxicity, with corresponding concentrations of MDMB-CHMICA as low as <1 ng/mL. Duration of hospitalisation ranged from 3 – 24 hours (mean: 12 hours, median: 8 hours). Discussion: The concentration range presented in this case series is indicative of MDMB-CHMICA having a high potency, as is known to be the case for other synthetic cannabinoid receptor agonists. The age range and gender representation were consistent with that reported for users of other drugs of this type. The clinical presentations observed were typical of synthetic cannabinoid receptor agonists and show the difficulties in identifying reactions potentially associated with drugs of this type. Conclusion: The range of MDMB-CHMICA concentrations in Emergency Department presentations (n=9) and post-mortem cases (n=2) was reported. No correlation between the concentration of this drug and clinical presentation or cause of death was reported in this sample. However, the potential for harm associated with low concentrations of MDMB-CHMICA and the symptoms of toxicity being non-specific was highlighted

    The present study investigated the effect of psycholinguistic variables on measures of response latency and mean interkeystroke interval in a typewritten picture naming task, with the aim to outline the functional organization of the stages of cognitive processing and response execution associated with typewritten word production. Onset latencies were modulated by lexical and semantic variables traditionally linked to lexical retrieval, such as word frequency, age of acquisition, and naming agreement. Orthographic variables, both at the lexical and sublexical level, appear to influence just within-word interkeystroke intervals, suggesting that orthographic information may play a relevant role in controlling actual response execution. Lexical-semantic variables also influenced speed of execution. This points towards cascaded flow of activation between stages of lexical access and response execution
