    Comparison of Backscattered Scanning Electron Microscopy and Microradiography of Secondary Caries

    Carious lesions are usually studied using light microscopy and/or microradiography which require preparation of thin sections. Backscattered scanning electron microscopy (BSEM) has received little attention although it provides information similar to that obtained with microradiographs, with the potential for higher resolution. Recently, microscopes have been introduced that can be used to study wet or nonconducting specimens, offering techniques for studying specimens without desiccation or preparation of thin sections. This investigation sought to determine if secondary carious lesions have the same characteristics when studied by microradiography as when using the wet BSEM mode. Microradiographs were made of thin sections from restored teeth with secondary caries induced in an artificial caries system. The thin sections were also studied by BSEM with a partial pressure in the specimen chamber to prevent specimen charging. Comparisons of the lesion size and shape were made using the two methods. Lesion depth measurements in enamel were the same; lesions that penetrated into dentin appeared to be of similar size and shape, but lesion depths measured by BSEM were slightly greater (paired t-test, p \u3c .05). This was a result of cracks at the carious enamel-dentin interface that probably developed during storage of the samples. Variations in the surface enamel rod structure and the development of subsurface lesions were apparent. Several zones were also apparent in the carious dentin, demonstrating loss of dentinal tubule detail in the depth of the lesion, collapse of tubules, and hypermineralized regions near the advancing front of the lesion. Several additional samples of natural carious teeth were examined. They demonstrated the characteristic structural features of the carious process. This method appears to have considerable promise for the study of such lesions

    RaLLe: A Framework for Developing and Evaluating Retrieval-Augmented Large Language Models

    Retrieval-augmented large language models (R-LLMs) combine pre-trained large language models (LLMs) with information retrieval systems to improve the accuracy of factual question-answering. However, current libraries for building R-LLMs provide high-level abstractions without sufficient transparency for evaluating and optimizing prompts within specific inference processes such as retrieval and generation. To address this gap, we present RaLLe, an open-source framework designed to facilitate the development, evaluation, and optimization of R-LLMs for knowledge-intensive tasks. With RaLLe, developers can easily develop and evaluate R-LLMs, improving hand-crafted prompts, assessing individual inference processes, and objectively measuring overall system performance quantitatively. By leveraging these features, developers can enhance the performance and accuracy of their R-LLMs in knowledge-intensive generation tasks.     SimplyRetrieve: A Private and Lightweight Retrieval-Centric Generative AI Tool

    Large Language Model (LLM) based Generative AI systems have seen significant progress in recent years. Integrating a knowledge retrieval architecture allows for seamless integration of private data into publicly available Generative AI systems using pre-trained LLM without requiring additional model fine-tuning. Moreover, Retrieval-Centric Generation (RCG) approach, a promising future research direction that explicitly separates roles of LLMs and retrievers in context interpretation and knowledge memorization, potentially leads to more efficient implementation. SimplyRetrieve is an open-source tool with the goal of providing a localized, lightweight, and user-friendly interface to these sophisticated advancements to the machine learning community. SimplyRetrieve features a GUI and API based RCG platform, assisted by a Private Knowledge Base Constructor and a Retrieval Tuning Module. By leveraging these capabilities, users can explore the potential of RCG for improving generative AI performance while maintaining privacy standards.     What impact does postgraduate clinical training have on empathy among Japanese trainee dentists?

    Background Enhancing empathy in healthcare education is a critical component in the development of a relationship between healthcare professionals and patients that would ensure better patient care; improved patient satisfaction, adherence to treatment, patients’ medication self-efficacy, improved treatment outcomes, and reduced patient anxiety. Unfortunately, however, the decline of empathy among students has been frequently reported. It is especially common when the curriculum transitions to a clinical setting. However, some studies have questioned the significance and frequency of this decline. Thus, the purpose of this study was to determine the impact of postgraduate clinical training on dental trainees’ empathy from cognitive, behavioral, and patients’ perspective. Methods This study included 64 trainee dentists at Okayama University Hospital and 13 simulated patients (SPs). The trainee dentists carried out initial medical interviews with SPs twice, at the beginning and the end of their clinical training. The trainees completed the Japanese version of the Jefferson Scale of Empathy for health professionals just before each medical interview. The SPs evaluated the trainees’ communication using an assessment questionnaire immediately after the medical interviews. The videotaped dialogue from the medical interviews was analyzed using the Roter Interaction Analysis System. Results No significant difference was found in the self-reported empathy score of trainees at the beginning and the end of the clinical training (107.73 [range, 85–134] vs. 108.34 [range, 69–138]; p = 0.643). Considering the results according to gender, male scored 104.06 (range, 88–118) vs. 101.06 (range, 71–122; p = 0.283) and female 109.17 (range, 85–134) vs. 111.20 (range, 69–138; p = 0.170). Similarly, there was no difference in the SPs’ evaluation of trainees’ communication (10.73 vs. 10.38, p = 0.434). Communication behavior in the emotional responsiveness category for trainees in the beginning was significantly higher than that at the end (2.47 vs. 1.14, p = 0.000). Conclusions Overall, a one-year postgraduate dental training program neither reduced nor increased trainee dentists’ empathy levels. Providing regular education support in this area may help trainees foster their empathy

    Titanium as an Instant Adhesive for Biological Soft Tissue

    A variety of polymer‐ and ceramic‐based soft‐tissue adhesives have been developed as alternatives to surgical sutures, yet several disadvantages regarding the mechanical properties, biocompatibility, and handling hinder their further application particularly when applied for immobilization of implantable devices. Here, it is reported that a biocompatible and tough metal, titanium (Ti), shows instant and remarkable adhesion properties after acid treatment, demonstrated by ex vivo shear adhesion tests with mouse dermal tissues. Importantly, in vivo experiments demonstrate that the acid‐treated Ti can easily and stably immobilize a device implanted in the mouse subcutaneous tissue. Collectively, the acid‐treated Ti is shown as a solid‐state instant adhesive material for biological soft tissues, which can have diverse applications including immobilization of body‐implantable devices

    On the Nature of AX J2049.6+2939 and AX J2050.0+2914

    AX J2049.6+2939 is a compact X-ray source in the vicinity of the southern blow-up region of the Cygnus Loop supernova remnant (Miyata et al. 1998a). This source was the brightest X-ray source inside the Cygnus Loop observed during the ASCA survey project. The X-ray spectrum was well fitted by a power-law function with a photon index of 2.1±0.1-2.1 \pm 0.1. Short-term timing analysis was performed and no coherent pulsation was found. Follow-up observations with ASCA have revealed a large variation in X-ray intensity by a factor of \simeq 50, whereas the spectral shape did not change within the statistical uncertainties. In the second ASCA observation, we found another X-ray source, AX J2050.0+2941, at the north east of AX J2049.6+2939. During the three ASCA observations, the X-ray intensity of AX J2050.0+2941 varied by a factor of \simeq4. No coherent pulsations could be found for AX J2050.0+2941. We have performed optical photometric and spectroscopic observations in the vicinity of AX J2049.6+2939 at the Kitt Peak National Observatory (KPNO). As a result, all objects brighter than BB-band magnitude of 22 in the error box can be identified with normal stars. Combined with the X-ray results and the fact that there are no radio counterparts, AX J2049.6+2939 is not likely to be either an ordinary rotation-powered pulsar or an AGN. The nature of AX J2049.6+2939 is still unclear and further observations over a wide energy band are strongly required. As to AX J2050.0+2941, the long-term X-ray variability and the radio counterpart suggests that it is an AGN.Comment: 23 pages, 4 figures, Accepted for publication by Astrophysical Journa

    Peptide-modified Substrate for Modulating Gland Tissue Growth and Morphology In Vitro

    In vitro fabricated biological tissue would be a valuable tool to screen newly synthesized drugs or understand the tissue development process. Several studies have attempted to fabricate biological tissue in vitro. However, controlling the growth and morphology of the fabricated tissue remains a challenge. Therefore, new techniques are required to modulate tissue growth. RGD (arginine-glycine-aspartic acid), which is an integrin-binding domain of fibronectin, has been found to enhance cell adhesion and survival; it has been used to modify substrates for in vitro cell culture studies or used as tissue engineering scaffolds. In addition, this study shows novel functions of the RGD peptide, which enhances tissue growth and modulates tissue morphology in vitro. When an isolated submandibular gland (SMG) was cultured on an RGD-modified alginate hydrogel sheet, SMG growth including bud expansion and cleft formation was dramatically enhanced. Furthermore, we prepared small RGD-modified alginate beads and placed them on the growing SMG tissue. These RGD-modified beads successfully induced cleft formation at the bead position, guiding the desired SMG morphology. Thus, this RGD-modified material might be a promising tool to modulate tissue growth and morphology in vitro for biological tissue fabrication

    Effects of Perioperative Management Training Program on the Awareness of Team Medicine among Trainee Dentists

     質が高く、安心・安全な医療を実現していくために、チーム医療の促進は重要な課題であり、チーム医療を担う医療人の育成が必要とされている。岡山大学病院歯科医師臨床研修では、平成23年度から周術期管理センターを研修の場とする「チーム医療研修プログラム」を開始した。本報告では、研修歯科医のプログラムに対する関心とチーム医療に関する理解に与えた効果をアンケート調査によって検討することを目的とした。対象は平成23年度本学病院研修歯科医44名とし、医師、看護師、薬剤師、理学療法士、管理栄養士など多職種によって行われる周術期管理チーム医療を体験させた。受講前には本プログラムに対する期待度を、受講後には満足度、有意義度について質問し、研修歯科医のプログラムに対する関心度を評価した。また、プログラム受講前後に周術期チーム医療に関する用語や職種について質問し、チーム医療に関する理解度を評価した。90.9%の研修歯科医がプログラムに対して期待し、受講後は88.6%が満足だった、84.1%が有意義だったと回答した。さらに、周術期管理に関する用語について主観的理解度が向上し、チーム医療にかかわる職種についての理解が深まった。歯科医師臨床研修におけるチーム医療教育の方法として、周術期管理チーム医療の現場に実際に関与させるプログラムは、研修歯科医の関心が高く、研修歯科医の周術期管理チーム医療に対する理解を深めることが示唆された
