Przypadek 81-letniej pacjentki z zespołem tachykardia–bradykardia po obustronnej mastektomii z infekcją loży stymulatora w wywiadzie
The case report presents a history of an 81-year-old woman with paroxysmal atrial fibrillation, treated with apixaban at a reduced dose, who has a DDD pacemaker implanted in March 2018 due to tachycardia-bradycardia syndrome. In the interview were left-hand mastectomy in 2012 and right-hand mastectomy in October 2022 caused by breast cancer. In November 2022 the patient was admitted to the Department of Cardiology to stimulate system removal because of a pacemaker pocket infection. Then, according to the interview of sick sinus syndrome, the electrocardiogram Holter monitoring was made and due to its results the patient was qualified for repeated electrotherapy. Considering the general clinical view it was decided to implant the leadless MICRA pacemaker.W niniejszej pracy przedstawiono przypadek 81-letniej pacjentki z wywiadem napadowego migotania przedsionków, leczonej przeciwkrzepliwie apiksabanem w dawce zredukowanej, po implantacji stymulatora typu DDD w marcu 2018 roku z powodu zespołu tachykardia–bradykardia. W wywiadzie stan po zabiegu mastektomii lewostronnej w 2012 roku, a w październiku 2022 roku — mastektomii prawostronnej z powodu raka piersi. W listopadzie 2022 roku chora została przyjęta do Kliniki Kardiologii Uniwersyteckiego Szpitala Klinicznego w Białymstoku w celu usunięcia układu stymulującego ze względu na zakażenie loży stymulatora. Ze względu na wcześniejszy wywiad zespołu chorego węzła po usunięciu układu wykonano badanie elektrokardiograficzne metodą Holtera i ponownie zakwalifikowano pacjentkę do elektroterapii. Heart failure with preserved ejection fraction in patient with chronic kidney disease
Częstość występowania przewlekłej choroby nerek oraz niewydolności serca wzrasta istotnie szczególnie u pacjentów w podeszłym wieku i stanowi bardzo istotny problem kliniczny i społeczny. Każda osoba z przewlekłą chorobą nerek powinna być traktowana jako chory z wysokim ryzykiem sercowo- naczyniowym, a u pacjentów z niewydolnością serca i przewlekłą chorobą nerek należy się liczyć ze znacznie gorszym rokowaniem. U pacjentów ze współistniejącą niewydolnością serca oraz przewlekłą chorobą nerek zarówno diagnostyka, jak i leczenie jest znacznie utrudnione i wymaga szczególnej uwagi, wielodyscyplinarnego podejścia oraz ścisłej współpracy nefrologa z kardiologiem, co w wielu przypadkach pozwala na uzyskanie wieloletniego stabilnego przebiegu klinicznego. W pracy przedstawiono opis przypadku pacjenta z rozkurczową niewydolnością serca oraz towarzyszącą przewlekłą chorobą nerek, omówiono postępowanie diagnostyczne i terapeutyczne.Patients with chronic kidney disease and heart failure have already become the clinical and social issue of great importance. Moreover, the improvement of health care and aging of the society contribute to the escalation of the scope of the issue. Every patient with chronic kidney disease should be considered at increased cardiovascular risk. Additionally, in patients with heart failure and chronic kidney disease worse prognosis should be taken into account. The treatment of such patients includes the elimination of reversible causes of the diseases and the prevention of their progression with optimal risk factors control. In patients with heart failure and coexisting chronic kidney disease diagnostic work-up and treatment are difficult and require much attention, multi-professional approach with constant cooperation of nephrologist and cardiologist, which in many cases results in stable clinical course of the diseases over the years. We present a case of heart failure with preserved ejection fraction. We also presented diagnostic and therapeutic strategy in this patient
A cross-national study of internationalisation barriers with reference to SME value chain
Research background: The study responses to the internationalisation issue, one of the essential factors of SMEs growth. Particularly to companies' efforts towards setting international cooperation and circumstances obstructing these struggles. The study takes into consideration that internationalisation in specific areas of company's operations differs noticeably. Similarly, different barriers may arise depending on the areas of the company's value chain that are the focus of the enterprises' internationalisation strategies.
Purpose of the article: This study aims to identify barriers to internationalisation regarding various areas of a company's value chain. The study employs the Value Chain Model by Porter.
Methods: The field data was collected using a questionnaire survey on a sample of small and medium enterprises (SMEs) located in countries in the Baltic Sea region (Denmark, Estonia, Finland, Germany, Latvia, Lithuania and Poland). A multiple regression analysis was performed to determine the impact of barriers on internationalisation.
Finding & value added: The study indicates three areas that are most often the subject of internationalisation in SMEs: operations, outbound logistics and marketing and sales. Barriers differ between value chain areas; however, cultural differences and competition are perceived as the most important hindering factors by companies experienced in internationalisation. At the same time, barriers regarding knowledge and finance diminish when companies becoming more experienced in internationalisation in particular value chain areas. Czy kontrast echokardiograficzny jest bezpieczny i użyteczny w ocenie funkcji lewej komory w okresie okołooperacyjnym po zabiegu kardiochirurgicznym? Badanie pilotażowe
Background: Myocardial contrast echocardiography (CE) improves the quality of standard echocardiography. The value of
CE during the early post-operative period after coronary artery bypass grafting (CABG) has not yet been well established.
Aim: To evaluate the accuracy and safety of CE used for the assessment of left ventricular (LV) function in patients after
CABG in the setting of a cardiosurgery post-operative unit (CPU) in comparison with conventional transthoracic echocardiography
(TTE).
Methods: Echocardiographic contrast agent Sono-Vue (Bracco, Italy) was administered in 30 consecutive patients with technically
difficult TTE, after CABG treated in the CPU. Improved quality of echocardiographic imaging was assessed by the
number of analysable LV segments. The LV end-diastolic and end-systolic volume (LVEDV, LVESV) and LV ejection fraction
(LVEF) were calculated before and after contrast administration.
Results: There were no side effects after contrast administration. The mean number of LV segments visualised after CE
increased from 8.0 ± 4 to 16.9 ± 0.1 in all patients (52.4% of improvement); 272 (52.3%) out of 510 segments were
described as poorly visible using standard TTE while only four (0.8%) segments were not visible after contrast administration.
Out of all visible hypokinetic, akinetic and dyskinetic segments, 63 (12%) segments were classified wrongly. The LV
volumes were smaller and LVEF significantly higher after CE compared to standard TTE (LVEDV 127 mL vs 98 mL; LVESV
65 mL vs 45 mL; p = 0.0002 and p = 0.0016, respectively). In all methods used: visual, Simpson’s method and biplane
method, LVEF was significantly higher compared to standard TTE (p = 0.012, p = 0.0088, p = 0.00065, respectively). In
patients after surgical LV restoration, CE enabled the assessment of LV geometry, patch localisation and the exclusion of
the presence of LV thrombus.
Conclusions: Contrast echocardiography is a rapid, simple and safe technique when performed at bedside in a cardiosurgery
post-operative unit setting, permitting accurate both segmental and global wall motion analysis. The use of contrast
echocardiography could help to identify causes leading to LV systolic dysfunction immediately after CABG surgery.

we wczesnym okresie po operacji pomostowania aortalno-wieńcowego (CABG) nie została dotychczas dostatecznie zbadana.
Cel: Celem pracy była ocena wartości diagnostycznej i bezpieczeństwa echokardiografii kontrastowej w ocenie funkcji lewej
komory u pacjentów po operacji CABG w warunkach kardiochirurgicznego oddziału pooperacyjnego w porównaniu z tradycyjną
echokardiografią przezklatkową.
Metody: Kontrast echokardiograficzny Sono-Vue (Bracco, Włochy) zastosowano u 30 kolejnych pacjentów z trudnym technicznie
obrazem przezklatkowym po CABG w warunkach kardiochirurgicznego oddziału pooperacyjnego. Poprawę jakości
badania echokardiograficznego określano na podstawie liczby segmentów lewej komory możliwych do oceny po podaniu
kontrastu. Objętość końcoworozkurczową i końcowoskurczową lewej komory (LVEDV, LVESV) oraz frakcję wyrzutową lewej
komory (LVEF) oceniano przed podaniem kontrastu i po jego zastosowaniu.
Wyniki: Nie zaobserwowano skutków ubocznych podania kontrastu echokardiograficznego. Liczba segmentów lewej komory
po podaniu kontrastu wzrosła z 8,0 ± 4 do 16,9 ± 0,1 (52,4% poprawy). Spośród 510 segmentów 272 (52,3%) było
opisanych jako źle widoczne, podczas gdy tylko 4 (0,8%) segmenty były niewidoczne po podaniu środka kontrastowego.
Spośród widocznych hipokinetycznych, akinetycznych i dyskinetycznych segmentów 63 (12%) segmenty były źle zakwalifikowane.
Objętości lewej komory były mniejsze, a LVEF istotnie statystycznie wyższa po podaniu kontrastu w porównaniu ze
standardowym badaniem (LVEDV 127 ml v. 98 ml; LVESV 65 ml v. 45 ml; p = 0,0002 i p = 0,0016, odpowiednio). We
wszystkich metodach użytych do oceny LVEF (metodzie wizualnej, metodzie Simpsona i metodzie dwupłaszczyznowej)
LVEF była istotnie wyższa w porównaniu ze standardowym badaniem (p = 0,012; p = 0,0088; p = 0,00065, odpowiednio).
U chorych po plastyce lewej komory podanie kontrastu umożliwiło ocenę jej geometrii, lokalizację naszytej łaty oraz wykluczyło
obecność zakrzepu w lewej komorze.
Wnioski: Kontrast echokardiograficzny jest szybką, prostą i bezpieczną metodą w warunkach kardiochirurgicznego oddziału
pooperacyjnego, która pozwala ocenić segmentalną i globalną kurczliwość lewej komory. Zastosowanie echokardiografii
kontrastowej może pomóc w różnicowaniu stanów przebiegających z dysfunkcją lewej komory tuż po operacji CABG.

Echocardiographic predictors of thrombus in left atrial appendage-The role of novel transthoracic parameters
INTRODUCTION: The left atrium appendage thrombus (LAAT) formation is a complex process. A CHA2DS2-VASc scale is an established tool for determining the thromboembolic risk and initiation of anticoagulation treatment in patients with atrial fibrillation or flutter (AF/AFL). We aimed to identify whether any transthoracic echocardiography (TTE) parameters could have an additional impact on LAAT detection. METHODS: That is a sub-study of multicenter, prospective, observational study LATTEE (NCT03591627), which enrolled 3,109 consecutive patients with AF/AFL referred for transesophageal echocardiography (TEE) before cardioversion or ablation. RESULTS: LAAT was diagnosed in 8.0% of patients. The univariate logistic regression analysis [based on pre-specified in the receiver operating characteristic (ROC) analysis cut-off values with AUC ≥ 0.7] identified left ventricular ejection fraction (LVEF) ≤ 48% and novel TTE parameters i.e., the ratios of LVEF and left atrial diameter (LAD) ≤ 1.1 (AUC 0.75; OR 5.64; 95% CI 4.03-7.9; p < 0.001), LVEF to left atrial area (LAA) ≤ 1.7 (AUC 0.75; OR 5.64; 95% CI 4.02-7.9; p < 0.001), and LVEF to indexed left atrial volume (LAVI) ≤ 1.1 (AUC 0.75, OR 6.77; 95% CI 4.25-10.8; p < 0.001) as significant predictors of LAAT. In a multivariate logistic regression analysis, LVEF/LAVI and LVEF/LAA maintained statistical significance. Calculating the accuracy of the abovementioned ratios according to the CHA2DS2-VASc scale values revealed their highest predictive power for LAAT in a setting with low thromboembolic risk. CONCLUSION: Novel TTE indices could help identify patients with increased probability of the LAAT, with particular applicability for patients at low thromboembolic risk
Left Atrial Thrombus in Atrial Fibrillation/Flutter Patients in Relation to Anticoagulation Strategy: LATTEE Registry
BACKGROUND: Atrial fibrillation (AF) and flutter (AFl) increase the risk of thromboembolism. The aim of the study was to assess the prevalence of left atrial thrombus (LAT) in AF/AFl in relation to oral anticoagulation (OAC). METHODS: LATTEE (NCT03591627) was a multicenter, prospective, observational study enrolling consecutive patients with AF/AFl referred for transesophageal echocardiography before cardioversion or ablation. RESULTS: Of 3109 patients enrolled, 88% were on chronic, 1.5% on transient OAC and 10% without OAC. Of patients on chronic OAC, 39% received rivaroxaban, 30% dabigatran, 14% apixaban and 18% vitamin K antagonists (VKA). Patients on apixaban were oldest, had the worst renal function and were highest in both bleeding and thromboembolic risk, and more often received reduced doses. Prevalence of LAT was 8.0% (7.3% on chronic OAC vs. 15% without OAC; p < 0.01). In patients on VKA, prevalence of LAT was doubled compared to patients on non-VKA-OACs (NOACs) (13% vs. 6.0%; p < 0.01), even after propensity score weighting (13% vs. 7.5%; p < 0.01). Prevalence of LAT in patients on apixaban was higher (9.8%) than in those on rivaroxaban (5.7%) and dabigatran (4.7%; p < 0.01 for both comparisons), however, not after propensity score weighting. CONCLUSIONS: The prevalence of LAT in AF is non-negligible even on chronic OAC. The risk of LAT seems higher on VKA compared to NOAC, and similar between different NOACs
