The global risk of pressure ulcers among elderly patients: initial diagnosis
Background. Prevention is the most effective method of reducing the risk of pressure ulcers. Prophylaxisinvolves several actions and procedures that can decrease the risk and aid implementation of rapidtreatment if pressure ulcers develop.Aim. To evaluate the risk of pressure ulcers in elderly patients.Material and methods. The study group involved patients hospitalized at the Department and Clinic ofGeriatric Medicine in Bydgoszcz: 82 females and 34 males (116 in total). The study used a diagnostic pollmethod with the application of an ADL (Activities of Daily Living) questionnaire to assess daily efficiencyon the basis of the Katz Scale, CDS (Care Dependency Scale) questionnaire to measure the level of caredependency and human needs, and a risk scale for pressure ulcer development devised by D. Norton.Results. The risk of pressure ulcers was observed in approximately one third of the study subjects (30.2%).Almost a half of the patient group had multiple morbidities. Problems concerning the care of patientswith a high risk of pressure ulcers were usually connected with a lack of independence in changing bodyposition, movement, dressing, and sphincter dysfunction.Conclusion. There is a correlation between the level of functional capability and the risk of pressureulcers. The accumulation of problems related to the lack of independence in fulfilling the patients’ ownneeds significantly increased the risk of pressure ulcers in the study group.Background. Old age as a challenge
Starość i sam proces starzenia się są zapewne dla człowieka trudnymi etapami w życiu. Wiele problematycznych
sytuacji, jakże częstych w okresie starości, negatywnie oddziałuje na psychikę człowieka, co skutkuje dużym
rozpowszechnieniem depresji w najstarszej grupie wiekowej. Świadczy to o tym, że wiele osób w starszym wieku
nie jest przygotowanych do okresu starości.
Celem niniejszej pracy, przygotowanej na podstawie przeglądu literatury, jest ukazanie starości jako ważnego
wyzwania dla osoby wkraczającej w starszy wiek. W artykule wskazano na potrzebę samorozwoju jednostki
w ciągu całego życia. Uzasadniono konieczność kształtowania postaw odpowiedzialności za własne życie, w tym
jakości starości. Ukazano osobę w starszym wieku wobec wielu problemów, z jakimi na co dzień musi się zmagać.Old age and the very process of ageing is a difficult time for people. Many difficult situations, which happen very
frequently in old age, negatively influence peoples’ mental state, which results in increased incidence of depression
among the elderly. This proves that many elderly people are not prepared for old age.
The aim of the study, based on the literature review, is to show that old age is a serious challenge for the elderly.
The article indicates that there is a need for life-long self development. It justifies the necessity of being responsible
for one’s own life including the quality of old age. It also shows everyday problems of the elderly and ways
of coping with these problems
Sense of coherence and life satisfaction in students of the University of the Third Age
Wstęp. Poczucie koherencji określa stopień radzenia sobie w trudnych sytuacjach. Jest ważnym wyznacznikiem
satysfakcji życiowej starzejącej się jednostki, a tym samym adaptacji do okresu starości. Celem pracy było zbadanie
związku między poczuciem koherencji a poziomem satysfakcji życiowej u słuchaczy Uniwersytetu Trzeciego
Wieku (UTW).
Materiał i metody. Przebadano 77 osób będących słuchaczami UTW w Bydgoszczy na przełomie marca i kwietnia
2008 roku. Uczestnictwo w UTW było podstawowym kryterium doboru osób do badań. Analizy prowadzono
metodą sondażu diagnostycznego przy użyciu standaryzowanych kwestionariuszy Ogólnej Orientacji Życiowej
SOC-29 i WHOQOL-Bref.
Wyniki. Poczucie koherencji bardzo silnie wiązało się z poziomem jakości życia na poszczególnych jej podskalach.
Średni globalny poziom SOC osiągnął wartość 134,06. Jakość życia badanych znalazła się na granicy 13,50–
–15,43 i stanowiła przeciętnie 3/4 maksymalnego wyniku. Wykształcenie i stan cywilny nie wykazały istotnego
wpływu na poziom poczucia koherencji i jakość życia słuchaczy UTW.
Wnioski. Wykazanie istotnego związku między poczuciem koherencji a zadowalającym poziomem jakości życia
świadczy o dobrym przystosowaniu się do okresu starości słuchaczy UTW. Całożyciowa edukacja jest ważnym
wyznacznikiem poczucia koherencji i satysfakcji życiowej starszych osób.

determinant of life satisfaction and adaptation to old age. The aim of the study was to examine the relationship
between the SOC and the level of life satisfaction in students of the University of the Third Age.
Material and methods. The study included 77 students of the University of the Third Age in Bydgoszcz and was
conducted in March and April 2008. The main inclusion criterion was attending the University of the Third Age.
A diagnostic survey was done using standardized questionnaires: life orientation questionnaire (SOC-29) and
WHOQOL-Bref.
Results. The sense of coherence was strongly related to the level life quality on the subscales. The mean global
SOC level was 134.06. Life quality was between 13.50 and 15.43, which was equal to 3/4 of the total score.
Education and marital status didn’t show any significant impact on the SOC level and quality of life of the
University of the Third Age students.
Conclusions. The study shows a significant relationship between the strong sense of coherence and satisfying
life quality which means that the students of the University of the Third Age are well adapted to old age. Whole
life education is an important determinant of SOC and life satisfaction in the elderly.

Effect of melatonin on the chosen markers of oxidative stress in the elderly patients with essential arterial hypertension (a preliminary study)
Wstęp. Stres oksydacyjny oraz hormon szyszynki - melatonina - są związane z patogenezą starzenia się
i chorób związanych z wiekiem podeszłym. Celem badań było oznaczenie wpływu suplementacji melatoniną na
stężenie dialdedydu malonowego (MDA) i melatoniny oraz aktywności wewnątrzkomórkowej Cu, Zn-dysmutazy
ponadtlenkowej (SOD-1) we krwi osób w podeszłym wieku z pierwotnym nadciśnieniem tętniczym.
Materiał i metody. W badaniach udział wzięło 10 pacjentów z pierwotnym nadciśnieniem tętniczym, leczonych
diuretykiem (śr. 80,2 ± 10 lat) oraz 8 osób z prawidłowym ciśnieniem bez chorób o poznanej etiologii wolnorodnikowej,
które stanowiły grupę odniesienia (średnia wieku 79,4 ± 6 lat). Badani przez 30 dni na godzinę przed
snem przyjmowali 5 mg melatoniny (firmy LE-KAM). Materiał do badań stanowiła krew pobierana o godzinie 8.00
przed rozpoczęciem eksperymentu i po 30 dniach suplementacji. W surowicy oznaczano stężenie melatoniny,
a w hemolizacie stężenie MDA oraz aktywność SOD-1.Wyniki. U osób z nadciśnieniem tętniczym stwierdzono niższe stężenie melatoniny (odpowiednio 6,06 ± 4,9
i 7,07 ± 3,6 pg/ml) oraz wyższe stężenie MDA (0,293 ± 0,03 i 0,275 ± 0,04 µmol/gHb) i niższą aktywność SOD-1
(2557,1 ± 300 i 2745,5 ± 279 U/gHb) niż w grupie pacjentów z prawidłowym ciśnieniem. Nie były to jednak
różnice znamienne statystycznie. Po suplementacji w grupie zarówno z nadciśnieniem tętniczym, jak i odniesienia
stwierdzono znamienny statystycznie spadek stężenia MDA (odpowiednio 0,219 ± 0,05 µmol/gHb,
p < 0,001 i 0,225 ± 0,04 µmol/gHb, p < 0,02) oraz wzrost aktywności SOD-1 (2974,8 ± 532 U/gHb, p < 0,01
i 3227,1 ± 37 U/gHb, p < 0,01), a także wzrost stężenia melatoniny (15,8 ± 11,1 i 14,37 ± 12,41 pg/ml),
znamienny statystycznie w grupie z nadciśnieniem tętniczym (p < 0,02).
Wnioski. Wyniki badań potwierdzają antyoksydacyjne właściwości melatoniny. Suplementację tym hormonem
można wykorzystać jako terapię wspomagającą leczenie hipotensyjne u osób w podeszłym wieku.Background. Oxidative stress and the pineal hormone - melatonin - are involved in the pathogenesis of aging
and aging-related diseases. The aim of our study was determination of the effect of melatonin administration on
malonyldialdehyde (MDA) and melatonin concentrations and the cellular Cu, Zn-superoxide dismutase (SOD-1)
activity in the elderly patients with essential arterial hypertension.
Material and methods. The study was carried out on 10 patients with essential arterial hypertension, who were
cured with thiazide diuretic (average 80.2 ± 10 years) and on 8 normotensive patients, who consituted the
reference group (average 79.4 ± 6 years). They were given 5 mg melatonin (LE–KAM) in one hour before sleep for
30 days. The material of study was blood, taken at 08.00 am before the beginning of experiment and after 30 days
of the melatonin administration. The melatonin concentration was determined in serum, and MDA content and
SOD-1 activity were determined in erythrocytes.
Results. Lower melatonin concentration (6.06 ± 4.9 and 7.07 ± 3.6 pg/ml, respectively) and higher MDA content
(0.293 ± 0.03 and 0.275 ± 0.04 µmol/gHb, respectively) and lower SOD-1 activity (2557.1 ± 300 and 2745.5 ± 279 U/gHb, respectively) were observed in patients with hypertension in comparison to the reference group.
These differences were statistically non-significant. After supplementation in the patients with hypertension and in
reference group statistically lower MDA concentration (0.219 ± 0.05 µmol/gHb, p < 0.001 and 0.225 ± 0.04
µmol/gHb, p < 0.02) and higher SOD-1 activity (2974.8 ± 532 U/gHb, p < 0.01 and 3227.1 ± 37 U/gHb, p < 0.01) were indicated. In examined groups higher melatonin content (15.8 ± 11.1 i 14.37 ± 12.41 pg/ml)
after administration was noted, but these changes were significant only in the patients with hypertension
(p < 0.02).
Conclusions. The antioxidative properties of melatonin were confirmed in the study. The supplementation of this
hormone may be used as supportive therapy in the hypertensive treatment of the elderly people
