Palmityl Coenzyme A Inhibition of Fatty Acid Synthesis
The effects of acyl-CoA derivatives (C8 to C20) on the activity
of the fatty acid synthetases from yeast and Corynebacterium
diphtheriae have been examined. Both enzyme
systems are inhibited by the longer chain acyl thioesters
(C16 to C20) and protected against this inhibition by bovine
serum albumin (BSA). Identical relief from acyl-CoA inhibition
is provided by the 6-0-methylglucose-containing
lipopolysaccharide (MGLP), from Mycobacterium phlei. It
is shown that MGLP forms a stable complex with palmitylCoA.
This interaction accounts for the BSA-like effects of
the polysaccharide. BSA and MGLP have two further effects
on the fatty acid synthetases under study, also attributable
to complex formation with palmityl-CoA. They
stimulate the rate of over-all synthesis from acetyl-CoA and
malonyl-CoAt and they cause a shift of the fatty acid pattern
towards products of shorter chain length. The observed effects
are discussed in terms of the regulation of fatty acid
synthesis both with respect to rate and product composition.
It is concluded that in the two microbial enzyme systems negative
feedback inhibition and its relief are important control
mechanisms

Observação fotográfica da ocorrência de bolhas em selantes de fóssulas e fissuras
The purpose of this study was to employ photographic evaluation to observe the presence or absence of bubbles in pit and fissure sealants Concise and Prisma Shield, at 7 days, 18 months, 36 months and 11 years after accomplishment of treatment. METHODS: A hematoxylin-based staining solution was applied to the occlusal surface at the study periods, which allowed assessment of the sealing material on the surface. All occlusal surfaces were photographed and the photographs corresponding to each period were subjected to photographic evaluation. RESULTS: There was statistically significant difference only at 18 months, with a significantly higher number of specimens with bubbles for the Prisma Shield sealant compared to the Concise, whereas the groups were similar at all other study periods. On the other hand, separate analysis of subsequent periods of the sealants did not reveal any significant difference. CONCLUSIONS: According to the results of the present longitudinal investigation, it may be concluded that the sealing materials showed that bubbles came into and went out with time. However, this fact did not interfere with its clinical purpose, provided the remaining material covers the expected region of pits and fissures.Avaliação fotográfica para observar a presença ou a ausência de bolhas nos selantes de fóssula e fissuras Concise e Prisma Shield aos 7 dias, 18 meses, 36 meses e 11 anos, após a realização dos selamentos. METODOLOGIA: Foi aplicado na superfície oclusal, em todos os períodos de análise, uma solução corante a base de hematoxilina, que possibilitou a verificação e a avaliação dos materiais seladores presentes nesta superfície. Todas as superfícies oclusais foram fotografadas e as fotografias, correspondentes a cada período em análise, foram submetidas à avaliação fotográfica. RESULTADOS: Verificou-se diferença estatisticamente significativa somente aos 18 meses de análise, com o selante Prisma Shield apresentando um número significativamente maior de bolhas superficiais, que o selante Concise; enquanto que os demais grupos apresentaram-se estatisticamente semelhantes entre si, em todos os outros períodos de análise. Por outro lado, quando os selantes foram analisados isoladamente, nos períodos subseqüentes, não foi observada a presença de diferença estatisticamente significativa. Referral management and the care of patients with diabetes: the Translating Research Into Action for Diabetes (TRIAD) study.
OBJECTIVE:
To examine the effect of referral management on diabetes care.
STUDY DESIGN:
Cross-sectional analysis.
PATIENTS AND METHODS:
Translating Research Into Action for Diabetes (TRIAD) is a multicenter study of managed care enrollees with diabetes. Prospective referral management was defined as "gatekeeping" and mandatory preauthorization from a utilization management office, and retrospective referral management as referral profiling and appropriateness reviews. Outcomes included dilated eye exam; self-reported visit to specialists; and perception of difficulty in getting referrals. Hierarchical models adjusted for clustering and patient age, gender, race, ethnicity, type and duration of diabetes treatment, education, income, health status, and comorbidity.
RESULTS:
Referral management was commonly used by health plans (55%) and provider groups (52%). In adjusted analyses, we found no association between any referral management strategies and any of the outcome measures.
CONCLUSIONS:
Referral management does not appear to have an impact on referrals or perception of referrals related to diabetes care
Self-Reported Cognitive Function and Mental Health Diagnoses among Former Professional American-Style Football Players
Clinical practice strongly relies on patients' self-report. Former professional American-style football players are hesitant to seek help for mental health problems, but may be more willing to report cognitive symptoms. We sought to assess the association between cognitive symptoms and diagnosed mental health problems and quality of life among a cohort of former professional players. In a cross-sectional design, we assessed self-reported cognitive function using items from the Quality of Life in Neurological Disorders (Neuro-QOL) Item Bank. We then compared mental health diagnoses and quality of life, assessed by items from the Patient-Reported Outcome Measurement Information System (PROMIS ®), between former professional players reporting daily problems in cognitive function and former players not reporting daily cognitive problems. Of the 3758 former professional players included in the analysis, 40.0% reported daily problems due to cognitive dysfunction. Former players who reported daily cognitive problems were more likely to also report depression (18.0% vs. 3.3%, odds ratio [OR] = 6.42, 95% confidence interval [CI] [4.90-8.40]) and anxiety (19.1% vs. 4.3%, OR = 5.29, 95% CI [4.14-6.75]) than those without daily cognitive problems. Further, former players reporting daily cognitive problems were more likely to report memory loss and attention deficit(/hyperactivity) disorder and poorer general mental health, lower quality of life, less satisfaction with social activities and relationships, and more emotional problems. These findings highlight the potential of an assessment of cognitive symptoms for identifying former players with mental health, social, and emotional problems
