Implementation of an Interprofessional Medication Therapy Management Experience
Objective. To measure the impact of an interprofessional experience (IPE) in medication therapy management (MTM) on students' attitudes and skills regarding interprofessional collaboration (IPC). Methods. This interprofessional MTM experience spanned three weeks, with health science students (medicine, nursing, nutrition, and pharmacy) meeting once weekly. The IPE facilitated interprofessional student collaboration via small-group sessions to conduct MTM consultations for patients with complex chronic conditions. Student learning and attitudinal changes were evaluated by comparing pre- and post-IPE survey responses and a qualitative summary of the students' clinical recommendations. Efficacy of student groups was measured via patient satisfaction surveys and was reported by frequency of response. Results. Twenty-seven students participated in the program and 22 completed both pre- and post-IPE surveys (81% response rate). The survey included open-ended and Likert-type items assessing students' attitudes and skills regarding the IPE as well as their reactions to the experience. Significant changes were observed for two attitudinal items regarding interprofessional teams: maintaining enthusiasm/interest and responsiveness to patients' emotional and financial needs. Patient-reported satisfaction and students' complex clinical recommendations provided further evidence of student learning. Conclusion. This novel IPE in MTM promoted interprofessional collaboration and education in this unique patient care area. Students' attitudes toward and skills in interprofessional collaboration improved, and the patients who received care reported positive experiences. Many health professions programs face challenges in meeting IPE requirements. Selective Theca Cell Dysfunction in Autoimmune Oophoritis Results in Multifollicular Development, Decreased Estradiol, and Elevated Inhibin B Levels
We describe the clinical course of three women with presumptive autoimmune oophoritis who developed multiple follicles but very low to undetectable estradiol levels. Multiple follicles developed spontaneously in all subjects and during pulsatile GnRH treatment for ovulation induction in subject 1. The development of multiple dominant follicles was accompanied by LH levels in the postmenopausal range and FSH levels at the upper limit for premenopausal women. Serum inhibin B levels were elevated appropriately in the setting of multifollicular development, but estradiol levels remained low. Measurement of estradiol precursors demonstrated androstenedione and estrone levels below the 95th percentile in normal women. Adrenal cortical antibodies, and antibodies to 21-hydroxylase and P450 side chain cleavage enzymes were identified in all subjects. All subjects met the criteria for premature ovarian failure during follow-up. Subject 1 later developed adrenal failure, whereas subject 3 had adrenal failure at the time of the study.
These subjects elucidate the hormonal pattern in autoimmune oophoritis, before the full criteria for premature ovarian failure are met. The elevated inhibin A and B levels, which accompany the development of multiple small and dominant follicles in these women, suppress FSH relative to LH levels, virtually independent of estradiol. These data provide further evidence for an important role of inhibin B and inhibin A in the negative feedback control of FSH. In addition, the normal inhibin A and inhibin B production in the absence of estradiol precursors and estradiol provide insight into the selective dysfunction of the theca cells in autoimmune oophoritis

Association of the functional ovarian reserve with serum metabolomic profiling by nuclear magnetic resonance spectroscopy:A cross sectional study of ~400 women
Background:
Women with diminished ovarian reserve are known to have increased cardiovascular risk, whether there is a continuous association between the ovarian reserve biomarkers; anti-Müllerian hormone (AMH), antral follicle count (AFC) and cardio-metabolic risk factors are unknown.
Methods:
A cross-sectional study of 398 women intending to undergo IVF with pre-treatment early follicular AMH and AFC measurements. Serum lipids, lipoprotein subclasses and low-molecular-weight metabolites were quantified by NMR spectroscopy (155 metabolic measures). Associations were analysed using multivariable regression.
Results:
Participants were mean 35.5 (SD 4.43) years old and had a median AMH of 16 pmol/l (IQR 8.8, 28.0 pmol/l) and a median AFC of 12 (IQR 7.16). AMH showed positive associations with HDL, omega-6 and polyunsaturated fatty acids and the amino acids isoleucine, leucine and tyrosine, with effects ranging from 0.11 (95%CI 0.004 to 0.21) for total lipids in small HDL to 0.16 (0.06 to 0.26) for isoleucine, for a mean difference of one SD of metabolite per one SD increment in AMH, and negatively with acetate: − 0.31(− 0.22, − 0.004) SD per 1 SD AMH. AFC was positively associated with alanine, glutamine and glycine. Results were consistent, though less precisely estimated, when restricted to those women who were preparing for treatment because of their partner’s infertility.
Conclusions:
In women intending to have IVF, AMH and AFC were not associated with traditional lipid measured but were associated with a number of novel cardiovascular risk factors. Prospective studies will be required for replication, determination of causality and confirmation that ovarian reserve is impacting on metabolism rather than variation in metabolism is influencing ovarian reserve
Implementation and evaluation of the new wind algorithm in NASA's 50 MHz doppler radar wind profiler
The purpose of this report is to document the Applied Meteorology Unit's implementation and evaluation of the wind algorithm developed by Marshall Space Flight Center (MSFC) on the data analysis processor (DAP) of NASA's 50 MHz doppler radar wind profiler (DRWP). The report also includes a summary of the 50 MHz DRWP characteristics and performance and a proposed concept of operations for the DRWP
Spectroscopy of Nine Cataclysmic Variable Stars
We present optical spectroscopy of nine cataclysmic binary stars, mostly
dwarf novae, obtained primarily to determine orbital periods Porb. The stars
and their periods are LX And, 0.1509743(5) d; CZ Aql, 0.2005(6) d; LU Cam,
0.1499686(4) d; GZ Cnc, 0.0881(4) d; V632 Cyg, 0.06377(8) d; V1006 Cyg,
0.09903(9) d; BF Eri, 0.2708804(4) d; BI Ori, 0.1915(5) d; and FO Per, for
which Porb is either 0.1467(4) or 0.1719(5) d.
Several of the stars proved to be especially interesting. In BF Eri, we
detect the absorption spectrum of a secondary star of spectral type K3 +- 1
subclass, which leads to a distance estimate of approximately 1 kpc. However,
BF Eri has a large proper motion (100 mas/yr), and we have a preliminary
parallax measurement that confirms the large proper motion and yields only an
upper limit for the parallax. BF Eri's space velocity is evidently large, and
it appears to belong to the halo population. In CZ Aql, the emission lines have
strong wings that move with large velocity amplitude, suggesting a
magnetically-channeled accretion flow. The orbital period of V1006 Cyg places
it squarely within the 2- to 3-hour "gap" in the distribution of cataclysmic

The analysis of rapidly developing fog at the Kennedy Space Center
This report documents fog precursors and fog climatology at Kennedy Space Center (KSC) Florida from 1986 to 1990. The major emphasis of this report focuses on rapidly developing fog events that would affect the less than 7-statute mile visibility rule for End-Of-Mission (EOM) Shuttle landing at KSC (Rule 4-64(A)). The Applied Meteorology Unit's (AMU's) work is to: develop a data base for study of fog associated weather conditions relating to violations of this landing constraint; develop forecast techniques or rules-of-thumb to determine whether or not current conditions are likely to result in an acceptable condition at landing; validate the forecast techniques; and transition techniques to operational use. As part of the analysis the fog events were categorized as either advection, pre-frontal or radiation. As a result of these analyses, the AMU developed a fog climatological data base, identified fog precursors and developed forecaster tools and decision trees. The fog climatological analysis indicates that during the fog season (October to April) there is a higher risk for a visibility violation at KSC during the early morning hours (0700 to 1200 UTC), while 95 percent of all fog events have dissipated by 1600 UTC. A high number of fog events are characterized by a westerly component to the surface wind at KSC (92 percent) and 83 percent of the fog events had fog develop west of KSC first (up to 2 hours). The AMU developed fog decision trees and forecaster tools that would help the forecaster identify fog precursors up to 12 hours in advance. Using the decision trees as process tools ensures the important meteorological data are not overlooked in the forecast process. With these tools and a better understanding of fog formation in the local KSC area, the Shuttle weather support forecaster should be able to give the Launch and Flight Directors a better KSC fog forecast with more confidence
Evaluation of the Emergency Response Dose Assessment System(ERDAS)
The emergency response dose assessment system (ERDAS) is a protype software and hardware system configured to produce routine mesoscale meteorological forecasts and enhanced dispersion estimates on an operational basis for the Kennedy Space Center (KSC)/Cape Canaveral Air Station (CCAS) region. ERDAS provides emergency response guidance to operations at KSC/CCAS in the case of an accidental hazardous material release or an aborted vehicle launch. This report describes the evaluation of ERDAS including: evaluation of sea breeze predictions, comparison of launch plume location and concentration predictions, case study of a toxic release, evaluation of model sensitivity to varying input parameters, evaluation of the user interface, assessment of ERDA's operational capabilities, and a comparison of ERDAS models to the ocean breeze dry gultch diffusion model
